A Case Report on HIVD-Cervical Spine Failed Back Surgery Syndrome Applied Chuna Treatment (추나 요법을 적용한 경추 추간판 탈출증 척추 수술 실패 증후군 환자의 경과관찰 1례)

  • Objects : This study was to report a clinical effect of Chuna Treatment for HIVD-cervical spine Failed Back Surgery Syndrome(FBSS) patient. Methods : In order to alleviate both arm tingling, numbness and neck stiffness, the patient was treated by acupuncture therapy, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, Verbal Numerical Rating Scale(VNRS) and Neck Disability Index(NDI) score were used. Results : VNRS and NDI were improved and Recovery rate was 100%. Conclusions : Korean Treatment can be effectively used for a patient with HIVD-cervical spine FBSS patient. Further clinical studies are needed to verify the findings.

Effective Low-Level Laser Therapy Including Laser Acupuncture Treatment Conditions for Non-Specific Chronic Low Back Pain: Systematic Review and Meta-Analysis

  • Low-level laser therapy including laser acupuncture has been widely used for non-specific chronic low back pain in primary Korean medical clinics. However, there is no critically appraised data regarding which treatment conditions are most effective. A systematic review and meta-analysis was conducted to determine effective treatment conditions using 12 databases (PubMed, Ovid, CENTRAL, KoreaMed, KMBASE, KISS, NDSL, KISTI, OASIS, CNKI, CiNII, and J-STAGE). There were 1,019 studies retrieved and 13 studies included in this review. It was determined that when the power output was ≥ 50 mW, the beam size was increased to ≥ cm2, the energy dose was increased to ≥ 4 J per point, the treatment interval was increased to ≥ 3 times a week, and the number of treatment sessions was increased to ≥ 10 treatments, these conditions appeared to increased treatment effectiveness.

The effects of active release technique on the gluteus medius for pain relief in persons with chronic low back pain

  • Objective: Low back pain is a primary of source of dysfunction and economic costs. Gluteus medius muscle co-activation and activity pattern change caused the low back pain. Active release technique (ART) is a patented, non-invasive, soft tissue treatment process that both locates and breaks down the scar tissue and adhesions. The purpose of this study was to assess the effects on chronic low back pain using ART on gluteus medius so that suggest usable treatment method for treating chronic low back pain. Design: One group pretest-posttest design. Methods: Twelve patients with chronic low back pain were participated in this study. Subjects in ART group were received 2 times a week for 3 weeks treatments with either ART on gluteus medius muscle trigger points. Outcome measures were conducted by pain intensity with a pain visual analogue scale and pressure pain threshold on gluteus medius. Results: Completion of the intervention, the visual analogue scale was decreased in ART group (p<0.05). Also pressure pain threshold was decreased in ART group (p<0.05). Conclusions: Our results suggest that the response to ART may be usable to treat low back pain. ART was presented to reduce pain level of low back in people with chronic low back pain. Further study is required to management for low back pain due to gluteus medius and more ART study.

The Effect of the Treatment with Venesection on Treating Low Back Pain of Acute Stage (급성기(急性期) 요통(腰痛)의 치료(治療)에 자락요법(刺絡療法)의 병행(竝行)이 미치는 영향(影響))

  • Objective : This study is designed in order to evaluate the therapeutic effect of venesection on acute low back pain. Methods : From Nov. 1st 2003 to Apr. 30th 2004, 40 cases of low back pain patients at acute stage were divided into 2 groups; one group(control group) took only acupuncture therapy, and the other group(test group) took acupuncture and venesection therapy. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. And visual analog scale(VAS) was used, too. PRS and VAS was checked twice, before the treatment and after the treatment. Treatment was done twice a day. Results : Both acupuncture therapy and acupuncture with venesection therapy showed good effect on low back pain of acute stage. And the latter group showed better effect on decreasing pain than former. It was proved by the difference between PRS and VAS checked before treatment and what checked after treatment. But it was not proved statistically. Conclusion : Only acupuncture and acupuncture with venesection can be recommended as a useful therapy to treat low back pain of acute stage.

