Factors Associated with the Middle-aged or the Old-aged Koreans' Enrollment in Private Health Insurance (국내 중고령층의 민간의료보험 가입에 대한 영향 요인)

  • In this study we investigated the factors determining people's decision on whether to subscribe to private health insurance, on how many private health insurances they subscribe to and the average amount of monthly payment from subscribers of private health insurances. For analysis, logistic regression analysis and multiple linear regression analysis were conducted on the sample of 8,167 people using 2008 Korean Longitudinal Study of Ageing(KLoSA) data. From the analysis, whether to enroll in private health insurance is found to be greatly influenced by population and socioeconomic factors as well as regular exercise, smoking, cognitive function scores, subjective health status, hospitalization, the number of outpatient services, free primary health screenings benefits. We also found that number of private health insurances purchased is affected by age, household income, subjective health status, drinking, free primary health screenings benefits and that the average amount of monthly payment for private health insurances purchased is influenced by age, marriage status, economic activities status, subjective sense of hierarchy, household income, drinking, hospitalization. This study is expected to contribute to show the healthy role of private health insurance so that the desirable direction in expansion of health security policy in Korea can be explored further.

Influence of dental services on satisfaction of old prosthetic patients (노인구강보철환자에게 미치는 치과 의료서비스 요인)

  • The main objective of this study is to investigate the improvement of the quality of dental health based on the dental service satisfaction factors of the old dental prosthesis patients of more than 60 years of age who use the dental institutes including dental hospital, clinic, and public health center and to identify the interest in the dental prosthesis as changing paint. Across the country through a questionnaire survey of a question and answer type conducted by the trained dental hygienists or dentist's. Major results of the empirical analysis are as follows. 1. looking at variation of the variables of socio-demographic features of the respondents, the group of female, age distribution of 60-65, average monthly income of 0.5-1 million Won, and schooling background of college showed significant difference. And the prosthesis treatment in terms of the purpose of isiting a dental institute, and the dental clinic in terms of the type of visiting dental institute showed a remarkably significant difference. 2. looking at variation of variables of the general features of the respondents, a prosthesis satisfaction service in terms of age showed significant difference: a prosthesis satisfaction service, dental staffs' service and dental facilities' service in terms of average monthly income showed significant difference: the entire conditions except for the dental facilities' service in terms of schooling and general features showed significant difference: and in terms of the purpose of visiting dental institute, it showed no significant difference. In terms of a visiting dental institute, the prosthesis satisfaction service, dental facilities, overall level of satisfaction, result of treatment, word of mouth and revisit showed significant difference. 3. looking at variation of the variables of post-installation prosthesis satisfaction of the respondents, the prosthesis satisfaction service, dentist's dental service, staffs' service, dental facilities' service and revisit in terms of the prosthesis type: and the prosthesis satisfaction service, word of mouth, revisit and overall level of satisfaction from the perspective of serious concern showed significant difference. The prosthesis satisfaction service and dentist's service in terms of the solving the economic burden: and the entire variables in terms of dissatisfaction elements showed a similar level of significant difference 4. in regard to the satisfaction of dental service and the change of the recognition of prosthesis patients, it was revealed that the level of dental prosthesis satisfaction of the respondents was closely related to the dentist's service, staffs' service, and dental facilities' service. Finally, looking at the influence of the dental service on the treatment result, satisfaction, word of mouth, revisit and the overall satisfaction level, it was revealed that they had a great impact on the prosthesis satisfaction service.

