Medical Expenses for Trauma According to the Type of Medical Insurance (외상환자의 보험체계에 따른 진료비 분석)

  • Purpose: In Korea, the nation's medical expenses were 12 billion won in 2010. The medical costs for individuals can also be overwhelming. If a patient has sustained severe trauma, his/her insurance company responsible may pay only part of the medical bills. In Korean, there are diverse types of medical insurance, such as health insurance, automobile insurance, and industrial accident compensation insurance. And each insurance system has a different type of payment system. Our study will be essential for establishing the optimal medical expense payment system. Methods: From January to December 2011, we retrospectively reviewed the medical charts of 161 patients who were admitted to our hospital's emergency room after having undergone severe trauma. Of those 161 patients, 125 were retrospectively reviewed. Written permission was obtained from all of the patients. We analysed the demographic characteristics, clinical outcomes, data of the trauma, type of the patient's insurance, and the entire bill when the patient was discharged. Results: Seventy-one patients had health insurance, 48 automobile insurance, and six industrial accident compensation insurance. High-deductible insurance included health insurance and industrial accident compensation insurance, with the deductibles up to 20.6% and 19.1%, respectively. We attempted to analyze the cause of the high deductible rate. In patients with health insurance, medicines, primarily sedatives, pain killers, antibiotics, and fluids. comprised a large proportion. On the other hand, industrial accident compensation insurance deducted for a high-grade hospital room charge. Conclusion: We found that medical expenses were diverse according to the type of insurance. In particular, health insurance forced patients to pay too much of the medical expenses. Therefore, in Korea we should try to identify the insurance problems and improve the wage system.

The Socioeconomic Cost of Injuries in South Korea (우리나라 손상의 사회경제적 비용)

  • Objectives : This study was conducted to estimate the socioeconomic cost of injuries in South Korea. Methods : We matched claims data from national health insurance, automobile insurance and industrial accident compensation insurance(IACI), and mortality data obtained from the national statistical office from 2001 to 2003 by patients unique identifier. Socioeconomic cost included both direct cost and indirect cost: the direct cost was injury-related medical expenditure and the indirect cost included loss of productivity due to healthcare utilization and premature death. Results : The socioeconomic cost of injuries in Korea was approximately 1.9% of the GDP from 2001 to 2003. That is, 12.1 trillion KRW(Korean Won) in 2001, 12.3 trillion KRW in 2002, and 13.7 trillion KRW in 2003. In 2003, direct medical costs were 24.6%(3.4 trillion KRW), the costs for loss of productivity by healthcare utilization were 13.0%(1.8 trillion KRW), and the costs for loss of productivity by premature death were 62.4%(8.6 trillion KRW). Conclusions : In this study, the socioeconomic cost of injuries in Korea between 2001 and 2003 was estimated by using not only health insurance claims data, but also automobile insurance, IACI claims and mortality data. We conclude that social efforts are required to reduce the socioeconomic cost of injuries in Korea, which represented approximately 1.9% of the GDP for the time period specified.

  • Korean insurance companies recently started to understand that the customer satisfaction can be a critical role affecting sales performance and profit. A number of authors have reported and analyzed service quality factors in the auto insurance. The purpose of this research is to examine influence factors for the customer satisfaction in the auto insurance market. This study is assumed that customer satisfaction factors are composed of prices of service and corporate image as well as service quality factors. 249 questionnaires are gathered and analyzed from persons. The main findings of the empirical study are : First, the influence from prices of service and corporate image is higher than that from service quality factors, Second, repurchase of auto insurance highly correlated to recommendation intention.

The Analysis on Annual Utilization Patterns of Inpatients in Korean Medical Hospitals for the Past 10 years (10년간 일개 한의대 부속 한방병원에 입원한 환자에 대한 연도별 이용실태 분석 : 침구의학과를 중심으로)

  • Objectives : This study was designed to clarify population-social characteristics that influence the utilization patterns of hospitalized patients in a traditional korean hospital, thereby providing clinical data which would help further improvements of traditional korean medical service in particular the Acupuncture and Moxibustion. Methods : We investigated population-social characteristics and annual utilization patterns of all patients who were hospitalized for more than 24 hours in a Korean Medical Hospital from January 2005 to December 2014. The obtained data were recorded in the EMR chart and statistical analysis was performed using SPSS 21.0. Additionally, data from the patients admitted to the department of Acupuncture and Moxibustion were analyzed separately. Results : 1. All inpatients had a significant annual difference in age, gender, hospitalized department, and disease code annually but not in re-hospitalization number. Inpatients of the department of Acupuncture and Moxibustion also varied in their age, gender, and disease code annually, but not in re-hospitalization number. 2. Pearson correlation analysis on all inpatients showed that the mean days of hospital treatments had a negative correlation with all variables except medical care insurance. Total cost, cost per day per person and recuperation cost had a positive correlation with all variables except medical care insurance. There was no meaningful relationship between nonrecuperation cost and the variables. 3. Stepwise multiple regression analysis on all inpatients showed that the mean days of hospital treatments had a negative correlation with all variables except automobile insurance. The total hospitalization costs had a positive correlation with both general insurance and medical care insurance. Cost per day per person and recuperation cost had a positive correlation with the females. There was no meaningful relationship between non-recuperation cost and the variables. 4.   • In this paper we consider the asymptotic mean squared error of positive part James-Stein estimators. In the normal-normal example, estimators of the mean squared error of these estimators are provided which are correct asymptotically up to O($m^{-l}$). Asymptotic estimators of the MSE's which correct up to O($m^{-l}$) are also provide. Here, m denotes the number of strata. A simulation study is undertaken to evaluate the performance of these estimators.

