Unilateral Augmented Pedicle Screw Fixation for Foraminal Stenosis

  • Objective: The purpose of this study is to evaluate the effectiveness of unilateral decompression and pedicle screw fixation for the unilateral symptomatic foraminal stenosis. Methods: The study group comprises consecutive 16 patients who underwent unilateral decompression and bone cement augmented pedicle screw fixation from May 2003 to January 2006. The patients were evaluated by visual analog scale (VAS) for pain and the scoring system of the Japanese Orthopedic Association (JOA) for low back pain. The result of surgery was also evaluated with McNab's classification. Excellent or good outcome was considered as successful. The patients were followed at postoperative 1 month, 3 month, 6 month, and 1 year with standing AP and lateral films. Results: The average VAS and JOA score of the 16 patients were 7.8(range, 6-9) and 5.8(range, 3 - 10) before surgery and 2.2(range, 0 - 5)and 12.3(range, 9 - 15) at the time of last follow up. Both VAS and JOA score improved significantly after the surgery (p<0.05, t-test). All patients improved after the operation and no revision surgery was required. No metal failure or pseudoarthrosis was observed during the follow-up. The success rate was 87.5%. Conclusion: Our data suggest that unilateral decompression and pedicle screw fixation for the unilateral symptomatic foraminal stenosis is an effect method for obtaining satisfactory clinical outcome. Its possible advantage is shorter operation time and reduced surgical extent. We believe that the reduced stiffness of unilateral fixation was compensated by pedicle screw augmentation and interbody fusion.

Analysis of GBAS Availability and Requirement with respected to Protection Level at Jeju International Airport (제주 국제 공항의 Protection Level 관점에서 GBAS 가용성 및 요구 조건 분석)

  • This paper presents the simulation results of GBAS availability and requirement (with respected to Vertical Protection Level) using simulated data at CAT I, CAT II/III DH point (Decision Height), which are generated using Jeju international GNSS reference position, aircraft horizontal velocity and reference/aircraft GNSS antenna performance index and so on. Two kinds of protection levels are presented, one is from a hypothesis (H0) and other is from a alternative hypothesis (H1). These protection levels are compared with AL (Alert Limit), and we analyse the GBAS availability and requirement for CAT I and CAT II/III at the airport.

Regulation of histamine H2-receptor mediated Mg2+ release by phosphodiesterase inhibitors in the guinea pig hearts (기니픽 심장에서 histamine H2-수용체 자극에 의한 Mg2+ 유리에 대한 phosphodiesterase 억제제의 효과)

  • Several recent studies demonstrate that receptor-mediated cAMP (adenosine 3',5'-monophosphate) production evokes marked change in magnesium ($Mg^{2+}$) homeostasis. The effects of dimaprit or/and phosphodiesterase (PDE) inhibitors on the $Mg^{2+}$ release from perfused guinea pig heart and collagenase-dispersed myocytes was studied to clarify an association of $H_2-histaminergic$ receptor-mediated $Mg^{2+}$ regulation with intracellular cAMP-degradation system. $Mg^{2+}$ efflux was stimulated in perfused hearts and myocytes by IBMX (3-isobutyl-1-methylxanthine), a calmodulin-sensitive PDE inhibitor, but not by RO 20-1724(4-(3-butoxy-4-methoxybenzyl)-2-imidazolidinone) or papaverine, cAMP-specific PDE inhibitors. $Mg^{2+}$ efflux was also be induced by dimaprit, a H-2-agonist. $Mg^{2+}$ effluxes induced by dimaprit were augmented by the presence of the PDE inhibitors. The augmentation of dimaprit-induced $Mg^{2+}$ effluxes by the PDE inhibitors were inhibited by ranitidine, a $H_2-antagonist$, and imipramine, a $Na^{+}-Mg^{2+}$ exchange inhibitor, in perfused hearts and myocytes and were also inhibited by amiloride in perfused hearts. These results suggest that the $H_2$-stimulated $Mg^{2+}$ effluxes from guinea pig heart can be regulated by the cytosolic nonspecific-dependent PDE systems and that it is induced by the $Na^{+}-Mg^{2+}$ exchanger stimulation.

