한국인 Cadaver Test에 대한 상완골 골수정 개발에 관한 연구 (A Study on Development of Humeral Intramedullary Fixation Nail based on Korean Cadaver Tests)

  • The aim of this study is to develop a humeral Intramedullary fixation nail (HIFN) suitable for Korean people. In this study, CT images were obtained from 72 Korean cadaveric humeral bones and 3D Korean humeral bone models were reconstructed based on the CT images to investigate anatomical characteristics. Major design parameters of HIFN were selected using the morphological measurement information of the Korean humeral bone models. Through finite element analysis and mechanical tests, the developed HIFN prototype was compared with the Polarus HIFN ($ACUMED^{(R)}$, USA), and it was found that the HIFN prototype showed similar and/or superior mechanical performance compared to the Polarus HIFN. Also, clinical validation for the HIFN prototype was carried out to check predictable troubles in surgical operations. Finally, optimal design modification was proposed to prevent the possible axillary nerve injury due to the locking screw system of the HIFN prototype.

테라테인먼트 스위스 볼 운동이 겨드랑신경 손상 환자의 상지기능, 통증 및 장애, 일상생활활동에 미치는 영향 : 개별대상연구 (The Effect of Theratainment Swiss Ball Exercise on the Upper Limb Function, Pain and disability, Daily Activities of a Patient with Axillary Nerve Injury: Single Subject)

  • 본 연구는 테라테인먼트 스위스 볼 운동이 겨드랑 신경 손상 환자의 상지기능, 통증, 일상생활활동에 미치는 효과를 알아보고자 하였다. 연구기간 및 대상은 2019년 11월 5일부터 2020년 2월 25일까지 G광역시에 거주하는 23세 여성 1명으로, 개별사례실험 연구 중 A-B-A' 설계를 사용하였다. 본 연구는 겨드랑신경 손상 환자에게 다양한 방향과 점진적 무게의 운동 형태로 반복 훈련을 제공하였다. 이는 어깨의 구조의 안정성, 가동성을 높이고, 사용가능한 어깨 기능을 강화시켜 통증을 완화에 효과적이었고 다양한 환경 안에서 활동에 따른 자세 변화의 적응력과 대응능력을 발휘하게 하여, 최대한의 독립적 생활의 수행이 가능하도록 하였다. 따라서 본 연구의 테라테인먼트 스위스 볼 운동은 겨드랑신경 손상 환자의 상지기능, 통증 및 장애, 일상생활활동에 긍정적 영향을 미쳤기에 중재방법으로의 가능성을 확인할 수 있었다. 이는 다양한 환자치료에 전문성을 요구하는 작업치료사의 치료적 접근법으로 발휘할 수 있도록 지속적인 조사와 반복연구를 통해 효과를 검증하고 발전 방안을 모색할 필요가 있다.

지속적 액와부 상완신경총 차단술 -I. 카테테르 삽입방법의 변형- (Continuous Axillary Branchial Plexus Block -I. Modification of catheter insertion method-)

  • Background: Authors modified the traditional continuous axillary brachial plexus block technique of Selander for purpose of increasing success rate and decreasing complications by use of commercial epidural anesthesia set. Method: Thirty-nine patients scheduled for upper extremity operations were injected with 40 ml of anesthetic solution by axillary perivascular technique, using 23~25G immobile needle at 2 cm from the pectoralis major. Tuohy needle was immediately introduced at 4 cm from the pectoralis major and pierced the expanded neurovascular sheath at an angle of 30 degree to the skin. The "pop" was well noted well. Needle was advanced 0.5 to 3.0 cm and epidural catheter introduced through the needle. After removal of needle, occlusive dressing was done. Tip of catheter and spread of solution were demonstrated by fluoroscopy with contrast dye after completion of procedure. Result: Catheter insertion was successful at first attempt for all case. Total length of insertion was from 6 to 13($10.0{\pm}1.7$) cm. Tip of catheter was placed in infraclavicular space(66.7%), about the humeral head(17.9%) and in upper arm in 3 cases as U-shape(7.9%). Catheters were maintained for $6.7{\pm}2.6$(3-12) days. There were no complications such as: perforation of major vessels, needle trauma to nerve, infection, bleeding or hematoma. Conclusion: This study demonstrated continuous axillary brachial plexus block with epidural anesthesia set is safe, easy and convenient modification of technique of Selander.

