자폐아를 위한 표정 훈련 디지털 치료제 (Facial Expression Training Digital Therapeutics for Autistic Children)

  • 최근 자폐 스펙트럼 장애가 있는 변호사를 주인공으로 하는 TV 드라마가 많은 화제를 불러일으키면서 그 영향으로 자폐 스펙트럼 장애인이 겪는 어려움에 관한 관심도 높아졌다. 자폐 스펙트럼은 조기에 발견해 교육과 치료를 제대로 받으면 예후를 좋게 할 수 있어 그 치료제 개발이 절실히 요구되고 있다. 현재 자폐 스펙트럼 치료에 사용되는 약물은 부작용이 있는 경우가 많아, 그에 비해 부작용이 없고 대량 공급이 가능한 디지털 치료제가 주목받고 있다. 본 논문에서는 자폐 스펙트럼을 가진 영유아를 위한 감정 및 표정 학습을 제공하는 애플리케이션이자 디지털 치료제인 '아이모션(AEmotion)'을 소개한다. 본 시스템은 훈련에 대한 흥미를 높이고 손쉬운 적용 테스트를 위해, 스마트폰용 애플리케이션으로 개발되었다. 머신 러닝 기법을 활용하는 본 시스템은 표정 카드를 통해 감정을 학습하는 '감정 학습' 단계, 감정과 표정을 잘 이해했는지 확인하는 '감정 식별' 단계, 그리고 감정과 알맞은 표정을 짓는 '표정 훈련' 단계 등 총 3단계로 이루어져 있다. 이 시스템을 통해, 표정 인식과 감정처리가 익숙하지 않아 사회적 상호작용이 어려운 자폐 스펙트럼 장애 아동들의 표정 훈련 및 치료에 효과가 있을 것으로 기대한다.

한국인 자폐스펙트럼장애에서 Glutamate Receptor, Ionotropic, N-methyl-D-Aspartate 2B(GRIN2B) 유전자 다형성-가족기반연구 (Polymorphisms in Glutamate Receptor, Ionotropic, N-methyl-D-aspartate 2B(GRIN2B) Genes of Autism Spectrum Disorders in Korean Population : Family-based Association Study)