The Protective Effect of Adenosine Included Cardioplegits in Myocardial Ischemia (심근의 허혈시 아데노신을 함유한 심정지액의 심근보호 효과)

  • Although the effects of adenosine on the heart, including the clinical suppression of cardiac arrhythmias, have been recognized for more than half a century, it is only in the last decade that the therapeutic potential of adenosine has been recognized. The objective of this study was to determine if augmentation of myocardial adenosine levels during global ischemia improves functional recovery after reperfusion. We used to modified Langendonf system to evaluate myocardial protective effect. Isolated rat hearts were subjected to 90 minutes of deep hypothermic arrest(15$^{\circ}C$) with modified St. Thomas'Hospital cardioplegic solution used to provide myocardial protection. Myocardial adenosine levels were augmented during ischemia by providing exogenous adenosine in the cardioplegic solution. Two groups of hearts w re studied: (1) control group(n=10) cardioplegia alone; (2) adenosine group(n=10) adenosine(0.75mg/kg/min) added to the cardioplegic solution. Significantly better percent recovery(p<0.01) in hemodynamics(except heart rate) at 60 minutes after reperfusion was evident compared to baseline values in the adenosine group. (systolic no란ic pressure : 78.5$\pm$3.6% vs 66.6$\pm$5.9%, airtic overflow volume : 61.7$\pm$ 11.6% vs 37.2$\pm$ 15.4%, coronary flow volume 77.1$\pm$7.5% vs 57.2$\pm$ 11.1%, and cardiac output : 65.6$\pm$ 11.5% vs 44.2$\pm$ 12.4%). Heart rate was similar in two groups(94.4$\pm$4.8% vs 95.3 $\pm$ 6.8%). Adenosine groups resulted in significantly rapid recovery time of heart beat after reperEusion(p<0.01) (24.5$\pm$7.6 sec. vs 179.0$\pm$ 131.1sec.). In biochemical study, CPK levels(0.1 $\pm$0.3U/L vs 1.4$\pm$0.8U/L) and lactic acid levels(0.08$\pm$0.Immol/L vs 0.34$\pm$0.2 mmol/L) were significantly low in adenosine groups(p<0.01). We concluded that adenosine included cardioplegia have better recovery effects after r perfusion in myocardial ischemia compared to adenosine free cardioplegia.

Electroacupuncture Delays Development of Hypertension through Increase of NO Level in Spontaneously Hypertensive Rats

  • Objective : Using a spontaneously hypertensive rat (SHR) model of essential hypertension, this study investigated whether electroacupuncture (EA) could reduce early stage hypertension by examining whether EA increased nitric oxide (NO) levels in plasma, which compensates for elevated blood pressure (BP). Methods : EA was applied to the acupoint, Baekhoe (GV20), and to a non acupoint in the tail at 10 Hz and an intensity of 1 mA for 10 minutes on the first and fourth day of the week for three weeks under isoflurane anesthesia. In conscious SHR and normotensive Wistar Kyoto (WKY) rats, blood pressure was determined the day after EA treatment by the tail cuff method using an automatic BP monitoring system. We also measured NO concentration of blood serum in SHR and WKY. Results : Systolic blood pressure (SBP) and diastolic blood pressure (DBP) were lower after 3 weeks of GV20 treatment than in non EA treated or non acupoint treated SHR rats. The NO level of plasma was significantly lower in hypertensive SHR than in normotensive WKY. EA prevented the augmentation of blood pressure, and also increased NO concentrations from $7.91{\pm}0.42$ ${\mu}M$ to $11.50{\pm}0.93$ ${\mu}M$ in SHR serum. Conclusions : We suggest that acupuncture may be an early intervention to delay the development of hypertension and enhance NO/NOS activity.

A Study for Verification of Hair Growth Effect of Azelaic Acid and Vitamin B6 (아젤라인산 및 비타민 B6의 육모효과 검증에 관한 연구)

  • Purpose: Interest in the augmentation of hair growth for functional and aesthetic purpose has increased dramatically in recent years. Many hair growth products have been released, but most of these have not been proven scientifically. This study aims to measure the hair growth effect of azelaic acid and vitamin $B_6$, which have been known as hair growth materials, in animal models. Methods: Six weeks old C57BL/6 mice were used in this study and hair of mice were removed by topical treatment. The mice were divided into five experimental groups according to the testing material such as saline (negative control), propylene glycol(vehicle control), azelaic acid, vitamin B6 and azelaic acid plus vitamin B6 in combination. Hair growth was documented photographically and histologically, and then analysed by the high quality hair analysis program system. The quantity of endocrine factors, IGF-I and TGF-${\beta}1$ in the skin of mice was measured by PCR analysis. Results: The topical treatment of azelaic acid and vitamin B6 in combination for 2 weeks to dorsal skin accelerated hair regrowth more than other groups. The azelaic acid and vitamin $B_6$-combined treatment also promoted hair follicle elongation and thickness compared to the others. Histologic studies showed increased number of basal cells in azelaic acid and vitamin $B_6$-combined treatment. Furthermore, the azelaic acid and vitamin $B_6$-combined group significantly increased the expression of IGF-I but decreased the expression of TGF-${\beta}1$ in the skin of mice compared to other groups. Conclusion: These results suggest that azelaic acid and vitamin $B_6$, when used together, have an additive effect and might be used as hair growth materials.

