Objectives: The objective of this study was to investigate factor analysis of low back pain patients. Methods: The data were collected from hospital located in Daegu. Observation was made on 40 low back pain patients and factor analysis study was also performed in terms of their abnormal physical findings. Results: The low back pain patients occupied 20% of all clinical patients in male group and 20% in female group. The occurrence of acute low back pain in the first factor tended to be higher among in male group than among in female group. While it was significantly higher muscle weakness of the back in female group. The most prevailing abnormal finding among low back pain patients in the first factor were tender point on the back, While it was significantly lower Decreased tendon reflex in both sexes Conclusions: The factor pattern of abnormal physical findings in low back pain patients was much different between male and female patients, suggesting the different pattern of etiology. Although low back pain is one of the most common symptoms causing limitation activity, as yet there is no known specific effective treatment.
Objectives : To review RCTs on acupuncture treatment for low back pain in order to establish a standard acupuncture treatment model in treating low back pain. Methods : RCT articles on traditional acupuncture treatment for low back pain were searched through online database. Study Quality was assessed using the FEAS. Results : Ten out of the one hundred six articles searched were reviewed. Among the ten articles reviewed, six articles compared acupuncture treatment with no treatment or non-penetrating sham acupuncture. All six articles concluded significantly positive effect of acupuncture compared to the control. Conclusions : The ideal acupuncture treatment model for low back pain was obtained as follows. A sterile disposable stainless steel(0.30mm${\times}$40mm) should be inserted to more than six acupuncture points on the BL, GV and GB meridians such as $BL_23$, $BL_25$, $BL_40$, $BL_60$, $GV_4$ and $GB_30$. Sparrow pecking method to obtain 'de-qi' is recommended and repeated stimulation during the 20 minute retention time is necessary. Ideal treatment frequency would be more than one a week for about 7 weeks.
Objectives : The purpose of this study was to observe the effect of Bong (a type of stick which is called bong) Chuna manual therapy (CMT) on the correction of posterior inferior (PI) ilium deviation, and evaluate changes of pain intensity and Oswestry Disability Index (ODI) for the treatment of low back pain. Methods : We measured leg length inequality (LLI), as an indicator of correction of PI ilium, before and after the Bong CMT twice immediately, and also checked the change of LLI in the course of 10 times of treatment. Ten times of Bong CMT with oriental medical treatment, such as acupuncture and bee venom pharmacopuncture, were performed not only to correct the PI ilium deviation but also to alleviate pain. Visual Analog Scale (VAS) and Oswetry Low-back Pain Disability Index (ODI) score were scored to evaluate the pain and function of low back. Results : In this study, LLI was decreased in general while we performed correction of PI ilium deviation immediately, also effectively decreased comparing between before and after 10 times of Bong CMT, Bong CMT with oriental medical treatment improved pain and function of low back. Conclusions : These results suggest that Bong CMT may be an effective method for the correction of PI ilium deviation. However more rigorous study should be followed because of small cases report.
Objective: This study was designed to investigate the difference of treatment effect between Oriental Medicine theraphy and Oriental and Western Medicine combination theraphy on low back pain patients. Methods: We divided 40 low back pain patients into 2 groups. The Group I was treated with Oriental Medicine theraphy and the Group II was treated with Oriental and Western Medicine combination theraphy. We evaluated visual analogue scale(VAS) score before and after 3, 5, 7 days of each treatment. Results: The VAS score of Group I and Group II after 3, 5, 7 days of each treatment was significantly lower than before treatment. But there was no significant difference between two groups after 3, 5, 7 days of each treatment. Conclusions: In this study, there was no significant difference between the two groups after 3, 5, 7 days of each treatment on low back pain patients. Further studies are needed for the comparison of Oriental Medicine theraphy and Oriental and Western Medicine combination theraphy.
