폐암환자에서 인체 부갑상선 호르몬 관련 단백에 대한 연구 (Human Parathyroid Hormone-Related Peptide Measurement in the Lung Cancer Patients)

  • 연구배경: 악성 종양 환자에서 PTHrp가 고칼슘혈증을 유발시킬 수 있음이 밝혀졌으며, 악성 종양 특히 편평상피세포 폐암에서 골 전이 없이 고칼슘혈증이 종종 관찰되므로 PTHrp가 암의 진행정도 및 예후와 관련이 있을 가능성이 있다. 정상 칼슘혈증인 폐암환자에서도 혈중 PTHrp를 측정하는 예민한 검사법을 이용하면 편평상피 폐암의 조기진단, 치료반응의 추적, 재발의 발견등에 도움을 받을 가능성이 있다. 방법: 조직학적으로 진단된 폐암 환자군 및 대조군의 혈중 PTHrp를 방사면역학 검사로 측정하여 폐암환자에서 각 세포형, 병기, 골 전이 유무에 따른 PTHrp의 혈중치와 혈청 칼슘 및 인, 24시간 요중 칼슘 및 인의 배설량과의 관계를 조사하였다. 결과: 혈청 PTHrp(평균$\pm$S.E.)는 대조군(n=22) $158{\pm}38.2pg/ml$, 전체 폐암군(n=63) $312{\pm}68.9pg/ml$으로 폐암군에서 높은 경향이지만 유의한 차이는 없었다. 각 세포형에 따라서는 편평상피세포암군(n=34) $356{\pm}103.9pg/ml$, 선암군(n=15) $281{\pm}148.7pg/ml$, 소세포암군(n=9) $316{\pm}140.8pg/ml$, 대세포암(n=1) 65.0pg/ml, 미분화 미분류 세포암군(n=4) $114{\pm}77.9pg/ml$로 편평상피세포암군에서 높은 경향이나 각 군 및 대조군 간에 유의한 차이가 없었다. 편평상피암의 병기에 따른 혈청 PTHrp(평균$\pm$S.E.)는 제 I병기(n=1) 143pg/ml, 제 II병기(n=3) $570{\pm}472.3pg/ml$, 제 IIIa 병기(n=9) $166{\pm}22.4pg/ml$, 제 IIIb 병기(n=12) $282{\pm}113.3pg/ml$, 제 IV 병기(n=9) $668{\pm}367.9pg/ml$로 제 II 병기와 제 IV 병기에서 대조군에 비해 유의하게 높았다(p<0.05). 골 전이가 있는 편평상피암(n=8)의 경우 혈청 PTHrp $1526{\pm}811.2\;pg/ml$로 대조군에 비해 유의하게 높았으나(p<0.0005), 골 전이가 없는 군은 대조군과 차이가 없었고, 혈청 및 24 시간 뇨중 calcium, phosphorus의 차이도 관찰되지 않았다. 고칼슘혈증은 편평상피암 1예에서 관찰되었으며, 이 예에서 PTHrp는 244pg/ml로 높지 않았다. 대조군, 폐암군, 폐암의 세포형 및 폐암의 병기에 따른 군간에 혈청 calcium, phosphorus, 24시간 뇨중 calcium, phosphorus에 유의한 차이가 없었다. 결론: 혈중 PTH게는 대조군에서도 상당량 검출이 되며, 정상 칼슘혈증인 폐암환자에서 다수 검출되고, PTHrp가 높아도 고칼슘혈증이 동반되지 않는 등 PTHrp로 종양의 체액성 고칼슘혈증 기전을 모두 설명할 수는 없으며, 폐암 조기발견의 표지자, 병기 및 예후 예측 인자로는 PTHrp 1-34 측정이 부적합하다고 판단 되지만 골 전이가 있는 진행된 예에서 증가하는 경향이므로 병기와 예후 예측인자, 치료 반응 및 재발 등에서의 응용 가능성에 대하여 적극적으로 검토할 가치가 있다. 앞으로 고캄슘혈증 환자에서 보다 많은 예를 시행하고 특이성이 높은 검사법을 개발하는 등의 추후 연구도 필요하다.