Tethered Hover Test for Small Scaled Tilt-rotor UAV (축소형 틸트로터 무인기의 안전줄 호버 시험)

  • Tilt rotor aircraft can take off and land vertically and cruise faster than any other helicopter. A scaled flight demonstration model of a tilt rotor aircraft has been developed by KARI. Because the flight characteristics of tilt rotor are not well known, the developed scaled model would be helpful to evaluate flight control algorithm of a full scale aircraft. The tethered hover test has been performed in order to improve hover flight characteristics of tilt rotor aircraft prior to flight test of the small scaled model. During the tethered hover test, the performance of rotor speed governor, rate SAS (Stability Augmentation System) and control surface mixers have been evaluated. We expect that the results of real flight hover test would be quite same as tethered hover test. Therefore the tethered hover test results will reduce the risk of flight test properly by fixing some of hidden problems which might occur during the flight test. This paper presents the results of tethered hover test in detail and shows how it could be final ground test before flight test. The control mixer gain and rate SAS feedback gains were modified in order to get higher controllability and stability during the tethered hover flight. The rotor governor showed that it could keep rotor RPM constant with very small deviation even during severe pilot collective input change. The tethered hover test results gave pilot and engineers confirmation and experience about the scheduled flight test.

Impact of the reform for separation between prescribing and dispensing of drugs upon financial situation of the National Health Insurance (의약분업이 건강보험 급여비에 미친 영향)

  • Korean health care system introduced the reform for separation between prescribing and dispensing of drugs (SPD reform) in the latter part of the year 2000. The objective of this paper is to look at what change this reform has brought about in the financial situation of Korean public health insurance scheme, particularly in terms of insurance benefit outlay. Since the inception of the reform is a development of more than five years ago, its impact on the finance situation would now start to become apparent. Hypothesis is set in this study for each of three components of drug reimbursement in health insurance, i.e. average price level, composition of drugs and their overall volume. In terms of the classification of health care services by mode of production, the impact of the SPD reform is confined mainly to the last two among three drug reimbursement fields including inpatient department, out-patient department and pharmacy. Pure impact of the SPD reform was estimated to be more or less than 1.7 trillion won, 13.1% of the total outlay of the Nation Health Insurance in 2001, and more than 2.0 trillion won, 14.9% of the total outlay of the Nation Health Insurance in 2003. Both dispensing fees for the pharmacists, which had been newly introduced on occasion of the SPD reform, and larger share of expensive drugs in the medicines prescribed by doctors were confirmed to be main drivers of the augmentation of drug reimbursement.

Phorbol 12-Myristate 13-Acetate Enhances Long-Term Potentiation in the Hippocampus through Activation of Protein Kinase $C{\delta}$ and ${\varepsilon}$

  • Many intracellular proteins and signaling cascades contribute to the sensitivity of N-methyl-D-aspartate receptors (NMDARs). One such putative contributor is the serine/threonine kinase, protein kinase C (PKC). Activation of PKC by phorbol 12-myristate 13-acetate (PMA) causes activation of extracellular signal-regulated kinase (ERK) and promotes the formation of new spines in cultured hippocampal neurons. The purpose of this study was to examine which PKC isoforms are responsible for the PMA-induced augmentation of long-term potentiation (LTP) in the CA1 stratum radiatum of the hippocampus in vitro and verify that this facilitation requires NMDAR activation. We found that PMA enhanced the induction of LTP by a single episode of theta-burst stimulation in a concentration-dependent manner without affecting to magnitude of baseline field excitatory postsynaptic potentials. Facilitation of LTP by PMA (200 nM) was blocked by the nonspecific PKC inhibitor, Ro 31-8220 ($10{\mu}M$); the selective $PKC{\delta}$ inhibitor, rottlerin ($1{\mu}M$); and the $PKC{\varepsilon}$ inhibitor, TAT-${\varepsilon}V1$-2 peptide (500 nM). Moreover, the NMDAR blocker DL-APV ($50{\mu}M$) prevented enhancement of LTP by PMA. Our results suggest that PMA contributes to synaptic plasticity in the nervous system via activation of $PKC{\delta}$ and/or $PKC{\varepsilon}$, and confirm that NMDAR activity is required for this effect.

  • MMORPG Quest plays a strong role of motivation through compensation related with Avatar such as items and experience points rather than a role of story transfer. This situation finally made MMORPG become a difficult game that players should study the compensation of Quest for growth of a character, so it causes fixation of the user class. As a solving method of the fixation of the users, the possibility of Quest capable of strengthening play motive of various users through clarification of motivation structure of the Quest play and pluralistic understanding about players was investigated. This is a direct response to the user within the game system or how to use the player log information. The pluralistic understanding method of players will have values as a tool that can guarantee the objectivity and actual production of the data collected utilizing.