Objectives : This study was performed to evaluate the general distribution and the efficacy of oriental medical treatment for low back pain. Methods: We have selected of the moxa bucket moxibustion. We make a comparative study of the thermodynamic characteristics of moxa bucket moxibustion. We examined combustion times, temperatures, temperature gradients in each period during a combustion of moxa bucket moxibustion made by oak wood. Results : The 80 cases of low back pain patients was analyzed according to the distribution of sex, age, the period of disease, contributing factor, the patient-condition on admission, the symptom on admission, the duration of admission, the reading of the X-ray and the treatment efficacy was evaluated respectively. 1. Female was more than male in the ratio of 1:1.2, and thirties and seventies 25% the most, the acutest phase 46.3% the most, reason unknown 33.8% the most, Grade III 50% the most, L.B.P. only 51.3% the most, the day of 6-10 30% the most, compression Fx 30% the most. 2. In the total treatment result, the good was 51.3%, the excellent 22.5%, the fair 17.5%, the poor 8.8% in order. About 'the effective rate'(the percentage of positive effective treatment cases) of each distribution, the fifties 100% the most, the acutest phase 97.3% the most, the slip down, weight lifting, overuse 100% the most respectively, Grade III 97.5% the most, L.B.P. only 95.1% the most, the day of 11-15 100% the most, HN.P. 93.3% the most. Conclusions : We have known the efficacy of oriental medical treatment for L.B.P. was good and early treatment was better than late treatment and main cause of L.B.P. was degenerative change.
The purpose of this study is to effectively treat low back pain for the out patients who have been treated at the oriental hospital. Sorting the low back pain patients into four types based on scapular and ilium tilting hyper/hypo mobility characteristics, it has used several kinds of acupuncture stimuli on the 4 motor points of limb skeletal muscles, analyzing the acupuncture treatment on the 12 acupuncture points with which oriental hospitals has given to the low back pain patients and comparing with this effect. It has been analyzed how effectively the acupuncture stimuli has been contributed to the low back pain patients who suffered from skeletal muscles of spinal limb thorax abdominal and spinal skeletal system. The results are as follows. 1. There has been changes after treatment in both the control and experimental groups for low back pain patients, but the latter has been more increased in changes than the former. 2. There were much effects on the balanced restorations for spinal skeletal muscle in both the control group and experimental one, but on terms of restoration degree, the degree of the experimental group was higher than that of the control group. 3. Both the contrastive treatment and the experimental one have been contributed to the balanced restoration for all of the limb thorax abdominal muscles, but there were a few differences in terms of the way for restoration and the effects for improvement according to body types. 4. On both the control and the experimental group, spinal skeletal system has been restored with balance, but the experimental group has been higher effective on the balanced restoration than the one of the control group. Considering these results both the control and the experimental treatments have been contributed to the balanced restorations for all of low back pain patients who were suffering from skeletal muscles of spinal limb thorax abdominal and spinal skeletal system, but the contribution degree for the experimental group has been higher than that of the control group.
Objective : This study is designed in order to evaluate the therapeutic effect of Scalp Acupuncture and Do-ma Acupuncture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) on acute low back pain Methods : We investigated 10 patients suffering from acute low back pain were treated from Mach. 1st 2005 to Jun. 30th 2005. 10 patients, were diagnosed only straightened curvature on lateral view of lumbar spine X-ray and get Scalp acupunture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) and Do-ma acupuncture. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. And visual analog scale(VAS) was used, too. Results : Both Scalp acupunture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) and Do-ma acupuncture therapy showed good effect on acute low back pain. It was proved by the difference between PRS and VAS checked before treatment and what checked after treatment. In the evaluation of treatment effect, Exellent was 2(20%), Good was 5(50%), Moderate was 3(30%), Poor was 0(0%). Conclusion : The therapeutic effect of Scalp Acupuncture and Do-ma Acupuncture on acute low back pain can be recommended as a useful therapy to treat acute low back pain.