급.만성 기관지염 환자에서 엘도스$^{(R)}$(Erdosteine)의 임상효과 -염산 암브록솔과의 무작위 이중맹검 비교시험- (Clinical Efficacy of Erdosteine in Patients with Acute or Chronic Bronchitis -A Randomized, Double Blind, Comparative Study vs. Ambroxol-)

  • 연구배경 : Erdosteine은 만성 기관지염 급성 악화시는 물론 만성 폐색성 기관지염 치료제로 개발된 Thiol 유도체로서 점액조절작용, 항산화작용이 있으며, 특히 위장관 부작용이 거의 없다. 본 연구에서는 현재 유효성 및 안전성이 입증되어 임상에서 많이 처방되고 있는 Ambroxol과의 비교임상연구를 통해 급성 및 만성 기관지염 환자에서 Erdosteine의 임상적 유용성을 알아보고자 하였다. 방 법 : 본 연구는 무작위 이중맹검 비교시험으로서 전향적 연구방법으로 수행되었다. 급 만성 기관지염으로 다량의 객담배출 혹은 객담배출곤란을 주증상으로 하는 80명을 무작위로 할당하여 Erdosteine 투여군에는 1일 2회, 1회 300mg, Ambroxol 투여군에는 1일 2회, 1회 30mg을 총 7일간 투여하였다. 1차 효과평가기준으로는 객담배출횟수, 객담량, 객담배출용이도, 객담점도, 기침정도, 호흡곤란을 포함한 자각증상이 평가되었고, 2차 효과평가기준으로는 동맥혈가스검사, 폐기능검사가 평가되었다. 안전성은 부작용과 임상병리검사로 평가하였다. 총 80명의 환자 중 61명(Erdosteine군 : 31명, Ambroxol군 : 30명)이 본 연구를 종료하였고 19명은 중도 탈락되었다. 결과의 분석은 paired t-test, ANOVA test, multivariate t2-test, Repeated measures analysis of covariance, two sample t-test, Loglinear-Logit model analysis, Fisher's exact test를 이 용하여 유의성을 검정하였다. 결 과 : 1) 두 군간의 환자배정에 있어서 인구학적, 임상적 특징은 차이가 없었다. 2) 동맥혈가스검사, 폐기능검사에서는 두 군간의 차이는 없었고, 자각증상에서는 Erdosteine군은 객담 배출횟수, 객담량, 객담배출용이도, 객담점도, 기침정도, 호흡곤란에서, Ambroxol군은 객담배출횟수, 객담량, 객담배출용이도, 객담점도, 기침정도에서 유의하게 호전을 보였다. 통계적으로 유의하지 않았으나 Erdosteine군이 Ambroxol군에 비해 더 유의하게 호전되는 경향을 보였다($\alpha$=0.05). 3) 혈액학적검사, 혈액화학적검사 및 소변검사에서는 각 군 모두 임상적으로 의미있는 변화는 없었고 두 군간의 차이도 없었으며, 자각증상에서는 Ambroxol군에서만 4명이 연구 종료시까지 부작용이 소실되지 않았고 2명은 연구를 중단하였으나 두 군간의 차이는 없었다. 4) 각 약제의 최종효과평가에서 Erdosteine군은 31명 중 아주좋은효과 11명, 좋은효과 12명, 경도효과 6명, 무효과 2명, 악화 0명이었고 Ambroxol군은 30명 중 아주좋은효과 6명, 좋은효과 14명, 경도 효과 5명, 무효과 4명, 악화 1명이었으며 Erdosteine군이 Ambroxol군에 비해 증상이 개선될 확률이 약 2.5배만큼 높았다 (p<0.05). 결 론 : Erdosteine은 급성 및 만성 기관지염 환자의 치료에 있어 그 효과와 안전성에 있어서 우수한 약물이라고 할 수 있다.

