Statistical Analysis on Hospitalized Patients with Thoracolumbar Compression Fracture at Single Korean Medicine Hospital: Retrospective Review (일개 한방병원에서의 흉요추 압박골절 입원 환자에 대한 통계적 분석: 후향적 차트리뷰)

  • Objectives: This study was designed to statistically analyze patients hospitalized for thoracolumbar compression fractures at Daejeon University Daejeon Korean Medicine Hospital from January 1, 2017 to December 31, 2021. Methods: A total of 62 patients were analyzed through electronic medical records in this study. The extracted data was analyzed using IBM SPSS ver.27.0. Results: 1. Traffic accident patients were more likely to reduce pain by more than half compared to falls and other patients. Patients without spinal disease were more likely to reduce pain by more than half compared to those without. The shorter the absolute bed rest and the longer the hospital stay, the higher the probability of pain reduction by more than half. 2. The duration of pain half-reduction was longer in patients with diabetes than in patients without diabetes. 3. The longer the hospitalization period and the shorter the absolute bed rest period, the less pain was reported upon discharge. Males complained of less pain at discharge than females, and patients without spinal disease complained less than those without. Patients who did not receive absolute bed rest complained of less pain upon discharge than those who did not. Conclusions: This study included patients hospitalized for thoracolumbar compression fractures and showed that etiology, absolute bed rest period, hospitalization period, gender, spinal disease, diabetes statistically affected the degree of pain reduction.

A Study on the Failed Rest After Work in Association with Cardiovascular and Other Diseases as Well as Physical disorders

  • This survey provides, at a participation rate of 70%, 4,790 examinees. The purpose of this study is to study the association of the failed rest after work with 34 diseases including cardiovascular diseases. The index of the failed rest after work was composed of 4 questions about "thinking of work for several hours", "feeling exhausted", feeling unsatisfied or depressed", and "needing to go to bed early for next day′s work". Estimation of correlation among 4 variables, factor analysis, and ANCOVA adjusted for sex, age and job were carried out. A self-rating questionnaire of one′s own disease history and the "London School of Hygiene Cardiovascular Questionnaire" were used in order to discriminate each morbid group from the opposite group. Brief explanations of the result are as follows: 1) Every variable of failed rest after work shows significant difference between the morbid group and the no morbid group for possible infarction; for angina pectoris in the total, and men. 2) Among 4 variables ′exhaustion′ best discriminates the infarction group from the no infarction group, and the angina group from the no angina group. 3) The factor of failed rest after work is a significant factor that distinguishes the infarction group from the no infarction group, and the angina pectoris group from the no angina group. Therefore, stress management through health education and promotion such as behavioral modification can be used to reduce cardiovascular diseases and stress as perceived by an individual.

Integrated Service Framework for Naval Ship using REST Architecture (REST 아키텍처를 이용한 함정 통합 서비스 프레임워크)

  • The functions of various systems inside the naval ships have been improved and diversified to prepare for the future battlefield and satisfy new requirements. The number of missions and operating displays with which a crew has to deal has increased. In this paper, Integrated Service Framework for Naval Ships is proposed. The proposed framework is based on REST architecture and it interfaces the networks of each systems in the ship. The connected data and functions are rebuilt as an object and they are provided as HTTP and JSON based APIs. The prototype of the framework was implemented in a combat system test-bed and its feasibility was proved with performance tests. With the proposed framework, operational efficiency for a naval ship can be achieved by designing new integrated displays and HCI which utilize whole ship's capabilities.

Effect of Positioning on Back Pain and Comfort of Bed Rest Patients after Transhepatic Arterial Chemoembolization (체위변경이 간동맥 화학색전술 후 침상안정기 환자의 요통과 안위에 미치는 효과)

  • Purpose: The study was done to investigate whether positioning relieves the back pain and enhances comfort during bed rest after transhepatic arterial chemoembolization(TACE). Methods: A quasi-experimental design with non-equivalent control group non-synchronized design was used. The experimental group was placed semi Fowler's position for the first 2 hours. The following 4 hours the position was rotated hourly between 30 degrees laterally inclined position and a semi Fowler's position. The control group was maintained in a supine position for the 6 hours. Data were analyzed using t-test, $x^2$-test, Fisher's exact test and repeated measures ANOVA. Results: 1) Back pain intensity significantly decreased in the experimental group compared to the control group. 2) Comfort level significantly decreased in both groups, but there was no significant difference between the two groups. 3) There were no hematoma and bleeding complication in either group. In addition, there was no significant difference in urinary retention between the two groups. 4) Analgesics were less frequently taken by the experimental group. Conclusion: These results suggest that positioning relieves back pain without causing an increased incidence of hematoma and bleeding formation after TACE and this nursing intervention might help patients be more comfortable during the treatment of TACE.