The length of hospital stay of the industrial workers with back injury (산업재해 요통근로자의 재원기간에 관한 연구)

  • Back injury is frequent in industry workers and is a common cause of productivity loss. It has been reported that the insured of industrial accident insurance tend to stay in hospital longer than that of other types of insurance. The purpose of this study was to identify factors affecting the length of hospital stay for the treatment of back injury in the workers under industrial accident insurance. The results of this study help insurers develop reasonable industrial accident insurance policy for back injury claims and prevention strategies of work-related back injury. A total of 2,949 patients whose industrial accident insurance claim has been approved for the treatment of work-related back injury from January to December 1999 were included in this study. Relationship between the length of hospital stay and characteristics of patient, work place, back injury, and hospital were assessed using ANOVA, t-test, simple linear regression and multiple resgression. The major findings of this study are as follows : 1. The average length of hospital stay(LOS) was 91.82 days, respectively. 2. Characteristics of Patient LOS of male patients was longer than that of female patients, there was positive correlation between age and LOS and between average wage and LOS. Working period was negatively correlated with LOS. Distance from resident to hospital was positively correlated with LOS and LOS was significantly different dependign on type of duty. 3. Characteristics of Work Place LOS was significantly different depending on types of industry and geographical region of work place. Size of work place was positively correlated with LOS. 4. Characteristics of Back Injury Occupational back pain required shorter LOS compared with back injury due to electric shock. Number of concomitant illnesses and severity of disability were positively correlated with LOS. 5. Characteristics of Hospital Patients treated in community hospitals required significantly longer LOS. Treatment in hospitals with rehabilitation program required decreased LOS. This was more prominent as number of physicians specialized in rehabilitation. 6. Multiple regression analysis revealed that distance form resident to hospital, geographical region of work place, size of work place, number of concomitant illnesses, severity of disability, and type of hospital were factors affecting LOS.

Interview Survey Methods for Moxibustion Treatment of Knee Pain, Neck Pain and Back Pain: Subject to Oriental Doctors in Seoul (슬통(膝痛), 경항통(頸項痛), 요통(腰痛)의 뜸 시술(施術) 방법(方法)에 대한 면접조사 -서울시 한의사를 대상으로-)

  • Objectives : The aim is to comprehend the difference of moxibustion used according to applied body parts (knee pain, neck pain, back pain) to oriental doctors running clinic in Seoul. Methods : A sampling of 288 oriental doctors running clinic in Seoul was done and the doctors were asked to fill out 20 questions by interviewing directly from Nov. 23. 2009 to Jan. 9. 2010. Results : The result shows that moxibustion was used for knee pain and back pain more than neck pain. The type of moxibustion mostly used was sticker moxibustion for all three symptoms. Burn marks were left mostly in knee and back pain. Most large volume of moxa was used to back pain. More than half of the oriental doctors used 1-3 acupoints for one procedure and on average 2-3 sessions was done. Patients with knee pain and back pain had more than 4 sessions compared to neck pain. Treatments were mostly done 3 times a week, and as for total treatment period, it took less than 1 week for acute symptom, and it took more than 1 week and less than 5 weeks for chronic symptom. Conclusions : This research shows that the method of moxibustion used differs according to the area of treatment(knee pain, neck pain, back pain) by survey of oriental doctors through direct interviews.

The Use of Ojeok-san in Low Back Pain Patients : Results of a Telephone Survey (요통진료 및 오적산 사용현황파악을 위한 전화설문조사)

  • Objectives : This survey was accomplished to find out how Korean medical doctors take Ojeok-san prescriptions for low back pain in clinical practice. Methods : The survey questionnaire was developed by the committee of experts who major in acupuncture & moxibustion or statistics for acupuncture clinical trial protocol development. 306 persons having more than 5 years experience were randomly selected from a list of Korean medical doctors. Telephone interviews with them were conducted by the well-trained interviewers between March 21 and April 4 in 2009 and the computerized data were analyzed by STATA 9.0 SE version(Stata Corp, Colleg Station Tex, USA) statistical program. Results : 1. 192 out of 306(65.1%) Korean medical doctors taked Ojeok-san prescriptions for musculoskeletal disorders patients and 183 out of 192(95.3%) Korean medical doctors taked Ojeok-san prescriptions for low back pain treatment. 2. The rate of low back pain patients, '20~30%' was 99 people(33.6%) and the rate of Ojeok-san prescriptions for low back pain treatment, 'below 10%' was 65 people(35.5%). 3. In Ojeok-san effect of low back pain treatment, 'greate effects' 29.5%, 'tolerable effects' 66.7%, 'no effects' 3.8% were selected. In granule extracts effect, 'similar to Original herbs' 9.8%, 'unlike to Original herbs' 90.2% were selected. Conclusions : In our telephone survey, Korean medical doctors taked Ojeok-san prescriptions for low back pain treatment. Further research on this issue is needed.

