변형 유방촬영술시 통증완화에 따른 요인 분석 (Analysis of Factors Related to Pain Relief on Modified Mammography)

  • 증가하는 유방암을 발견하고 치료하기 위한 기본검사인 유방 촬영은, 대부분 여성에게 암이나, 방사선 피폭으로 인한 불안을 주게 된다. 게다가 두려운 압박 통증을 주게 되므로, 불안과 통증의 정도를 파악하고, 통증을 완화할 방법을 실험 연구하였다. 임상 경험을 바탕으로 변형된 촬영 방법과 기본 방식의 촬영방법을 각각 비교 분석하여, 그 효과를 평가하는데 목적이 있다. 연구대상은 2017년 2월부터 3월까지 서울대병원운영 서울특별시보라매병원에 유방촬영을 목적으로 내원한, 30대부터 60대 이상까지의 여성 160명이었다. 변형된 유방촬영 방법 80명과, 기존의 유방촬영 방법 80명의 두 그룹으로 나누어 검사하고, 순차적으로 연구대상자 모르게 각각에게 설명하고 설문지를 작성하게 하였다. 연구방법은 첫 번째는 자동페달을 4회 이상 나누어 압박하고, 수동압박을 동시에 이용하며, 자세히 설명하는 태도로 시행하는 변형 방법이다. 두 번째는 기본 방법인 자동 페달 압박 방식만 사용하며, 압박 횟수를 1-3회 이내로 제한하고, 단순히 설명하는 태도로 촬영하였다. 통증 분포의 평균을 살펴보면 실험군(변형촬영방법)은 2.49이고, 대조군(기본촬영방법)은 평균 4.86으로 48.8%로 통증이 감소하는 효과를 나타냈다. 이처럼 변형된 방법으로 촬영한 경우, 실험군이 대조군보다 통증 지수가 낮게 나타났으며, 그 정도는 통계적으로 유의한 결과를 보여주었다 (p<0.001).

근로자의 트리클로로에틸렌 폭로 농도와 요중 총삼염화물 및 삼염화초산 농도와의 관계 (The Relationship Between Airborne Trichloroetnylene Concentrations and Total Trichloro-compounds and Trichloroacetic Acid in Urine)

  • This study was designed to establish the relationship between airborne trichloroethylene concentrations and total trichloro-compounds and trichloroacetic acid in urine samples of the trichloroethylene exposed workers, to examine if the biological screening value for trichloroacetic acid in urine set by the Ministry of Labor is appropriate, and to suggest a suitable biological screening value for total trichloro-compounds in urine. Seventy male workers from the cleaning, the packing, and the inspcetion areas were selected as the study group and eighty male office workers were chosen as the control group. The results were as follows: 1. The mean values of total trichloro-compounds and trichloroacetic acid in the exposed group ($48.1{\pm}1.5mg/{\ell}$, $19.7{\pm}1.9mg/{\ell}$) were significantly higher than those in the control group($4.3{\pm}1.5mg/{\ell}$, $1.8{\pm}1.2mg/{\ell}$). 2. The airborne tichloroethylene concentrations were significantly related with the concentrations of total trichloro-compounds in urine(r=0.8212) and the concentrations of trichloroacetic acid in urine(r=0.7216). 3. The average trichloroethylene concentrations in the manual cleaning plants and that in the automatic cleaning plants were 40.1 ppm and 7.7 ppm, respectively. The difference between two groups was statistically significant. 4. The geometric mean of 49.6 ppm trichloroethylene concentration was resulted in the $185.4mg/{\ell}$ total trichloro-compounds in urine, and the 50 ppm trichloroethylene concentration was expected to produce $170.4{\pm}28.5mg/{\ell}$ total trichloro-compounds in urine. 5. With the geometric mean of 49.6 ppm trichloroethylene concentration, the corresponding geometric mean concentration of trichloroacetic acid in urine was $74.7mg/{\ell}$. In conclusion, the level of personal exposure to trichloroethylene concentration was significantly correlated with the concentrations of total trichloro-compounds and trichloroacetic acid in urine. Current biological screening value of $75mg/{\ell}$ for trichloroacetic acid in urine set by the Ministry of Labor was thought to be appropriate, and a biological screening value for total trichloro-compounds in urine should be set in the range of $170.4{\pm}28.5mg/{\ell}$ as a reference value for trichoroethylene exposure.