Epidemiology, Incidence and Mortality of Bladder Cancer and their Relationship with the Development Index in the World

  • Background: Bladder cancer is an international public health problem. It is the ninth most common cancer and the fourteenth leading cause of death due to cancer worldwide. Given aging populations, the incidence of this cancer is rising. Information on the incidence and mortality of the disease, and their relationship with level of economic development is essential for better planning. The aim of the study was to investigate bladder cancer incidence and mortality rates, and their relationship with the the Human Development Index (HDI) in the world. Materials and Methods: Data were obtained from incidence and mortality rates presented by GLOBOCAN in 2012. Data on HDI and its components were extracted from the global bank site. The number and standardized incidence and mortality rates were reported by regions and the distribution of the disease were drawn in the world. For data analysis, the relationship between incidence and death rates, and HDI and its components was measured using correlation coefficients and SPSS software. The level of significance was set at 0.05. Results: In 2012, 429,793 bladder cancer cases and 165,084 bladder death cases occurred in the world. Five countries that had the highest age-standardized incidence were Belgium 17.5 per 100,000, Lebanon 16.6/100,000, Malta 15.8/100,000, Turkey 15.2/100,000, and Denmark 14.4/100,000. Five countries that had the highest age-standardized death rates were Turkey 6.6 per 100,000, Egypt 6.5/100,000, Iraq 6.3/100,000, Lebanon 6.3/100,000, and Mali 5.2/100,000. There was a positive linear relationship between the standardized incidence rate and HDI (r=0.653, P<0.001), so that there was a positive correlation between the standardized incidence rate with life expectancy at birth, average years of schooling, and the level of income per person of population. A positive linear relationship was also noted between the standardized mortality rate and HDI (r=0.308, P<0.001). There was a positive correlation between the standardized mortality rate with life expectancy at birth, average years of schooling, and the level of income per person of population. Conclusions: The incidence of bladder cancer in developed countries and parts of Africa was higher, while the highest mortality rate was observed in the countries of North Africa and the Middle East. The program for better treatment in developing countries to reduce mortality from the cancer and more detaiuled studies on the etiology of are essential.

The Effects of perceived health status, sleep, depression and pain on quality of life by Gender in Community-dwelling Older Adults (저소득층 재가노인의 지각된 건강상태, 수면, 우울, 통증이 삶의 질에 미치는 영향)

  • In this study, we have investigated the effect of perceived health status, sleep, depression and pain on quality of life by Gender in Community-dwelling Older Adults. A cross-sectional descriptive study was conducted with a survey of elderly aged 65 years or older that registered in the health care center located in J city from december 20, 2012 to february 15. Data were analyzed with descriptive statistics, t-test, ANOVA. Pearson correlation coefficients and multiple regression analysis using the SPSS Win 18.0 program. The result of perceived health status on average $9.89{\pm}2.21$ points, sleep $39.72{\pm}8.79$ points, depression, $7.13{\pm}7.54$ points, pain $5.37{\pm}1.90$ points and quality of life, the average was $7.61{\pm}2.25$ points. Perceived health status of low-income that perceived health status, sleep, and pain. explained 44.6% of variance in their quality of life. Based on the findings of the study, low-income elderly in home in order to improve the quality of life of the systematic development of intervention programs for health care and nursing needs to be applied.

A Study on the Characteristics of Cancer Patients with Radiotherapy and Social Support, Psychosocial Status (방사선치료를 받는 암환자의 특성과 사회적지지 및 심리상태에 관한 연구)

  • By studying on the characteristics of cancer patients who receive radiotherapy and the attitudes of medical social workers, this thesis aims to suggest of requirements for medical social work intervention. The study was conducted using SPSS 10.0 for Windows to analyze data taken from a survey involving 90 cancer patients receiving radiotherapy in 4 general hospitals in Daejeon city. The data were collected form Oct. 1st to 15th, 2003, and its analyses used averages, correlation, and regression. The results were summarized as follows ; 1. The average family income in 67.8% of the cases was less than 2,000,000 won, which is a lower income bracket. In the disease characteristics, fatigue ware the highest (2.78) in the side effects of radiotherapy, which most patients were receiving alongside other forms of treatment. 2. It was shown that cancer patients receiving radiotherapy receive a high level of psychological and social support from doctors and other medical staffs, and that they also received a high average (4.38) of individual care and encouragement from family members. 3. In the psychosocial status, the need for financial assistance in the form of a national aid program or an expansion of medical insurance was great, especially in the need for house keeping service and night nurses. The need for psychosocial counselling rose following rises in treatment side effects and depression. Information for cancer patients was especially needed in the areas of treatment plans, treatment costs, and side effects of radiotherapy. The need for information rose in accordance with an elongation of hospital treatment and an escalation of care from family members.

