Prognosis of patients with postural tachycardia syndrome: a follow-up study

  • Background: Postural tachycardia syndrome (POTS) refers to the presence of orthostatic intolerance symptoms associated with a heart rate increment of greater than 30 beats/min, usually up to 120 beats/min, on head-up tilt test. Symptoms related to POTS are usually light-headedness, palpitations and tremor, but syncope can also occur. The pathophysiology of POTS is heterogeneous and its prognosis is uncertain. Methods: We prospectively evaluated patients who met the criteria for POTS, at baseline and follow-up, using composite autonomic symptom scores and autonomic tests to assess the autonomic function. We compared the clinical and autonomic test results between baseline and follow-up. Results: Sixty-eight patients met the inclusion criteria for POTS and forty-five patients were ultimately followed up for at least 1 year after baseline. The patients were predominantly young females (84%), with a mean age of 21 years. Most patients showed improved orthostatic symptoms and more than a quarter of patients had no longer met the criteria for POTS at follow-up. Conclusions: Most patients had a benign outcome in that they could resume their daily activities without great limitations. Our results demonstrated a relatively favorable prognosis in most patients with POTS.

HRV(Heart Rate Variability) 측정을 통한 희발월경 환자의 자율신경기능에 관한 연구 (A Study on the Autonomic Nerve Function of Oligomenorrheic Patients by Using HRV(Heart Rate Variability))

  • Purpose: Oligomenorrhea is clinically important because it can be progressed to amenorrhea. The purpose of this study is to analyse Autonomic Nerve Function of some oligomenorrheic patients by using HRV(Heart Rate Variability). Methods: We studied 35 patients visiting OO Oriental Hospital from 1st November 2006 to 31th May 2007. One woman who takes antidepressant is excluded. 17 oligomenorrheic group and 17 normal menstrual cycle group were compared with HRV. The SPSS 12.0 for windows was used to analyze the data and Mann Whitney U-test were used to verify the results. Results: Mean PR, LF norm, LF/HF ratio of oligimenorrheic group are higher than normal menstrual cycle group. But there is no statistically significance. SDNN, RMS-SD, ln TP, ln VLF, ln LF, ln HF, HF norm of oligimenorrheic group are lower than normal menstrual cycle group. But there is no statistically significance. Conclusion: It can be suggested that oligomenorrhea can be related to increased activities of sympathetic nerve and decreased activities of general autonomic nerve function. But, we need study with more subjects for settling this.

청소년의 사이버 여가활동과 자율신경계 기능의 관계 (The Relationship between Cyber Leisure Activity and Autonomic Function in High School Students)

  • Purpose: The purpose of this study was to explore the relationship between cyber leisure activity and autonomic function in high school students. Methods: The subjects of this study were 242 second graders at high school located in D city. The research design was descriptive correlation study. Data were collected from August 26 to September 17, 2010, using a cyber leisure questionnaire and heart rate variability (SA-6000, Medicore, Korea) for autonomic function. Results: Total time for the cyber leisure activity was $391.3{\pm}262.0$ (min/week). Time for internet usage and computer game was $234.5{\pm}140.1$ (min/week) and $156.8{\pm}164.3$ (min/week), respectively. There were significant positive correlation between total cyber leisure time and sympathetic activity (r=.247, $p$ <.001). Conclusion: Excessive cyber leisure activity could increase sympathetic activities in high school students. Thus, nursing intervention to reduce the total cyber leisure time will be helpful to improve their neurophysiological status.

근로자(勤勞者)의 양생수준(養生水準)과 스트레스와의 관계(關係) (A Study on the Relations between Yangseng Level and Stress in Industrial Workers)

  • This study aims at grasping the relativity between laborers' yangseng level and stress so as to present the groundwork for preventive oriental medicine approach in industrial health. The researcher prepared the questionnaire on general character, health-related yangseng level and chose 632 people to execute Autonomic balance test through heartbeat change. The collected material was analyzed by SPSS and tested by T-test, ANOVA. The general yangseng level average is 3.27, morality yangseng 3.91, sleep yangseng 3.39, mind yangseng 3.32, sex life yangseng 3.29, exercise yangseng 3.14, activities and rest yangseng 3.07, diet yangseng 2.95, seasonal yangseng 2.84. The highest is morality yangseng and seasonal yangseng is the lowest. In the aspect of yangseng level : Having a spouse, Non-smoking, Non-drinking, Regular exercising, Sufficient sleeping have higher yanseng levels. In Autonomic balance test, age from 35 to 40 has the highest autonomic activity and job period from 5 years to 10 years has the highest stress resistance. Non-smokers have high autonomic activity and stress resistance. Further, have low autonomic balance, stress index and fatigue strength. In the case of regular exercising person, he/she has high autonomic activity and stress resistance with low stress index. Regarding sleeping time, autonomic balance is the highest when he/she sleeps less than 6 hours. Stress resistance is the highest when he/she sleeps 7 hours and stress index is the highest when he/she sleeps 6 hours. After comparing the relativity between yangseng level autonomic balance test, only exercise yangseng has plus factor on autonomic activity and stress resistance and minus factor on stress ind ex and fatigue strength. Yangseng level has been affected by individual character and daily habits. Also stress is more influenced by daily habits than by individual character. We can find the relativity of exercise yangseng between yangseng and stress which suggests that we need to take measures to win over stress in individual health in the near future.