상강화기의 임상평가 (Clinical Evaluation for System Performance of Image Intensifiers)

  • 상강화기는 형광 영상장치에서 영상의 특성을 결정하는 주된 요소이다. 본 연구는 두가지의 상강화기를 임상에서 평가하기 위하여 간단하고 비파괴적이며 적당한 시간내에 행할 수 있는 계의 활동 프로그램을 제시하였다. 이 프로그램은 크게 세 부분 즉, X-선 발생장치, 영상의 질, 조준으로 되어있다. X-선의 발생장치에 대하여 관전압의 정확도와 자동노출 조절기능을 비교하였다. 영상의 질을 위해서는 저대비 및 고대비 분해능 측정, 체격자 실험 등을 수행하였으며 이 실험에서는 비디오 모니터 및 순간영상을 필름화한 정량적인 분석을 분석하였다. 조준에 대해서는 상강화기의 유용한 영역의 직경과 상의 찌그러짐을 측정하고 정량적인 분석을 하였다. 이 실험들의 과정 및 결과들이 상강화기의 인수검사 및 계의 활동지수를 평가하는데 이용되기를 바란다.

지상형 탄약고의 내부온도 감소 방안 연구 (A Study on the Deduction of Internal Temperature of the Ground Magazine)

  • Among ammunitions which are stored in a war field, the lifespan of propelling gunpowder is affected by storage environment such as storage temperature, humidity, and exposure to sunlight. These are because Nitrocellulose (NC) which is the main component of propelling gunpowder can be naturally disassembled to unstable substances similar with other nitric acid ester. We can't prevent it fundamentally from being disassembled, but to restrain induction of automatic disassembly by decomposition product, a decomposition product ($NO_2$, $NO_3$, and $HNO_3$) and tranquillizer DPA (Diphenylamine), having high reactivity, are added into a propellant. For this, it will decrease the velocity of tranquillizer which can also affect the velocity of producing the decomposition product of NC, storage temperature or humidity of propelling gunpowder is higher, drop of tranquillizer content is much faster. Therefore, to extend storage lifespan of propelling gunpowder, it is really important to control storage temperature or humidity inside the magazine. Hereupon, according to the manufacture of small scale model magazine and the result of performing experiments and measuring variation of inside temperature (storage temperature), using roof types that have a steel slate structure of magazine among ground magazines, this research shows the differences in details.

디지털 유방촬영장치에서 다엽 조리개 적용으로 인한 화질과 선량의 변화 (Variation of Image Quality and Dose by Applying Multi-Leaf Collimator for Digital Mammography)

  • X선 조사야(X-ray beam field)는 X선관 앞에 부착된 선속제한기구(beam limiting device)인 조리개(collimator)에 의해 조절된다. 조리개는 1차 X선을 조절하여 환자의 피폭선량을 감소시키고 산란선량을 감소시켜 영상의 화질을 향상시키는 중요한 기능을 한다. 그러나 유방촬영장치의 조리개에 대해서 따로 규정되어 있지 않으며 상업화된 장치에서는 조사야가 사각형으로 조절되는 조리개를 사용하고 있다. 최근 디지털 유방촬영장치의 보급이 확대되고 있으나 film/screen방식에서부터 적용되던 사각형으로 조사야가 조절되는 조리개가 사용되고 있다. 따라서 본 연구에서는 디지털 유방촬영에서 조사야 조절 사용에 대한 실태조사 후 유방의 크기와 모양에 따라 조사야를 조절할 수 있는 다엽조리개를 제작 후 적용했을 때 영상의 화질변화와 평균유선선량, 면적선량 값의 변화를 알아보았다. 사각형으로 조사야가 조절되는 조리개와 비교하여 다엽 조리개를 적용한 영상에서 통계적으로 유의한 차이가 없었으며 DAP값 또한 50.72% 감소되었다. 따라서 디지털 유방촬영장치에서 조사야를 최적화하기 위한 자동조사야조절장치 개발이 환자 피폭선량 감소에 도움이 될 것으로 판단된다.