Health Inequity among Waged Workers by Employment Status (고용형태의 변화에 따른 건강불평등)

  • Objectives: The purpose of this study was to examine the relationship between the differences in employment status and self assessed health in Korea. Methods: We analyzed 4 year follow-up data generated by the Korean Labor and Income Panel Study(KLIPS), which was conducted on 1,207 men and 582 women who had undergone a change in employment status. The study subjects were placed into 1 of the following 4 groups based on their employment history; Non-precarious workers, Precarious to non-precarious workers, Non-precarious to precarious workers and Precarious workers. Logistic regression was then used to examine the relationship between the changes in employment status and self assessed health. Results: When males were considered, self assessed health was better among the precarious to non-precarious workers (OR 1.58, 95% CI=1.57-1.60) and the precarious workers (OR 1.29, 95% CI=1.28-1.30) than in the non-precarious workers, after adjusting for age, socioeconomic status (education level, occupational class, marital status, average equivalent household income and average number of hours worked per week), health behavior (smoking, drinking and exercise) and medical service access (regular medical examination, have chronic disease or hospitalized within 1 year). When female workers were considered, the precarious to non-precarious workers (OR 1.89, 95% CI=1.86-1.92), non-precarious to precarious workers(OR 1.24, 95% CI=1.23-1.26) and precarious workers (OR 1.27, 95% CI=1.25-1.28) all reported poorer health than the non-precarious workers after adjusting for the aforementioned factors. Conclusions: This study showed that changes in employment status were associated with differences in self assessed health among men and women. Specifically, the results of this study showed that a corresponding positive outcome based on self assessed health was greater for employees that changed from precarious to non-precarious jobs and for male employees with precarious jobs., whereas female employees with non-precarious jobs had higher self assessed health. However, additional longitudinal studies on the health effects of employment status should be conducted.

Health Behavior Motivators about Quit-Smoking in Some Outpatients of University Hospitals (일부 병원 내원 환자의 금연에 관한 건강행위 동기요소)

  • This study was aimed to investigate the motivators of smoking in 300 subjects using convenient extraction in one hospital. After informed consent, the survey that consisted of general characteristics, health behavior, stage of change, health behavior-related-perceived benefit, health behavior-related-perceived barrier, health behavior-related-family support, oral health behavior, subjective oral health and others was carried out. The smoking rate was higher in case of male (sex), high school graduates (education) and 2~3 million Korean won (KRW) (monthly income). The present smokers responded lower oral health behavior (p<0.001), lower health behavior-related-perceived benefit (p=0.021), higher health behavior- related-perceived barrier (p<0.001) and lower stage of change (p<0.001). The oral health related variables were not influenced by smoking. In logistic regression model, odds ratios in smoking were 0.378 in below average (health behavior-related-perceived barrier), 4.746 in below average (stage of change), 12.743 in male (sex) and 3.271 in 2~3 million KRW (monthly income/standard : over 3 million KRW). It was suggested that oral health related manpower should take account of not only smoking-related oral health but also motivators of smoking as to deliver effective no-smoking program.

