Effects of Sagittal Spinopelvic Alignment on Motor Symptom and Respiratory Function in Mild to Moderate Parkinson's disease

  • Purpose: This study examined the effects of sagittal spinopelvic alignment on the clinical parameters, motor symptoms, and respiratory function in patients with mild to moderate Parkinson's disease (PD). Methods: This study was a prospective assessment of treated patients (n=28, Hoehn and Yahr (H&Y) stage 2-3) in a PD center. Twenty-eight subjects ($68.5{\pm}5.7yrs$) participated in this study. The clinical and demographic parameters, including age, sex, symptoms duration, treatment duration, and H&Y stage, were collected. Kinematic analysis was conducted in the upright standing posture with a motion capture system. A pulmonary function test (PFT) was performed in the sitting position using a spirometer. The motor symptoms were assessed on part III of the movement disorder society sponsored version of the unified Parkinson's disease rating scale (MDS-UPDRS). SPSS 18.0 was used to analyze the collected data. Results: The exceeding 12 degrees group of the lower trunk showed significantly higher on the clinical parameters than the below 12 degrees group. In addition, the exceeding 12 degrees group of the lower trunk showed a significantly lower forced expiratory volume at one second (FEV1) / forced vital capacity (FVC) (%) and 25-75% forced mid-expiratory flow (FEF) (L/s) than in the below group. On the other hand, there was no difference in the upper trunk and the cervical pelvis between the groups. Conclusion: These findings suggest that the sagittal balance in the lower trunk is related to the clinical parameters and respiratory function, but not the motor symptoms in patients with mild to moderate PD.

대학생의 배가로근과 뭇갈래근 두께와 척추정렬간의 상관관계 (Correlations between the Muscle Thickness of the Transverse Abdominis and the Multifidus Muscle with Spinal Alignment in College Students)

  • Purpose: The transverse abdominis and themultifidus muscle are located in the core. They surround one's trunk and help in body stabilization. Specifically, they control spine articulation to maintain posture and balance. Therefore, weakened deep muscle in the trunk may cause spinal malalignment. This study aims to compare the correlation between the thickness of the transverse abdominis and the multifidus muscle and the spine alignment among college students in their 20s. Methods: This study measured the thickness of the transverse abdominis and the multifidus muscle of 42 healthy college students in their 20s using ultrasonic waves. The thickness of the muscle was measured for the length of the cross-section except for fascia. The thickness of the left and right muscles was measured, and the mean value was calculated. As the thickness of the transverse abdominis can increase because of pressure during exhalation, it was measured at the last moment of exhalation. Spinal alignment was measured by the kyphosis angle, lordosis angle, pelvic tilt, trunk inclination, lateral deviation, trunk imbalance, and surface rotation using Formetric III, which is a three-dimensional imaging equipment. They were measured for three times, and the mean values were calculated. The general characteristics of the subjects were analyzed using descriptive statistics. The correlations between each factor were analyzed using Pearson's correlation analysis. Results: The transverse abdominis showed asignificant correlation with trunk inclination (p<.05). The multifidus muscle showed a significant positive correlation with pelvic tilt and a negative correlation with surface rotation (p<.05). Conclusion: The thickness of transverse abdominis and the multifidus muscle appears to influence spinal alignment. Specifically, the multifidus muscle, which plays an important role on the sagittal plane, influences surface rotation, thus making it an important muscle for scoliosis patients. Therefore, a strengthening training program for the transverse abdominis and the multifidus muscle is necessary according to specific purposes among adults with spinal malalignment.

Intervention Strategy Applied ICF Checklists for Sitting Cross-Legged in Patient with Multiple Ligament Knee Injury: Single Subject Study