Usefulness of Balloon-type Hemostatic Device After Transarterial Chemoembolization (간동맥 화학색전술 후 풍선형 지혈기구의 유용성)

  • Transarterial chemoembolization is one of the most representative procedures for puncture of the femoral artery. In addition, the same procedure is often repeatedly performed many times, and Hepatocellular carcinoma patients due to cancer is significantly lowered blood tests, regardless of platelet counts are not good enough to stop bleeding. More importantly, hepatocellular carcinoma has a high degree of complication and disease severity, which makes it less likely that the condition of the body will be relatively inferior to other patients. In order to prevent delayed hemorrhage of the femoral artery puncture site after the procedure, it is advised to absolutely stabilize the limb so that it does not bend the limb for 3 hours after climbing in the ward. Therefore, I have been complaining about inconvenience. In addition, in order to prevent delayed hemorrhage after hemostasis, balloon type hemostatic device was used instead of sand bag which was placed on hemostatic site. The results of this study were compared with the results of actual application. The use of a balloon-type hemostatic device to increase the effectiveness of continuous hemostasis and to minimize the inconvenience during the time of patient's absolute bed rest, rather than raising the sandbag to prevent primary delayed hemorrhage by various methods in transarterial chemoembolization. It can be used as a substitute for existing sand bags because it can alleviate pain, increase satisfaction, and can be used as a disposable one.

A Study on the Safety of Continuous Hemostasis after Arterial Puncture Intervention (동맥천자 인터벤션 시술 후 지속지혈 안전성에 대한 연구)

  • Most of the vascular procedures performed for various diagnoses and treatments of various abdominal intervention procedures performed by the Department of Radiology and Angiography are performed by puncture of the femoral artery. For this reason, patients should undergo blood-related tests such as prothrombin time (PT) and partial thromboplatin time (PTT). Therefore, many patients are instructed to take precautions such as putting a sandbag on the puncture site to prevent delayed hemorrhage after hemostasis of the femoral artery puncture site, and not to bend the leg of the treated area for about 3 hours. Because of this, many patients have complained of pain during the procedure and inconvenience during the absolute bed rest time in the ward. The purpose of this study was to compare the safety of balloon ancillary devices with sandbags placed on the hemostasis site to prevent delayed hemorrhage after arterial puncture. We compared the safety of each patient with the results of medical records in consideration of the problem that the patient could not press with the focus, the position of the patient was changed depending on the patient's body shape, and the problem of falling down according to the location of the puncture site. As a result, the use of a balloon type ancillary device improves the effect of continuous hemostasis, reduces discomfort during the patient's absolute stabilization time, increases the patient's satisfaction, and is a good alternative to the existing sandbag.

Lumbar herniated disc: spontaneous regression

  • Background: Low back pain is a frequent condition that results in substantial disability and causes admission of patients to neurosurgery clinics. To evaluate and present the therapeutic outcomes in lumbar disc hernia (LDH) patients treated by means of a conservative approach, consisting of bed rest and medical therapy. Methods: This retrospective cohort was carried out in the neurosurgery departments of hospitals in KahramanmaraŞ city and 23 patients diagnosed with LDH at the levels of L3-L4, L4-L5 or L5-S1 were enrolled. Results: The average age was $38.4{\pm}8.0$ and the chief complaint was low back pain and sciatica radiating to one or both lower extremities. Conservative treatment was administered. Neurological examination findings, durations of treatment and intervals until symptomatic recovery were recorded. $Las{\grave{e}}gue$ tests and neurosensory examination revealed that mild neurological deficits existed in 16 of our patients. Previously, 5 patients had received physiotherapy and 7 patients had been on medical treatment. The number of patients with LDH at the level of L3-L4, L4-L5, and L5-S1 were 1, 13, and 9, respectively. All patients reported that they had benefit from medical treatment and bed rest, and radiologic improvement was observed simultaneously on MRI scans. The average duration until symptomatic recovery and/or regression of LDH symptoms was $13.6{\pm}5.4$ months (range: 5-22). Conclusions: It should be kept in mind that lumbar disc hernias could regress with medical treatment and rest without surgery, and there should be an awareness that these patients could recover radiologically. This condition must be taken into account during decision making for surgical intervention in LDH patients devoid of indications for emergent surgery.

