Background: To evaluate the diagnostic performance of IOTA simple rules in differentiating between benign and malignant ovarian tumors. Materials and Methods: A study of diagnostic performance was conducted on women scheduled for elective surgery due to ovarian masses between March 2007 and March 2012. All patients underwent ultrasound examination for IOTA simple rules within 24 hours of surgery. All examinations were performed by the authors, who had no any clinical information of the patients, to differentiate between benign and malignant adnexal masses using IOTA simple rules. Gold standard diagnosis was based on pathological or operative findings. Results: A total of 398 adnexal masses, in 376 women, were available for analysis. Of them, the IOTA simple rules could be applied in 319 (80.1%) including 212 (66.5%) benign tumors and 107 (33.6%) malignant tumors. The simple rules yielded inconclusive results in 79 (19.9%) masses. In the 319 masses for which the IOTA simple rules could be applied, sensitivity was 82.9% and specificity 95.3%. Conclusions: The IOTA simple rules have high diagnostic performance in differentiating between benign and malignant adnexal masses. Nevertheless, inconclusive results are relatively common.
Recently, we experienced a case of rare neurilemmoma originated from intercostal nerve [9th] in the right chest wall in a 25 year old male officer. The tumor was incidentally found in the routine chest X ray, where the round well circumscribed mass tumor the ninth rib with notching and sclerotic margin, suggesting slowly growing benign benign of chest wall was revealed and the tumor mass was easily extirpated in the exploratory thoracotomy, with uneventful recovery. Grossly, the tumor was firm, partly soft and well circumscribed, measuring 4.5X3.0X 3.0 cm with yellowish smooth outer surface, attached with intercostal nerve trunk. Cut surface exhibits partly grayish white and largely hemorrhagic areas. Microscopically, the characteristic palisading arrangement of schwann cells and Verocay bodies are seen but dominant features are cystic degeneration and hemorrhage with organization and fibrosis. The sheath of intercostal nerve and capsule of neurilemmoma were con joined. There is no evidence of malignancy. The tumor was confirmed as neurilemmoma of intercostal nerve, Antony type B.
Ganglioneuroma (GN) is a rare benign tumor of neural crest origin usually found in the abdomen, but may occasionally present at uncommon sites including the cervical, lumbar, or sacral spine. However, GNs of thoracic spine are extremely rare. In this report, we describe a 12-year-old girl with giant GN in the thoracic spine, who underwent successful resection (T1-4 level) of the tumor. Histopathological examination confirmed the diagnosis. GN should be considered in the differential diagnosis of any paraspinal mass. A high index of suspicion and correlation of clinico-radiological findings is necessary in differentiating a large benign tumor from a malignant growth. Complete surgical excision is the treatment of choice; however tumor size and location need to be considered for the surgical approach (one-step or multiple surgeries). Close follow-up after surgery is mandatory.
Studies on the relationship between blood fatty acids and the risk of breast cancer have not yielded definite conclusions. The role of fatty acids in the development and progression of breast cancer is unclear. We conducted a case-control study to determine serum phospholipid fatty acid composition in benign breast tumor and breast cancer. Subjects consisted of 27 benign breast tumor and 68 breast cancer patients, and 28 matched controls. The levels of fatty acids were measured by gas chromatography. Higher arachidonic and palmitic acids were observed in breast cancer patients as compared with control and benign breast tumor patients. The percentage of total saturated fatty acids in breast cancer was higher than in control and benign breast tumor patients. The level of stearic acid was lower in benign breast tumor and breast cancer patients. Saturation index, the ratio of stearic to oleic acid, was lower in benign breast tumor and breast cancer patients compared to the control. Moreover, stearic acid was negatively and arachidonic acid was positively correlated with the cancer stage. In conclusion, our results support that serum phospholipid compositions of specific fatty acids are associated with the risk of benign breast tumor as well as breast cancer. Further studies are necessary to investigate mechanisms linked to the breast cancer etiology.
The diagnostic value of membrane glycolipid biochemistry index, the lipid-bound sialic acid (LSA) and total sialic acid (TSA) in cerebrospinal fluid (CSF) was evaluated in 30 intracranial and 65 gastrointestinal tumors. The plasma LSA, TSA and red cell membrane sialic acid (R-SA) in were determined according to the method of Sevenmerhulm. Our results showed that the levels of LSA and TSA in CSF of intracranial tumor patients was higher than that of normal group(p<0.01). The concentration of TSA and LSA in patients with malignant glioma was higher than that of benign meningioma patients(P<0.01). No significance was found between intracranial halmatoma patients and normal control group for levels of membrane glycolipids (p>0.05). Results also found that the plasma LSA, TSA and R-SA of gastric carcinoma were significantly higher than those of control group (p<0.05); while no significant difference was found in the plasma LSA, TSA and R-SA levels between chronic gastritis, gastrohelcoma and normal control group (p>0.05). Plasma LSA, TSA and R-SA levels of gastric carcinoma patient were significantly higher than those of chronic gastritis patients and gastrohelcoma patients(p<0.05). It was also found that plasma LSA, TSA and R-SA contents were significantly higher in large intestine carcinoma patients than in benign in stestine tumor patients (p<0.05) while no significant difference was found between intestine benign tumor and normal control group (p>0.05). The levels of LSA, TSA and R-SA were obviously higher in the patients with metastasis than in the ones without (p<0.05.) The membrane glycolipid biochemistry index LSA and TSA in CSF are sensive markers for diagnosing intracranial tumors. For gastrointestinal malignant tumors the plasma LSA TSA and red blood cell membrane SA may be considered as auxiliary indicators for diagnosis. They can be used for distinguishing benign from malignant tumors.