  • Purpose: This study applies the ICF to identify the patient's body function, structure, and participation, evaluates the patient's environmental factors and individual factors, and is a high level of movement to return to the society of patients with multiple ligament injury of the knee joint. Methods: Progressive strength training and ROM exercise were performed 30 minutes a day, 5 times a week for 6 weeks. The evaluation was performed by examining the ROM, length, MMT, instability, dynamic balance, pain and depression. Results: The ROM of the knee joint was improved from 110° to 135° after intervention, and the knee flexion length decreased from 69 cm to 45 cm. Knee flexor is Good after intervention from Poor-, and knee extensor is Good+ after intervention from Poor, and the plantar flexor of the ankle joint improved from Poor- before intervention to Good after intervention and dorsi-flexor of the ankle joint improve to Good from Poor. Pain index was moderate before and after the intervention, with a score of 3, 2 after the intervention, and when maintaining the sitting cross-legged, the before intervention score was 7 to 4 after the intervention. Conclusion: The patient's posture of sitting cross-legged was maintained from 30 seconds before intervention to 14 minutes after intervention. These results were able to set the hypothesis design, intervention method and goal that the multifaceted approach of environment and individual factors as well as body function and structure area, activity and participation area using ICF checklists, it is helped the patient to return to daily life.

체성 실행장애 아동을 위한 감각통합 중재가 체성 실행과 작업수행 능력에 미치는 영향: 단일대상연구 (The Effects of the Sensory Integration Intervention for Somatosensory and Occupational Performance Abilities in Children with Somatodyspraxia: Case Study)

  • 목적 : 본 연구는 체성 실행장애가 있는 학령기 아동에게 개별 맞춤 감각통합 중재가 체성 실행과 작업수행 능력에 미치는 효과에 대해 알아보고자 한다. 연구방법 : 단일대상연구의 A-B설계를 적용하여 총회기는 11회기로 기초선 3회기와 중재는 8회기로 구성하여 실시하였다. 중재 전후 효과 비교를 위해 수행영역과 수행 요소로 나누어 측정하였으며, 매회기마다 중재 후 목표 활동(배와 위 굴곡 자세, 손수레 걷기, 밸런스 볼에서 우세 발로 서있기)의 목표 달성 척도를 시행하였다. 결과 : 연구에 참여한 아동은 중재 전·후 수행영역과 수행 요소에서 전반적인 향상을 보였으며, 회기별 중재 후 목표 활동 측정 시 목표 달성 척도가 증가한 것을 확인할 수 있었다. 결론 : 고유감각, 전정감각, 촉각을 중심으로 구성된 감각통합 중재가 체성 실행장애 아동의 목표 활동 수행 및 또래와의 관계 개선에 긍정적인 효과가 있는 것으로 나타났다.

유형별 운동프로그램이 선 자세에서의 균형능력과 근활성도에 미치는 영향 (Effects of Exercise Program by Type on Balance Ability and Muscle Activity In A Standing Posture)

  • 본 연구는 20대 성인남성을 대상으로 세 운동군이 정적 및 동적 균형능력과 근활성도에 미치는 효과를 규명하기 위하여 수중 운동군, 체간 안정화 운동군, 균형 운동군으로 각각 15명씩 배치하였다. 2013년 6월부터 2013년 9월까지 연구가 진행되었으며, 6주 간 1주일에 3일, 1회 30분 동안 중재 후, 동일 대상자들의 균형능력 및 근활성도(앞정강근, 장딴지근)를 측정하여 비교하였다. 그 결과 집단 내 중재 전과 중재 후의 비교에서 세 운동군 모두 신체중심 이동면적과 총궤적길이의 변화는 유의한 차이가 있었고(p<.05)(p<.01), 동적 균형에서도 안정성 한계의 변화는 유의한 차이가 있었다(p<.05)(p<.01). 앞정간근의 근활성도의 변화에서 세 운동군의 좌 우측 모두 통계학적으로 유의한 차이가 있었고(p<.05)(p<.01), 장딴지근은 체간 안정화 운동군의 좌측을 제외한 모두 통계학적으로 유의한 차이가 있었다(p<.05)(p<.01). 수중 운동이 균형능력의 향상과 근활성도 증가에 효과적임을 규명하였고, 본 연구결과를 토대로 균형능력이 좋지 않은 노인이나 환자들에게 객관적인 수중 운동 효과를 밝힐 수 있는 연구가 진행되어 구체적인 수중 운동 프로그램 개발을 제안하고자 한다.

