The fire not to back to its origin(火不歸原) is said that source yang(元陽) of sea of qi(氣海) rises because fire(火) of lower energizer(下焦) can't return to its origin. Successive medical men regarded the cause of it as yang deficiency(陽虛) or yin deficiency(陰虛) generally, but Jangseoksun(張錫純) presented eight kinds of cause, they are syndrome of upcast yang(戴陽證), deficiency of qi(氣虛), yin deficiency, yin and yang deficiency(陰陽虛), thoroughfare qi ascending counterflow(衝氣上衝), heart fire(心火), yang deficiency with cold fluid retention(寒飮) in middle energizer(中焦寒飮), yang deficiency with sunken cold locked in(沈寒錮冷). The method of conducting fire back to its origin may be the treatment of fire not to back to its origin as an interpretation of the phrase in a broad sense, but it is limited to yang deficiency with sunken cold locked in besides syndrome of upcast yang as the treatment based on pathological conditions. By this standpoint Eunsuryong(殷壽龍) used conducting fire back to its origin to remove hidden cold(伏寒) and make rising false fire(假火) settle. The meaning of conducting fire back to its origin is not just raise yang qi(陽氣) but break sunken cold locked in by using the drugs like Buja(附子), Yukgye(肉桂). Jakyak(芍藥) can concentrate yang qi on the life gate(命門) by converging it, Sukjihwang(熟地黃) can supply yin essence(陰精) and check the intense nature of tonifing yang(補陽) drugs. So if we want to use the method of conducting fire back to its origin, we should confirm the symptoms of sunken cold locked in and yang deficiency not to misdiagnose yin deficiency.
Objective : To explore and describe chronic low back pain (cLBP) patients' experience of acupuncture treatment Methods : Eleven patients with cLBP who agreed and completed the informed consent to take part in this qualitative study were interviewed thoroughly and tape-recorded. Transcribed data were read independently by 4 researchers and analysed thematically with ground theory. Results : All participants chose acupuncture treatment for their cLBP because either they preferred, other people recommended, or felt conventional treatment being more comfortable. Most of the participants experienced improvement during acupuncture treatment and continued treatment motivated by mere expectations. Participants also expressed the specific physical responses/sensations to the effect of acupuncture; various adverse effects during the course of acupuncture treatment. Economic burden was another concern when combined with other therapies in Korean Medicine hospitals. It is considered as a specifically important to those who give acupuncture treatment to obtain a full treatment effect. Conclusions : The emotional facts that Korean patients with cLBP receiving acupuncture treatment benefited by the treatment and considering specific sensations from doctor-patient relationship were turned out to be important to have the maximum treatment effect. Findings from this study should be considered in both acupuncture research and practice.
Objectives : This research was conducted to find the subjective symptoms of musculoskeletal system for main jobs and job posture. Methods : A survey was conducted from September 15, 2011 to October 7, 2011 and 357 dental hygienists were responded. The survey was consisted of the working environment and medical equipments, psycho-social characteristics, health habits and education, job posture, subjective symptoms of musculoskeletal system pain, and so on. Results : In the survey on the frequency of job posture of dental hygienists by jobs, said they cast down or tilt their head by 15 degrees or more or twist or bend their back. The result of the analysis on the subjective symptoms of musculoskeletal system by main job showed that they experienced a pain in neck, shoulder, and back, although the order differed. The order of subjective symptoms of musculoskeletal system for casting down or turning head by 15 degrees or more, twisting or bending back, having shoulders not parallel to the ground, sitting at the end of a chair, and supporting the whole body with one leg or pressing on a pedal for treatment was from the shoulders to the neck to the back. The order of subjective symptoms of musculoskeletal system for having arms too far from the body or hunching for treatment and bending wrists inward or outward for treatment was from the shoulders to the back to the neck. Conclusions : Due to the characteristics of the job which requires one to have a proper view of a patient's oral cavity, most of the postures included casting down of head or bending of back. Therefore a development of new postures is needed to replace the existing job postures which pressure the musculoskeletal system. And shift rotation can be used as a preventive measure.
