Correlation of Temporary Lateral Recumbency and Lung Opacity on Ventrodorsal Radiograph (일시적인 횡와 자세와 흉와위 영상에서 폐 불투과도의 상관관계)

  • The lung opacity on radiography is influenced by various factors. The physical density of the lung and the attenuation ensured on computed tomography (CT) scans is determined by three components : lung tissue, blood, and air. Temporary right lateral recumbency may responsible for the increase of opacity on ventrodorsal projection view. Thus, our aim is to demonstrate that the effect of right lateral recumbency posture on right lung opacity using radiograph and CT scan. In this study, 62 dogs without clinical or radiologic signs of cardiopulmonary disease are selected. Thorax radiographs per 30 seconds for 2 minutes (30s, 60s, 90s, 120s) were performed for 62 dogs. After discussion of the radiographic findings of lung field by two radiologists and a student at Chungbuk national university veterinary medical center a consensus opinion was recorded. Computed tomography per a minute (1 min, 2 min) for 2 minutes were performed for 2 dogs. Mean x-ray attenuation of lung was measured quantitatively using software at two levels (aortic arch and basal level). Among 62 dogs with radiograph comparison, 9.3% of dogs showed influence by postural effect. However, all 2 dogs with computed tomography comparison, showed influence by postural effect. In conclusion, position dependent changes of lung density in CT exam are not consistent with thoracic radiograph.

  • It is well recognized that a cross-bite tends to occur more frequently among Japanese, Chinese and Koreans more than among Europeans. A variety of functional appliances have already invented for the treatment of cross-bites. One of them is the FR III(by Rolf $Fr\ddot{a}nkel$) which applies a quite different philosophy based on the using vestibular region compared with other methods which utilize muscular forces applied directly on the teeth. The FR III also increases both the lack of muscular tension against the dental arch and the dentition and muscular forces which can then influence the teeth indirectly. This mechanism can achieve favorable developments with the basal bone, teeth, and alveolar bone. After using FR III to the anterior cross-bite patient with the eruption of a maxillary incisor the results were as follows: 1. Forward growth of maxilla and proclination of the upper incisor 2. Downward and backward rotation of the mandible 3. Increase the facial height 4. The case which was normal mandible, underdevelopmental maxilla, deeper overbite and more nagative overjet exhibited good prognosis

  • The elastic open activator is one of the modified myodynamic activator. The reduced size of the appliance mass motivates the patients' comfort and longer time of wearing. Its peculiarities in loose fitting and the lack of appliance stabilization in the mouth draws the tongue and the surrounding functional matrices on close interaction with the appliance, consigns the physiologic exertion to target structures, and eventually makes it feasible to the inland of non-extraction treatment In the context of the sagittal malocclusion, the orthodontic trench is dependent upon the growth of basal structure aimed, therefore, it is contemplated to grabble the effects of Elastic Open Activator upon the class II malocclusion of growing child retrospectively. The cephalometric headfilms and study models of nine Class II malocclusion of growing child retrospectively. The cephalometric headfilms and study models of nine class II division 1 and five division 2 patients were evaluated and analyzed, and the following observations were drawn, 1. The maxilla maintained a normal growth pattern in both groups. 2. The mandible grew anteroinferiorly in both groups. 3. The upper incisors tipped ligually in Class II division 1 and tipped labially in Class II division 2 and anterior vertical alveolar growth was interrupted in both groups. 4. The lower incisors tipped labially. 5. There was an arch expansion in both groups and increase of available space in Class II division 2

Assessment of Computed Tomographic Lung Density in Beagle and Shihtzu Dogs : Influence of Position and Positive End Expiratory Pressure (비글과 시츄견에서 호기말 양압에 따른 전산화 단층촬영상의 폐밀도의 평가)