스쿼트 동작 시 발뒤꿈치 보조물 경사각에 따른 하지근과 척추기립근의 근육활동 비교 (Effect of Different Heel Plates on Muscle Activities During the Squat)

  • The purpose of this study was to determine the effect of three different plates($0^{\circ}$, $10^{\circ$}, $20^{\circ}$)under heels on the lower limb muscles and erector spinae during squat exercise. Ten high school korean traditional wrestling players(age: $18.5{\pm}0.7$, weight: $1972.2{\pm}128.5N$, height: $177.8{\pm}6.0cm$, weight of barbell: $1004.5{\pm}132.4N$) performed squat exercise using three different tilting plates under heels at a cadence of 40beats/sec with 80% one repetition maximum load. Surface electrodes were placed on the participants' left and right erector spinae, and rectus femoris, vastus medialis, vastus lateralis, tibialis anterior, biceps femoris, medial gastrocnemius, and lateral gastrocnemius in the right lower extremity. One S-VHS camcorder(Panasonic AG456, 60fields/s) was placed 10m to the side of the participant. To synchronize the video and EMG data, a synchronization unit was used for this study. Average and Peak IEMG values were determined for each participant. For each variable, a one-way analysis of variance was used to determine whether there were significant differences among three different tilting plates under heels. When a significant difference was found in plates type, post hoc analyses were performed using the Tukey procedure. A confidence level of p<.05 was used to determine statistical significance. As a result of this study, maximum nEMG values of the tibialis anterior in $0^{\circ}$ plates was significantly higher than the corresponding values for the other plates during the knee extension. This increased activation in the tibialis anterior muscle indicates an increase in displacement of center of gravity of body. It is very likely that additional muscle activation are needed to stop the forward and backward movement. The results also showed that muscular activities of quadriceps femoris and erector spinae were decreased with increasing angle of plates. This suggests that increasing angle of plate may help to sustain the balance and posture of squat exercise. It is considered that very few significant differences were found among three different plates($0^{\circ}$, $10^{\circ}$, $20^{\circ}$) since elite players with much experience in squat exercise, were chosen as a participant of this study. In order to obtain meaningful results regarding the tilting angle of heel plates in squat exercise, kinetic and 3D kinematic analysis will be needed in the future study.

후족부 경사각을 이용한 신발에 관한 생체역학적 연구 (A Biomechanical Research for Incorates a Rounded Sole with a 20 Degree Heel Lift in Functional Shoes)

  • 본 연구는 9명의 피험자가 2주간 하루 6시간동안 후족부 경사각을 이용한 신발인 S사(社) 신발을 착용하여 보행하였으며 신발을 처음 신었을 때와 2주간 하루 6시간 이상 착용한 후의 데이터를 통해 기존의 운동화와 S사(社) 신발간의 비교를 수행하였다. 1. 기립자세에서의 S사(社) 신발의 착용 시 근전도의 활동인 증가하고 압력분포 중심 움직임은 약 60% 정도 증가하였다. 2. 피험자의 보행 시 S사(社) 신발을 착용하였을 때 신발의 힐 부부분의 $20^{\circ}$ 경사각으로 인해 heel strike 시 발목이 약 $14^{\circ}$ 정도 발등 쪽으로 굽혀짐을 확인하였으며 무릎에서는 이를 보완하기 위해 $2^{\circ}$ 정도 굴곡 됨을 보였다. 3. 이로 인해 최대 지면반력이 보다 빠르게 나타났으나 크기는 변하지 않았다. 4. S사(社) 신발의 착용하고 보행 시 지지기에서 발목과 무릎의 부하는 21-61%의 감소하는 결과를 나타내었다. 결과적으로 S사(社) 신발 착용시 목과 무릎각의 굴곡 각을 증가시켜 보행 시 몸이 곧게 서도록 하였다. 또한 보행 시 부하를 감소시킬 수 있음을 나타냈으나 2주간의 신발 착용으로 인한 신체의 변화는 나타나지 않음을 보였다.

