증례보고 : 보존적 치료를 통한 연골무형성증을 가진 요통 환자 (A Case Report : Low Back Pain with Achondroplasia Treated with Conservative Treatment)

  • Objectives : The purpose of this study is to investigate the efficacy of conservative treatment on Achondroplasia patient who has low back pain with radiation. Methods : The patient was treated by conservative treatment including acupuncture, pharmaco acupuncture, chu-na manipulation, herbal medicine. Numeric rating scale(NRS), Oswestry disability index(ODI) were used to evaluate pain reduction. Physical examination also used to compare with admission state and discharge state. Results : After 1 month admission treatment, NRS decreased 9 to 6, ODI dcreased 56 to 42, ROM improved as normal person. Conclusions : It was efficient that Achondroplasia patient who has low back pain with radiation treated as conservative treatment including acupuncture, pharmaco acupuncture, chu-na manipulation, herbal medicine. But It was just one case and treatments were carried out in Integrative Package. So further studies are required to compare that which treatment is efficient.

배수혈의 내장기 치료 기전에 관한 연구 (Study on the Treatment Mechanism of Back-Shu Points for Organ Dysfunction)

  • Objectives : This study aims to overview the therapeutic mechanism of back-shu points in terms of sympathetic visceral motor nervous system. Methods : Studies about autonomic nervous system, and studies and ancient texts about back-shu points were reviewed. We interpreted possible mechanism of back-shu points considering similarities of anatomical and physiological characteristics of back-shu points and visceral motor nervous system. Results : Afferent signals for organ lesions that can develop the symptoms of autonomic neurological symptoms, pain, hyperalgesia through the skin segment. Through a physical examination of the myotome and dermatome, it is possible to diagnose segmental disorders. Treatment stimulation of the thick fibers of the disorder segment skin can reduce abnormal autonomic influence over the sympathetic reflex mechanism. In addition, if spinal muscles are relaxed, the pressure on the nerve roots could be reduced and consequently the hyperactivity of the sympathetic visceral motor signal would be suppressed. Conclusions : The back-shu points treatments work through the mechanism of the sympathetic nervous reflex. Moreover, segmental acupuncture can reduce tension of the spinal muscles, thereby improving pathological conditions of the sympathetic nervous system.

임신 중 요통에 관한 고찰 (A Review of Pregnancy Related Low Back Pain)

  • Pregnancy and puerperium are associated with significant changes in pschological and physiologic health status. Back and pelvic pain is common in pregnancy with prevalence figures in the range of $48\%-90\%$. The pain starts during pregnancy and often disappears soon after childbirth. But the prevalence of such pain four to six months post postpartum is report to be $25-40\%$. In $10-15\%$ of the case the pain become chronics, that is persisting for more than three months after childbirth. Low back pain and pelvic pain maybe caused by several factors related to changes that occur naturally during pregnancy. Changes in the center of gravity can create a strain on weight-bearing structures in bone. Pregnancy related hormones, relaxin, create general laxity of collagenous tissue. Another factor found to be a possible primary or contributing cause for law back and hip symptoms are leg length inequality, weight gain and changes in foot function. This article outlines the physiological and biomechanical changes that occur during pregnancy which have been reported to be possible causes of low back and pelvic pain And then, examination, diagnosis, evaluation and treatment of the woman during pregnancy are described. Standard treatment for low back pain and pelvic pain in pregnancy includes education in anatomy and kinesiology, back-strengthening exercise, training of the abdominal muscles and body posture correction. So, most woman during pregnancy require individual consultation and physiotherapist.

만성 요통환자의 수지요법 적용에 대한 효과 (The Effects of Koryo Hand-Acupuncture on the Patients with Chronic Low Back Pain)

  • Purpose: The purpose of this study was to identify the effects of Koryo Hand-Acupuncture on health status(pain, trunk flexion, IADL, depression) of patients with chronic low back pain. Method: This study used a quasi experimental pre-test and post-test design. Data were collected from December 1st, 2000 to December 20th, 2001. 63 chronic low back pain patients(35 experimental group, 28 control group) admitted to the Back-School and consented to this study. The experimental group participated in treatment: Koryo Hand-Acupuncture and AB-Bong. Two groups was homogeneity. After 4 weeks the effects of treatment on the health status was measured between experimental and control group. Data were analyzed using SPSSWIN 10.0 with crosstab, t-test, and paired t-test. Result: In the experimental group, pain(t=4.85, p=.000) and IADL difficulty(t=2.05, p=.045) was significantly lower than those in the control group. It makes no difference trunk flexion(t=-1.60, p=.114) and depression(t=1.50, p=.138) between experimental and control group. Conclusion: These findings indicate that Koryo-Hand Acupuncture is an effective method for reducing pain and IADL difficulty in patients with chronic low back pain, and is considered as a independent nursing intervention for chronic low back pain.

