Feasibility of Household Surveys for Population Risk Assessment of Cancer and Cancer Registration Support

  • Cancer is a major health problem in the Arab region including Iraq. An adequate database is essential for effective cancer control strategies. Such a database may be provided through cancer registration but supportive household surveys may be useful. This article reports selected results on the feasibility of household surveys to support and validate cancer registration in Basrah governorate - southern Iraq. A large scale multi-stage cluster sample household survey was carried out in Basrah during 2013. It covered 6,999 households and involved gathering data on demographic characteristics and both incident cancer cases and cancer-related deaths among members of these households during a three-year recall period (2010-2012). The data obtained yielded an average annual incidence rate of 91 per 100,000 population (age-standardized incidence rate of 148.8 /100,000) and cancer specific mortality rate of 68 per 100,000 population (age-standardized mortality rate of 126.3/100,000). The results showed an overall pattern of cancer similar to that reported according to cancer registration but the household survey results were consistently higher than those of the cancer registration by a margin of approximately 20- 30% with respect to incident cancer and about 70 % with respect to cancer-specific mortality. Household surveys on cancer, while costly and time consuming, are a very useful additional source of information on cancer at the population level. They can be performed for specific purposes with effective resource mobilization.

  • Studies on the relation between socio-economic factors and metermal and child health have found that poverty, lack of edcation, inappropriate health serives are affecting to maternal and child health. The Gender Related Development Index (GDI) focuses on equality between men and women as well as on the average achiement of all people taken together, using same cariables as the Human Development Index (HDI) which are life expectancy, literacy rate, and per capita GDP. This research is to inverstigate whether HDI and GDI are useful determinants for maternal infant mortality. Using 146 UN member countries date, we condented multiple regression analysis for maternal and infant mortality with three models which are Model(individual variables-literacy rate, per capita GDP), Model(HDI) and Model(GDI). The results showed that HDI and GDI are powerful determinants of both maternal and infant mortality, respectively HDI($\beta$=-1.18, t=3.3; $\beta$=1.04, t=5.1) GDI($\beta$=-1.44, t=3.9; $\beta$=1.28, t=6.5) The higher power in model with GDI for both maternal and infant mortalities represented that GDI was more powerful determinant of maternal and infant mortality, than HDI respectively HDI($R^2$=0.824, $R^2$=0.842), GDI($R^2$=0.834, $R^2$=0.865). In conclusion, the maternal and infant mortalities are explained by GDI than HDI and may be lower in the societies where there are less discimination between men and women.

단일심실증의 수술요법 (Surgical management ofuniventricular heart)

  • Univentricular heart is a rare congenital cardiac anomaly in which the atrial chambers are connected to only one ventricular chamber and it consists of a diverse group of cardiac malformation characterized by both AV valves or a common AV valve opening into the same ventricle, or the presence of only a solitary AV valve. In spite of recent development in cardiac surgery, corrective operations for univentricular heart still have high mortality and complication rate. Twenty eight patients underwent corrective operation for univentricular heart at Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital from February 1979 to July 1986. Of the 28 patients, 7 patients were operated on by ventricular septation and 21 patients by modified Fontan operation. Of the 28 patients, 19 patients were male and 9 patients female and ages ranged from 5 months to 18 years old with the average age of 7.3 years. There were 2 mortalities in 7 patients operated on by septation with the mortality rate of 28.6% and 5 complications, 3 complete AV block, 1 low cardiac output and 1 arrhythmia. All survived patients are being followed up without specific problem till now. There were 10 mortalities in 21 patients operated on by modified Fontan operation with the mortality rate of 47.6% and 10 complications, 2 low cardiac output, 2 respiratory failure necessitating tracheostomy, 2 persistent cyanosis, 2 arrhythmia, 1 missing of left AV valve in situs inversus patient due to misdiagnosis and one rupture of closed right AV valve. Incremental risk factors for operative mortality are young age less than 5 years old, anomalous pulmonary and systemic venous drainage and atrial septation procedure. In 11 survived patients, 9 patients show good follow-up results but one patient complains of persistent cyanosis and another one patient is suffered from CHF. In our series, results of corrective operation for univentricular heart shows continuing improvement but still high mortality and complication rate. So there must be continuing improvement in surgical result by selection of patient, by adequate decision making for timing and method of operation and by improving operative methods.