Efficient simulation using saddlepoint approximation for aggregate losses with large frequencies

  • Aggregate claim amounts with a large claim frequency represent a major concern to automobile insurance companies. In this paper, we show that a new hybrid method to combine the analytical saddlepoint approximation and Monte Carlo simulation can be an efficient computational method. We provide numerical comparisons between the hybrid method and the usual Monte Carlo simulation.

A Comparative Study of Solvency Margin Regulation System : Focusing on Non-Life Insurance (지급여력제도의 국제적 정합성 연구 - 손해보험을 중심으로 -)

  • This paper aims to find a reasonable solvency margin system in non-life insurance industry and also to evaluate the appropriateness of the current solvency margin regulation system in Korea. The current solvency margin system in Korea, based on EU's solvency margin model, was introduced during the 1997 financial crisis. The solvency requirement is not based on non-life insurer's risk, but simply on written premiums. The current solvency margin for general insurance, such as fire, marine, and automobile insurance, is determined by the greater between a premium-based amount and a claim-based amount, where the premium-based solvency margin is calculated by multiplying the net written premium for the preceding year by the premium based solvency margin ratio. Also, the amount of solvency margin for long term insurance is set at 4% of the policy reserve of the long term insurance. Still, there exist many differences between the current solvency margin regulation system in Korea and EU's model. This paper focuses on the rationality of the solvency margin regulation system, and compares the current system in Korea with EU's model and the RBC(Risk Based Capital) system in U.S. and Japan. Finally, this paper suggests a more specific and reasonable solvency margin system to be developed in Korea.

A Study on Human Injury Characteristics and Vehicle Body Deformation with Car to Car Crash Test for Crash Compatability (${\cdot}$${\cdot}$대형 중고 승용차량에 대한 차 대 차 충돌시험을 통한 차체변형 및 인체상해 특성에 관한 연구)

  • Currently many safety assessment tests are conducted by crashing a vehicle against a rigid or deformable barrier. It is quite rational to evaluate crash performance of a vehicle in a barrier test in terms of vehicle stiffness and strength. However, there has been a lot of debate on whether barrier testing is a duplicate of real world crash collisions. One of the issues is car to car compatability. There are two essential subjects in compatability. One is partner-protection when crashing into another vehicle and the other is self-protection when struck by another vehicle. When considering a car to car frontal crash between a mini car and a large heavy car, it is necessary to evaluate human body stiffness of each vehicle. In this study, in order to evaluate the compatability of cars in car-to-car crashes, four tests were conducted. Test speed of each car is 48.3km/h, and the overlap of the mini and large car is $40\%$, and the overlap of the small cars is $100\%$. In all tests, only a drive dummy is used. The test results of the car to car crash test show that vehicle safety standard of mini car is not satisfied compared with large heavy car and HIC value of mini car is higher than large car. In this case observed that the relatively lower stiffness and weight of the mini car resulted in absorbing a large share of the total input energy of the system when crashed into the large heavy car.

Machine Learning-Based Prediction Technology for Medical Treatment Period of Automobile Insurance Accident Patients (머신러닝 기반의 자동차보험 사고 환자의 진료 기간 예측 기술)

  • In order to help reduce the medical expenses of patients with auto insurance accidents, this study predicted the treatment period, which is the most important factor in the medical expenses of patients in their 40s and 50s, and analyzed the factors affecting the treatment period. To this end, a mechine learning model using five algorithms such as Decision Tree was created, and its performance was compared and analyzed between models. There were three algorithms that showed good performance including Decison Tree, Gradient Boost, and XGBoost. In addition, as a result of analyzing the factors affecting the prediction of the treatment period, the type of hospital, the treatment area, age, and gender were found. Through these studies, easy research methods such as the use of AutoML were presented, and we hope that the results of this study will help policies to reduce medical expenses for automobile insurance accidents.

  • 'No-Automobile driving Day' is one of Special option of Automobile Insurance in Republic of Korea. It option is used a system for collect to automobile driving information. However, Its system gives discomfort to users of this system. A Discomfort of this system to users is the OBD device's(Personal Car Driving Check Device) installation. A frequent installation of this device provides this OBD device's breakage to the users of this system. In this paper, for a solve this problem, studied new system design to supplement the existing system using Smartphone and WPAN(Wireless Personal Area Network).