Objective : Since ancient times venepuncture has been widely used as a useful treatment for low back pain. This study is made in order to objectively evaluate the therapeutic effect of Weizhong(BL40) on low back pain. Methods : Total 46 cases of low back pain patients were divided into 2 groups; one group took only acupuncture therapy, and the other group took acupuncture and venepuncture therapy together. Each patient's pain was evaluated before the treatment and the 3 or 4 days later after the treatment. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. Results : Both acupuncture therapy group and acupuncture & venepuncture group showed good effect on low back pain. Though the latter group showed better pain release effect than the former, there was no statistical significance between pain rating score of two groups. Sciatica group showed no significant difference from non-sciatica group in pain rating score. Palpation on Weizhong(BL40) group showed significant difference only in pain duration from non-palpation group. Exposed vessel around Weizhong(BL40) group showed significant difference in pain rating score from non-exposed vessel group. Conclusion : Weizhong(BL40) venepuncture can be highly recommended as a useful therapy for low back pain patients especially who show exposed vessels on the popliteal area.
Objective : Psychological factors may have a major influence on the outcome of treatment for back pain. We investigated the relationship between the outcome and some psychological factors, such as self-reported pain intensity, disability, sleep disturbance and fatigue. Method : The study was conducted as a survey using a questionnaire and telephone interviews. The survey included consecutive 294 patients who visited the neurosurgical out-patient department complaining of low back pain and contacted by telephone on average seven months after the first visit. Pain intensity was measured by visual analog scale, and disability was assessed by Waddell's chronic disability index. Results : The outcome of treatment for back pain was recovered in 36.7%, improved in 30.6%, almost same in 28.6%, and aggravated in 4.1%. Overall rate of improvement was 67.3%. The rate of improvement was related to the duration, patterns and intensity of the symptom, and Waddell index. It was not influenced by the doctors, special studies, and methods of treatment. When the duration was more than 6 months, there were the symptoms of both back and legs, and the self-reported Waddell index was 1-3, the rate of improvement was relatively low. Although the intensity of the pain and disability was closely related to the degree of sleep disturbance, fatigue, appetite, or indigestion, the outcome of treatment for back pain was not always bad in patients with high psychological stress. Actually the outcome of the patients who complained severe pain and disability was better than the outcome of the others. Conclusion : The outcome of the back pain can be predicted by the duration, patterns and intensity of the symptom, and Waddell index. The multidisciplinary treatment will be necessary for the patients whose expected outcome is not good to reduce not only the physical symptoms but also the psychological stress.
Objection : A medicine which is patient-initiative and preventive is required in Korea, which is going to an aging society. On this, we started this study to make a new rule to diagnose and treat more definitely by linking traditional Oriental medicine to Sasang Constitional medicine, which thought to be suitable because of Sasang Constitional medicine has been valued on staying, diet and human's nature which means preventive medical propensity. Methods : On this study, we discriminated Sasang Constition of 161 students, who is in college of oriental medicine, Wonkwang university, by using QSCCII program; and in those students, we checked 30 volunteers's five vicera's Back-su point on riging and depression, tenderness reaction, and color reaction after cupping treatment. Results : As a result of study of riging and depression of five vicera's Back-su point, we could confirm that defecient organ's Back-su point were more depressed than excess organ in Seum-in, Taeeum-in, Soyang-in. As a result of study of tenderness reaction of five vicera's Back-su point, we could confirm that excess organ's Back-su point showed stronger tenderness reaction than defecient organ in Seum-in, Taeeum-in; and excess organ's Back-su point showed weaker tenderness reaction than defecient organ in Soyang-in. As a result of study of color reaction after cupping treatment of five vicera's Back-su point, we could confirm that excess organ's Back-su point showed stronger color reaction than defecient organ in Seum-in, Taeeum-in; and excess organ's Back-su point showed weaker color reaction than defecient organ in Soyang-in. Those data suggest that there are close correlation between Sasang Constition and on riging and depression, tenderness reaction, and color reaction after cupping treatment of five vicera's Back-su point in treditional Oriental medicine. Conclusions : From this study, we can make a new rule to differentiate Sasang Constition by clearfy a relation between Sasang Constition and five vicera's Back-su point; and with this rule, we could contribute to popularization and objectification of Sasang Constition medicine and further more, Oriental medicine by making a Sasang Constition diagnosis instrument. With this, a trait of Sasang Constition medicine that is preventive and valued on character of individuals will contribute to building a patient-initiative and preventive medical system and will expend the domain of Oriental medicine.
