The effects of intensive gait training with body weight support treadmill training on gait and balance in stroke disability patients: a randomized controlled trial

  • Objective: The purpose of this study was to investigate the effects of intensive gait training with body weight support treadmill training on gait and balance in stroke disability patients. Design: Randomized controlled trial. Methods: Twenty-six stroke patients (20 men and 6 women) participated in this study. All subjects were hospitalized patients. They were randomly divided into two groups: the experimental group (body weight supported treadmill training group, n=14) and control group (treadmill group, n=12). The mean ages were 52.07 years (experimental group) and 53.83 years (control group). Subjects in both groups received conventional training 10 times/wk. Subjects in the experimental group practiced body weight supported treadmill training for 30 minutes a day, 3 day/wk. Subjects in the control group practiced treadmill training for 30 minutes. The Berg Balance Scale (BBS) and GAITRite were used to evaluate balance and gait parameters (step length, cadence and gait speed) before and after the intervention. Results: BBS scores in the experimental group showed significantly greater improvement ($4.33{\pm}1.54$), compared with the control group (p<0.05). Significantly greater improvement in the gait speed ($24.13{\pm}4.53$ cm/s), affected side step length ($10.40{\pm}3.42$ cm), sound side step length ($11.97{\pm}3.29$ cm), and cadence ($23.88{\pm}5.52$ step/min), compared with the control group (p<0.05). Conclusions: Intensive gait training with Body Weight Support Treadmill Training may improve gait and balance in subacute stroke.

Effects of Gait Training Using Functional Electrical Stimulation on Stroke Patients' Balance and Gait Velocity

  • The purpose of this study is to examine the effects of gait training using functional electrical stimulation on the improvement of hemiplegic patients' functions for balance and gait velocity. The subjects of the experiment were determined to be 10 each hemiplegic patients who had been diagnosed with stroke or brain damage six months or longer earlier assigned to an experimental group and a control group respectively. The subjects were evaluated before the experiment using Tetrax and 10M gait tests, received gait training five times a week for four weeks using functional electrical stimulation and were evaluated after the experiment in the same method as used in the evaluation before the experiment. In order to examine differences between the experimental group that received gait training using functional electrical stimulation and the control group that was treated by functional electrical stimulation and received gait training thereafter, differences between before and after the experiment were analyzed using paired sample t-tests and differences in changes after the experiment between the experimental group and the control group were analyzed using independent sample t-tests in order to compare the two groups with each other. Experimental results showed significant differences in weight bearing, balance and gait velocity between before and after the experiment in the experimental group(p<.05). In the control group, whereas weight bearing and gait velocity did not show any significant difference between before and after the experiment(p>.05), balance showed significant differences(p<.05). Weight bearing, balance and gait velocity change rates showed significant differences between the experimental group and the control group(p<.05). In conclusion, it was indicated that gait training using functional electrical stimulation is effective for enhancing stroke patients' weight bearing rates, balance abilities and gait velocity.

무릎 폄근 강화 운동 동안 압력 생체되먹임 장비 적용이 무릎관절 전치환술 환자들의 근력과 균형에 미치는 영향 (Effects of Pressure Biofeedback Unit Application on Muscle Strength and Balance in Total Knee Arthroplasty Patients during Exercise for Strengthening the Knee Extensor Muscle)

  • Purpose : The purpose of this study is to verify the effect of selective muscle strengthening of the knee joint extensor muscles using a pressure biofeedback unit to improve knee extensor strength and the balance ability of total knee replacement patients. Through this, we tried to provide clinical information. Methods : In this study, 12 patients with total knee replacement were recruited from a rehabilitation hospital. They were divided into two groups: a feedback group (n=6) and a control group (n=6). All patients received 30 minutes of continuous passive motion and leg-strengthening exercises for 15 minutes five times a week for two weeks. Subjects performed knee extension exercises with or without biofeedback units in the sitting position. The knee extensor strength and balance ability were measured before and after exercise. Knee extensor strength was measured by Biodex system 3 and balance ability was measured by Balancia software. Results : Both the experimental group and the control group showed a significant difference in the muscle strength of the knee joint extensor muscles after intervention (p<.05). In comparison, the experimental group showed a significant difference than the control group (p<.05). Both the experimental group and the control group showed a significant difference in the velocity average, path length, area 95 % center of pressure (COP), weight distribution, five times sit to stand test (FTSST) after intervention. In comparison, the experimental group showed a significant difference in velocity average, area 95 % COP, and FTSST than the control group (p<.05). Conclusion : In order to strengthen the knee extensor muscle and improve the balance ability in total knee replacement patients, it is necessary to consider providing pressure biofeedback unit during leg strengthening exercises.

