A Case Report of Schizophrenia with Acute Cerebral Infarction Treated with Guibi-tang (급성 뇌경색을 동반한 정신분열병 환자의 귀비탕(歸脾湯) 치험1례)

  • Schizophrenia is a disturbance condition that lasts for at least 6 months and includes at least 1 month of active-phase symptoms(i.e., two [or more] of the following : delusions, hallucinations. disorganized speech, grossly disorganized or catatonic behavior, negative symptoms) In this study, we report one case of schizophrenia with acute cerebral infarction. The patient has been diagnosed with schizophrenia more than 20 years ago, and after this recent cerebral infarction, all of her schizophrenic symptoms such as auditory hallucination, delirium, depression, insomnia were aggravated. Her schizophrenic symptoms, PANSS and NIHSS total scores were considerably reduced after oriental treatment(Guibi-tang(歸脾湯)), so we report this as a potential treatment.

  • Traumatic asphyxia, also called 'Perthes syndrome', is characterized by subconjunctival hemorrhage, cervicofacialpetechiae and cyanosis resulting from venous hypertension caused by an abrupt, severe, compressive force to the thoracoabdominal region. A 37-year-old male patient who was transferred to the emergency room due to chest trauma by overturning of a forkcrane. His head, neck, and shoulders showed severe ecchymosis, and his upper chest was cyanotic. There was bilateral subconjunctival hemorrhage and bilateral ear bleeding without tympanic rupture. Perthes syndrome is a rare condition and we treated a patient with typical and atypical symptoms; thus we report this case of Perthes syndrome.

  • Effects of the music and white noise on recovery of the autonomic and cortical responses evoked by aversive visual stimulation were analyzed in 20 subjects. It was suggested that the music is able to exert modulatory influence on the physiological activity resulted from exposure to unpleasant IAPS based stimuli. Spectral power of DDG, heart rate(HR)respiration rate (RSR) and electrodermal activity(EDA)were recorded and analyzed for each experimental condition. It was observed HR and RSR deceleration, increased EDA and electrocortical activation expressed in decreased alpha power and increase of delta activity ao occipital and frontal areas. Obtained results suggest that audutory stimulation both with pleasant and sad music lead to restoration of pre-stimulation activation levels of most physiological parameters during listenning to music and in post-stimulation period. White noise evoked short-term physiological responses typical for orienting reaction and quite distinct from changes produced by music. Available data to differentiate effeces among pleasant and sad music, due toqualitative similarities of physilolgical patterns, but suppert an assumption that music is capable to facilitate the process of recovery of physilolgical responses elicited by visual stimulation of negative valence, thus positively modulate post-stress state.

  • The cochlear implantation(CI) as an useful tool for aural rehabilitation in bilateral severe to profound hearing impairment. However, CI prefer to usually one ear in spite of bilateral hearing impaired. because of the various characteristics of hearing loss, the hearing conservation for the future possibility, and socioeconomic condition of hearing impaired person and their families. The unilateral CI has limitations such as a directional loss, a difficult speech understanding in noise and a neural plasticity. These limitations will be overcome by hearing aid(HA) which is familiar with hearing impairer. but HA fitting for bimodal-binaural hearing are difficult because the difference output characteristic of HA and CI. This study will be confirm realities of use of HA in unilateral cochlear implantee. For this goal, 25(m:f=10:15) child participated who are used to HA for 1 to 17 months. We had telephone interviews with their mother about use of HA, change of auditory performance and own voice. As the results, hearing threshold levels of unimplanted ear, the use of a appropriate HA, implanted and aided hearing threshold level(HTL) are must be considered for successful biomodal-binaural hearing. Especially, implanted and aided HTL should be very useful parameter for a prediction of HA effect and a criterion of selection for bilateral cochlear implantation.

The Research of Railway Noise through Auditory Experiments Focused on the Autonomic Nervous System and Cardiovascular System (청감실험을 통한 철도소음의 자율신경 및 심혈관계통 영향도 연구)

  • According to the conventional studies on the noise and cardiovascular effect, railway noise is better associated with hypertension and adverse cardiovascular events than road traffic noise. But the underlying mechanisms remain unclear. We investigated the hypothesis that exposure to acute railway noise would the unfavorable effect of cardiovascular and autonomic system in healthy young subjects. Using a randomized, sham-controlled cross-over design, ten subjects were assigned to receive either an exposure to high speed train noise (84 dB) for 30 minutes or a control condition (non noise), separated by two days. Blood pressure, heart rate, augmentation index and heart rate variability as indices of cardiovascular and autonomic system function were measured at baseline, during, and recovery from two trials. The results show that exposure to acute railway noise significantly increased diastolic blood pressure and augmentation index, which may cause of adverse cardiovascular effects.