근막동통 증후군의 요통환자에 대한 고찰 (Observation on the Low Back Pain with Myofascial Pain Syndrome)

  • Myofascial pain syndrome is a common cause of physical disability and reduces the activity of the patient. The purpose of this study was review and analysis efficiency of myofascial pain syndrome with low back pain on 50 case who were treated at the Tae Jon Nam, Myung Ho rehabilitation clinic, from January 1, 1994 to May 31, 1994. The results of this study are as follows : 1. of the 50 cases, 24(48.0%) were male and 26(52.0 %) were female. The most common age group was 40 to 30 years old. 2. The most common duration of the treatment and onset were more than 25 months with 22(44.0 %) and less : 3. As for a major causative disease of low back pain with myofascial pain sndrome, without known cause(30.0 %), lifting object(16.0 %), post exercise(14.0 %) and heavy work(12.0 %). 4. Involved muscle with low back pain reviewed I. C. L(31.0 %), Q. L, Gluteus and others muscle(23.0 %). 5. Disease history and treatment duration were proportioned. 6. Involved muscle distribution for causative disease was 26.0 % post exercise to I. C. L, 33.5 % lifting object to Q. L, 40.0 % without known to gluteus. 7. Low back pain with syndrome was buttock and thigh Pain with 44.0 %, only low back pain with 36.0 %. 8. The improvement by physical therapy on the low back pain was good 24.0 %, fair 56.0 % etc respectively. * I.C.L : Iliocostalis lumborum * Q.L : Quadratus lumborum.

건식수치료가 요통환자의 통증과 요부장애에 미치는 영향 (The Effects of Dry-Hydrotherapy on The Low Back Disability and Pain for Patients with Low Back Pain)

  • Purpose: The purpose of this study was to evaluate the effects of dry-hydrotherapy to release pain in patient with low back pain. Methods: The subjects of the study were fifty patient with low back pain in orthopedic clinic. Fifty patients who had low back pain were randomly divided into 2 groups, 1) dry-hydrotherapy group 2) control group. Visual analog scale(VAS) and Oswestry low back pain disability questionnaire were measured before and after treatment. The treatment plan Was as follows; 15 min. exercise per day and 5 days per week for 2 weeks. Results: 1. Before dry-hydrotherapy, there was no significant difference in VAS score and Oswestry low back pain disability questionnaire score between two groups. 2. The VAS score was significantly decreased in both groups after 2-week dry-hydrotherapy and the decrease in the AS score in dry-hydrotherapy group was significantly lower than in control group. 3. The Oswestry low back pain disability questionnaire score was significantly decreased in both groups after 2-week dry-hydrotherapy and the decrease in Oswestry low back pain disability questionnaire score dry-hydrotherapy group was significantly lower than in control group. Conclusion: In conclusion, the dry-hydrotherapy was effective to alleviate pain in patients with low back pain.

요부통증에 대한 운동치료 (Therapeutic Exercise for Low Back Pain)

  • Even though back pain therapy has greatly improved as spinal bio-mechanics is introduced, many patients still have difficulties due to low back pain. At the initial therapeutic stage, the aim of rehabilitation therapy for low back pain is pain control, but, at the later therapeutic stage, the prime aims are to reduce the late complication and to prevent the recurrence of low back pain. Accurate diagnosis should be a first step before any therapy is planned. Thus, accurate physical, neurologic, E.M.G. and radiologic tests are required to give prescription for therapeutic exercise to the patients. In addition to this, the roles of theraphists and therapeutic exercise should be re-evaluated after the therapeutic exercise is performed. Fist of all, the most important things are to educate the patients to understand the low back pain and to let the patients join the therapeutical planning. 1. Bed rest and muscle relaxing exercise for releasing the muscle tention are required for the treatment of acute low back pain. An active exercise is recommended rather than a passive exercise. If the therapeutic exercise depravate the low back pain, the exercise should be immediately terminated and the therapeutical exercise should be replanned. 2. For the treatment of the chronic back pain, stretching exercise and para-spinal muscle strengthening exercise should be performed steadily and actively to prevent the recurrence of low back pain and the low back injury due to minor damage. The patients should be educated to do proper exercise and to maintain good posture in everyday life. 3. As the low back pain is released and the body function is recovered, control of whole body function is necessary. Swiming, bicycling and walking for $30\sim40$ minutes a day and $3\sim4$ days a week are recommended. Other exercise could be recommended depending on the patients condition.

