Study on trends of cancer study in TKM and its research strategy in future (한의학계(韓醫學界)의 암연구동향(癌硏究動向)과 연구(硏究) 전략(戰略)에 대한 연구(硏究))

  • For the purpose of designing more successful cancer research, the strategy for cancer study in the field of Traditional Korean Medicine(TKM) during the 21th century was examined from the analysis of trends on cancer study in traditional korean medicine. The results were summarized as follows: 1. So far cancer research in TKM was chiefly done on cytotoxicity, side-effects by chemotherapy, tumor immunology, apoptosis, survival time with S-180 and pulmonary colonization assay and also clonogenic assay, cell adhesion assay, angiogenesis, cell-differentiation and side-effect by radiotherapy were partly performed. 2. It may be ideal that we should study synergistic effect between constituent drugs of prescriptions, tumor immunology, combined therapy between western and oriental medicines by reducing side-effect by radiotherapy and chemotherapy and antimetastasis according to the characteristics of oriental medicine chiefly and also supplement the studies on molecular biology, gene therapy, angiogensis and signal transduction. 3. We had better do specific-field research in cooperation between oriental medical colleges and Korea Institute of Oriental Medicine(KIOM) as well as study a target cancers such as hepatic cancer, pulmonary cancer and gastric cancer more intensively than all cancers domestically. 4. Our country must keep communication with China having many clinical data, Taiwan chiefly doing the combined tharapy between oriental and western medicines, Japan having done basic study actively on cancer.

Postural Risk Assessment of Young Tree Tending Operations in for Managing Pinus Densiflora and Larix Kaempferi Plantations (소나무와 일본잎갈나무 조림지 관리를 위한 어린나무가꾸기의 작업 자세 위험도 분석)

  • Pinus densiflora and Larix kaempferi forests not only extended the plantation management area but also cultivated to the sustainable wood supply chain during the last 5-years. These prescriptions are primarily done by manual operation activities, particularly in young tree tending. Two types of tending activities, brashing by brush cutter(BB) and pruning by pruning shear and saw(PP), were selected for the investigate of musculoskeletal disorders' level and postural risks using Ovako Working Posture Analysis System (OWAS). The postural risk indexes (BB and PP) ranged from 115 to 125 and 102 to 105, respectively. There is no significant difference between the species. The young tree tending operations had low postural risks. The results may be used as basic data to develop technical guideline for safe young trees.

Bibliographic Study on the Book Dongeuisasangjinryoeuijeon (동의사상진료의전(東醫四象診療醫典)에 대한 서지학적 고찰)

  • Objective The purpose of this study was to examine the composition, bibliographic characteristics, and contents of the book, Dongeuisasangjinryoeuijeon(東醫四象診療醫典). Method The images of this book published in 1941 at Haenglim publishing company, stored in the National Library of Korea were acquired and used as basic data, and books related to Sasang constitutional medicine such as Dongeuisasangshinpyun (東醫四象新編), Dongmuyugo(東武遺稿), and Sasanggeumgyebibang(四象金匱秘方) were compared. Result Many of the data were based on the Dongeuisasangshinpyun(東醫四象新編), and were made with reference to Dongmuyugo(東武遺稿) and Sasanggeumgyebibang(四象金匱秘方) partly. This book is thought to be made to be used conveniently for clinicians at the time when data on Sasang Constitutional Medicine was insufficient. In particular, it can be said that the mention of well-established diagnostic method, drugs that cause adverse reactions when a patient of a different constitution takes them and the quatrains of prescriptions using seven Chinese characters in each line are very unique. Conclusion Although most of the books, including Dongeuisasangshinpyun(東醫四象新編), were cited, it is thought that the book was well-organized in order to provide good information to the clinicians practicing Sasang Constitutional Medicine at the time. For the original part of this book, further research should be conducted.

  • The prevalence of respiratory diseases is increasing due to social and environmental factors such as increased environmental pollution and air pollution, and among them, chronic obstructive pulmonary disease (COPD) in particular has a high mortality and morbidity rate worldwide. As a result, medical expenses are rapidly increasing, creating a social and economic burden. In response to this, there is a need to discuss ways to reduce the risk from diseases and manage them appropriately, and the most basic starting point in this process is how these chronic lung disease patients are treated in actual clinical settings and how to improve the quality of treatment. There is a need to look into whether there are effective drugs. Western treatment for chronic obstructive pulmonary disease is basically a disease in which the airway narrows, so bronchodilators are used to widen the bronchi, and corticosteroids and antibiotics are mainly used to relieve the inflammatory response in the lungs. However, since the mainly used Western medicine does not serve as a fundamental therapeutic drug and contains many side effects, there is a need for drugs that improve the quality of life of patients and are more effective in managing symptoms as symptomatic prescriptions. Therefore, Western and Oriental medicine treatments are needed. The purpose is to suggest better treatments through comparative analysis.