An Study on the Sound Attenuation and the Improvement in Hearing Condition of Fire Alarm Device in Apartment Buildings (공동주택 화재 경보음 전달특성과 개선방안 고찰)

  • In many fire emergencies, the auditory fire alarm signals are very important to save the occupant's life. But as the sound insulation of building elements has been improved, it is more difficult for occupant to recognize the fire alarm signals when the audible fire alarm worked. This is the first study to show the sound attenuation of audible fire alarm device in apartments. We measured and analyzed the sound attenuation level in seven units. The result showed that it was not sufficient to detect the sound from the fire alarm device in bedrooms. Whether the fire alarm device worked or not, the differences of sound level in bedrooms were below 1$\sim$10 dBA. To give the minimum sound level 60 dBA in bedrooms, the proper sound levels from alarm device installed in livingrooms were suggested using computer simulation.

Effects of Text Types and Working Memory on Text Comprehension in Reading Normal and Reading Deficient Children (텍스트 유형과 작업기억이 읽기 정상 아동과 읽기 지진 아동의 텍스트 이해에 미치는 영향)

  • Two experiments were conducted to investigate the effects of verbal working memory and the spatial working memory on children's text comprehension. The reading span and the operation span of the reading deficient children were smaller than that of the reading normal children, but the two groups did not differ in the visual span. Reading deficient children got lower score in the comprehension tests than reading normal children, and the difference was larger for the expository text than the narrative text. The involvement of visual working memory in reading narrative texts were more directly tested in Experiment 2 by asking the children do a secondary memory task before they answered the comprehension test. Reading normal children suffered more under auditory secondary memory condition for both narrative and expository texts, whereas reading deficient children suffered under visual secondary memory condition for narrative texts as well. The results of the two experiments suggested that the spatial working memory can be involved in text comprehension process, especially with reading deficient children.

Dual task interference while walking in chronic stroke survivors

  • Objective: Dual-task interference is defined as decrements in performance observed when people attempt to perform two tasks concurrently, such as a verbal task and walking. The purpose of this study was to investigate the changes of gait ability according to the dual task interference in chronic stroke survivors. Design: Cross-sectional study. Methods: Ten chronic stroke survivors (9 male, 1 female; mean age, 55.30 years; mini mental state examination, 19.60; onset duration, 56.90 months) recruited from the local community participated in this study. Gait ability (velocity, paretic side step, and stride time and length) under the single- and dual-task conditions at a self-selected comfortable walking speed was measured using the motion analysis system. In the dual task conditions, subjects performed three types of cognitive tasks (controlled oral word association test, auditory clock test, and counting backwards) while walking on the track. Results: For velocity, step and stride length, there was a significant decrease in the dual-task walking condition compared to the single walking condition (p<0.05). In particular, higher reduction of walking ability was observed when applying the counting backward task. Conclusions: Our results revealed that the addition of cognitive tasks while walking may lead to decrements of gait ability in stroke survivors. In particular, the difficulty level was the highest for the calculating task. We believe that these results provide basic information for improvements in gait ability and may be useful in gait training to prevent falls after a stroke incident.

  • In the past, objective usability was one of the most important aspects when user used system. But nowadays user's subjective experiences are getting more critical element than objective usability in HCI(human-computer interaction). Most people own their mobile phone and use it frequently these days. It is especially important to make user's subjective experiences more positive when using devices like mobile phones people frequently carry and interact with. This study investigates whether the interfaces which express the emotion give more positive experiences to users. Researchers created mobile phone prototypes to compare the effect of mechanical melody feedback(the major auditory feedbacks on mobile phones) and emotional voice feedback(recorded human voice). Participants experienced four kinds of mobile phone prototypes(no feedback, mechanical melody feedback, emotional voice feedback and dual feedback) and evaluated their experienced usability, hedonic quality and preference. The result suggests that person's perceptional fun and hedonic quality were getting increased in the phone which gave the emotional voice feedback than the mechanical melody feedback. Nevertheless, the preference was evaluated lower in the emotional voice feedback condition than the others.

Hand Proximity Effect on Task Switching Performance Through Cue Modality (손 근접성이 단서양상에 따라 과제전환 수행에 미치는 효과)

  • The present study examined how processing features of visual information near the hand would affect task switching. Recent studies reported enhanced cognitive control of visual information presented the near hands. To investigate the enhancement of cognitive control based on the relationship between hand proximity and attention, we implemented 2 experiments. In the task switching performance experiment, the hand proximity effect depended on modality of cue and target. The first experiment showed that stimuli near the hand received greater cognitive control than stimuli far from the hand, resulting in smaller switch cost. The result could rule out the feature-binding problem, which identifies reduced switch cost as the cause instead of hand proximity. Our results show that hand proximity actually reduced switch cost. In the second experiment, we examined the effects of hand nearness, modality, and their interaction on switch cost. In task switching, the target was always visual, and the cue was presented either visually or auditorily. In addition, we manipulated the cue-target interval to observe the preparation effect of cue. The results showed that a visual cue near the hand reduced switch cost by shortening task preparation time. Also, modality switching between an auditory cue and visual target was remarkable in a hand-near condition. The results for the visual cue could be interpreted as a benefit of rapid visual attention orienting. On the other hand, the results for the auditory cue could be interpreted as the cost of interference of modality switching by slower attentional disengagement of stimuli near the hands. Finally, modulation of switch cost by attention induced by hand nearness was discussed.
