The Effect of Wearing a Soft Knee Brace and Balance Training on Paretic Side Foot Pressure and Knee Joint Muscle Strength in Stroke Patients

  • The purpose of this study was to investigate the effect of wearing a soft knee brace during balance training on paretic side foot pressure and knee joint muscle strength in stroke patients. The recruited 20 stroke patients were randomized into 10 experimental group and 10 control group. All subjects were subjected to balance training, and only the experimental group was trained in balance while wearing a soft knee brace. Experimental group and the control group before and after the intervention showed significant increases in foot pressure and knee joint muscle strength on the paralyzed side (p<0.05), experimental group showed a significant increase in foot pressure and knee joint muscle strength compared to the control group (p<0.05). This study confirmed that wearing a soft knee brace had a positive effect on paretic side foot pressure and knee joint muscle strength in stroke patients.

Effects of Ankle Invertor and Plantar Flexor Stretching on Balance and Walking Ability of Stroke Patients

  • Purpose: This study was to applied to stretching plantar flexor and invertor at the same time to observe the effect of balance and walking ability. Methods: Subjects were instructed patients with ankle limited of motion, 6 months after stroke. We classified 20 subjects into two groups (experimental group: stretching plantar flexor and invertor at the same time, control group: only stretching plantar flexor). each group included 10 subjects and applied ankle stretcher for 20 minutes, 5 times/week during 4 weeks (total 20 times). Results: Both experimental group and control group showed significant increases in static balance, however, the significantly increases in step length and gait speed was observed only in the experimental group. There was no significant increases in step length and gait speed in the control group. Conclusion: Simultaneous stretching of ankle invertor and plantar flexion is effective in improving balance and walking ability in chronic stroke patients.

앉은 자세에서의 시각을 차단한 균형훈련이 아급성기 뇌졸중 환자의 체간 위치감각에 미치는 영향 (Effects of Trunk Position Sense through Visual Cue Deprivation Balance Training in Subacute Stroke)

  • PURPOSE: The purpose of this study is to investigate effects of trunk position sense through visual cue deprivation balance training in subacute stroke patients. METHODS: The subjects were randomly allocated to two groups: experimental(n=10) and control(n=10). Both groups performed balance training on sitting for 30minute after measurements. Trunk position sense test was assessed using the David back concept to determine trunk repositioning error for four movement(flexion, extension, affected side lateral flexion, non-affected side lateral flexion). Measurements on each test were assessed prior to the balance training and then immediately following the balance training. RESULTS: In comparison of the difference of the trunk position sense between groups, the experimental group decreased significantly in trunk repositioning error of flexion, extension and affected side lateral flexion than control group(p<.05). CONCLUSION: The trunk position sense of the experimental group showed more improvement after the balance training program compared to the control group, Therefore, these results suggest that visual cue deprivation training is considered an effective exercise method for individuals with subacute stroke.

Transient Effects of Calf Muscle Fatigue and Visual Control on Postural Balance During Single Leg Standing

  • PURPOSE: Muscle fatigue is a cause to change proprioception. The purpose of this study was to investigate the effects of calf muscle fatigue and visual control on postural balance during single-legged standing in healthy adults. METHODS: Nineteen healthy adults (male) were participated in this study (mean age: 24.36 years; mean height: 171.32 cm; mean weight: 64.58 kg). The postural balance (sway length, sway area, sway velocity of COG displacement) was measured by Balance Trainer System (BT4) in before and after calf muscle fatigue feeling in single legged stance. In this study, repetitive single-legged heel rise test was used to induce fatigue of the calf muscle. Paired t- test was used to compare the postural balance between before and after calf muscle fatigue. Data of subjects were analyzed using SPSS 22.0 (SPSS Inc., Chicago, IL, USA). Level of significance was set to .05. RESULTS: The sway length, sway area, sway velocity of COG (center of gravity) displacement after calf muscle fatigue feeling was significantly increased compared to before calf muscle fatigue feeling during single leg standing both eye open and close conditions (p<.05). CONCLUSION: This study suggested that calf muscle fatigue feeling has affected on postural balance when standing one leg both eye open and close conditions and postural control was disturbed by muscle fatigue and visual feedback in single leg standing.

시뮬레이션 과제 수행이 뇌졸중 환자의 균형 능력에 미치는 효과 (The Effect of Simulation Task Oriented on Balance in Patients with Stroke)

  • Purpose : This study was performed to examine the effect of stimulation task-oriented training on the balance ability of the hemiplegic patients caused by stroke. Methods : We made a random sampling of 25 hemiplegic patients caused by stroke. 10 patients(experimental group)were treated by simulation Task-oriented training and Conventional training used by balance pad. The other 10 patients(control group) were only treated by Conventional training used by balance pad. During the training, 3 patients from the experimental group and 2 patients from the control group were excluded by private affairs. The control group has done Conventional training 6 times a week for 6 weeks. And experimental group has done Simulation task-oriented training two times, conventional training four times a week for 6 weeks. Balance ability was assessed by Fuctional Reaching Test (FRT) : unilateral and bilateral reaching. Results : In comparison of FRT before and after training, two groups all was significantly improved(p<.05). But bilateral reaching variation was significantly improved in experimental group. Conclusion : we can use simulation Task-oriented training valuably to increase balance ability of hemiplegic patients.