  • Purpose : The purpose of this study was to identify whether gym-ball exercise in standing position was an effective intervention for improving muscle strength, balance, gait, and fall efficacy in stroke patients. Methods : Twenty-four stroke patients were randomized into three groups: experimental group 1 (n=8), experimental group 2 (n=8), and control group (n=8). Experimental groups 1, 2 and the control group performed the gym-ball exercise in standing position, same exercise without a gym-ball, and general physical therapy for 4 weeks, five times a week in 30-minute sessions. Muscle strength, balance, gait, and fall efficacy were assessed using a handheld dynamometer, the Berg Balance Scale (BBS), the wearable BTS G-WALK® sensor, and the Korean version of the Falls Efficacy Scale (K-FES), before and after training, respectively. Comparisons within and between groups were analyzed using the Wilcoxon signed rank test, Kruskal Wallis H test, and Mann-Whitney U test. Bonferroni correction was performed when significant differences between groups were identified (p<.017, .05/3). Results : Regarding muscle strength, BBS score, cadence and FES-K were significantly improved after intervention in all three groups. The weight bearing rate, gait speed and step length in experimental group 1 and 2 were significantly improved after the intervention. The stride length in experimental group 1 were significantly improved after the intervention. Experimental group 1 had significantly improved BBS score and stride length after intervention than experimental group 2 and control group. Experimental group 1 and 2 improved muscle strength, weight bearing rate, and FES-K score more than the control group. Experimental group 1 showed significant improvement in cadence, gait speed, and step length after the intervention than control group. Conclusion : This study showed that exercise with gym-ball in standing position can be an effective intervention to improve balance and gait in stroke patients than the same exercise without gym-ball.

정상인과 뇌졸중 환자의 체간 위치감각 비교 및 보행과 균형에 미치는 영향 (Comparison Between Stroke Patients and Normal Persons for Trunk Position Sense and It's Relation to Balance and Gait)

  • Background: Stroke patients have reduced trunk control compared to normal people. The ability to control the trunk of a stroke patient is important for gait and balance. However, there is still a lack of research methods for the characteristics of stroke control in stroke patients. Objects: The aim of this research was to determine whether trunk position sense has any relation with balance and gait. Methods: This study assessed trunk performance by measuring position sense. Trunk position sense was assessed using the David back concept to determine trunk repositioning error in 20 stroke patients and 20 healthy subjects. Four trunk movements (flexion, extension, lateral flexion, rotation) were tested for repositioning error and the measurement was carried out 6 times per move; these parameters were used to compare the mean values obtained. Subjects with stroke were also evaluated with clinical measures of balance and gait. Results: There were significant differences in trunk repositioning error between the stroke group and the control group in flexion, lateral flexion to the affected side, lateral flexion to the unaffected side, rotation to the affected side, and rotation to the unaffected side. Mean flexion error: post-stroke: 7.95 ± 6.76 degrees, control: 3.32 ± 2.27; mean lateral flexion error to the affected side: 6.13 ± 3.79, to the unaffected side: 5.32 ± 3.15, control: 3.57 ± 1.92; mean rotation error to the affected side: 8.25 ± 3.09, to the unaffected side: 9.24 ± 3.94, control: 5.41 ± 1.82. There was an only significant negative correlation between the repositioning error of lateral flexion and the Berg balance scale score to the affected side (-0.483) and to the unaffected side (-0.497). A strong correlation between balance and gait was found. Conclusion: The results of this study indicate that stroke patients exhibit greater trunk repositioning error than age-matched controls on all planes of movement except for extension. And lateral flexion has correlation with balance and gait.