Background and Objectives : Salivary gland neoplasms are unique because of their infrequency, inconsistent classification, and highly variable biologic behavior. The aim of this study is to analysis the histopathologic classification of salivary glnad neoplasm and to suggest a guideline of management. Materials and Methods : The medical records of 310 patients with salivary gland neoplasm who treated at Asan medical center between 1992 and 2001 were analyzed retrospectively. Among the 310 patients, 138 patients were male and 172 patients were female. Mean age was 50.5 years. Results : Benign salivary neoplasms were 213 cases. They consisted of 153 cases (71.8%) of parotid tumor, 41 cases (19.2%) of submandibular gland tumor and 19 cases (8.9%) of minor salivary gland tumor. Pleomorphic adenoma was the most common benign neoplasm. Malignant salivary neoplasms were 97 cases. They consisted of 45 cases (46.4%) of parotid tumor, 26 cases(26.8%) of minor salivary gland tumor, 24 cases(24.7%) of submandibular gland tumor and 2 cases(2.1%) of sublingual gland tumor. Adenoid cystic carcinoma was the most common malignant neoplasm. Conclusions : The most commonly involved gland was parotid (64%) and the most frequent tumor was pleomorphic adenoma (52%). Although the majority of minor salivary gland neoplasms are malignant, three of parotid tumors are benign.
Benign lung tumors compose a heterogeneous group of solid growths that present variations in clinical features, depending on whether the origin is within the bronchus on lung parenchyma or from visceral pleura. Benign tumors of the lung are relatively uncommon, and series are to be found in the literature and the classification of benign tumors of the lung continues to be controversial because of disagreement concerning the origin and prognosis of many common lesions. We adopt Liebows original classification but excluded bronchial adenoma which no longer considered as benign tumor and added pulmonary A-V fistula and congenital cystic adenomatoid malformation. We analyzed 61 cases of benign tumors which were composed of 16 original Korean cases and 45 cases which were reported on journal of Thoracic & Cardiovascular Surgery. The results were. [1] Incidence; Of 61 cases, chondromatous hamartoma was 2 cases [41 %], congenital cystic adenomatoid malformation 10 cases [16.4 %], pulmonary A-V fistula 5 cases [8.1 %], sclerosing hemangioma 4 cases [6.5 %], teratoma, plasma cell granuloma & mesothelioma were 3 cases [4.9%], Castlemans disease 2 cases [3.3%], and mucous gland adenoma, paraganglioma, and leiomyoma 1 case [1.6 %]. [2] Age & Sex distribution; Male 30 cases and female 31 cases. Mean age was 31.4 years old. [3] Main symptom; was coughing, 32.8%, and no symptom, 24.6%. [4] Sixty eight percentage of chest film showed mass density, and 4 cases showed calcification, 2 cases had lobulation. [5] Size of mass was large and multiple mass was 2 cases. Endobronchial tumors were 9 cases, 14.9 %. [6] Three cases of endobronchial tumor were preoperatively diagnosed by bronchoscopy and 2 cases of pulmonary A-V fistula were diagnosed by pulmonary arteriography. [7] Seven cases, 11.5%, had associating diseases such as bronchogenic cyst, thymic cyst, Schwannoma, situs inversus, bronchiectasis and bronchogenic carcinoma. [8] Minor resection such as excision 8i: wedge resection were 15 cases, 26.2 %, and 6 cases, 75.4 %, of lobectomy were performed including 5 cases of pneumonectomy 5 cases had. [9] Postoperative complications; One case, 1.6 %, expired due to respiratory insufficiency. Two cases had re-operation due to bleeding and hemoptysis.

Benign osteoblastoma, a rare tumor which forms osteoid and woven bone, represents 3% of benign and 1% of all primary bone tumors. It can involve anywhere in the axial skeleton, which is most common in the spine (34%). But the lesion of the calcaneus is extremely rare. The ratio of males to females is 2:1, with 80 percent of the patients being under thirty years of age. Pain is the most common symptom and rarely pathologic fracture. Osteoblastoma is a benign tumor but has a high recurrence rate after intracapsular malignant change. Therefore, choice of surgical procedure is the 'enbloc' resection. Authors have experienced a case of osteoblastoma developed in calcaneus. The treatment was wide resection and triple arthrodesis. We have reported here the case in which osteoblastoma occured in calcaneus of the right foot.
56 dogs that had undergone surgical excision of skin lesion tumors at Veterinary Medical Teaching Hospital in Seoul National University from 2000 to 2005 were evaluated retrospectively. The ratio of cases with benign versus malignant tumor types were 1.5:1.48 cases received surgical excision alone and 8 cases received adjuvant treatments. 42 cases were followed postoperative recurrence time and survival time (25 benign, 17 malignant). 25 cases (25/42, 60%) had no recurrence after surgical excision. In malignant tumor types, 5 cases (5/17, 29%) had recurrence and the median recurrence time was 157 days, and 8 cases (8/17, 47%) died due to tumors and survival time was 125 days. In benign tumor types, only 2 cases (2/25, 8%) had recurrence and the median recurrence time was 273 days.

Undifferentiated pleomorphic sarcoma (UPS) in the mandible region is a rare malignant neoplasm. We encountered the case of a patient who had been diagnosed with deep benign fibrous histiocytoma (DBFH) prior to surgery, but the final biopsy revealed the presence of tumor tissues in the resected margin, which indicated a UPS. Therefore, radiotherapy was conducted post-operatively, but the tumor recurred approximately 3 years later and a second surgery was performed. The difference between the initial and final diagnoses was considered to be due to the characteristics of the tumor, as it involved both benign and malignant components. DBFH and UPS can occur in young women. DBFH can transform to a malignancy and UPS may also appear as a benign tumor during early biopsy. Hence, attention should be paid to diagnosis and treatment methods, and ongoing close observation is required.