A Case of Brachial Plexus Schwannoma

  • Brachial plexus schwannoma is rare. A case of a 28-year old woman who complained of a palpable mass in the right axilla is presented. She had previous incisional biopsy on the axillary mass. Brachial plexus magnetic resonance imaging revealed a well circumscribed contrast enhancing mass on the right distal cord of the brachial plexus. EMG revealed normal. Tumor resection was performed with the transaxillary approach. Though dense granulation tissue obscured normal brachial plexus nerve anatomy, using the surgical microscope and nerve stimulator, grossly total tumor resection was performed. She is free of any neurologic symptom at three months postoperatively.

Exacerbation of spasticity in ipsilateral shoulder after right brachial plexus block in a patient with right hemiplegia

  • Spasticity is a common impairment in patients with central nervous system disease. Clinical observation has demonstrated that spasticity can be aggravated by various factors such as emotional state as well as noxious stimuli. A 51-year-old male patient was scheduled for arteriovenous fistula surgery. He had right hemiplegia including motor weakness and spasticity. It was decided that the surgery would be performed under an axillary brachial plexus block (BPB). He appeared nervous when blockade was terminated. The spasticity of the right shoulder increased after ipsilateral BPB. However, when we administered sedative drugs and performed interscalene BPB 2 days later, spasticity did not occur. Exacerbation of spasticity might be evoked by an anxious emotional state. Thus, it seems to be good to consider removing of anxiety and using an appropriate approach when it is tried to perform nerve blocks in individuals with spasticity.

급성 상행대동맥 박리증 수술에서 액와동맥 삽관술의 역할 (The Role of Axillary Artery Cannulation in Surgery for Type A Acute Aortic Dissection)

  • 배경: 급성 상행대동맥박리증 수술 시 대퇴동맥이 동맥삽관 장소로 주로 이용되어 왔으나 최근 관류 장애 및 역행성 색전증의 발생 위험이 적은 액와동맥삽관이 증가하고 있다. 이에 기존의 대퇴동맥 삽관술과 액와동맥 삽관술을 이용한 수술성적을 비교하여 액와동맥 삽관술의 안전성과 유용성을 고찰하고자 하였다. 대상 및 방법: 1995년 10월부터 2001년 9월까지 급성 상행대동맥박리증으로 수술을 받은 71명을 대상으로 의무기록을 후향적으로 검토하여 수슬 전의 상태, 수술범위 및 방법, 수술 후 경과를 분석하였다. 대퇴동맥삽관은 40명으로 평균 연령은 57세, 액와동맥삽관은 31명으로 평균 56세였다. 결과: 액와동맥군의 평균 체외순환시간과 순환정지시간은 각각 207분과 39분으로 대퇴동맥군의 263분과 49분보다 의미있게 짧았고(p<0.05), 수술 후 입원기간 역시 액와동맥군에서 15일로 대퇴동맥군의 35일에 비하여 의미있게 단축되었다(p<0.05). 수술 후 사망률(액와동맥군 6.5%, 대퇴동맥군 10%)과 영구적 신경계 합병증 발생률(액와동맥군 3.2%, 대퇴동맥군 2.5%)은 의미있는 차이를 보이지 않았으나 일시적 신경계 합병증의 발생률은 액와동맥군에서 12.9%, 대퇴동맥군에서 25%로 의미있는 차이를 보였다(p<0.05). 대퇴동맥군 중 2명의 환자에서 수술 중 대동맥궁 분지의 관류 부전이 발견되어 삽관 위치의 변경이 필요하였다. 액와동맥 삽관에 따른 합병증으로는 1명의 환자가 정중신경 손상을 경험하였다. 결론: 급성 상행대동맥박리증 수술에서 액와동맥 삽관은 뇌허혈시간의 단축과 이에 따른 수술 후 신경계 합병증의 발생률을 낮추는 데 기여하는 안전한 술식으로 판단된다. 또한, 이에 따라 대동맥궁에 내피 손상부위가 있는 환자에 대하여 보다 적극적이고 광범위한 수술 적용을 가능하게 한 것으로 판단된다.

