기본 심폐소생술 교육이 대학생의 심폐소생술에 대한 지식과 태도에 미치는 효과 (The Effects of Basic Life Support Education on CPR Knowledge and Attitude of Undergraduates)

  • Purpose: This study is a non-equivalent control group pre-post design, which attempted to observe the effects of Basic Life Support Education on undergraduate students' CPR Knowledge and Attitude. Methods: The subjects were 96 undergraduates in Jeonbuk. They were divided into two groups: an experimental group of 46 students and a control group of 50 students. The BLS education was given by three BLS instructors certified by the American Heart Association using a DVD. Data were collected from October 20 to November 15, 2010 and analyzed, using the ${\chi}^2$ test, t-test, and ANCOVA. Results: There were significant differences between the experimental group and the control group in the levels of CPR Knowledge and Attitude. The 1st hypothesis, that the CPR knowledge scores for the experimental group would be higher than those of the control group, was supported (F=157.58, p=.000). The 2nd hypothesis, that the CPR attitude scores for the experimental group would be higher than those of the control group, was supported (F=23.78, p=.000). Conclusion: This study showed that BLS education had a positive influence on undergraduate students' knowledge and attitudes toward CPR. We expect that BLS education will contribute to spreading knowledge of CPR and reducing mortality among heart attack victims, and we can see from this study how CPR training in combination with theory and clinical practice develops knowledge and practical ability.

호흡응급환자의 적절한 헬스케어를 위한 마스크 유형별 환기효과 비교 (Comparison of Ventilation Effects by Mask Type for Proper Health Care of Respiratory Emergency Patients)

  • 본 연구의 목적은 기도관리의 중요한 장비 중 하나인 bag-mask ventilation (BMV)의 환기 적절성이 상업화된 마스크의 종류에 따라 그리고 처치자의 손의 특성에 따른 영향요인을 분석해 가장 적확한 bag-mask의 종류를 제언해보기 위한 연구이다. 이를 위해 전남지역 D 대학의 기본인명 소생술 교육센터(대한심폐소생협회 등록기관)에 응급의료종사자를 위한 기본소생술 과정 및 한국형 전문심장구조술 과정을 수료한 학생을 39명을 모집하여, 손의 신체적 특성을 측정함은 물론, bag-mask를 이용한 분당 평균환기량을 측정 및 분석하였다. 결과적으로 손의 특성에 가장 영향을 받지 않고 적절한 환기량이 제공되는 마스크 유형은 Soft type(tube, silicon)의 마스크였다. 반면 Hard type(tube, silicon)의 마스크는 처치자의 손의 특성에 유의미한 영향을 받아 보편적으로 사용하기에 부적절함을 알 수 있었다. 현재 COVID-19로 인해 구급대원과 환자들에게 감염의 위험도는 증가하고 있다. 이러한 상황들을 고려했을 때 감염방지는 물론, 적절한 환기량에도 불리한 반영구 Hard type 마스크의 보편적 사용은 지양되어야 할 것으로 생각되며, 이에 환자 및 처치자의 감염 방지(1회 용) 용이성은 물론 적절한 환기량에서도 안정적인 환기량을 제공 가능한 Soft type 마스크의 '장점 인식'과 공급을 통해 현장에서 적극적으로 활용돼야 할 것이다.

심폐소생술 수행자의 엉덩관절 각도가 심폐소생술 결과에 미치는 영향 (Effect of Chest Compression Position Depending on the Rescuer's Hip Joint Angle During Basic CPR)

  • 이 연구는 기본심폐소생술 교과목을 이수한 응급구조과 학생을 대상으로 수행자 엉덩관절 각도에 따라 기본심폐소생술의 질이 향상되는지 알아보고자 하였다. 기본심폐소생술 중 엉덩관절 각도에 따른 기본심폐소생술 질(가슴압박 깊이, 완전한 이완, 압박속도 등)을 측정하기 위해 SimPad SkillReporter와 Resusci Anne® QCPR®을 이용하여 비교 분석하였고, 2015 AHA Guideline 따랐으며, 가슴압박과 인공호흡의 비율은 30:2로 5주기 시행하였다. 실험군과 대조군의 일반적인 자세에서는 엉덩관절 각도, 인공호흡정도, 가슴압박 깊이, 가슴압박 속도, 가슴압박 이완, 가슴압박 정확도, 가슴압박 중단시간(Hands off time), 가슴압박 적용시간 비율은 큰 차이를 보이지 않았다(p > 0.05). 엉덩관절 각도를 90도로 유지하는 기본심폐소생술은 유지하지 않았을 때와 큰 차이는 없었으나, 적정깊이와 100% 이완에서는 좋은 결과를 보여 주어 보다 더 나은 심폐소생술 질 향상을 기대할 수 있을 것이다.