방사선촬영 보조기구를 이용한 어린이 흉부 엑스선 검사에 관한 연구 (A Study on Chest X-ray Using Ancillary Device for Child Radiography)

  • In this study, We developed a Ancillary device for child radiography for X-ray of children under 5 years old and verified its effectiveness. Chest X-rays of children younger than 5 years of age were performed by Supine method at the position of Table detector, Short - Source to Image Receptor Distance(SID). Existing Supine and Short -SID imaging methods cause many problems, such as errors in image reading and excessive radiation exposure dose to patients, but the use of an Ancillary device for child radiography(ADCR) solves these problems. A total of 160 children were divided into the Upright group using ADCR and Supine group without ADCR. The chest X-ray image was visually evaluated by two radiologists with reference to the European Commission's List of Quality Criteria for Diagnostic Radiographic Images in Pediatrics. The total score of the qualitative evaluation was 5.15% higher in the chest upright method using ADCR than in the chest supine method without ADCR, and the chest upright method score was higher than that of the chest supine method in items 1 to 7. whether infants have deep inspiration or not, 4.87% higher for item 1, whether infants rotate or not and the degree of tilting, 0% higher for the item 2, the reproduction of image from just above apices of lungs to T12/L1, 0% for the item 3, reproduction of the vascular pattern in central 2/3 of the lungs, 6.92% higher for the item 4, reproduction of the trachea and the proximal bronchi, 12.9% higher for the item 5, visually sharp reproduction of the diaphragm and costo-phrenic angles, 10% higher for the item 6, reproduction of the spine and paraspinal structures and visualisation of the retrocardiac lung and the mediastinum, and 3.65% higher for the item 7. Items 2 and 3 showed no statistically significant differences(P > 0.05), and items 1, 4, 5, 6, and 7 showed statistically significant differences(P < 0.05). In conclusion, Upright method using ADCR in pediatric chest X-ray is considered as a good alternative to existing Supine method.

On the Breeding of “CSR18${\times}$CSR19”- A Robust Bivoltine Hybrid of Silkworm, Bombyx mori L. for the Tropics

  • Earlier breeding experiments undertaken at Central Sericultural Research and Training Institute, Mysore, India since a decade had resulted in the development of many productive and qualitatively superior bivoltine hybrids. However, the hot climatic conditions of tropics prevailing particularly in summer are not conducive to rear these high yielding bivoltine hybrids. This has necessitated breeding of compatible bivoltine hybrids for year-round rearing. Accordingly, the Japanese hybrid, B2Ol ${\times}$ BCS12 which was found to be tolerant to high temperature was used as breeding resource material. Following hybridization and selection rearing of silkworms was taken up in SERICATRON (Environmental chamber with precise and automatic control facilities for uniform maintenance of temperature and humidity) at high temperature of $36{\times}1^{\circ}C$ and 85${\times}$5% RH in fifth instar and the control batches at $25{\times}1^{\circ}C$ and 65{\times}$5% RH. Directional selection was resorted to the batches reared at 36$\pm$1$^{\circ}C$ till F$_{5}$ keeping pupation rate as important selection criteria. From $F_{2}% onwards the experiment was modified in such a way as to conduct normal rearing every alternate generation to regain the lost vitality due to continuous exposure to high temperature and high humidity stress. At $F_{2}$ , Oval and dumb-bell cocoons were separated out and designated as CSR18 and CSR19, respectively. By utilizing these lines at $F_{12}$, the hybrid CSR18$\times$CSR19 was prepared and studied for the thermotolerance by subjecting to stress condition at high temperature of 36$\pm$1$^{\circ}C$ and 85$\pm$5% RH in fifth instar and the control batches at $25{\times}1^{\circ}C$ and 65${\times}$5% RH. The better performance of CSR18${\times}$CSR19 (survival > 88%) at $36{\times}1^{\circ}C$ clearly indicates the general superiority of CSR18${\times}$CSR19 with regard to high temperature tolerance over the productive hybrids and CSR18$\times$CSR19 can perform well in varied agro-climatic conditions of the tropics with optimum qualitative and quantitative characteristics.s.