기관지 반응성의 임상적 의의 및 메타콜린으로 유도된 천식 발작시 폐기능 변화와의 관계 (Clinical Impact of Bronchial Reactivity and Its Relationship with Changes of Pulmonary Function After Asthmatic Attack Induced by Methacholine)

  • 연구배경 : 기관지 반응성은 기관지 수축제의 연속적인 흡입에 따르는 기도 수축반응의 증가로서 용량-반응곡선의 기울기로 표현되고 기관지 감수성과 더불어 기관지 천식의 가장 중요한 특정인 기관지 과민성을 구성하는 요소이다. 그러나 일반적으로 기관지 수축 유발 검사 시에는 기관지 감수성만을 측정하고 있으며, 기관지 반응성에 대해서는 아직까지 정립된 이론이 없다. 이에 본 연구에서는 기관지 감수성과 기관지 반응성의 관계와 임상증상과의 관련성, 치료경과 및 폐기능 변화와의 관계를 비교 분석하여 기관지 반응성의 임상적 유용성 및 의의를 밝히고자 하였다. 대상 및 방법 : 천식의 중증도 분류에서 중증군을 제외한 경증 간헐성, 경증 지속성, 중등증의 천식 환자들과 기침형 천식환자, 총 116명을 대상으로 하였다. 모든 환자에서 기저 메타콜린 천식유발검사로 $FEV_1$이 비교 기준치보다 20% 이상 감소하는 메타콜린 PC20에 도달하면, 메타콜린 농도를 1회 더 높여 흡입시켜 $FEV_1$을 측정하여, log 용량-반응곡선을 그려서 PC40를 구했고, PC20와 PC40를 연결한 기울기로 기관지 반응성을 측정했다. 이 중에 43예 에서는 메타콜린 천식유발검사 전후에 body plethysmography와 single breath for DLCO를 시행하였다. 결 과 : 1) 지관지 반응성은 각각 경증 간헐성군 $38.0{\pm}18.16$, 경증 지속성군 $49.8{\pm}9.77$, 중등증군 $61.0{\pm}13.07$, 기침형 천식군에서 $43.8{\pm}3.73$ 이었다. 2) 기관지 감수성과 기관지 반응성은 경미한 역 상관관계를 보였다(r=-0.34, p<0.01). 3) 기저 폐기능이 유사하면서 기관지 감수성이 비슷한 환자들에서 기관지 반응성이 큰 군이 작은 군에 비해 증상의 경중도와 임상경과가 심한 경향을 보였다. 4) 기저 폐기능과 기관지 반응성과는 sGaw, Raw, $FEV_1$/FVC, MMFR 순으로 유의한 상관관계를 보였다. 5) 메타콜린으로 유도한 천식 발작 후의 폐기능 변화량과 기관지 감수성 사이에는 유의한 관계를 찾아 볼 수 없었으나, 기관지 반응성은 PEF, $FEV_1$, DLCO 순으로 유의한 상 관관계를 보였다(p<0.05). 결 론 : 기관지 반응성은 기관지 감수성과 경미한 역 상관관계를 보였으며, 기관지 감수성에 비하여 임상증상의 정중도와 더 관련이 켰다. 기관시 감수성은 메타콜린으로 유도한 천식발작시 폐기능의 변화정도와 유의한 상관성이 없었으나, 기관지 반응성은 메타콜린으로 유도한 기관지 폐색과 유의한 상관관계를 보였다. 따라서, 기관지 반응성과 기관지 감수성은 서로가 보완적이며 독립적인 기관지 과민성의 구성요소로서, 천식환자의 진단을 위한 메타콜린 천식 유발검사시에 PC20와 함께 기관지 반응성을 측정하는 것이 환자의 임상적 평가와 치료경과 및 예후 판정에 도움이 될 것으로 생각하며, 반응성을 측정하는 못하는 경우 sGaw의 측정이 도움이 될 것으로 사료된다.

폐암환자에서 혈청 CEA, SCC, Cyfra21-1, TPA-M 측정의 의의 (Clinical Application of Serum CEA, SCC, Cyfra21-1, and TPA in Lung Cancer)