캡사이신 섭취와 UCP1 과 β3- adrenergic Receptor Polymorphism의 다양성에 대한 자율신경활동의 변화 (Alterations of Human Autonomic Nervous System Activity on Capsaicin Ingestion, and Variants of UCP1 and β3- -adrenergic Receptor Polymorphism)

  • 본 연구의 목적은 1) 캡사이신 섭취 (100 mg) 가 에너지 대사 조절자로서 열발생 교감신경활동과 관련된 자율신경활동을 향상시키는지, 2) 본 연구의 대상자들의 UCP1 과 ${\beta}_{3}$-AR 유전자 다양성이 자율신경활동에 영향을 주는지를 조사한 것이다. 8명의 대상자 (24.7 ${\pm}$ 1.8 세) 가 이 실험에 자발적으로 참여하였다. 심박수변이성 파워 스펙트랄 분석에 의해 평가된 심장자율신경활동과 에너지 대사는 총 90분간 매 30분마다 5분간 측정하였으며, 캡사이신 또는 위약 그룹은 무작위로 대상자에서 섭취되었다. 본 연구의 결과에서, 두 그룹간의 안정시 심박수에서는 유의한 차이가 없었다. 자율신경활동은 캡사이신 그룹에서 증가하였으나, 통계적으로 유의한 차이는 없었다. 총 대상자중, UCP1의 GG유전자 타입을 가진 대상자는 37.5% 였다. 그러나 캡사이신 그룹은 Test3에서 측정시 호흡 교환율이 유의하게 낮았다 (CAP: 0.80 ${\pm}$ 0.02 vs. CON: 0.85 ${\pm}$ 0.02, means ${\pm}$ SE, p<0.05). 이상의 결과로서, 캡사이신 (100 mg) 의 섭취는 지방분해를 향상시킴으로서 비만 과/또는 고지혈증을 가진 개개인에서 유익한 성분임을 시사할 수 있다. 또한 자율신경에 대한 비만과 관련된 UCP1 과/또는 ${\beta}_{3}$-AR 과 같은 유전자 다양성은 앞으로의 연구에 고려되어져야 할 것이다.

  • The discrete state theory on emotion postulated that there existed discrete emotions, such as happiness, anger, fear, disgust, and so forth. Many investigators who emphasized discreteness of emotions have suggested that discrete emotions entailed their specific activities in the autonomic nervous system. The purposes of this study were to develop a model of emotion-specific physiological response patterns. The study postulated six emotions (i.e., happiness, sadness, anger, disgust, fear, and surprise) as the basic discrete emotions. Thirty eight college students participated in the present study. Twelve slides (2 for each emotion category) were presented to the subjects in random order. During resting period of 30 s prior to the presentation of each slide, four presentation of each slide, four physiological measures (EEG, ECG, EDA, and respiration) were recorded to establish a baseline. The same physiological measures were recorded while each slide was being presented for 60 s (producing an emotional sate). Then, the subjects were asked to rate the degree of emotion induced by the slide on semantic differential scales. This procedure was repeated for every slide. Based upon the results, a model of emotion-specific physiological response patterns was developed: four emotion (fear, disgust, sadness, and anger) were classified according to the characteristics of EEG and autonomic responses. However, emotions of happiness and surprise were not distinguished by any combination of the physiological measures employed in this study, suggesting another appropriate measure should be adopted for differentiation.

근골격계 통증 환자에서 침 치료 후 심박수 변이도(Heart rate variability) 변화에 관한 연구 (Study on the Change of the Autonomic Nervous System by HRV Test in Musculoskeletal Pain Patients Treated with Acupuncture)

  • Objectives : This study was designed to evaluate the changes of autonomic nervous system by heart rate variability(HRV) test on the musculoskeletal pain patients treated with acupuncture on the Back-su points and the unfixed points, and untreated healthy adults. Methods : This clinical study have been carried out with 64 cases of musculoskeletal pain patients and 26 healthy adults. 35 patients were treated with acupunctureon on the Back-su points(Group I) and 26 patients were treated on the Unfixed points(GroupII), and two groups were tested by HRV at pre and post acupuncture treatment. 26 healthy adults(GroupIII) were tested twice by HRV without any stimulation. Results : In Group I, VAS was decresased $5.83{\pm}1.62$ to $5.17{\pm}1.89$ significantly(p<0.05) after acupuncture treatment, and LF/HF ratio was decreased significantly(p<0.05) as compared with healthy control Group. In GroupII, SDNN & RMSSD were increased singnificantly(p<0.05) as compared with other Groups. in Group I & II, MHRT was decreased significantly(p<0.05) as compared with healthy control Group. Conclusions : These results suggest that acupuncture treatment on the Back-su points was able to reduce the pain, suppress the sympathetic activities and enhance the parasympathetic activities in musculoskeletal patients.

원적외선 온열이 인체 자율신경기능에 미치는 영향에 관한 연구 (A Study on Human Autonomic Nervous System Activities by Far-Infrared Ray Hyperthermia)

  • 본 논문에서는 원적외선 온열효과가 자율 신경계에 미치는 영향을 심박변동(HRV)을 이용하여 평가 하였다. 측정된 심박변동신호를 FFT 의한 전력스펙트럼을 구함으로써 자율 신경계의 반응 특성을 고찰하고 Poincare plot 분석 방법을 이용하여 결과를 비교 분석하였다. 실험을 통해 심박변동 신호분석해보면 원적외선 온열에 노출되기 전에 비해 노출된 후에 매우 복잡한 모습임을 관찰 할 수 있었는데 이것은 자율신경계가 관여하는 심혈관계 제어 메커니즘이 원적외선 온열에 노출되기 전보다 노출된 후에 더욱 복잡함을 반영하는 것으로 분석된다.