만성 요통 환자의 통증, 지식 및 교육 요구 (Degrees of Low Back Pain, Knowledge of and Educational Needs for Low Back Pain in Patients with Chronic Low Back Pain)

  • Purpose: The purposes of this study were to identify degrees of low back pain, knowledge of and educational needs for low back pain of patients with chronic low back pain and to investigate their relationships. Methods: Data were collected from questionnaires distributed to 83 patients with chronic low back pain at a hospital. Results: The low back pain score was $4.70{\pm}2.22$ out of 10. The degree of low back pain was a statistically significant difference according to gender, smoking, radiating pain and frequency and duration, daily life disturbance degree, sleep disturbance and depression. The knowledge score was 8.29 out of 13. The knowledge was a statistically significant difference according to smoking and degree of sleep disturbance. The educational needs score was 39.83 out of 50. The educational needs was a statistically significant difference according to age, duration of disease, radiating pain, standing time, depression, pain treatment experience, and treatment institutions. As the low back pain increased, the educational needs increased (r=.254, p=.021). There were no correlations between low back pain and knowledge (r=-.040, p=.720) and knowledge and educational needs (r=.061, p=.581). Conclusion: It is important to focus on items with statistically significant differences in pain, knowledge, and educational needs, and to select low knowledge and high educational needs items to develop a systematic education plan.

만성요통의 치료에서 중재적인 치료가 보존적 치료보다 우세한가?: 긍정적인 입장에서 (Is Interventional Therapy Superior to Medical Treatment in Chronic Low Back Pain?: Yes, in Considerable Cases)

  • The British guideline for early management of persistent low back pain, published in 2009, indicated that physicians should offer exercise or medication, rather than radiological interventions or injections, as first choice of treatment in the patients with chronic low back pain (CLBP). However, there had been great controversies regarding the effectiveness of interventional treatment of patients with CLBP. Both somatic (discogenic, instability, etc) and psychosocial factors contribute to the pathophysiology of chronic low back pain (CLBP). Although it can be difficult in many occasions, thorough interview with the patients and specific diagnostic approaches can help us to identify which is the main etiology in individual patient. With the recent progress in medical radiology and development of new therapeutic modalities, some subgroups of patients of CLBP caused by somatic factors appear to be good candidates of interventional therapy. Interventional therapy can be considered in patients with CLBP caused by annulus rupture, facet joint degeneration, disc degeneration, and vertebral column instability. Among other subgroups of CLBP, carefully selected patients with disc degeneration show the most favorable result by interventional therapy. In this regard, discogenic pain, either as a form of CLBP or acute discogenic radiculopathy, seems to be a good indication of interventional therapy. Because many spine specialists generally consider those with radiculopathy are easier to be treated, patients with CLBP tend to be subjects of conventional conservative therapy. For these reasons, clinicians should make their best effort to identify every possible somatic cause in patients with CLBP before regarding them as hypochondriacs. In this review, some of the recent evidence on the role of interventional treatment in patients with CLBP will be discussed, and some of our cases who showed favorable results by interventional therapy will be presented.

Three-dimensional analysis of tooth movement in Class II malocclusion treatment using arch wire with continuous tip-back bends and intermaxillary elastics

  • Objective: The aim of this study was to analyze three-dimensional (3D) changes in maxillary dentition in Class II malocclusion treatment using arch wire with continuous tip-back bends or compensating curve, together with intermaxillary elastics by superimposing 3D virtual models. Methods: The subjects were 20 patients (2 men and 18 women; mean age 20 years 7 months ${\pm}$ 3 years 9 months) with Class II malocclusion treated using $0.016{\times}0.022-inch$ multiloop edgewise arch wire with continuous tip-back bends or titanium molybdenum alloy ideal arch wire with compensating curve, together with intermaxillary elastics. Linear and angular measurements were performed to investigate maxillary teeth displacement by superimposing pre- and post-treatment 3D virtual models using Rapidform 2006 and analyzing the results using paired t-tests. Results: There were posterior displacement of maxillary teeth (p < 0.01) with distal crown tipping of canine, second premolar and first molar (p < 0.05), expansion of maxillary arch (p < 0.05) with buccoversion of second premolar and first molar (p < 0.01), and distal-in rotation of first molar (p < 0.01). Reduced angular difference between anterior and posterior occlusal planes (p < 0.001), with extrusion of anterior teeth (p < 0.05) and intrusion of second premolar and first molar (p < 0.001) was observed. Conclusions: Class II treatment using an arch wire with continuous tip-back bends or a compensating curve, together with intermaxillary elastics, could retract and expand maxillary dentition, and reduce occlusal curvature. These results will help clinicians in understanding the mechanism of this Class II treatment.