  • 목 적: 본 연구의 목적은 자폐스펙트럼장애를 가진 아동들과 그 부모로 이루어진 trio를 대상으로 이 장애와 NMDA 수용체 유전자, 그 가운데 GRIN2B 유전자와의 관련성을 규명하고자 하는 것이다. 방 법: 발달지연을 주소로 가천의대 길병원과 경상대학교병원 소아정신과 외래를 내원한 아동을 선별 대상으로 하였다. DSM-IV 진단체계를 이용하여 2명의 소아정신과 의사가 자폐 스펙트럼 장애로 진단한 아동이 일차적인 연구 대상으로 선별되었다. 선별된 아동과 부모들에게는 한국판 자폐증 진단 관찰 스케줄(Autism Diagnostic Observation Schedule, 이하 ADOS) 및 자폐증 진단 면담-개정판(Autism Diagnostic Interview-Revised, 이하 ADI-R)를 실시하였다. PCR-RFLP법을 이용, GRIN2B 유전자에서 모두 4개의 단일 염기 다형성을 분석하였다(rs7301328, rs1806201, rs1805247, rs1805502). 각각의 SNPs에 대한 allelic association 을 평가하기 위하여 TDT 방법이 시행되었으며, 이를 통해 자폐장애 아동이 부모로부터 후보유전자의 특정 alleles들을 유의하게 더 많이 전달받았는지의 여부를 관찰한 뒤 McNemar chi-square test(df=1)에 의거하여 분석하였다. 결 과: 1) 연구 대상군의 특성 : 총 126명의 자폐 스펙트럼장애 아동과 그들의 생물학적 부모가 최종 분석 대상에 포함되었다. 전체 대상자 중 109명(86.5%)이 남아였으며 여아는 17명(13.5%)으로, 남아 대 여아의 비율은 6.41:1이었다. 대상군의 진단 분포는 자폐장애 107명(85.1%), 달리 분류되지 않는 전반적 발달장애(PDD, NOS) 17명(13.5%), 아스퍼거 씨 장애(Asperger's disorder) 2명(1.6%)이었다. 대상군 아동의 평균 연령은 $71.9{\pm}31.6$개월(range : 26~185개월)이었으며 한국판 사회성숙도 검사로 측정된 평균 사회지수(Social Quotient)는 $61.2{\pm}20.6$(range : 23.1~126), 측정 가능한 아동들의 평균 지능은 $65.0{\pm}27.7$(range : 25~126)이었다. K-CARS 점수는 $31.5{\pm}5.4$(range 18.5~46)로 나타났다. 2) 유전자 분석 : 분석한 GRIN2B 유전자의 4개 SNPs 가운데 하나의 SNP(rs1805247)에서 의미 있는 allelic transmission의 차이를 보였다. 이 SNP에서 transmission ratio(transmitted alleles/non-transmitted alleles)는 A allele과 G allele에서 각각 2.03과 .49로, A allele이 G allele에 비해 부모로부터 환자군에게 더 빈번하게 전달(preferential transmission) 되었음이 확인되었다(TDT ${\chi}^2$=12.89, p=.0003). 이는 Bonferroni correction 후에도 여전히 유의미한 수준을 유지하였다(p=.0009). 기타 3개의 SNP(rs7301328, rs1806201, rs1805502) 들에서는 의미 있는 transmission의 차이가 나타나지 않았다(p<.05). 결 론: 본 연구에서 GRIN2B 유전자의 단일유전자 다형성과 자폐스펙트럼장애 사이에 유의한 연관성을 보였다. 이는 glutamate NMDA 2B수용체 유전자가 이 질환의 발생에 관여할 가능성을 시사하는 것이라 생각된다.

멜라토닌과 정신과 질환 (Melatonin in Psychiatric Disorders)

  • The secretion of melatonin exhibits a circadian rhythm entrained with the sleep-wake cycle. An alteration of this secretory rhythm has been found in various psychiatric disorders. This review summarizes the regulation of melatonin and its relationship to the circadian rhythm, major depressive disorder, bipolar disorder, seasonal affective disorder, Alzheimer's disease and autism. The review also looks at the effect of melatonin and melatonin agonist on sleep and symptoms of depression, bipolar disorder and seasonal affective disorder. In Alzheimer's disease, the circadian rhythm alterations are associated with the change of melatonin levels and melatonin receptors. It has been reported that melatonin and melatonin synthetic enzyme levels decrease in autism spectrum disorder.

  • In this paper, we propose and examine the feasibility of the robot-assisted behavioral intervention system so as to strengthen positive response of the children with autism spectrum disorder (ASD) for learning social skills. Based on well-known behavioral treatment protocols, the robot offers therapeutic training elements of eye contact and emotion reading respectively in child-robot interaction, and it subsequently accomplishes pre-allocated meaningful acts by estimating the level of children's reactivity from reliable recognition modules, as a coping strategy. Furthermore, for the purpose of labor saving and attracting children's interest, we implemented the robotic stimulation configuration with semi-autonomous actions capable of inducing intimacy and tension to children in instructional trials. From these configurations, by evaluating the ability of recognizing human activity as well as by showing improved reactivity for social training, we verified that the proposed system has some positive effects on social development, targeted for preschoolers who have a high functioning level.