Objective Evidence for the Effectiveness of Single-session Treatment with a Spinal Thermal Massage Device: A Pilot Study (척추온열마사지기기의 1회 치료의 효과에 대한 객관적 증거: 선행 연구)

  • Individuals often report significant relief from pain and stress even after a single session of massage therapy; however, no previous studies have provided objective evidence supporting the effectiveness of a solitary massage therapy session. In the present study, we aimed to investigate the effectiveness of one-time treatment with a spinal thermal massage device reported to exert the same therapeutic effects as massage therapy in terms of pain reduction and stress relief. A man with chronic low back pain (LBP) underwent two rounds of experiments involving spinal massage treatment and bed rest, respectively. Pain was assessed using a visual analog scale, and heart rate variability (HRV) was measured in real-time to examine autonomic nervous system (ANS) activity. Blood samples were obtained at five points during each round of the experiment to examine changes in cortisol, epinephrine, and norepinephrine. Spinal massage significantly reduced pain and enhanced parasympathetic activity when compared with the bed rest condition. In addition, both epinephrine and norepinephrine levels were lower following spinal massage than following bed rest. These results are consistent with the reported effects of conventional massage therapy and support the effectiveness of one-time treatment using a spinal thermal massage device.

  • The microgrid system is the combination of photovoltaic(PV) array, load, and battery energy storage system. The control strategies were defined as multi-modes of operation, including rest operation without use of battery, power charging, and power discharging, which enables grid connected mode or islanded mode. Photovoltaic power is a problem of the uniformity of power quality because the power generated from solar light is very sensitive to variation of insolation and duration of sunshine. As a solution to the above problem, energy storage system(ESS) is considered generally. There fore, in this study, we did basic research activities about optimization method of the amount of energy used, using a smart microgrid test-bed constructed in building. First, we analyzed the daily, monthly and period energy pattern amount of power energy used, and analyzed PV power generation level which is built on the roof. Utilizing building energy pattern analysis data, we was studied an efficient method of employing the ESS about building power consumption pattern and PV generation.

The Study of Effect on Oxygen Chamber for Recovery of Muscular Fatigue (산소 챔버의 근육 피로회복효과에 대한 연구)

  • Objectives: This Study was designed to investigate the effect of Oxygen chamber to recover muscle fatigue. Methods: Twenty Subjects were divided into Oxygen Chamber Group (n=10) and Rest group (n=10). Subjects visited hospital two times, blood tests were performed 3 times for each visit. 1st blood test was performed in 4 hours hunger state. 2nd blood test was performed within 5 minutes after the 6 minutes bike exercise. 3rd blood test was performed after the 40 minutes Oxygen treatment or 40 minutes rest in bed. Blood test items were Lactate, Glucose, Lactate Dehydrogenase, Creatine Kinase, Free Fatty Acid. 2nd visit was applied 1st visit process, But Oxygen Chamber group was crossed over to Rest Group, Rest Group was crossed over to Oxygen Chamber group. The Blood test result was analyzed with paired T-test using SPSS for Windows version 21. Results: The reduction of Lactate in Oxygen Chamber Group (6.86±2.07 mmol/ℓ) was higher than Rest group (6.57±2.33 mmol/ℓ), but it was not statically significant (p=0.68). The reduction of Glucose in Oxygen Chamber Group (6.85±12.14 mg/dl) was lower than Rest group (7.60±9.83 mg/dl), but it was not statically significant (p=0.83). The reduction of Lactate Dehydrogenase in Oxygen Chamber Group (16.10±14.91 mmol/ℓ) was lower than Rest group (19.75±12.46 mg/dl), but it was not statically significant (p=0.41). The reduction of Creatine Kinase in Oxygen Chamber Group (13.40±5.69 U/ℓ) was lower than Rest group (15.25±8.01 U/ℓ), but it was not statically significant (p=0.41). The reduction of Free Fatty Acid in Oxygen Chamber Group (285.50±174.13 uEq/ℓ) was higher than Rest group (196.15±131.58 uEq/ℓ), but it was not statically significant (p=0.07). Conclusions: This study showed Oxygen chamber therapy could be effective to recover muscle fatigue.