  • 연구배경 : 종양 표지자는 각종 악성 종양의 진단, 진행정도, 절제 가능성, 예후추정, 치료후 추적 검사 등에도 사용될 수 있으나 민감도와 특이도가 만족스럽지 못한 경우가 많아서 임상에서 보조적인 수단으로 이용되고 있는 경우가 대부분이다. 최근 Cyfra21-1이 비소세포 폐암의 진단에 CEA, SCC보다 우수하다는 보고가 있었고 단일클론 항체를 이용한 TPA-M(Tissue polypeptide antigen)이 비소세포 폐암은 진단하는데 상당히 우수한 결과를 보인다고 보고하고 있다. 이에 폐암환자에서 혈청 CEA, SCC, Cyfra21-1, TPA-M의 임상적 유용성을 서로 비교해 보고자 이 연구를 시행하였다. 방 법 : 1996년 4월 1일 부터 19961건 8월 31일까지 계명대학교 의과대학 병원에 내원한 환자중 조직 생검으로 확진된 비소세포 폐암 37예와 소세포 폐암 12예를 총 49예를 폐암 환자군으로 같은 기간의 입원 환자 중 양성 양성 폐질환으로 확진된 29예를 대조군으로 선정하였고 폐암 환자의 경우 수술, 항암요법 및 방사선치료를 전혀 시행하지 않은 상태에서 혈청을 채취하여 검사를 시행하였다. CEA는 일본 Eiken사의 Ab bead CEA, SCC는 미국 DAINABOT사의 SCC RIA BEAD kit, Cyfra21-1은 일본 TFB사의 CA 21-1 kit, TPA-M은 일본 DAIICHI사의 TPA-M kit를 사용하였다. 결 과 : 전체 폐암환자와 대조군을 비교해보면 CEA가 $10.05{\pm}38.39{\mu}/L$, $1.59{\pm}0.94{\mu}/L$, SCC가 $3.04{\pm}5.79{\mu}/L$, $1.58{\pm}2.85{\mu}/L$, Cyfra21-1이 $8.27{\pm}11.96{\mu}/L$, $1.77{\pm}2.72{\mu}/L$, TPA-M이 $132.02{\pm}209.35U/L$, $45.86{\pm}75.86U/L$으로 나타났으며 Cyfra21-1과 TPA-M은 폐암환자에서 대조군보다 종양 표지자의 측정치가 유의하게 높았다.(p<0.05) CEA, Cyfra21-1, TPA-M, SCC의 cutoff value를 회사에서 권장하는 $2.5{\mu}/L$, $3.0{\mu}/L$, 70.0U/L, $2.0{\mu}/L$로 하였을 때 전체폐암에 대한 민감도와 특이도는 CEA 33.3%와 78.6%, Cyfra21-1 50.0%와 89.7%, TPA-M 52.3%와 89.7%, SCC 23.8%와 89.3%이었으며 비소세포 폐암에 대하여서는 CEA 36.1%와 78.1%, Cyfra21-1 50.1%와 89.7%, TPA-M 53.1%와 89.7%, SCC 33.8%와 89.3% 소세포 폐암의 경우에도 CEA 25.0%와 78.5%, Cyfra21-1 50.0%와 89.6%, TPA-M 50.0%와 89.6%, SCC 0%와 89.2%로 폐암환자에서 Cyfra21-1과 TPA-M이 민감도와 특이도에 있어 더 우수한 결과를 보였고 비소세포 폐암환자에서 SCC도 높은 특이도를 나타내었다. ROC(Receiver operating characteristic) curve에 따라 민감도와 특이도를 얻어 가장 정확도가 높은 cutoff value를 CEA $1.25{\mu}/L$, Cyfra21-1 $1.5{\mu}/L$, TPA-M 35U/L, SCC $0.6{\mu}/L$로 하여 민감도와 특이도, 정확도 및 kappa index를 비교해 보아도 Cyfra21-1과 TPA-M이 더 우수한 결과를 나타내었다. 병기에 따른 비소세포 폐암에서의 종양 표지자를 3A 병기까지와 3B 병기이상으로 나누어 비교해보면 3B 병기이상에서 SCC에서만 통계적 유의성을 보였다. (p<0.05) 그러나 종양크기에 따른 종양표지자의 측정치는 통제적 유의성이 없었다.(p>0.05) 결 론 : 혈청 TPA-M과 Cyfra21-1은 폐암환자에 있어 CEA나 SCC보다 민감도와 특이도가 높으며 SCC의 경우 비소세포 폐암에 있어 높은 특이도를 보였으며 SCC의 측정치가 병기와 유관하게 나타났으나 종양의 크기와 병기의 진행과의 관계에 있어서는 더 많은 환자들을 대상으로 한 연구가 행해져야 할 것으로 생각된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.