The Levels of Vitamin D, Vitamin D Receptor, Homocysteine and Complex B Vitamin in Children with Autism Spectrum Disorders

  • Objective: Autism spectrum disorder (ASD) is a complex neurodevelopmental syndrome with an increasingly prevalent etiology, yet not fully understood. It has been thought that vitamin D, complex B vitamin levels and homocysteine are associated with environmental factors and are important in ASD. The aim of this study was to examine serum vitamin D, vitamin D receptor (VDR), homocysteine, vitamin B6, vitamin B12 and folate levels in ASD. Methods: In this study, serum vitamin D and VDR, homocysteine, vitamins B6, B12 and folate levels were determined in 60 patients with ASD (aged 3 to 12 years) and in 45 age-gender matched healthy controls. In addition, calcium, phosphorus and alkaline phosphatase, which are associated with vitamin D metabolism, were measured from serum in both groups. ASD severity was evaluted by the Childhood Autism Rating Scale (CARS). Results: Serum vitamin D and VDR were substantially reduced in patients with ASD in comparision to control group. However, homocysteine level was significantly higher and vitamin B6, vitamin B12 and folate were also reduced in patients with ASD. Total CARS score showed a positive association with homocysteine and a negative correlation with vitamins D,B6, B12, folate and VDR. Conclusion: This comprehensive study, which examines many parameters has shown that low serum levels of vitamins D, B6, B12, folate and VDR as well as high homocysteine are important in the etiopathogenesis of ASD. However, further studies are required to define the precise mechanism(s) of these parameters and their contributions to the etiology and treatment of ASD.

자폐범주성장애 아동의 나이에 따른 식행동 및 식품 기호도와 학부모의 영양교육 요구도 (Dietary Behavior and Food Preferences according to Age and the Parents' Nutrition Education Needs of Children with Autism Spectrum Disorder)

  • This study examined the eating habits and preferences of foods according to the age of children with autism spectrum disorders and identified the nutritional needs of parents to support their healthy eating habits and provide basic data for the development of educational programs. A survey of 100 parents of children with autism spectrum disorders was conducted on their children's snack intake, food preferences, and parents' nutrition education needs. As a result, there were four times more boys than girls, and the body mass index differed according to age (p<0.001). Most children ate snacks more than once a day, and parents found that they wanted to develop nutritious snacks that could replace high-protein, low-calorie meals with the minimal use of additives. For the food preference, there was a significant difference in grains and starches, vegetables/seafood and fruits, milk and dairy products (p<0.05), fats and sugars, and Korean fermented pastes (p<0.01) between the two age groups. Regarding whether nutrition education was required, many parents had a high demand for nutrition education (4.24±0.70), and the intention to participate was 3.62±1.09. The most important content to be dealt with in parent education was 'Inducing a change in bad eating habits and behaviors', which was the highest (63%), and mostly face-to-face education was preferred. In conclusion, young children require diet education, and older children need to focus on weight control and obesity.

시냅스 접착 단백질과 구조 단백질의 정신과적 의의 (Psychiatric Implication of Synaptic Adhesion Molecules and Scaffold Proteins)

  • Synaptic adhesion molecules mediate synapse formation, maturation and maintenance. These proteins are localized at synaptic sites in neuronal axons and dendrites. These proteins function as a bridge of synaptic cleft via interaction with another synaptic adhesion molecules in the opposite side. They can interact with scaffold proteins via intracellular domain and recruit many synaptic proteins, signaling proteins and synaptic vesicles. Scaffold proteins function as a platform in dendritic spines or axonal terminals. Recently, many genetic studies have revealed that synaptic adhesion molecules and scaffold proteins are important in neurodevelopmental disorders, psychotic disorders, mood disorders and anxiety disorders. In this review, fundamental mechanisms of synapse formation and maturation related with synaptic adhesion molecules and scaffold proteins are introduced and their psychiatric implications addressed.

