골수이식 환자의 영양지원 실태에 관한 연구 (A Study on the Status of Nutrition Support in Bone Marrow Transplantation Patients)

  • Recently, the number of patients who received Bone Marrow Transplantation(BMT) has been increased dramatically and the diseases for which BMT if efficacious are increasing. Adequate nutritional card for BMT patients is crucial for the success of BMT because nutritional deficiency could provoke deteriorative effects. However, little is known about nutritional status among BMT patients in Korea. This study was conducted to assess oral and parenteral intake of BMT patients and compare the change of nutritional status before and after BMT. Twenty-two BMT patients who were admitted to the Severance hospital from December in 1995 to September in 1997 participated in the study. Total calorie requirements were calculated for each patients individually and nutritional support for each patients consisted of oral and parenteral feeding. To assess oral intake of BMT patients, each patients recorded the amount of food they have eaten from 6 days before BMT to 28 days after BMT. The medical records of each patients were used to assess parenteral intake. To compare the nutritional status before and after BMT, the results of anthropometric and biochemical test from 14 days before BMT to 28 days after BMT were used. At the time of admission, the patients were in allowable nutritional status and their total calorie intake was 93% of total calorie requirement. When the preparative regimen for BMT was started, the patients' oral intake was dramatically decreased below 400kcal/day. Even though their oral intake was increased after BMT, their oral intake at the 4th week after BMT was only 752kcal/d, which is only 35.8% of total calorie requirement. The patients' mean oral intake during BMT period (from 6 days before BMT to 28 days after BMT) was only 439kcal/d. Although Total Parenteral Nutrition(TPN) was added when the preparative regimen was started, the patients' mean total caloric intake during BMT period was 111% of basal energy expenditure and 83% of total calorie requirement. The mean total protein intake was only 58% of total protein requirement. In the comparison of nutritional status between pretransplant phase and posttransplant phase in BMT patients, their body weight and serum albumin level were significantly decreased(p<0.001). These results show inadequacies in nutritional intake among BMT patients, and indicate the need of TPN during BMT period.

골수이식생존자의 삶의 질 의미 (The Meaning of Quality of Life for Bone Marrow Transplant Survivors)

  • The purpose of this study was to explore the concept of quality of life for bone marrow transplant (BMT) survivors and to gain understanding of nursing interventions that may improve QOL in this population. The data was gathered from 32 BMT survivors using seven open-ended questions. The items were based on previous research of Ferrell et al., (1992). Content analysis was performed on written responses to seven questions regarding BMT and QOL. The results were as follows : 1. The meaning of QOL for BMT survivors were "being healt", "being able to take a role", "having relationships", "self-accomplishment", "peace of mind", "spiritual well-being", "economic stability" and "being alive". 2. The impact of BMT on physical well-being were "skin impairment", "digestive problems", "infections ", "fatigue/weakness", "arthralgia", "eye dryness". "weight gain", "amenorrhea" and "hand tremor". 3. The impact of BMT on psychological well-being were "fear of recurrence", "sence of peace" and "hope". 4. The impact of BMT on socioeconomic status were "financial burden", "limitation of social activities" and "sence of withdrawal". 5. The impact of BMT on spiritual well-being were "dependency on Supreme Being", "spiritual arousal " and "spiritual maturity". The findings of the study will be useful in constructing a instrument to measure QOL in BMT and in understanding the conceptual basis of QOL for the BMT population.ency on Supreme Being", "spiritual arousal " and "spiritual maturity". 골수이식환자의 교육요구도 (Learning Needs in Patients undergoing Bone Marrow Transplantation)

  • The active treatment phase in preparation for bone marrow transplantation(BMT) of che- motherapy regimen and total body irradiation (TBI) containing regimen requires considerable teaching. There have been researches that are related to treatment onto BMT patients and to psychological change during BMT process. However, it was hard to find researches focused on learning needs of patients undergoing BMT. The purpose of this study was to provide the basic data for effective educational program about BMT by investigating the learning needs in patients undergoing BMT. The subjects consisted of 90 BMT patients have been admitted to the department of BMT at three university hospitals. Data were obtained from October 1998 to March 1999 and analyzed by SAS program for unpaired t-test, ANOVA, Duncan test. The results were as follows : 1. Learning needs related to demographic characteristics was identified as below. That of male was higher than that of female. That of under age 29, unmarried, religious and university graduated group was higher than that of opposite group but it didn't show significant difference. Learning needs of group of patients who were employed was significantly higher then that of unemployed patients. 2. According to types of diagnosis, learning needs of myelodysplastic syndrome(MDS) patients was the higher than that of others, but admission frequency was the least. Learning needs of unrelated matched BMT(UBMT) patients was higher than that of autologous BMT patients. However, it didn't show significant difference. With regard to learning needs according to process of BMT, learning needs of Pre- BMT period or Post-BMT period was significantly higher than that of BMT day. 3. Learning needs related to BMT was relatively high (total mean: 3.11 of 4.0). The order of the mean score of leaning needs was shown as follows : Restricted activities after discharge, Relapse symptom, Complications of BMT, Kinds of available drugs at home. Therefore the learning needs that is related to life after discharge and to relapse and complications after BMT was high. 4. Learning needs related to radiation therapy was high (total mean: 3.35 of 4.0). The learning needs in radiation therapy items was the Skin care of radiation therapy and Purpose of radiation therapy. 5. Learning needs related to graft versus host disease(GVHD) therapy was high (total mean: 3.55 of 4.0). The highest learning needs in GVHD therapy items was the Preventive method GVHD. less admission frequency and UBMT patients. It is necessary that education for BMT patients should be focused on life after discharge and on relapse and complications after BMT. Especially education for allogeneic BMT patients should be emphasized on GVHD. For all of these, it is necessary to develop systematic and concrete educational program.