유방조직등가 팬텀을 이용한 Screen-Film과 Digital Mammography에서의 평균 유선선량 (Comparison of Average Glandular Dose in Screen-Film and Digital Mammography Using Breast Tissue-Equivalent Phantom)

  • 유방촬영장치가 screen-film system에서 최근에는 빠른 속도로 digital system으로 교체되고 있는 추세이다. Digital system이 screen-film system에 비해 여러 가지 장점을 가지고 있다. 그 중의 하나가 digital detector가 screen-film보다 X선 흡수효율이 더 높으며, digital system 중에서도 direct digital detector가 indirect digital detector보다 흡수효율이 높다. 이에 입각하면 촬영 시 적은 선량을 주어도 되기 때문에 환자피폭선량이 감소한다. 그러나 임상 방사선사들이 느끼는 바는 달랐다. 따라서 이번 연구에서는 screen-film mammography system, computed mammography system, indirect digital mammography system, direct digital mammography system에서 glandularity가 30%, 50% 및 70%인 세 종류의 유방조직 등가팬텀을 이용하여 평균유선선량(AGD)을 측정하여 다음과 같은 결론을 얻었다. Screen-film보다 digital detector가 X선 흡수효율이 높지만, glandularity가 30%인 경우의 AGD는 오히려 SFM에서 가장 낮았고, 그다음 CM, IDM, DDM 순으로 증가되었다. 자동노출장치로 설정하였지만 두께나 glandularity에 따라 target/filter의 조합이 바뀌지 않고 Mo/Mo으로 계속 사용된 SFM에서는 AGD가 급격히 증가되었다. 따라서 높은 glandularity를 가진 두꺼운 팬텀을 위한 고에너지 촬영에서는 SFM에서 AGD값이 가장 높았고, 저에너지 촬영에서는 DDM에서 AGD값이 가장 높았다. 그러나 target/filter의 조합이 다양하게 바뀐 CM에서는 두께나 glandularity에 따른 AGD의 곡선이 가장 완만하게 증가하였으며, AGD값이 가장 낮았다. 이와 같이 두께와 glandularity가 증가함에 따라 target/filter의 조합이 Mo/Mo에서 Mo/Rh으로 더 증가하면 Rh/Rh으로 바뀌는 것이 AGD의 증가량이 적어 환자의 피폭선량 측면에서 바람직하다. 따라서 유방촬영에서 환자의 피폭선량을 평가하기 위해서는 검출효율 외에도 여러 가지 촬영 변수가 중요한 역할을 한다.

보건소(保健所)의 방사선과(放射線科) 업무(業務)에 관한 조사연구(調査硏究) (Survery on Business of the Departments of Radiology in Health Centers)