식도암의 수술에서 transhiatal 술식의 역할 (The Role of Transhiatal Operation at Esophageal Carcinoma Operation)

  • Background : Controversy exists whether patients with esophageal carcinoma are best managed with classical Ivor Lewis esophagectomy(ILO) as combined thoracic and abdominal approach or transhiatal esophagectomy(THO). The THO approach is known to be superior with respect to operative time, morbidity and mortality, and length of stay, especially at poor pulmonary function patient, but may represent an inferior cancer operation due to inadequate mediastinal clearance compared with ILO. Accordingly, we estimated the THO role at esophageal cancer to compare each operative approach. Material and Method : From January 2002 to December 2007, we performed a retrospective review of all esophagectomies performed at Keimyung University Dongsan Medical Center; 36 underwent THO, and 11 underwent ILO. Result : There were all men and squamous cell carcinoma but 1 woman at ILO group, 2 women at THO group. There were no significant differences between THO and ILO with age, sex, location of tumor, mean tumor length. There were significant differences at preoperative pulmonary function test(In ILO group, average FEV1 is $2.65{\pm}0.6\;L/min$ and iIn THO group, average FEV1 is $2.07{\pm}0.7\;L/min$). The amount of blood transfusion, hospital stay, leak rates and respiratory complication, hospital mortality rate were not significantly different. Conclusion : There was no significant difference in the post-operative complication, hospital mortality rate, long-term survival of patients of both operative method. THO method had lower mobidity and mortality at poor pulmonary function patient than ILO method. Hence, THO is a valid alternative to ILO for patients with poor general condition or expected post-operative respiratory complication.

일개 지방사립대학병원에서의 중증외상팀 운영경험 (Experience with Operating a Trauma Team at a Local Private University Hospital)

  • Purpose: This hospital has operated a trauma system of the inclusive trauma system under the sponsorship of this hospital and with financial support from the government from 2011, and it has been designated as a specialized trauma center (candidate) since November 2008. Therefore, this emergency medical center evaluated the influence of the inclusive trauma system on the course of healing and on the results for trauma patients within the region. Methods: The medical records of all patients who were registered as trauma patients from among those who visited the emergency medical center of this hospital from April 2009 to May 2012 were retrospectively reviewed. The monthly and the annual averages of important indices, such as the time in the emergency room and preventable mortalities, were calculated, and patterns of change were sought. The preventable mortality rate was calculated by using the Trauma Injury Severity Score (TRISS) for each patient. Results: The total number of patients registered from April 2009 to May 2012 was 601, and male patients accounted for a larger proportion(432 males(71.88%) vs. 169 females(28.12%)). Their average age was 46.2 years, the average Revised Trauma Score (RTS) was 5.74 points, and the average Injury Severity Score (ISS) was 26.99 points. The preventable mortality rate during the entire period, which was calculated using the TRISS, appeared lower than the preventable mortality rates reported in past studies in the Republic of Korea. Conclusion: These results for the operation of a new trauma system are limited in that they are only for a local private university hospital. However, results show greater changes and developments in and out of the hospital due to multilateral endeavors by the trauma team and the hospital. These endeavors include increased communications among the departments and development of a complementary patient registration system.

급성심근경색증 환자의 진료 질 평가를 위한 병원별 사망률 예측 모형 개발 (Development of a Model for Comparing Risk-adjusted Mortality Rates of Acute Myocardial Infarction Patients)

  • Objectives: To develop a model that predicts a death probability of acute myocardial infarction(AMI) patient, and to evaluate a performance of hospital services using the developed model. Methods: Medical records of 861 AMI patients in 7 general hospitals during 1996 and 1997 were reviewed by two trained nurses. Variables studied were risk factors which were measured in terms of severity measures. A risk model was developed by using the logistic regression, and its performance was evaluated using cross-validation and bootstrap techniques. The statistical prediction capability of the model was assessed by using c-statistic, $R^2$ as well as Hosmer-Lemeshow statistic. The model performance was also evaluated using severity-adjusted mortalities of hospitals. Results: Variables included in the model building are age, sex, ejection fraction, systolic BP, congestive heart failure at admission, cardiac arrest, EKG ischemia, arrhythmia, left anterior descending artery occlusion, verbal response within 48 hours after admission, acute neurological change within 48 hours after admission, and 3 interaction terms. The c statistics and $R^2$ were 0.887 and 0.2676. The Hosmer-Lemeshow statistic was 6.3355 (p-value=0.6067). Among 7 hospitals evaluated by the model, two hospitals showed significantly higher mortality rates, while other two hospitals had significantly lower mortality rates, than the average mortality rate of all hospitals. The remaining hospitals did not show any significant difference. Conclusion: The comparison of the qualities of hospital service using risk-adjusted mortality rates indicated significant difference among them. We therefore conclude that risk-adjusted mortality rate of AMI patients can be used as an indicator for evaluating hospital performance in Korea.

중환자실 환자의 사망 관련 요인에 관한 연구 - APACHE III 도구를 중심으로 - (A Study About the Factors Concerned with Death of ICU patients by the APACHE III tool)

  • Using the APCHE III tool, this study was about the factors related to the death of ICU-patients. From 1999. 12. 1 to 2000. 9. 30, the 284 patients admitted to ICU at P university who were over 15 years of age were selected for the subjets. The data was analyzed through SPSS WIN program for frequency, percentile, $x^2$-test, t-test and logistic regression. The results are summarized as follows: 1) Of the 284 patients, 88died. The mortality is 31.0 percent. The average APACHE III point was $48.62{\pm}32.32$. The average point of non-survivors was higher than that of survivors. 2) There are the significant difference between APACHE III marks and mortality. The mortality rate were over 50 percent 60 points of the mark. When the marks were over 100 points, the mortality were over 90 percent. Below 40 points, the mortality was below 10 percent. Among the variables in the APACHE III, the most significant variables in explaining death were neurologic abnormalities, pulse, $PaO_2$/$AaDO_2$, creatinine, sodium, glucose, chronic health state and age. According to the variables, the models explained the 42.43 percent of the variance in patient's death. In conclusion, the APACHE III tool can be used to predict the progress of ICU patients, and can also be used for the selection of patients for ICU admission/discharge criteria.