Implementations of Geographic Information Systems on Sewage Management for Water Resources Protection

  • Taipei Watershed Management Bureau (WRATB) is a government agency entitled for water resources protection at two major watersheds in order to provide drinking water for about four millions population in Taipei on a sustainable basis. At WRATB, there are two major public sewage treatment facilities which can convert sewage in each watershed into an acceptable state before they were discharged into rivers. More than 82% of household wastewater have been collected and treated by the two public sewage systems. However, households at remote area still need more effective sewage management prescriptions. The objective of this paper is to implement geographic information systems in order to provide more effective approaches that sewage management can be easier and cost effective. ArcIMS was implemented for Internet browsing and map server of those sewage facilities on personal computers, laptop computers. In the open field, ArcPAD was implemented with personal digital assistant (PDA) such that compact flash type's global positioning systems (GPS) and digital camera can be utilized with PDA. All sewage facilities digital files were convert into ArcMap format files. MapObjects and visual BASIC were used to create sewage application modules to meet every single technician personal flavor. ASP.NET was implemented for Internet database manipulations of all sewage databases. Mobile GIS was the key component of GIS applications in the open field for sewage management on a basis of house by house. Houses at remote area, which can not cover by the two public sewage systems, were managed by PDA and laptop computers with GPS and digital camera. Sewage management at Taipei Watershed Management Bureau is easier both in the open field and in the office. Integration of GPS, GIS, and PDA makes sewage management in the open field much easier. ArcIMS, MapObjects, ASP.NET and visual BASIC make sewage management can be done in the office and over Internet.

  • The first medical text in which detailed treatments of diseases by combinations of materia medica are mentioned is the "Shanghanzabinglun(傷寒雜病論)". True meaning of the combinations of materia medica used in this text lies in the fact that the efficacy of a prescription transcends that of the linear sum of each materia medica. This kind of concept regarding the composition of a prescription has come to contribute to the formation of theories in prescriptionology; However, it is difficult to analyze and interpret the meaning of each prescription separately because of differences in interpreting methods, points of view, and terminologies used by members of different academical branches. Therefore, it is desirable that one should understand a prescription as having been modified from a basic prescription, and then bring the interactions of ingredients into the picture, finally understanding the efficacy and chief virtues of the targeted prescription. Nevertheless, with the massive information of prescriptions, which exist in the format of texts, it is impossible to efficiently take advantage of prescription analyzing methods, and therefore the range of analysis extremely restricted. In order to overcomes these weaknesses, this paper suggests that object-oriented prescription database be constructed and that a search program for education and research that could facilitate an efficient access to the database be developed as well.

The Effects on the Aerobic Function of Major in Dance and Common Students after Treadmill Gait Exercise (트레드밀 보행운동이 무용전공학생과 일반학생의 유산소 운동능력에 대한 비교)

  • The purpose of this study was to examine the effects of treadmill exercise on the aerobic function of dance majors in their 20s. The subjects consisted of 10 female Korean dance majors in their 20s as an experimental group and 10 normal female college students as a control group, and both groups conducted treadmill gait training. Their heart rate recovery (HRR), maximal oxygen uptake (MOU), maximal voluntary ventilation (MVV), and lactate recovery rate (LRR) were measured before and after the experiment. The comparison of changes in the subjects' aerobic function before and after the experiment showed that the experimental group gained more significant effects on HRR and LRR than the control group. Therefore, treadmill gait training may help Korean dance majors to improve their basic physical strength by enhancing their aerobic function, and the findings of this study are likely to provide basic data to support scientific exercise prescriptions.

The Oriental medical study for Constitutions and Anticancer effect on Cancer therapy (암치료에 있어서 체질과 항암효과에 관한 한의학적 연구)

  • As one of the chemical and experimental study on cancer treatment in Korea oriental medicine, we investigated the clinical characteristics on the treatment of cancer patients in oriental medical hospital. Furthermore, we carried out the cytotoxicity test of extracts of herbal medicine. The results are as follows: About Clinical Aspects of the Orient낄 Medical Cancer Treatment- (1) Clinical experience is that 420 Oriental medical hospitals were surveyed. Number of cancer patiens treated by one oriental medical doctor was under 10(about 60%)or from 10 to 20(about 30%). (2) Diagnostic Procedure Is that more than 80% of cancer patients were diagnosed by medical doctor of western hospital. (3) Therapeutic Method is that combination of oriental and western medical treatment was used in oriental medical hospital. (4) Relationship between cancer and constitutions is that to the relationship between cancer and constitutions, about 75% responded and most were considered to be Um. About Experimental Study- Toevaluate the direct cytotoxicity of hot water and methanol extracts of 34 oriental medical prescriptions on stomach cancer cell, SRB assay was carried out. (1) The $IC_{50}$ value of water extract of Samsoum was $19{\mu}g/ml$ and that of methanol extract was $15{\mu}g/ml$ so that we confirmed Samsoum has a strong direct cytotoxicity. (2) The $IC_{50}$ values of water extract of Coptidis Rhizoma, methanol extract of Caesalpiniae Lignum and Draba Semen were $10{\mu}g/ml$, $9{\mu}g/ml$ and $12.5{\mu}g/ml$ respectively, so that we confirmed Coptidis Rhizoma, Caesalpiniae Lignum and Drabs Semen have a strong direct cytototoxicity. (3) The $IC_{50}$ lalue of combinatory prescription of Rhei Rhizoma, Forsythie Fructus and Caesalpiniae Lignum was about $40{\mu}g/ml$.