  • We serveyed the actual condition of business of the departments of radiology of 45 health conters (except 3) in the area of Seoul, Kyungki and Inchon from March, 1984 to November, 1984. The results are summarized as follows : 1. T.O. of the radiologic technologist is three persons in each health center of Seoul area, and one person in each one of Kyungki and Inchon area. P.O. is 2-5 persons in Seoul area, 1-2 persons in Kyungki or Inchon area. 2. The number of all the radiologic technologists employed now is 75 persons, and among all of them, when analized by position class 7th is 54.7%, class 8th 28.0%, class 9th is 13.3%, and class 6th is 2.7%, and by sex, female is 68.0%, male is 32.0%, by educational background, for the most part, junior college graduates come to 73.3%, by age group 60% of them is in their twenties, 16.0% is in their thirties and forties, 8.0% is in their fifties, and by career after certificate 60% have the career of 1-5 years, 13.3% have the one of 6-7 years or mor than 21 years, and 6.7% have the one of 11-15 years of 16-20 years. 3. All the diagnostic x-ray equipment being kept is 62, and among them flxing equipment is 71.0%, portable equipment is 29.0% and by rating of X-ray equipment, maximum tube current 100 mA is 46.8%, maximum KV 100KVP is 72.6%, the most part. 4. Photofluorographic camera and hood are equipped in every health center. While, as to the radiographic cassettes, $14{\times}14"$ cassetts are equipped in every health center, but cassettes of other sizes are in half of them. 5. Bucky's table is equipped in 11.9% health centers, the automatic processor is in 21.4%, the photofluorographic film changer is 9.5%, the grid is 73.8%, the protective apron is in 88.1%, and the protective glove is in 57.1% health centers. 6. The number of the people who got the x-ray examination for one year (by the year 1989) is the most, 1,000-6,000 in direct radiography of the chest, or 15,0001-45,000 in the health centers of Seoul area, 5,000-20,000 in Kyungki and Inchon area in photofluorography of the chest. Moreover, other radiographies are being taken extremely limitedly in all health centers. 7. In processing types of x-ray film, automatic processing is used in 9 health centers (21.4%), manual tank processing is in 30 (71.4%), and manual tray processing in 3 (7.2%). 8. As for collimation of x-ray exposure field, "continual using restricted by a subject size" has the most part, 78.6% "restricted using at every radiography" has 19%, and the case of "never considered" has 2.4% response. 9. As for the dosimeter used for radiation control, film badge (35.7%) and pocket dosimeter (26.2%) are used, and in 38.1% health centers the dosimeter is not equipped at all. Consideration of the previous radiation exposure is being done in only one health center. 10. Reading of radiographs is mainly depended on the radiologists electively (45.2%) or on the genral practitioners(45.2%).

Consecutive automated production of carbon-11 labeled radiopharmaceuticals by sharing 11C-methylation reagent from one 11C-synthetic module

  • Increasing clinical demand for carbon-11 labeled radiopharmaceuticals has triggered technological advances in fields of radiochemistry and automated modules. Even though carbon-11 has a short half-life ($t_{1/2}=20.4min$), the consecutive second production of carbon-11 labeled radiopharmaceutical in one $^{11}C$-synthetic module should be delayed at least over 4 h to avoid the high radiation exposure. We herein aimed to produce two different carbon-11 labeled radiopharmaceuticals ([$^{11}C$]PIB and [$^{11}C$]methionine) by sharing of [$^{11}C$]methylation source in one $^{11}C$-synthetic module. The synthesis of $^{11}C$-labeling reagents ($[^{11}C]CH_3I$ or $[^{11}C]CH_3OTf$) is fully automated using the commercial TRACERlab $FX_{C-pro}$ module and is readily adaptable to $^{11}C$-labeling reactor for [$^{11}C$]PIB as well as another $^{11}C$-labeling apparatus for [$^{11}C$]methionine via the three-way valve. After completing the [$^{11}C$]PIB production, the re-synthesized $[^{11}C]CH_3I$ was passed through the three-way valve connected the polyetheretherketone (PEEK) line and loaded into the C18 Sep-Pak cartridge including the methionine precursor. The labeled product [^${11}C$]methionine was purified by a simple cartridge separation and reformulated into saline. The radiochemical yield of [$^{11}C$]PIB and [$^{11}C$]methionine were $5.3{\pm}0.6%$ and $18.7{\pm}0.8%$ (n.d.c.), respectively, with over 97% of radiochemical purity. The specific activity of [$^{11}C$]PIB was over $110GBq/{\mu}mol$. Total production time of two radiopharmaceuticals needs about 2 h from $1^{st}$ beam irradiation including quality control tests. Final [$^{11}C$]PIB and [$^{11}C$]methionine were satisfied all quality control test standards.
