Study on the Antiinflammatory, Anticoagulative and Analgesic effects of Sambitang in the experimental animal model (삼비탕(三痺湯)의 항염(抗炎), 항응고(抗凝固) 및 진통효과(鎭痛效果)에 대(對)한 연구(硏究))

  • This study was designed to elucidate the antiinflammatory, cardiovascular, antithrombotic, and analgesic effect of Sambitang. The antiinflammatory effects was measured by the method of carrageenin induced edema, protein leakage test using CMC-pouch, and the effect of Sambitang on the cardiovascular system was observed by the change of flow rate of Ringer solution in the vascular system in the ear of rabbit, and the contraction and dilatation of rat tail artery. Death rate, platelet aggregation, plasma coagulation activity was observed for the measurement of the anticoagurative effect of Sambitang, and the analgesic effect was measured by the acetic acid method and hot plate method. The result was as follows: 1. Sambitang administration, edema and protein leakage was significantly decreased. 2. The drug increased the auricular blood flow in rabbit. 3. The drug relaxed the artery contraction by pretreated norepinephrine in rat. 4. The drug inhibited the death rate of mouse which was led to thromboembo- lism by serotonin and collagen. 5. The drug inhibited the platelet aggregation in rat. 6. The drug prolonged the prothrombin time and activated partial thromboplastin time on the test of plasma coagulation factor activity in rat, but was not valuable. 7. The slight anagesic effect of Sambitang extract was confirmed by the observation of writhing syndrome, paw licking time, and escape time.

Evaluation of Local Lymph Node Assay as an Alternative Method for Skin Sensitization Potential in Baltic Mice (Balb/c 마우스에서 Local Lymph Node Assay(LLNA)를 01용한 피부 감작성 시험 대체시험법 연구)

  • Allergic contact dermatitis (skin sensitization) may be caused by a wide variety of chemicals. A murine local lymph node assay (LLNA) has been developed as an alternative to guinea pig models for assessing the contact sensitization potential of chemical. This study was carried out to evaluate the skin sensitization potential for chemicals in Balb/c mice by LLNA. Contact allergen, dinitrochlorobenzene (DNCB), respiratory allergen, toluene diisocyanate (TDI) and a weak allergen, $\alpha$-hexlycinnamaldehyde (HCA) were wed as positive chemicals and irritant, sodium lauryl sulfate(SLS) also wed as a reference chemical in this study. The weights of lymph node in the mice treated with DNCB, TDI, and HCA were increased compared to vehicle control. There was a significant increase in lymph node weight of mice treated with high concentration of SLS compared to vehicle control. The stimulation index (SI) of Lymph node cell in the mice treated with DNCB, TDI, and HCA revealed over three-fold increase compared to vehicle control by $3^H$-thymidine uptake. All allergens correctly identified in this LLNA study wing Balb/c mice. These results suggest that LLNA wing Balb/c mice could be a useful method for screening the allergenic potential of chemicals. The expression of IL-2 mRNA was slightly increased in draining auricular lymph node cell of the mice treated with TDI and HCA by RT-PCR. However the IL-2 levels in DNCB and SLS of treated animals were not significantly changed.

A Case Study on a Visceral Fat Changes in Obese Patient Treated by the Oriental Medical Therapy Program (내장형 고도비만 환자에 대한 한방 체형관리 프로그램의 치료 증례보고)

  • Objectives : The purpose of this study was to investigate the clinical effect of oriental medical short term therapy on visceral fat obesity. In present study, we report 20-year-old visceral fat obesity woman who experienced oriental medical therapy program. Methods : The obese patient was treated from the 20th of June to the 3rd July. Herbal medicine, low calorie diet, electrolipolysis, auricular acupuncture, and aerobic exercise were done during the treatment period. Results : Her body weight decreased from 77.30kg to 71.15kg. The BMI decreased from $32.6kg/m^2$ to $29.5kg/m^2$. First of all, visceral fat tissue was effectively decreased after oriental obesity therapy evaluated by CT. Visceral fat tissue decreased from $123.44cm^2$ to $83.06cm^2$, subcutaneous fat tissue from $327.88cm^2$ to $323.09cm^2$. Short term oriental medical treatment could be effective in visceral fat obesity treatment. Conclusions : The patient with visceral fat obesity was improved using short term oriental medical treatment.