Trends and Future Direction of the Clinical Decision Support System in Traditional Korean Medicine

  • Objectives: The Clinical Decision Support System (CDSS), which analyzes and uses electronic health records (EHR) for medical care, pursues patient-centered medical care. It is necessary to establish the CDSS in Korean medical services for objectification and standardization. For this purpose, analyses were performed on the points to be followed for CDSS implementation with a focus on herbal medicine prescription. Methods: To establish the CDSS in the prescription of Traditional Korean Medicine, the current prescription practices of Traditional Korean Medicine doctors were analyzed. We also analyzed whether the prescription support function of the electronic chart was implemented. A questionnaire survey was conducted querying Traditional Korean Medicine doctors working at Traditional Korean Medicine clinics and hospitals, to investigate their desired CDSS functions, and their perceived effects on herbal medicine prescription. The implementation of the CDSS among the audit software developers used by the Korean medical doctors was examined. Results: On average, 41.2% of Traditional Korean Medicine doctors working in Traditional Korean Medicine clinics manipulated 1 to 4 herbs, and 31.2% adjusted 4 to 7 herbs. On average, 52.5% of Traditional Korean Medicine doctors working in Traditional Korean Medicine hospitals adjusted 1 to 4 herbs, and 35.5% adjusted 4 to 7 herbs. Questioning the desired prescription support function in the electronic medical record system, the Traditional Korean Medicine doctors working at Korean medicine clinics desired information on 'medicine name, meridian entry, flavor of medicinals, nature of medicinals, efficacy,' 'herb combination information' and 'search engine by efficacy of prescription.' The doctors also desired compounding contraindications (eighteen antagonisms, nineteen incompatibilities) and other contraindicatory prescriptions, 'medicine information' and 'prescription analysis information through basic constitution analyses.' The implementation of prescription support function varied by clinics and hospitals. Conclusion: In order to implement and utilize the CDSS in a medical service, clinical information must be generated and managed in a standardized form. For this purpose, standardization of terminology, coding of prescriptions using a combination of herbal medicines, and unification such as the preparation method and the weights and measures should be integrated.

Study on the Food Therapy for the Aged as Discussed in 『천금방』 (『천금방』의 노인식이요법연구)

  • On the basis of the widely collected fork prescriptions, long clinical practice of himself, together with integration of all the good points of earlier medical specialists, 孫思邈 wrote 『千金方』 of thirty volumes. Among them Volumes 『食治』 (Food Therapy) and 『養性』(Cultivating the Character) are especially meaningful for in gerontology. Thirty years after its completion, as a supplement, he wrote another thirty volumes of 『千金翼方』. Mr. Sun integrated his discoveries in the treatments of the aged patients, and his opinions for longevity into the Volumes 『養性』 (Cultivating the Character), 『?穀』(Fasting), 『退居』(Retirement), 『補益』(Supplementation). Thus he laid down the basic foundation for the gerontology of Chinese medicine. 『千金要方』 and 『千金翼方』 by Mr. Sun discuss mainly the prevention of illness for aged patients. As for the function of food therapy, he explains: 'Food can dispel evil elements of the body and settle down its organs. It can further bring pleasure to man's spirit and help the circulation of blood and breath.' He then propose the major method of food therapy: 'For a medical doctor, he should understand the causes of disease, then treat it with food. Only after the food treatment fails, medicine is employed.' He thinks that medicine does work in treatment, but it also creates imbalance in the body, and makes the latter vulnerable to outside harms. Therefore, a good doctor is the one who cures the disease and bring pleasure to the patient with food. He stresses that food therapy is always a priority. Mr. Sun prefers animal's organs in the food therapy for aged. For example, he uses powder of sheep's kidney to treat the lumbago. He suggests the viewpoint of 'Curing man's organs with animal's organs' and 'Strengthening man's organs with animal's organs,' and sets the theoretical foundation of 'Organic Treatment.' Mr. Sun's gerontology and food therapy received further development latter. The prescriptions contained therein had been widely used in 王懷隱's 『太平聖惠方』 and 陳直's 『養老奉親書』, both in the Song Dynasty. Eventually they had become a science of food therapy for aged patients.