척추재활운동을 위한 디지털 압력바이오피드백 장치의 사용성 평가 (Usability Testing of Digital Pressure Bio-feedback for Spinal Rehabilitation Exercise)

  • 임상현장에서 압력 바이오피드백 장치는 요통 환자들의 척추재활운동에 사용되고 있으나, 여러 가지 단점을 갖고 있다. 본 연구는 사용자에게 신체의 움직임과 균형을 센싱하고 모니터링하여 사용자에게 바이오피드백을 제공함으로써 척추재활운동시 실시간으로 정확한 운동방법과 자세를 제공하는 디지털화된 압력 바이오피드백 장치를 개발하고자 하였다. 그 후 사용성 평가를 진행하여 안전성, 수행성, 조작성, 만족성 등의 문제점을 파악하고 개선방향을 제시하고자 한다. 본 연구에 참여한 대상자는 총 33명으로 전문가 평가 집단 11명, 사용자 평가 집단 22명으로 실제 개발된 장비를 사용 후 5점 척도로 평가하였다. 사용자 집단에서는 안전성 3.59점, 조작성 4.38점, 만족성 4.49점으로 나타났으며, 전문가 집단에서는 안전성 2.86점, 조작성 3.91점, 수행성 4.28점으로 나타났다. 본 사용성 평가를 바탕으로 태블릿PC 거치대의 안정성 문제, 공기주입, 화면 표시 등의 문제점을 개선한다면 척추안정화 운동을 하면서 실시간 자신의 운동상태를 확인하여 정확한 운동과 기능 평가가 가능한 운동장치가 될 것이다.

게임 기반의 자세수직 훈련이 급성 뇌졸중 환자의 밀기행동, 자세조절, 그리고 일상생활동작에 미치는 영향: 사전연구 (Effects of Game-based Postural Vertical Training on Pusher Behavior, Postural Control, and Activity of Daily Living in Patients With Acute Stroke: A Pilot Study)

  • Background: Visual and somatosensory integration processing is needed to reduce pusher behavior (PB) and improve postural control in hemiplegic patients with acute stroke. Objects: This study aimed to investigate the effects of game-based postural vertical training (GPVT) on PB, postural control, and activity daily living (ADL) in acute stroke patients. Methods: Fourteen participants with acute stroke (<2 months post-stroke) who had PB according to the Burke lateropulsion scale (BLS) (score>2) were randomly divided into the GPVT group ($n_1=7$) and conventional postural vertical training (CPVT) group ($n_2=7$). The GPVT group performed game-based postural vertical training using a whole-body tilt apparatus. while the CPVT group performed conventional postural vertical training to reduce PB (30 minutes/session, 2 times/day, 5 days/week for 3 consecutive weeks). The BLS was evaluated to assess the severity of PB. And each subject's postural control ability and ADL level were assessed using the postural assessment scale for stroke (PASS), balance posture ratio (BPR), and Korean-modified Barthel index (K-MBI). Outcomes were measured pre- and post-intervention. Results: Comparison of the pre- and post-intervention assessment results showed that both interventions led to the following significant changes: decreased severity of PB scores and increased PASS, BPR, and K-MBI scores (p<.05). In particular, statistical analysis between the two groups, the BLS score was significantly decreased in the GPVT group (p<.05). And PASS, BPR, and K-MBI scores were significantly improved in the GPVT group than in the CPVT group (p<.01, respectively). Conclusion: This study demonstrated that GPVT lessened PB severity and improved postural control ability and ADL levels in acute stroke patients.

스쿼트 운동 시 발목 관절에 키네시오 테이프의 적용이 넙다리 네갈래근과 뒤넙다리근의 활성에 미치는 영향 (Effects of Kinesiology tape in ankle joint of quadriceps and hamstring muscles activation during squat exercise)

  • Background: During squat exercises, Ankle joint stability during squat movement transfers forces produced by unstable supports or various loads to the leg joints and trunk, reducing the risk of injury; therefore, a reference is needed for correct ankle joint posture during squat exercises. The purpose of this study was to examine the effect of ankle joint stability on quadriceps femoris and hamstring activation during squat exercises. Design: Quasi-experiment one group pretest-posttest design. Methods: The participants in this study were 20 volunteers who performed squat exercises with and without kinesiology tape. We measured quadriceps femoris and hamstring activation using surface electromyography. The ankle joint was stabilized with kinesiology tape using the ankle balance taping (ABT) method. A paired t-test was performed to compare differences between taping conditions. Results: Vastus medialis and vastus lateralis activation were greater following squat exercises with kinesiology tape than without; however, the difference was not significant. Medial and lateral hamstring activation was not significantly different between taping conditions. Conclusion: Although it is difficult to maintain stability using kinesiology tape alone, ankle joint stability is believed to affect quadriceps femoris muscle activity, which increases the effectiveness of the squat exercise.