심정지 환자 이송 시 구급대원의 자동제세동기 사용현황 (Use of Defibrillator(AED) on Prehospital Cardiac Arrests)

  • Purpose: There has been an increase in the number of prehospital cardiac arrests due to the increasing number of cardiovascular diseases and the increase in the average age of the population. We performed this study to identify the proper resuscitation technique and AED to be used to increase the survival rate in prehospital cardiac arrests. Methods: This studied 159 victims with prehospital cardiac arrests(VF or VT rhythm) by EMT's Reports from January to August, 2005. Results: 108 of 159 victims(67.9%) were shocked by AED. Eighty of 159 victims(50.9%) were recorded with AED shock in prehospital cardiac arrests. A number of shocks is averaged 2.19; 46.2% of one-shock and 86.1% of 1-3 shock. EMS first-tier response interval from time of dispatch to scene arrival was 5.88 minutes, from scene arrival to scene start was 7.36 minutes, from scene start to hospital admission was 9.91 minutes and from scene arrival to AED shock was 6.84 minutes. EMT provided advanced care to prehospital cardiac arrests: 97.5% in CPR, 10.1% in advanced airway management, 67.9% in AED shock. Conclusion: With the increase in cardiovascular disease and old age, the number of prehospital cardiac arrests has risen gradually. However, there were lack of CPR by bystander, defibrillation and advanced cardiac life support(ACLS) in prehospital stage. To improve the adequacy of basic life support and to increase the performance of ACLS, especially AED, we must create challenges to develop new protocols in prehospital care.

간호학생의 기본 심폐소생술에 대한 태도, 지식, 능숙도에 미치는 교육의 지속효과 (Retention Effect of Basic Life Support Education Program on Attitude, Knowledge, Skillfulness for Nursing Students)

  • Purpose: This study was done to identify the retention effects of a basic life support training program for nursing students on their attitude, knowledge, and skillfulness. Methods: A one-group repeated posttest design was employed for this study. The subjects included 44 junior nursing students from Gachon University in Incheon. Data were collected by a structured questionnaire test immediately after, 3 months after, 6 months after, and 9 months after the training respectively from May 2014 through March 2015. Results: The respective attitude, knowledge and skillfulness score on BLS of the nursing students had reduced significantly at 3 months after (p<.001), 6 months after (p<.001), and 9 months after (p<.001, p=.011 for Attitudes) the training compared to immediately after the training. Conclusion: Re-training on basic life support for nursing students should be implemented within 3 months for retention of educational effects. Thus, it is recommended that basic life support training programs, including a curriculum for nursing students, is developed to support re-training. And it is expected that the improved skills of nursing students on basic life support would contribute to the successive first aid nursing for patients at risk of cardiac arrest.

간호대학생의 심폐소생술 수행의도에 영향을 미치는 융합적 요인 (Willingness to Perform Cardiopulmonary Resuscitation (CPR) in Emergency Situation: Convergence factor in Undergraduate Nursing Students)

  • 본 연구는 기본간호술 수행자신감과 심폐소생술 관련 융합적 요인들의 상관관계를 알아보고 간호대학생을 대상으로 응급상황에서의 심폐소생술 수행 의도에 영향을 미치는 요인을 파악하고자 수행된 연구로서 BLS와 핵심기본간호술을 교육받은 간호학과 재학생을 대상으로 하였다. 자료는 2018년 1월부터 2018년 3월까지 수집되었다. 연구분석은 SPSS(ver. 24.0)을 이용하여 Independent t-test, Pearson's correlation과 Logistic regression을 사용하였다. 연구결과 심폐소생술관련 변수들 사이에서는 중간 이상의 높은 상관관계가(r=.575~.738) 확인되었으며, 자동제세동기 활용의 경우 기본간호술 자신감, 심폐소생술 자신감, 숙련도, 자기효능감과 낮은 상관관계가 확인되었다(r=.163~.301). 핵심기본간호술과 심폐소생술 관련 변수들 간에는 중간정도의 상관관계(r=.415~453)가 확인되었다. 로지스틱 회귀분석 결과 심폐소생술 자기효능감과 (OR=1.22, 95% CI: 1.07~1.38) 자동제세동기 활용이 (OR=1.72, 95% CI: 1.09~2.75) 영향요인으로 확인되었다. 이러한 연구 결과는 전반적인 간호술기 능력 향상이 심폐소생술 술기 능력향상과 관련이 있음을 알 수 있었고, 심폐소생술에서 주요 사용 도구인 자동제세동기와 상관관계가 있으므로 이에 대한 자기효능감 증진과 실제상황에서 심폐소생술과 밀접한 관련이 있는 자동제세동기 사용을 교육의 필수요소로 포함시키는 노력이 필요하다.