Advantages of Scorpion Suture Passer and 70 Degrees Arthroscope in Arthroscopic Bankart Repair: Usefulness for Inferior Labral Repair

  • Background: The blunted tip of a reusable with multiple uses can cause problems with the passing procedure in arthroscopic Bankart repair. This study assessed the advantage of Scorpion with a $70^{\circ}$ arthroscope in arthroscopic Bankart repair compared to hook typed suture passer. Methods: Scorpion in 19 patients, the hook type suture passer (conventional group) in 18 patients were used. All patients underwent the same procedure except for the type of suture passer used. Another different point of the procedure were telescopes and the number of portals used; three arthroscopic portals (posterior, anterorsuperiorlateral, and mid-anterior) and a $30^{\circ}$ arthroscope in the conventional group, but two portals and a $70^{\circ}$ arthroscope as well as the $30^{\circ}$ one in the Scorpion group. The surgery time and the surgical complications including an iatrogenic axillary nerve injury were recorded. Results: The Scorpion group showed a significant decrease in surgery time compared to the conventional group. In contrast to the conventional group, Scorpion provided an easy estimation of the exit of suture passing, no iatrogenic labral injury during the passing procedure with straight movement and the sharp tip of the knife installed. Iatrogenic supraspinatus injuries could be avoided when making an accessory anteosuperiorlateral portal due to the $70^{\circ}$ arthroscope. Conclusions: In arthroscopic Bankart repair, the use of the Scorpion suture passer and a $70^{\circ}$ arthroscope can reduce the surgery time, avoid unnecessary supraspinatus injury, and avoid iatrogenic axillary nerve damage through the relatively easy and precise suture passing and saving of the anterosuperior portal.

Minimally Invasive Percutaneous Plate Osteosynthesis via a Deltoid-splitting Approach with Strut Allograft for the Treatment of Displaced 3- or 4-part Proximal Humeral Fractures

  • Background: This study introduces a surgical technique with good clinical outcome useful in the treatment of osteoporotic displaced 3- or 4-part proximal humeral fractures. Methods: From May 2014 to February 2016, 16 patients with displaced 3- or 4-part proximal humeral fractures were treated by application of a locking plate with an endosteal strut allograft via a deltoid splitting approach with a minimum follow-up of 12 months. The allograft was inserted through a fractured gap of the greater tuberosity to support the humeral head and then fixed by a locking plate with meticulous soft tissue dissection to protect the axillary nerve. Surgical outcomes were evaluated by the American Shoulder and Elbow Surgeons (ASES) and visual analogue scale (VAS) scores, radiological imaging, and clinical examination. Fixation failure on radiographs was defined as a >$5^{\circ}$ loss of neck shaft angle (NSA) compared to that on an immediate postoperative radiograph. Avascular necrosis (AVN) of the humeral head was also evaluated. Results: In all cases, complete union was achieved. The ASES and VAS scores were improved to $85.4{\pm}2.1$ and $3.2{\pm}1.3$, respectively. Twelve patients (75.0%) had greater than a $5^{\circ}$ change in NSA; the average NSA change was $3.8^{\circ}$. Five patients (31.3%) had unsatisfactory ranges of motion exhibiting a <$100^{\circ}$ active forward flexion. No axillary nerve injuries or AVN were observed at the last follow-up. One patient was converted to reverse total arthroplasty due to severe pain and functional deficit. Conclusions: Minimally invasive fixation via a locking compression plate and an endosteal fibula strut allograft in Neer classification 3-or 4-part fractures with severe osteoporosis in elderly patients can achieve good clinical results.

The Axillary Approach to Raising the Latissimus Dorsi Free Flap for Facial Re-Animation: A Descriptive Surgical Technique

  • The latissimus dorsi flap is popular due to the versatile nature of its applications. When used as a pedicled flap it provides a robust solution when soft tissue coverage is required following breast, thoracic and head and neck surgery. Its utilization as a free flap is extensive due to the muscle's size, constant anatomy, large caliber of the pedicle and the fact it can be used for functional muscle transfers. In facial palsy it provides the surgeon with a long neurovascular pedicle that is invaluable in situations where commonly used facial vessels are not available, in congenital cases or where previous free functional muscle transfers have been attempted, or patients where a one-stage procedure is indicated and a long nerve is required to reach the contra-lateral side. Although some facial palsy surgeons use the trans-axillary approach, an operative guide of raising the flap by this method has not been provided. A clear guide of raising the flap with the patient in the supine position is described in detail and offers the benefits of reducing the risk of potential brachial plexus injury and allows two surgical teams to work synchronously to reduce operative time.