고유전율의 BMT 기판을 이용한 소형 헤어핀 구조의 듀플렉서 설계 (The Compact Hairpin-Shaped Duplexer using a BMT Substrate with a High Dielectric Constant)

  • 본 논문에서는 고유전율의 BMT 물질을 기판에 적용하여 기판 위에 소형화된 평면형 구조의 마이크로웨이브 듀플렉서를 설계, 제작하였다. Ba(Mg$_{1}$3/Ta$_{2}$3/)O$_3$(BMT)는 품질계수, 온도계수 측면에서 뛰어난 유전 특성을 보이며 유전상수가 23인 고유전 물질로서 회로의 크기를 줄이기 위한 기판에 적용하기가 적합하다. BMT 기판은 tape casting 공정에 의해 제작되었으며, 회로 패턴은 실크스크린을 이용하여 전극 패턴을 입혔다. Open-loop ring type의 듀플렉서를 BMT 기판 위에 설계, 제작하였으며 유전상수가 6.15인 상용 기판에 동일한 규격의 듀플렉서를 제작하여 비교한 결과, 특성의 저하 없이 약 80 % 정도 크기를 줄일 수 있었다. 따라서 제안된 BMT 기판은 예시된 듀플렉서 소형화를 구현하였으며, 마이크로웨이브 수동 소자의 소형화에 기여할 수 있을 것으로 기대된다.

BMT 구동장치의 유한요소해석 및 형상변수 최적화 (Finite Element Analysis and Geometric Parameter Optimization for BMT Driving Assembly)

  • Base-mounted type(BMT) driving assembly in CNC machine tools is an indispensable part to improve productivity by reducing tool changeover time and to meet the ever-increasing demand of precision machine tools. This study aimed to perform finite element analysis and geometric parameter optimization to improve the efficiency of BMT driving assembly. First, simulations for three-dimensional structural and vibration analysis were performed using ANSYS/Workbench on the initial geometric models of BMT driving assembly. After analyzing stress and deformation concentration zones, several new geometrical models were designed and evaluated by design of experiments and ANSYS/DesignXplorer. Through a series of analysis-evaluation-modification cycles, it was seen that designed models were effective in determining optimal geometry of BMT driving assembly.

  • 정보산업이 발달함에 따라 유사한 제품이 많이 출시되어, 기업은 목적, 용도 및 비용 등을 고려해서 최적을 제품을 선택하기 위해, 여러 제품을 대상으로 BMT를 실시하고 있다. 그러나 기존 BMT는 소프트웨어 운영 중에 지속적으로 발생하는 비용을 고려하지 않고 기능과 성능을 위주로 하여 절반의 정보만을 제공하였다. 그래서 본 논문에서는 품질 특성을 바탕으로 운영 중 발생하는 운영비용을 고려한 BMT 항목 도출 방법을 제안하였다.

  • Effect of MgO content on microstructural evolution and microwave dielectric properties of Ba(Mg1/3Ta2/3)O3 system was investigated. MgO content was varied from 10 mol% deficiency to 10 mol% excess of stoichiometric composition, respectively. It was found that MgO-deficient specimen showed faster grain growth rate than stoichometric and MgO excess BMT system. Besides, sandwich type precipitates of Ba5Ta4O15 which was formed within the BMT grain, might lead to the anisotropic grain growth of BMT grain. On the contrary, in MgO excess specimen, BMT grain growth rate was retarded by precipitations of MgO phase in grain boundary. Besides, the Q values of MgO-deficient showed lower than MgO-excess due to precipitations of Ba5Ta4O15 within BMT grain.

Leucostim, A Human Granulocyte-Colony Stimulating Factor, Facilitates Granulopoiesis After Bone Marrow Transplantation In Mice

  • In vivo administration of Leucostim, a human recombinant granulocyte colony-stimulating factor (G-CSF), was evaluated for the effects on survival, hematologic recovery, and colony forming unit- spleen (CFU-5) in murine bone marrow transplantation (BMT) model. Sublethally irradiated (9 Gy) mice received bone marrow cells from untreated mice, and then were treated with G-CSF subcutaneously at doses of 2.5,5, or $10\mu\textrm{g}$/kg or vehicle solution (control) for 14 days from one day after BMT. There was no effect of irradiation and BMT on mortality. The repeated subcutaneous injections of Leucostim for 14 days post- BMT significantly facilitated hematologic recovery compared with vehicle control in a dose-dependent manner. Moreover, mice treated with Leucostim had significantly increased numbers of CFU-s colonies on day 10 post-BMT. These results suggest that Leucostim, a new G-CSF, has beneficial effects on hematologic reconstitution after BMT.

Factors Determining Intention to Use Banking Technology in Indonesian Islamic Microfinance

  • This study aims to determine the perceptions of Islamic Financial Cooperative (BMT) managers in the Indonesian BMT Association on the acceptance of core banking technology. The Technology Acceptance Model (TAM) is used because it has simple theoretical characteristics (parsimony) and is supported by data (verifiability). This study develops the TAM model by integrating new variables -perceptions of maqashid sharia, perceptions of economies of scale, perceptions of market structure, and perceptions of technology procurement costs. These new variables are used to measure intention in using technology and actual usage in BMT operations. This study used PLS-SEM with smartPLS 3. The study was conducted in Central Java in six ex-Residency at 35 BMT with 300 respondents consisting of six levels of position level. The research found that maqashid sharia and market structure directly influenced the intention of BMT managers in using core banking technology. This new finding strengthens a theoretical model regarding the role of maqashid sharia in the acceptance of information technology in BMT. In addition, the perception of economies of scale has no significant effect on intention in using technology or its actual usage. The perception variable of technology procurement costs was found to have no significant effect on intention in using technology.