타이치 운동 중재 후 균형 증진에 관한 논문분석 (Analysis of the Effectiveness of Tai Chi Exercise for improving Balance)

  • Purpose: This study aims to analyze the effectiveness of Tai chi exercise for improving balance and its tendency and helps to identify directions for future research. Method: 19 articles from Medline search of foreign journals(1981-2003) and 5 from Korean nursing journals (1981-2003) were surveyed. The contents analyses were focused on outcome measures and relative factors regarding balance. Result: Variable measures for balance were used in Tai Chi studies relating to balance. They included the functional measures such as 14 single-leg stance, 7 walking in physiological measures and 7 laboratory-based balance measures were done with platform stability test. The measure of effective Tai Chi exercise depended on the health status of subjects and the methods. Health status of subjects were only on physically inactive older adults in 17 articles and arthritis patients in 7 articles in the Tai Chi study. The significant effects of Tai Chi exercise on balance was revealed after 10weeks duration. Conclusion: Considering the results, Tai Chi exercise is more effective than any other studies for walking balance. But the reports on the outcome are inconsistent with wide variations in the choice of balance measures. It is suggested that future studies analyze systemic change through meta analysis and have a wide variety of subjects who need postural control and balance control. The methods of measures should be distinct from the balance state of subjects for the effectiveness of Tai chi exercise.

건측발목고정을 동반한 균형운동이 아급성기 뇌졸중 환자의 균형 조절능력 및 보행 기능에 미치는 영향 (Effect of Balance Exercise with Sound Side Ankle Fixation on Balance and Gait Function of Subacute Stroke Patients )

  • PURPOSE: This study examined the effects of balance exercises accompanied by ankle fixation on the healthy side on the balance control ability and gait of patients with subacute stroke. METHODS: The study was conducted on 23 patients with subacute stroke. Eleven people were assigned randomly to the experimental group who performed balance exercises with ankle fixation on the healthy side, and 12 were assigned to the control group who performed balance exercises without ankle fixation. The intervention was conducted for 30 minutes three times a week for four weeks. The Berg Balance Scale, a weight-bearing ratio measurement, was performed to evaluate the balance ability before and after intervention. The gait symmetry, walking speed, cadence, step length, and 10-meter walk test were conducted to evaluate the walking ability. RESULTS: A significant difference in the Berg Balance Scale was observed between before and after the intervention in the experimental group. A comparison of the two groups also revealed a significant difference. Significant differences in the gait symmetry, walking speed, and step length measurements were observed before and after the intervention in the experimental group, and significant differences in the gait symmetry and step length measurements were observed between the two groups. CONCLUSION: Through this study, balance exercises with the healthy side ankle fixed showed qualitative improvement in the balance and walking ability, suggesting future directions for the rehabilitation treatment of stroke patients.

  • Load master is authorized to be controlled all of loading stuffs for safety of flight such as passenger, baggage, cargo and e.t.c. There are many things are missed even though the weight and balance is the most important process. This study analyzes how the differences of C.G. by among ten load masters of each careers. This study is tested how load-master takes load-control by the respective result based on differences of each practical experiences, gender and a number of certification. In result, all of load masters set C.G. on the stability region. But the practical experience of load master is relative to set better C.G. for economical effectiveness of weight and balance control work.

  • Position balance control refers to ability that is kept bodily center of gravity under minimum position shake on surface of land. Patients who an injury of brain, a wound of the central nervous system or weakening of leg muscular power is fallen are trend increasing steadily by the car accident body balance function is on an increasing trended. These patients have difficulty in position balance control, receive big restriction walk and life style. This research composed to do body balance function rehabilitation training which elevate leg muscular power using step machine. And expect to become effective rehabilitation training device because measuring bodily pressure center using Force Platform and do so that can confirm training result easily as that is monitor.

  • Using the gyro effect, Balance Beam Controller is developed with Samsung Corporation. Balance Beam Controller is possible to control the position of an object in air by controlling the attitude of inner gimbal. But in the unknown load inertia case, even a skilled worker it is not easy to operate a Balance Beam. That is caused by the difficulty to estimate the load inertia. If the amount of the gimbal operation is set excessively with a wrong load estimation, it can often cause accidents. To solve this problem, the control function which is revolving with velocity of the revolution has to be added to developed equipment. In this research, we analyze the characteristics of a Balance Beam(the smaller load inertia, the bigger force of restitution) using the angular velocity sensor, and present constant velocity revolving controller with estimating value of the load using this moving characteristics.