Considerations for the Management of Cryptotia Based on the Experience of 34 Patients

  • Background Cryptotia is a congenital ear deformity in which the upper pole appears buried beneath the mastoid skin. Cryptotia is a common auricular malformation among Asians. The aim of this paper is to examine the surgical techniques for and complications of 34 cryptotic patients. Methods Surgery was performed for 34 cryptotic deformities (January 2005 to January 2012). Twenty-two patients (64.7%) were classified as having type I cryptotia, and 12 patients (37.5%) type II cryptotia. Among the type I cryptotia patients, 8 patients had mild deformity and 14 severe deformity. Among the type II cryptotia patients, 10 patients had mild deformity and 2 severe deformity. Results The mild deformities were corrected via Z-plasty, V-Y plasty, full-thickness skin graft, and transposition flap, while the severe deformities were corrected via cartilage graft or Medporfor the spread of cartilage adhesion of antihelix. There were two cases of reinvagination in the autologous cartilage graft group. Implant exposure occurred with Medpor (two cases). There were two cases of hypertrophic scar on the previous surgical wound with Medpor. There were no complications in the 18 patients who had mild deformities. Conclusions The type I cryptotia patients had more severe deformities than the type II cryptotia patients. As most of the type II cryptotia patients had only mild deformities, their deformities were corrected without using autologous conchal cartilage graft or Medpor, except for two patients. Through more case analyses, researchers should make an effort to identify methods for recurrence and prevention of complication.

Comparison of Rectal and Infrared Thermometry for Obtaining Body Temperature of Gnotobiotic Piglets in Conventional Portable Germ Free Facility

  • Gnotobiotic piglets (n = 10) were hand-reared in conventional germ-free facilities. Piglet body temperatures were measured with rectal and non-contact infrared thermometry (NIFT) on the lower eyelid, auricular center and margin, parietal regions, axilla, central abdomen and dorsum, and the perianal region. Body temperature measurements at central abdomen, cranial dorsum, and perianal regions had NIFT values which had a significant linear relationship (p<0.0001) with rectal thermometry. The predicted equations of between-subject formulas were calculated as follows: rectal temperature, 28.07489+0.30372${\times}$central abdominal surface temperature; rectal temperature, 34.02799+0.15197${\times}$central dorsum surface temperature; and rectal temperature, 33.87937+0.15676${\times}$perianal temperature. These results suggested that NIFT could serve as a valid alternative to rectal thermometry in a portable germ-free facility without disturbing experimental animals. The development of a NIFT body temperature evaluation that does not require animal restraint is clinically advantageous, particularly in gnotobiotic piglets, and would be significantly less stressful for experimental procedures in germ-free facilities.

Effects of Non-pharmacological Interventions on Primary Insomnia in Adults Aged 55 and Above: A Meta-analysis (수면장애가 있는 중장년 환자에게 적용한 비약물적 중재의 효과: 메타분석)

  • Purpose: This study was performed to evaluate the effects of non-pharmacological interventions on sleep disturbance amongst adults aged 55 and above. Methods: PubMed, Cochrane Library, EMBASE, CINAHL and several Korean databases were searched. The main search strategy combined terms including non-pharmacological interventions and presence of insomnia. Non-pharmacological interventions included cognitive behavioral therapy, auricular acupuncture, aromatherapy, and emotional freedom techniques. Methodological quality was assessed using Cochrane's Risk of Bias for randomized studies and Risk of Bias Assessment tool for non randomized studies. Data were analyzed by the RevMan 5.3 program of Cochrane Library. Results: Sixteen clinical trials met the inclusion criteria with a total of 962 participants. Non-pharmacological interventions was conducted for a mean of 5.5 weeks, 7.7 sessions, and an average of 70 minutes per session. The effects of non-pharmacological interventions on sleep quality (ES=-1.18), sleep efficiency (ES=-1.14), sleep onset latency (ES=-0.88), awakening time after sleep onset (ES=-0.87), and sleep belief (ES=-0.71) were significant, and their effect sizes were ranged from moderate to large. However, the effects on total sleep time and insomnia severity were not significant. Conclusion: The findings of the current study suggest that non-pharmacological interventions have a positive impact on attitudes and beliefs about sleep, sleep quality, sleep duration, and sleep efficiency. Therefore, the findings of the study provide an evidence to incorporate various non-pharmacological interventions into nursing practice to improve both sleep quality and quantity in patients with insomnia.

Review of Korean Clinical Studies on Treatment for Childhood Obesity (소아비만의 치료에 대한 국내 임상연구 동향)

  • Objectives The purpose of this study is to investigate Korean clinical studies on treatment of childhood obesity and to propose for better treatment options for childhood obesity. Methods Based on RISS, KISS, OASIS, KMbase with the keyword 'childhood obesity', 'child obesity', 'obese child', 'overweight child', total of 21 randomized controlled trials (RCTs), 11 single clinical studies, 3 chart reviews and 3 case reports have been found, and were analyzed. Results and Conclusion 1. Some study subjects were classified based on their sex - only men or women in a group or sometimes both. Another way of classify the study subjects were based on their age - preschooler, primary school students, middle school students, and high school students. The diagnostic criteria of the subjects were BMI (body mass index), Body fat percentage, Obesity index, Weight and unknown criteria. 2. The treatments used in 38 studies were exercise alone, education alone, both exercise and education or herbal therapy. The 7 studies that used herbal therapy as part of their study intervention had a study group with herbal intervention only and the other 2 study group with herbal treatment in addition to exercise and education. 3. 9 studies included herb medicines, electroacupuncture, auricular acupuncture, cupping, aroma massage, infrared light, and abdomen pad. Herb medications used in 5 studies included Chegameuiin-tang 2 kinds, Sobieum, Biman-tnag, and I-razin. 4. The methods of assessment used in 38 studies were classified by 7 categories and anthropometry parameters which is the basic methods are used in all studies 5. Almost studies (29 studies in all 38 studies) were using anthropometric parameters results on significantly effectiveness of childhood obesity. 6. More studies are needed to prove true effectives from various treatments, especially herbal therapy for childhood obesity.