자폐스펙트럼장애의 치료에 대한 한의 임상 가이드라인 개발을 위한 기초연구 -기존에 개발된 자폐스펙트럼장애 가이드라인을 중심으로- (A Basic Study for Development of Clinical Practice Guidelines of Korean Medicine in Autism Spectrum Disorder -Based on Pre-existing Clinical Practice Guidelines of Autism Specturm Disorder-)

  • Objectives The purpose of this study is to review pre-existing clinical practice guidelines for autism spectrum disorders, and refer those in developing a new practice guideline. Methods A total of 9 existing clinical practice guidelines for autism spectrum disorder developed from 2010 to 2016 were searched by Google scholar and Pubmed, and were reviewed those literatures in three parts: general, diagnosis & evaluation, and intervention. Results There were no consistency in the recommendation methods of 9 clinical care guidelines (such as the method of rating and recommendation intensity for diagnosis, evaluation, and treatment). However, in the diagnosis and evaluation section, frequently used evaluation and diagnostic tools are mentioned in most clinical practice guidelines, and the types of pharmacologic and non-pharmacological treatments that are mainly recommended in treatment are equally mentioned in most clinical practice guidelines could confirm. Conclusions 1. Some guideline recommendations are graded according to each criterion. Recommendations presented in various databases were based on systematic reviews or other literatures. The most utilized database were PsycINFO, CINAHL, Cochrane. 2. DSM-5 and ICD-10 were the most common used diagnostic criteria, and DSM-IV was used as a diagnostic standard in the guideline published before 2013. The tools used for diagnosis and evaluation were also varied. However, most recommended ones were ADI-R, ADOS-G, and DISCO. 3. Treatment was largely divided into pharmacological intervention and non-pharmacological intervention. In some guideline, the interventions were divided into pediatric and adult. Most of the pharmacological interventions were not recommended due to lack of evidence, but in cases in which specific symptoms were aimed, they recommended to seek professional help. 4. In addition to interventions, each guideline referred to supportive interventions that may be helpful in the daily life of patients with ASD, which may need to be addressed in future clinical guidelines.

The effect of game-based dual-task training for executive function and repetitive behaviors in patients with autism

  • Exergames are playing an important role in healthcare/rehabilitation. It has also been used to improve motivation among patients with reduced cognition. The purpose of this pilot study was to evaluate the feasibility of using augmented reality (AR) with game-based cognitive-motor training programs for executive function, restricted and repetitive behaviors (RRBs) in children with autism spectrum disorder. Sixteen children aged 6 -16 years were randomly allocated to the experimental group and control group. Outcome measures were performed before and after the intervention and included executive function, restricted and repetitive behavior. A satisfactory survey was conducted post-intervention. A statistically significant improvement was observed in working memory and cognitive flexibility in the experimental group (P<0.05). However, despite no statistical improvements in cognitive inhibition and four subscales of RRBs, promising changes were observed in all the subscales of the executive function and the behavioral outcomes. Parents appreciated the program and children enjoyed the interaction with the AR game-based training. The findings of this preliminary feasibility study showed that AR using Kinect v2 motion with a cognitive-motor game content can be used for children with autism. However, there is a need for conducting a large-scale study to evaluate his effectiveness on executive function and restricted and repetitive behaviors.

자폐스펙트럼장애아동을 대상으로 한 국내 로봇활용 융합연구동향 (Research Trends and Considerations in The Clinical Use of Robots for Children with Autism Spectrum Disorders)

  • 본 연구의 목적은 국내에서 이루어진 자폐스펙트럼장애아동을 대상으로 한 로봇 활용 융합연구동향을 살펴보는 것이다. 이를 위하여 최근에 장애 아동의 교육적, 치료적 중재에 연구되고 있는 로봇연구 동향을 살펴보고자 하였다. 연구방법으로, 자폐를 키워드로 2009년 이후에 발표된 논문을 검색하여 총 17편의 연구 논문을 최종 선정하였고, 활용로봇, 독립변인, 종속변인 유형, 연구방법을 바탕으로 하여 주제별 범주로 묶어 내용 분석하였다. 연구결과, 그 중 자폐아동을 위한 로봇개발은 발전이 빠른 분야 중 하나이며, 자폐스펙트럼장애아동의 중재에 유용한 것으로 나타났다. 연구를 통해 향후 로봇활용의 일반화를 촉진할 수 있는 적용 절차 및 단계 등에 대한 세분화된 접근이 필요함을 알 수 있었다.