유방암 환자 사망의 역학적 특성과 건강증진 방안 : 국가 암등록 자료를 이용하여 (Epidemiologic Characteristics of Death in Breast Cancer Patients and Health Promotion Plans : Using Korean Cancer Registry data)

  • Objectives: The purpose of this study was to identify the major influencing factors of breast cancer death and to suggest policy measures to promote the health of breast cancer patients. Methods: The method of this study performed statistical analysis by applying weights to 2,300 cases of breast cancer registration statistics in Korea collected in 2018 due to the relatively small number of mortality data compared to survival. Statistical processing of the collected data was analyzed using SPSS 26.0. Results: The epidemiologic characteristics of death in breast cancer patients were 31.8% in those aged 70 years or older, and the mortality rate was 5.25 times higher in patients aged 70 years or older than those aged 39 years or younger. The anatomical site code was 36.4% in C50.4~C50.6, and the mortality rate was 1.82 times higher in C50.4~C50.6 than in C50.0~C50.1. The tumor size was 40.4% and larger than 4cm, and the mortality rate was 4.53 times higher in tumors larger than 4cm than those smaller than 1cm. The degree of differentiation was 13.9% in the poorly differentiated group, and the mortality rate was 4.38 times higher in the poorly differentiated group than in the highly differentiated group. In the hormone receptor test, non-triple negative cases were 59.6%, and the mortality rate was 0.57 times lower in non-triple negative cases than in triple negative cases. As for lymph node involvement, the presence or absence of lymph node involvement was 78.8%, and the mortality rate with lymph node involvement was 1.36 times higher than that without lymph node involvement. The survival period of 13 to 24 months was the highest at 26.5%, and the average survival period was 25.68 months (±14.830). Conclusion: A policy to advance the timing of national health examinations for early detection of breast cancer is necessary. In addition, a bill for the mandatory placement of health educators in medical institutions for patients with special diseases such as breast cancer should be prepared.

한국인의 생명표 작성에 관한 연구 (A Study of Life Table of Korean People : Based on 1986 Data)

  • In recent years the life expectancy of the Korean people has been increasing appreciably because of the improvement in living conditions and public health facilities as well. However, there exist considerable constraints in the measurement of Korea's mortality rate. In elaboration, there are quite a few persons who do not submit birth and death reports, thereby lowering the reliability of statistical data. The 1978-1979 life table of the Korean people is still considered the latest one. Nevertheless, a decade has passed since its publication, and the need for a better life table reflecting realities is inc-reasingly growing. Capitalizing on the annual report of demographic statistics and demographic sample survey data m 1988, I have worked out the 1986 tentative life table of the Korean people and found out the following : 1. The male life expectancy in the 1978-1979 life table of the Korean people is 62.7 years old, and that of the female sex is 69.1 years old whereas the average life expectancy of the male sex in the 1986 life table stands at 66.3 years old, and that of the female sex is 74.5 years old. The average life expectancy is up by 3.5 years old for the male sex and also, up 5.4 years old for the female sex, res-pe ctively. 2. A gap between the male and female life expectancies in the 1978-1979 life table was 6.4 years old, while that of the 1986 life table was 8.2 years old. It means the female life expectancy has increased substantially. 3. The infant mortality rate has decreased, compared with the 1978-1979 level, yet it is still above those of Japan and Taiwan. 4. The mortality rate of the middle - aged men in the forties remains high, as was the case in the 1978-1979 period.

종격동 종양 및 낭포 (Mediastinal tumors and cysts)

  • For the purpose of evaluation of clinical characteristics and histopathological properties in mediastinal tumors, 130 patients with mediastinal tumors treated during the period from 1958 to 1982 were reviewed. Cancers of unidentified primary site of origin, nonneoplastic lesions and the cases with clinical diagnosis only were excluded from this report. There were 69 males and 61 females, and their ages ranged from 2 months to 66 years, with the average age of 31.3 years. 19.2% of patients were younger than 15 years of age. The most frequently encountered tumors were teratodermoids, followed by neurogenic tumors, thymic tumors, benign cysts, malignant lymphomas and miscellaneous tumors in order of decreasing frequency. 75.4% of the mediastinal tumors were benign and 24.6% were malignant. 16.6% of the patients were asymptomatic at admission. There were 2 postoperative deaths with 1.7% of operative mortality rate, and 2 other deaths who were not operated upon, showing overall hospital mortality rate of 3.1%. Postoperative follow up was possible in 84 cases, and there were 4 late deaths.