Effects of the Acupuncture on the Smoking-cessation Clinic Program of Public Health Center in Urban Area: Retrospective Case Control Study (금연클리닉 프로그램을 통한 금연침 효과의 후향 연구)

  • Objectives : This study identified the effects of the acupuncture associated with successful cessation at a smoking-cessation clinic of public health center in an urban area. Methods : The subjects of this study were 1121 patients to the smoking cessation clinic of the Anjung Public Healthcare Center in Pyeongtaek, from June 1, 2007 to December 31, 2008. The 123 patients of them were treated by the auricular acupuncture. The experimental group were treated by the acupuncture, and the control only participated in the smoking-cessation clinic program. The data were analysed by SPSS(version 17.0). Chi-square analysis were conducted to see the different of effects between experimental and control group. Also, to find out the factors that affect the success of smoking cessation, logistic regression and Fisher's exact test were done. Results & Conculusions : 1. As the result of the analysis, it showed that 73.2% of the experimental were able to quit smoking for 4 weeks and 6months. 2. Logistic regression and Fisher's exact test showed that number of Cigarette per day, Nicotine Dependence, CO figure(p=.004<.01), and Number of Acupuncture(p=.000<.001) were the factors that affect the successful smoking cessation. 3. The smoking cessation were more likely to success as patients have lower CO figure(B=-.074), more number of the acupuncture(B=1.718). The more number of acupuncture increased 1 time, the more smoking cessation success did 5.576 times(e$^{(1.718{\times}x)$=e$^{(1.718{\times}1)$ = 5.576). 4. As the result of the analysis between experimental and control group, the success rate have the significance difference(p=.000<.001). 73.2% of the experimental were able to quit smoking, but 45.1% of the control did.

Slot Correction by the Frechet Flaps in Hair Restoration Surgery (두발재건 환자에서 Frechet 피판술을 이용한 선상 반흔제거)

  • Purpose: A common side effect of the scalp reduction is a creation of a 'slot' with the hair growing in the opposite directions away from the scar. Overcoming the unnatural appearance of the slot has been a vexing problem in the scalp reduction surgery. None of the conventional corrective surgical techniques provides a complete and satisfactory aesthetic result. The Frechet flap is a triple transposition flap used for the correction of the slot defect secondary to scalp reduction surgery, seldom needing further scar revision. The Frechet technique provides a solution to the problem of the central slot concealment that is unattainable by other means, such as; Z-plasty and mini-graft. Methods: Authors applied the Frechet technique to Asian patients who had undergone scalp reduction and operated on 4 patients from March, 2000 to January, 2001. Average follow-up period was 13 months. Patients with long scars passing through the temporoparietoccipital zone were excluded. All the undermining was performed in the subgaleal plane, reaching the upper auricular sulcus and stopping just above the nuchal ridge. Results: None of the patients experienced infection, hematoma, nor any permanent hair loss. Transient telogen effluvium at the distal end of flap 2 and 3 was noticeable in one case. Conclusion: In conclusion, the results are aesthetically satisfactory without any significant complications.

Correction of Cup Ear using the Mattress suture (매트리스봉합을 이용한 수축귀의 교정)

  • Purpose: Constricted ear, which is named by Tanzer includes lop ear, cup ear, and sort of prominent ear. It has classified three groups by Tanzer, especially grou I and IIA have been corrected by banner flap, Musgrave's technique, tumbling concha - cartilage flap, reversed banner flap and others. However, these techniques were too invasive for correcting mild degree of deformity. Therefore, we corrected the ear with mattress suture which is simple and less invasive. Method: The operations were done against 5 patients from 2005 March to 2008 April. All the cases were unilateral ears with constriction included helix and scaphoid fossa without difference in length between both ears. Though a posterior auricular skin incision, the folded cartilage is exposed and two parallel incisions on superior crus were made. After mattress suturing in cartilage, the superior crus of antihelix was formed and its force enables the folded portion to be in a normal anatomic position. Result: All of 5 patients got satisfactory results. There were no complications like hematoma or skin necrosis, and no recurrence during follow - up period(the average period was 9 months). And we couldn't recognize the difference between height of both auricles. Conclusion: Mattress suture is simple, less invasive, and suitable on correcting mild deformity of constricted ear with better result, so here we suggest the method.
