Research Trends of Randomized Controlled Trial Studies on the Herbal Treatment of Avascular Necrosis of the Femoral Head Using China National Knowledge Infrastructure Database (대퇴골두 무혈성 괴사의 탕약 치료에 대한 무작위대조군 연구의 최신 경향 분석: 중국지망(中國知網) 검색을 중심으로)

  • Objectives This study was conducted to investigate a randomized controlled trial study on the clinical treatment of herbal medicines for avascular necrosis of the femoral head. Methods By March 2022, the China National Knowledge Infrastructure (CNKI) was reviewed about randomized controlled trial studies on the herbal treatment of avascular necrosis of the femoral head through. The publication year, evaluation criteria, treatment period, treatment method and effectiveness were analyzed for articles published from 2017 to 2022. Results 16 articles were selected and 14 kinds of herbal medicines were used. In most studies, Western medical treatments such as injection therapy, oral drug administration, traction therapy and mid-frequency electric therapy were treated alone in the control group, and a combination of Western medical treatment and herbal treatment was administered to the experimental group. Conclusions In China, there have been many studies on herbal treatment for avascular necrosis of the femoral head. In Korea, more clinical studies on the herbal treatment of avascular necrosis of the femoral head are needed. This study will be helpful for future research in Korean medicine on avascular necrosis of the femoral head.

Analysis of Domestic and Foreign Research Trends on the Korean Medical Treatment of Avascular Necrosis of the Femoral Head (대퇴골두 무혈성 괴사의 한의학적 치료에 대한 국내·외 연구동향 분석)

  • Objectives This paper is an analysis of domestic and foreign trends for the Korean medical treatment of avascular necrosis of the femoral head. Methods Domestic and foreign databases were searched and analyzed for Korean medical treatment interventions, research types, and treatment effects for avascular necrosis of the femoral head. Results There were 10 case reports in Korea, 2 case reports and 3 randomized controlled studies abroad. A total of 12 treatment interventions were used, and the types include acupuncture, herbal medicine, phamacopunture, electroacupuncture, cupping, physical therapy, chuna, acupotomy, moxibustion, warm acupuncture, laser acupuncture, and motion style treatment. As for the evaluation index, visual analogue scale, range of motion and harris hip score were the most common with 7 cases each. Conclusions Through this study, it was possible to understand the trends of domestic and foreign research on the treatment of avascular necrosis of the femoral head with Korean medicine. However, due to the small number of literature and no randomized control studies in Korea, treatment evidence is insufficient. It is considered that further research is needed in the future.

Three cases of affections of the hip treated with Korean Bee-Venom therapy (봉약침요법(蜂藥鍼療法)을 이용한 고관절병변(股關節病變) 치험(治驗) 증례보고(症例報告))

  • Objective : In this study, we have discovered that Korean Bee-Venom therapy is effective in treating various hip joint diseases. For example, A vacular Necrosis, Degenerative Arthritis and Rheumatoid Arthritis. Methods & Results: We have treated three cases of affections of the hip(eg. Degenerative arthritis, Rheumatoid arthritis and Avascular Necrosis of Femoral Head) with Korean Bee-Venom therapy and herbal acupuncture treatments. For acupuncture, Korean Bee-Venom therapy was observed for its pain relieving effects. Korean Bee-Venom therapy was treated on the following acupuncture points: GB29(Koryo), GB30(Hwando:環跳), ST36(Chok-samni:足三理). As the results of these treatments, a little change of inflammation around the hip joint on X-ray scan study was observed. but the degree of pain and range of motion were improved. in addition to general conditions of the patients. Conclusions: Based on the clinical results, Korean Bee-Venom therapy is believed to be effective for treating Avascular Necrosis of Femoral Head, Degenerative arthritis and Rheumatoid arthritis of hip joint. However, it is expected that further studies should be conducted to provide more objective information.

The Complication of the Talar Neck Fracture (거골 경부 골절의 합병증)

  • Purpose: The purpose of this study fracture. Materials and Methods: The clinical and radiological analysis were performed on 19 cases of the talus neck fractures who had been treated with conservative treatment or variable methods of operative treatment. Each cases followed up more than 1 year and 6 months from May 1989 to June 2001. The clinical results were analyzed according to the age, cause of injury, fracture type of Hawkins classification, associated soft tissue injury, method of treatments, complications, and Hawkins scoring system. Results: According to Hawkins classification, type I was 6 cases(32%), type II was 5 cases(26%), typeIII was 7 cases(37%), and typeIV was 1 case(5%). In all cases, complete bony union was obtained. According to the Hawkins scoring system, 8 cases(42%) were excellent and good. Avascular necrosis was 4 cases(25%). Traumatic arthritis was 10 cases which were occurred in type II, III and IV. An ankle fusion was 1 case. Conclusion: In talar neck fractures, non-displaced fracture treated by the cast immobilization and displaced fracture treated by early open reduction and internal fixation were expected good results. The complications were 77% of traumatic arthritis and 31% of avascular necrosis in type II, III and IV. We should preoperatively explain to the patient for high complication rates of traumatic osteoarthritis and avascular necrosis in the talus neck fractures.