심페소생술 시행 시에 휴대용 압박 센서 활용이 흉부압박의 질에 미치는 영향: 마네킹 기반 시뮬레이션 연구 (The effect of portal compression sensor on the quality of chest compressions during cardiopulmonary resuscitation (CPR): A mannequin based simulation study)

  • 본 연구는 평균 이송시간에 따른 흉부압박을 심정지 환자에게 수기로만 이루어진 압박과 깊이 보조 장치를 사용한 압박을 적용하여 그 결과를 분석하고 그에 따른 영향을 규명하여 심정지 환자의 심폐소생술을 위한 기초자료를 파악하고자 시도되었으며 2011년 9월부터 2012년 5월까지 청주시 소재 소방서에 근무하고 있는 1급 및 2급 응급구조사 20명과 BLS provider를 이수한 응급구조과 학생 20명을 대상으로 수기로만 이루어진 흉부압박과 깊이 보조장치를 사용한 흉부압박의 결과 기록지를 비교 분석하였다. 수기로만 이루어진 흉부압박은 시간이 지남에 따라 압박깊이는 떨어지고 횟수는 증가했으며 깊이 보조 장치를 사용한 흉부압박은 압박깊이를 유지하고 횟수는 일정하게 유지하는 기능을 나타내었고 두 그룹 모두 깊이 보조 장치를 사용하였을 때 압박 정확도는 의미 있는 차이를 보여 심폐소생술 시행 시에 깊이 보조 장치를 사용하는 것이 심정지 환자에게 유용한 영역이라 사료된다.

간호대학생의 기본심폐소생술 자격과정 교육 경험 (The Meaning of BLS-Provider Certification Course Experienced by Nursing Students)

  • Purpose. The purpose of this study was to explore the meaning of BLS-Provider Certification Course experienced by nursing students. Methods. The phenomenological method developed by Giorgi was used for this study. The participants were 7 students who had experienced BLS-Provider Certification Course. The data were collected between February, 2013 and May, 2013 by face to face interview. The interview was recorded and then transcribed. Results. The constituents associated with the meaning of the Nursing students's experiences on BLS-Provider Certification Course were as followings 'To obtain the information', 'Complicated preparation process', 'Unfamiliar education', 'Fear on the BLS ', 'Satisfaction after getting of BLS-Provider Certification ' Conclusions. The result of this study may contribute to a deeper understanding of the meaning of the BLS-Provider Certification Course experienced by nursing students. The highlights of this study are that, although the students had much inconvenience on the preparation process of BLS-P and fear on the BLS-Provider Certification Course, they were satisfied much after getting the BLS-Provider Certificate. It can give the better quality of BLS-Provider Certification Course if these results are provided in the program.

응급처치에 대한 초.중.고등학교 교과서 내용 분석 (Contents Analysis of First Aid in Elementary, Middle and High School Textbooks)

  • Purpose: The purpose of this study was to provide basic data to develop first aid education programs for elementary, middle and high school students. Method: Forty nine textbooks adopted by first to six grade elementary schools and first to third grade of both middle and high schools were analyzed for content on first aid. Results: 1) First aid content was covered only in courses on and 2) Content on first aid was included in the categories, "General of first aid", "Rescue & moving", "Fire & Burns", "Cardiopulmonary resuscitation", "Musculoskeletal injuries", "Thermal injuries", "Wounds", "Removing foreign bodies", "Disaster & Accidents" and "Others". Categories that were not included were "Poisoning" and "Bites". The content did not follow a progression related to the level of intellectual and physical development of the students. Conclusion: These results suggest that the content on first aid should be revised to be practical and rational for the benefits of student safety.

노인요양시설에서 노인수발 담당자의 응급처치에 대한 교육수요도 조사 - 일개 도시를 중심으로 - (A Study on the Need for Emergency Care Education in Nursing Homes)

  • Purpose: This study was conducted to evaluate the need for education for emergency care in nursing homes. Method: 312 people who were engaged in caring for the elderly at nursing homes in D-metropolitan c! ity answered the questionnaires about the experiences, the need, and the ideal methods of education for basic emergency care, from Jul. 1. 2007 to Jul. 30. 2007. Result: 69.9% were female. Only 7.1% were nurses and the rest did not have majors in the medical field. 83.0% had experienced education for emergency care, and 89.2%, 78.4% and 44.8% of them had been educated about cardiopulmonary resuscitation, airway obstruction care, and fracture/sprain/dislocation care respectively. Fifty-three who had not experienced education answered 'no opportunity' as a cause of non-education. The need for emergency care education was 4.53/5.00, and the desire to receive education was 2.81/3.00. Mainly workers wanted to be educated abo! ut airway obstruction (88.1%), and breathing assistance & cardiac massage (72.4%). Workers preferred to be educated 1-2 times per year for 2-3 subjects, through programs including both lecture and practice. Conclusion: It is essential to create an adequate program and apply it to the people engaged in caring for the elderly.