  • The objective of this study was to measure densities in various areas of the normal canine lung with computed tomography (CT) depending on influences of gravity and the degree of lung inflation and to determine optimal positions and positive end expiratory pressure of canine lung for CT scanning. In each eight normal Beagle and Shihtzu dogs, a respiratory breathhold maneuver without spontaenous breathing at different positive end expiratory pressure (PEEP) of 0 mmHg, 10 mmHg and 20 mmHg was applied with the position of right and left lateral recumbency, sternal recumbency, and dorsal recumbency and spiral-CT scans of the total lung were acquired. Slices were selected at three levels through the apex, middle and basal lung at the aortic arch, carina and just above the diaphragm and lung density was measured in the dorsal, ventral, and lateral portions of the peripheral lung field. Lung density in dependent areas was higher than in nondependent areas (p < 0.05) regardless of species, positions, anatomic locations at the PEEP of 0 mmHg and 10 mmHg. However, no significant difference of lung density was found at PEEP of 20 mmHg in both species except the dorsal recumbency in Shihtzu dogs. This density gradient in the dependent areas is strongly influenced by PEEP (p < 0.05). In the four positions on the CT gantry, the lung density at the dependent and nondependent location of the lung was greater at the aortic arch than at the base (p < 0.05). Lung density decreased on identical location according to increase of PEEP (p < 0.05). There was no significant difference between right and left lung density at sternal and dorsal recumbency and no significant difference of the dorsal, ventral, and lateral portions of lung density at the right and left recumbency under identical pressure. It is implied that during chest CT scan with 20 mmHg of positive end expiratory pressure with right or left lateral recumbency, canine lung density do not influenced by gravity or anatomic location.

The Changes of Cerebral Metabolic Parameters, Serum Levels of Neuron-Specific Enolase and S-100$\beta$ Protein During Retrograde Cerebral Perfusion Under Profound Hypothermic Total Circulatory Arrest (초저체온하 완전순환정지 시에 이용되는 역행성 뇌관류의 시간에 따른 뇌대사 지표, 혈청 내 neuron-specific enolase, 및 S-100 베타단백의 변화)

  • Background: Retrograde cerebral perfusion(RCP) is one of the methods used for brain protection during aortic arch surgery. The author previously published the data, however, for the safety of it, there still remains many controversies. The author performed RCP and checked various parameters to clarify the possibility of early detection of cerebral injury. Material and Method: The author used pigs(Landrace species) weighing 25 to 30kg and performed RCP for 120 minutes. After weaning of cardiopulmonary bypass, we observed pigs for another 120 minutes. Rectal temperature, jugular venous oxygen saturation, central venous pressure were continuously monitored, and the hemodynamic values, histological changes, and serum levels of neuron-specific enolose(NSE) and S100$\beta$ protein were checked. Central venous pressure during RCP was maintained in the range of 20 to 25 mmHg. Result: Flow rates(ml/min) during RCP were 224.3$\pm$87.5(20min), 227.1$\pm$111.0(40min), 221.4$\pm$119.5(60min), 230.0$\pm$136.5(80min), 234.3$\pm$146.1(100min), and 184.3$\pm$50.5(120min). Serum levels of NSE did not increase after retrograde cerebral perfusion. Serum levels of S100$\beta$ protein(ng/ml) were 0.12$\pm$0.07(induction of anesthesia), 0.12$\pm$0.07(soon after CPB), 0.19$\pm$0.12(20min after CPB), 0.25$\pm$0.06(RCP 20min), 0.29$\pm$0.08(RCP 40min), 0.41$\pm$0.05(60min), 0.49$\pm$0.03(RCP 80min), 0.51$\pm$0.10(RCP 100min), 0.46$\pm$0.11(RCP 120min), 0.52$\pm$0.15(CPBoff 60min), 0.62$\pm$0.15(60min after rewarming), 0.76$\pm$0.17(CPBoff 30min), 0.81$\pm$0.20(CPBoff 60min), 0.84$\pm$0.23(CPBoff 90min) and 0.94$\pm$0.33(CPBoff 120min). The levels of S100$\beta$ after RCP were significantly higher than thosebefore RCP(p<0.05). The author could observe the mitochondrial swellings using transmission electron microscopy in neocortex, basal ganglia and hippocampus(CA1 region). Conclusion: The author observed the increase of serum S100$\beta$ after 120 minutes of RCP. The correlation between its level and brain injury is still unclear. The results should be reevaluated with longterm survival model also considering the confounding factors like cardiopulmonary bypass.