Background: We evaluated the role and effects of prolotherapy in patients presenting with lower back pain and detected sacral asymlocation, by retrospectively analyzing the results of prolotherapy performed at our institute. Methods: Twenty-three patients with referred pain in the lower back rather than distinct radiculopathy, were detected to have sacral asymlocation by simple X-ray from May 2004 through July 2005. The patients were treated with prolotherapy and manipulation by the Ongley's method around the lumbosacral junction, iliolumbar ligament, and sacroiliac joint. They were treated for approximately one to two week intervals, and during this period were rechecked by X-ray and evaluated using the visual analogue scale (VAS). Results: A total of 23 patients were included in the study (10 male and 13 female), and the average age was 41 years. The average VAS at the time of visit was B.5, the average treatment time was 4,7 days, and the average VAS after treatment was 2.1. Conclusions: Back pain, and associated leg and buttock pain, originate from several causes. In these case analyses, instability around the lumbosacral area and sacral asymlocation might have been important causes of patient back pain and associated buttock and leg pain. We therefore applied prolotherapy as well as manipulation techniques devised by Ongley to these patients, and obtained good results.
Objectives This study aimed to analyze the medical utilization of low back pain (LBP) patients after back surgery and estimate the medical costs of Korean and Western medicine collaborative treatment, odds ratio, and hazard ratio between the two groups using the 2019 Health Insurance Review and Assessment Service-National Patient Sample (HIRA-NPS-2019). Methods Data management and descriptive analysis, logistic regression, and survival analysis were conducted for defining and estimating the LBP patients after back surgery in the NPS 2019 dataset. Results A total of 216,424 patients out of 991,189 were identified as having LBP. Among the patients with LBP, 1,734 were treated with surgery while 214,690 were not. Among those who had surgery, 937 were treated with conventional treatments only and 797 underwent Korean medicine treatments. The odds ratio of the logistic regression analysis was 0.7129, suggesting that Korean medical treatment experience group had a 28.7% lower risk of reoperation than the Western medical treatments only group. The hazard ratio of the survival analysis was 0.9145; thus, the risk probability of reoperation was estimated to be approximately 8.55% lower. The 50% risk of reoperation was 69 days (0.5044) for the conventional group, and 97 days (0.5008) for the Korean medical group in the survival analysis using the Kaplan-Meier graph. Conclusions These results could be utilized in future studies in conducting economic evaluation for estimating cost-effectiveness of Western medicine and Korean medicine treatment compared to Western medicine alone in LBP patients after back surgery in a South Korean perspective. mended and should be applied while taking the necessary precautions.
Purpose:To investigate the effect of the home exercise program on pain, scoliosis, pelvic alignment of low back in chronic back pain patients, and suggest optimal method for home exercise program. Methods:I divided into two groups who has chronic back pain; one is control group who was given a treatment at the hospital only and the other is experimental group who did another exercise after treatment at the hospital, and there were 10 people in each group. The manual therapy were given to all the patients in each group after applying a stupe and an electric treatment, but the experimental group conducted another exercise program at homes. All the exercise programs were applied to patients 12 times for 4 weeks totally. Results:SPSS for win version 12 was used for statistic analysis and independent t-test was used to find changes between two groups. VAS scale was used to show changes in pain between each group. The grade of pain was decreased between pre&post test to -5.60 in control group and -4.80 in experimental group but there wasn't significant difference between each group. Cobb's degree was used to compare the changes of scoliosis in lumbar and it was improved in each group but there wasn't significant difference between each group. The measurement of pelvic misalignment was decreased between pre&post test to -1.00 in control group and -2.00 in experimental group but it wasn't enough to show significant difference between each group. Conclusion:As you read the results above, for a chronic low back pain patient, application of the manual therapy showed that it has effect on decrease of low back pain, improvement of scoliosis in lumbar and pelvic misalignment. However, the effect of home exercise treatment was not sure about improvements for chronic low back pain patient. So I think there should need further study about the effect of home exercise treatment except the treatments at hospital and the thorough education for the exercise of lumbar should be done before the study for the accurate experiment.