The Results of Distal Chevron Osteotomy with Lateral Soft Tissue Release for Hallux Valgus Deformity (무지 외반증에서 외측 연부 조직 유리술을 함께 시행한 원위 갈매기형 절골술의 치료 결과)

  • Purpose: To evaluate the results and effectiveness of distal chevron osteotomy combined with lateral soft tissue release for the correction of hallux valgus deformity. Materials and Methods: 31 patients who underwent distal chevron osteotomy with lateral soft tissue release between July 2002 and June 2003, were analyzed in terms of hallux valgus angle, intermetatarsal angle, subluxation of sesamoid, AOFAS score and the occurrence of avascular necrosis of the first metatarsal head. Results: The mean amount of the correction of hallux valgus angle was $26.3^{\circ}$. The mean amount of the correction of intermetatarsal angle was $6.6^{\circ}$. The mean amount of the correction of sesamoid subluxation was 1.2 points. The mean improvement of AOFAS score was 25.8 points. Avascular necrosis of the first metatarsal head was not found in any cases. Conclusion: For correction of hallux valgus deformity, distal chevron osteotomy with lateral soft tissue release showed improvement of hallux valgus angle, sesamoid subluxation and AOFAS score with no evidence of avascular necrosis of the first metatarsal head.

Operative Treatment of Displaced Talar Neck Fracture (전위된 거골 경부 골절의 수술적 치료)

  • Purpose: To evaluate the results of open reduction and internal fixation for displaced talar neck fracture. Materials and Methods: Fourteen patients were followed for more than 1 year after open reduction and internal fixation of displaced talar neck fracture. The mean age was 41.5 years, and the mean follow-up period was 3.8 years. There were 10 type II, 3 type III and 1 type IV fractures per Hawkins. There were 3 open fractures. Clinically AOFAS ankle-hindfoot scale and Hawkins criteria were utilized. Radiologically quality of reduction, duration of bony union, avascular necrosis and posttraumatic arthritis were evaluated. Results: At last follow-up, AOFAS scale was mean 89.1 points. There were 8 excellent, 4 good, and 2 fair results according to Hawkins criteria. Radiologically anatomical reduction was obtained in 13 cases. Mean duration of bony union was 11.8 weeks. There were 3 avascular necrosis and 2 post-traumatic arthritis. There was no significant difference in the incidence of avascular necrosis between early operation group and surgically delayed group. Conclusion: Accurate open reduction and rigid internal fixation seem to be prerequisites for satisfactory treatment of a displaced talar neck fracture.

  • Objectives : The aim of this study is to report the improved case of bilateral avascular necrosis(AVN) of femur head treated with Korean medicine and Fascia Chuna Therapy Methods : The patient diagnosed with bilateral AVN is hospitalized at department of korean rehabilitation medicine, Samse hospital of korean medicine. The subject is treated by herbal medicine, acupuncture, SBV and hip joint gyoung-geon chuna. This study was measured by visual analogue scale(VAS) score, walking time without pain per 6 minutes and harris hip score(HHS). Results : The patient showed decreased VAS score, HHS and improved walking time without pain per 6 minutes after treated with korean medicine and hip joint Fascia Chuna Therapy. Conclusions : The patient showed reduced pain and positive effect on activities of daily living.

Core decompression for early-stage avascular necrosis of the humeral head: current concepts and techniques

  • Avascular necrosis (AVN) of the humeral head is a rare, yet detrimental complication. Left untreated, humeral head AVN frequently progresses to subchondral fracturing and articular collapse. Cases of late-stage humeral head AVN commonly require invasive procedures including humeral head resurfacing, hemiarthroplasty, and total shoulder arthroplasty (TSA) to improve clinical outcomes. However, in cases of early-stage AVN, core decompression of the humeral head is a viable and efficacious short-term treatment option for patients with pre-collapse AVN of the humeral head to improve clinical outcomes and prevent disease progression. Several techniques have been described, however, a percutaneous, arthroscopic-assisted technique may allow for accurate staging and concomitant treatment of intraarticular pathology during surgery, although further long-term clinical studies are necessary to assess its overall outcomes compared with standard techniques. Biologic adjunctive treatments, including synthetic bone grafting, autologous mesenchymal stem cell/bone marrow grafts, and bone allografts are viable options for reducing the progression of AVN to further collapse in the short term, although long-term follow-up with sufficient study power is lacking in current clinical studies. Further long-term outcome studies are required to determine the longevity of core decompression as a conservative measure for early-stage AVN of the humeral head.

  • Talar fracture and total dislocation, each known as one of rare injuries mainly caused by high-energy trauma, tend to cause avascular necrosis, post-traumatic arthritis, non-union, skin necrosis and infection because of weak blood supply. The authors have experienced and reports a total talus dislocation of 16 year old male associated with medial malleoalr fracture, who showed relatively good result by early reduction and operation.