Influence of Non-compliance of Treatment and Family Support on Depression in Diabetic Patients with Hypoglycemia (저혈당을 경험한 당뇨병 환자의 치료지시 불이행, 가족지지가 우울에 미치는 영향)

  • This study was to identify the effective factors on the Depression in Diabetic patients with hypoglycemia. This study used 119 questionnaires from Diabetic patients with hypoglycemia in B city to collect data from October 1, 2015 to March 15, 2016. Data were analyzed by SPSS 20.0 program using descriptive statistics, t-test, ANOVA, Scheffe's test. Pearson's correlation coefficient and Stepwise Multiple Regression. The Depression in Diabetic patients with hypoglycemia showed a positive correlation with Non-Compliance of Treatment(r=42, p<.001), a negative correlation with Family support(r=-.38, p<.001). According to the result of multiple regression analysis, the significant factors influencing depression were Family support(${\beta}=-.29$, p<.001), Drug therapy(${\beta}=.21$, p=.020), Dietetic therapy(${\beta}=.23$, p=.014), Monthly average income over 3,000,000won (${\beta}=-.20$, p=.014), Monthly average income 2,000-3,000 thousand(${\beta}=-.17$, p=.033), it was explained by 30.0%. Based on these findings, it is suggested to develop and apply a family support program to reduce the depression of diabetic patients with hypoglycemia.

Management Strategies for Apolygus spinolae(Hemiptera: Miridae) in Grapevine Yards (포도원에서 애무늬고리장님노린재(Apolygus spinolae(Meyer-Dur))방제체계)

  • Studies were conducted to investigate the control effects of several insecticides on Apolygus(=Lygocoris) spinolae(Meyer-Dur) and to develop its management strategies in grapevine yards. Of insecticides which were applied by farmers in commercial grapevine yards, chlorpyrifos, parathion, fenvalerate, and esfenvalerate$.$fenitrothion showed good control effects against A. spinolae. In designed field experiments of fenitrothion and chlorpyrifos for the evaluation of A. spinolae control, both insecticides showed high control efficacy of 94.8 and 91.6%, respectively. The damage of A. spinolae was examined in a grapevine yard, where five different combinations of application timing and number were treated with fenitrothion to assess yield losses (average cluster weight) by A. spinolae. There were no significant differences among average cluster weights of sprayed vines; 466.0 g in sprayed vines on 2 to 3 leaves unfolded stage, 460.7g in sprayed vines on flower separating stage, 465.0 g in sprayed vines each on 2 to 3 leaves unfolded and flowers separating stage, and 487.4 g in sprayed vines each on 2 to 3 leaves unfolded, flowers separating and fruit set stage. Weights of fruit clusters of unsprayed vines was significantly lower than those of sprayed vines. Based on yield and spray cost, chemical control on 2 to 3 leaves unfolded stage produced higher net income than each spray on 2 to 3 leaves unfolded and flower separating stage (i.e., two times of spray), and produced similar net income as each spray on 2 to 3 leaves unfolded, flowers separating and fruit set stage (i.e., three times of spray). Thus, the 2 to 3 leaves unfolded stage was economical spray timing to controlA. spinolae in grapevine yards. Also, management strategies for A. spinolae were discussed.

Estimating the Determinants of Loan Amount of Housing Mortgage : A Panel Data Model Approach (주택 담보 가계 대출액 결정요인 추정에 관한 패널 데이터 모형 연구)

  • Loan amount of housing mortgage is composed of various factors. This study paper studies focuses on estimating the determinants of a loan amount of housing mortgage. The region for analysis consist of seven groups, that is, metropolitan city (such as Busan, Daegu, Incheon, Gwangiu, Daejeon, Ulsan.) and Seoul. Analyzing period be formed over a 45 time points(2007. 01.~ 2010. 09). In this paper the dependent variable setting up loan amount of housing mortgage, explanatory(independent) variables are composed of the consumer price index, unemployment rate, average monthly household income per household, expenditure rate of health care, composite stock price index and overdue rate of household loans for commercial bank. In looking at the factors which determine loan amount of housing mortgage, evidence was produced supporting the hypothesis that there is a significant positive relationship between the consumer price index and unemployment rate. The study also produced evidence supporting the view that there is a significant negative relationship between expenditure rate of health care. The study found that average monthly household income per household, expenditure, composite stock price index and overdue rate of household loans for commercial bank were not significant variables. The implications of these findings are discussed for further research.