Advanced Water Treatment of High Turbidity Source by Hybrid Process of Photocatalyst and Alumina Microfiltration: Effect of Organic Matters at Nitrogen Back-flushing (광촉매 및 알루미나 정밀여과 혼성공정에 의한 고탁도 원수의 고도정수처리: 질소 역세척시 유기물의 영향)

  • Effect of humic acid (HA) with periodic nitrogen back-flushing was investigated in hybrid process of alumina microfiltration and photocatalyst for drinking water treatment. It was compared and investigated with the previous results of microfiltration water back-flushing or ultrafiltration nitrogen back-flushing in viewpoints of membrane fouling resistance ($R_f$), permeate flux (J), and total permeate volume ($V_T$). As results, the trends of membrane fouling were different depending on nitrogen or water back-flushing, and depending on ultrafiltration or microfiltration made with the same material. Also, the nitrogen back-flushing using microfiltration was more effective membrane fouling inhibition than ultrafiltration, and the nitrogen back-flushing was more effective than water back-flushing using the same microfiltration membrane. Turbidity treatment efficiencies were almost constant independent of HA concentration, but HA treatment efficiency was the maximum at HA 10 mg/L. From this results, it was shown that the treated water HA quality increased as increasing HA concentration, but HA could be removed the most effectively by photocatalyst beads adsorption and photo-oxidation at HA 10 mg/L.

Systematic Review of High Frequency of Acupuncture Point and Self Exercise Therapy for Lower Back Pain (요통 치료의 빈용 경혈과 자가 운동 요법에 대한 체계적 문헌 고찰)

  • Objectives The aim of this review is to figure out which acupoints are frequently used to treat low back pain and which exercises are effective to low back pain. Methods We searched the three electronic database (RISS, KISS, KTKP) and manually checked related Korean journals and reference lists up to June 2013. We investigated the frequency of acupoints for using treatment of low back pain. We Introduce exercise therapies we can do easily at home without professional skills for treatment low back pain. Results We included 72 articles in this study. The most frequently adopted acupoints were BL25, BL23, BL24, GB30, BL40, BL26, BL60, GB34, BL52, BL57, GB39. The most frequently adopted meridian pathways were BL, GB, GV, ST. The most frequently adopted disease causing low back pain were HIVD (Herniated intervertebral disc), Lumbar sprain, Spinal stenosis, Compression fracture, Cauda equina syndrome, Spondylolisthesis, Ankylosing spondylitis. There are many exercise therapies (Lumbar stabilizing exercise, Resistance exercise, Bareunmom physical exercise, Mckenzie exercise, Williams exercise, Emblass exercise, Swiss ball exercise, Thera band exercise, Yoga) for treatment low back pain. Conclusions These results suggest that most frequently adopted acupoints were foot taeyang urinary bladder channel acupoints. Beacuse foot taeyang urinary bladder channel is passes through the waist, it will be effective for treatment of low back pain. Also exercise therapy is effective for enhance Muscular strength. In order to treatment of low back pain, Acupuncture treatment is also important, as well as exercise therapy.

A Case of Dural Ectasia with Low Back Pain and Sciatica Treated with Integrative Korean Medical Treatment

  • Dural ectasia is defined as ballooning or expansion of the dural sac surrounding the spinal cord. This report describes a rare case of low back pain and sciatica, suspected as being dural ectasia. The patient was hospitalized for 45 days, and underwent integrative Korean medical treatment, including pharmacopuncture, acupuncture, herbal medicine, Chuna therapy, cupping therapy, and physiotherapy. The effect of the treatment was evaluated using the numerical rating scale, Oswestry disability index, European quality of life 5 dimensions, and subjective symptoms. After inpatient treatment, the pain the patient experienced was significantly reduced and the evaluation indices improved. This case report suggested that integrative Korean medical treatment could be an effective therapeutic choice for low back pain and sciatica, with dural ectasia. Further clinical studies are needed to support this observation.