외발서기 시 시각정보 차단에 따른 인체 균형 특성 변화 분석 (Changes in Balance Characteristics Affected by the Visual Information during Single Leg Stance)

  • The purpose of study was to analyze how the visual information affects balance control of individuals during single leg stance. A total of 27 young normal people (20 males and 7 females, age: $13.7{\pm}2.6$, height: $162.3{\pm}13.2$ cm, weight: $53.9{\pm}13.9$ kg) was voluntarily involved in the experiment. The subjects were requested to maintain balance for 20 seconds with eyes both open and closed on a force plate and then foot ground reaction data were collected for that duration. Results showed that mean velocity of COP in closed eyes condition was larger 1.84 times than that of the open-eyes condition and range of vertical angle was increased approximately one degree in the closed eyes condition. To accomplish a balance, the frequency power in mediolateral and anteroposterior components of the foot-ground reaction force was increased by 1.3~1.4 times. Consequently, visual absence during single leg stance can result in critical loss of balance and lead to instability of body control.

Effects of Virtual Reality Exercise Program on Balance, Emotion and Quality of Life in Patients with Cognitive Decline

  • Purpose: In this study, we investigated the effectiveness of a 12-week virtual reality exercise program using the Nintendo Wii console (Wii) in improving balance, emotion, and quality of life among patients with cognitive decline. Methods: The study included 30 patients with cognitive decline (12 female, 18 male) who were randomly assigned to an experimental (n=15) and control groups (n=15). All subjects performed a traditional cognitive rehabilitation program and the experimental group performed additional three 40-minute virtual reality based video game (Wii) sessions per week for 12 weeks. The berg balance scale (BBS) was used to assess balance abilities. The short form geriatric depression scale-Korean (GDS-K) and the Korean version of quality of life-Alzheimer's disease (KQOL-AD) scale were both used to assess life quality in patients. Statistical significance was tested within and between groups before and after treatment, using Wilcoxon signed rank and Mann-Whitney u-tests. Results: After 36 training sessions, there were significant beneficial effects of the virtual reality game exercise on balance (BBS), GDS-K, and KQOL-AD in the experimental group when compared to the control group. No significant difference was observed within the control group. Conclusion: These findings demonstrate that a virtual reality-training program could improve the outcomes in terms of balance, depression, and quality of life in patients with cognitive decline. Long-term follow-ups and further studies of more efficient virtual reality training programs are needed.

재활승마가 노인의 균형과 노인 삶의 질척도에 미치는 영향 (Effects of Rehabilitation Horse Riding on the Balance and Geriatric Quality of Life Scale of the Elderly)

  • Purpose The purpose of this study was to investigate the effect of rehabilitation horse riding exercise on the balance and Geriatric Quality of Life Scale (GQOL) of the elderly. Methods The subjects were 14 elderly males who consisted of 7 in the experimental group and 7 in the control group. The subjected performed rehabilitation horse riding exercise twice a week for 12 weeks. For balance measurements, leftright and anteriorposterior weight bearing, and BBS were tested using GymPlate. In addition, the GQOL was evaluated. Results The results of this study show that in the experimental group, there were significant differences in balance ability and GQOL after the rehabilitation horse riding exercise (P<0.01), but there was no significant difference in the control group (P>0.05). Furthermore, the experimental and control groups showed significant differences in balance ability and GQOL (P<0.01). Conclusion Rehabilitation horse riding exercise is effective in improving the balance ability of the elderly, which also had an effect on the GQOL.

PNF 목 패턴을 병행한 체간안정로봇훈련이 만성 뇌졸중 환자의 체간 안정성 및 균형능력에 미치는 영향 (The Effects of PNF and Trunk Stabilization Robot Training on Trunk Stability and Balance in Patients with Chronic Stroke)

  • Purpose: The purpose of the study was to identify the effects of proprioceptive neuromuscular facilitation (PNF) training and robot rehabilitation training on trunk stability and standing balance in individuals with chronic stroke. Methods: There were 30 patients with chronic stroke, divided into two groups: 15 subjects who received PNF and robot training (the experimental group) and 15 subjects who received standard conservative training (the control group), that participated. The experimental group received treatment for 60 min: 30 min of conventional physical therapy, 15 min of PNF training, and 15 min of robot training. The control group received conventional physical therapy for 60 min. Trunk stabilization (trunk impairment scale) and standing balance (center of pressure, limit of stability, modified functional reach test, and Berg balance scale) were measured before and after intervention. Results: Within each group, both the experimental and control groups significantly improved after the intervention in all tests; however, the experimental group showed greater improvement in scores on the trunk impairment scale, the center of pressure, the limit of stability, the modified functional reach test, and the Berg balance scale. Conclusion: The study verified that PNF training and robot training had a positive influence on trunk stability and standing balance indices in patients with chronic stroke.

Relationships among Lower Extremity Muscle Circumference, Proprioception, ROM, Muscle Strength, and Balance Control Ability in Young Adults

  • Purpose: The purpose of this study was to analyze the correlation between balance control ability and leg circumference, proprioception, range of motion (ROM), and muscle strength in young adults. Methods: The subjects of this study were 30 university students who were enrolled in D university in Gyeongbuk province. We measured the dynamic balance and static balance using the Biorescue. The muscular strengths of the hip, knee, and ankle joints were measured using a muscle contraction dynamometer. The ROM and proprioception were measured using an inclinometer. Pearson correlation analysis was used to test the correlations between balance control ability and variables. Results: Sway length was significantly correlated with knee and hip joint muscle strength, ROM, and proprioception of hip and ankle joints (p<0.05). Sway speed was significantly correlated with ROM and proprioception in hip joints (p<0.05). Limit of stability was significantly correlated with muscle strength and ROM in ankle joints, and proprioception in hip, knee, and ankle joints (p<0.05). Conclusion: The sway length was most related to hip extension and ankle joint plantar flexion in the range of motion and ankle joint plantar flexion in proprioception. Overall, balance training for young adults will be of effective help if the treatment focuses on the knee and hip joints, range of motion and the ankle and hip joints' proprioception.