Herniation of intervertevral disc(HIVD) is the most common disease causing low back pain. Acupuncture, electroacupuncture and moxibustion has been used for treatment of HIVD. Bee venom (BV) also has been traditionally applied to relieve pain. Recently, it has been reported that BV is effective in treating HIVD. But more objective methods were required to assess the efficacy of BV on HlVD. So we assessed of BV-acupuncture effect on herniated disc patients by use of rating scale for low back pain. We researched 22 patients who were treated at Department of Acupuncture & Moxibustion, Kyung Hee Medical Center from January 1999 to June 1999. The patients were diagnosed by MRI as having a herniated disc. The results of treatment using rating scale for low back pain in HIVD showed that $43.77{\pm}14.39$ point of pre-treatment score and $84.5{\pm}3.97$ point post-treatment score. The improvement index showed $0.43{\pm}0.15$. Especially, clinical score of neurologic deficits showed most highest improvement index of $0.7{\pm}0.45$. Assessment of BV-acupuncture Effect by MRI finding were showed that protruded disc and extruded disc got more higher improvement index than bulging disc. Among 22 patients, satisfactory assessment of BV-acupuncture effect showed that 15(68.2%) were excellent, and 7(31.8%) were good. The result suggest that BV-acupuncture is good method for treatment of HIVD.
Background: Because of fear of reoperation and low efficiency, some patients with failed back surgery syndrome (FBSS) opt for Korean medicine treatment. Gold standard treatment is not present for FBSS, therefore both surgical and non-surgical treatment are possible. Studies of Korean conservative medicine for FBSS had a short duration of treatment and mainly pre- and post-treatment comparisons. And case of female patients who have worsened after childbirth is rare. The purpose of this study was to report long-term follow-up and rare cases of FBSS. Case summary: A 33-year-old female patient diagnosed with lumbar disc herniation complained with low back pain and radicular pain in the left leg after surgery in 2011. Acupuncture and decoction (Cheongpa-jeon, GCSB-5) were administered to her twice a day for about 6 years and 6 months. The patient's complaints improved with each hospitalization, and Magnetic Resonance Imaging (MRI) showed a slight decrease in the size of the recurrent disc (L4/5) and a newly developed disc (L5/S1) that had deteriorated after delivery. Conclusion: Korean medicine could be used to manage the pain of lumbar spine FBSS patients for 6-7 years and to alleviate lumbago after delivery.
Objectives : This study was designed to compare the effect of Shinbaro pharmacopuncture treatment and Jakyakgamcho Decoction pharmacopunture treatment in patients with low back pain caused by traffic accidents. Methods : This study was conducted on 64 cases of patients with low back pain caused by traffic accidents, who were admitted to Bucheon Jaseng Hospital of Korean Medicine from May 3rd, 2015 to October 31th, 2015. The patients were divided into two groups : Shinbaro pharmacopuncture treatment group and Jakyakgamcho Decoction pharmacopunture treatment group. We measured the efficacy of treatment with a numerical rating scale(NRS) and Oswestry disability index(ODI). The NRS which was checked at pre-tretment and after 4th and 8th treatments, and the ODI which was checked at pre-treatment and after 8th treatment were collected and analyzed. Results : In both Shinbaro and Jakyakgamcho Decoction group, NRS and ODI decreased significantly. However there was no statistically significant difference between the groups. At the 4th treatment, However, the Jakyakgamcho Decoction group showed a greater decrease in NRS with a statistically significant difference. Conclusions : In a short period of time, Jakyakgamcho Decoction Pharmacopuncture is more efficient than Shinbaro pharmacopuncture in reducing the pain of patients with low back pain caused by trafficic accidents, but in the end there is no statistically significant difference between the groups.
Objectives : The purpose of this study was to evaluate the effect of electro-acupuncture treatment for low back pain and radicular pain in patients diagnosed with Lumbar HIVD. Methods : This clinical study was carried out on 92 patients with low back pain and radicular pain diagnosed with HIVD, who had been admitted from Mar. 2011 to Dec. 2011. 60 patients were divided into two groups, group A and group B, of 46 patients each. Group A was treated with electro-acupuncture treatment and general acupuncture treatment twice per day for more than 2 weeks. Group B was treated with general acupuncture treatment twice per day for more than 2 weeks. Verbal numerical rating scale(VNRS) and Oswestry disability index(ODI) were used to evaluated the effectiveness of the Oriental medical treatment. Results : In both two groups, VNRS, ODI, patient's condition grade decreased significantly in ststistics as treatment was perfomed. Between the two groups there was no significant difference in the VNRS, patient's condition grade. But there was significant difference in the ODI. Conclusions : These results suggest that both electro-acupuncture and general acupuncture were effective in reducing low back pain and radicular pain caused by HIVD. More systematic clinical stuty on electro-acupuncture is needed.
