Drinking Pattern and Nonfatal Injuries of Adults in Korea (성인에서 AUDIT와 손상의 연관성)

  • As alcohol use is one of the most important risk factors for injuries, this study was intended to clarify and evaluate any relationship between drinking patterns and the incidence rates/specific characteristics of injuries in adult populations, using a widely accepted tool, the Alcohol Use Disorders Identification Test (chronic alcohol drinking behaviors measurement, hereinafter the AUDIT) developed by the World Health Organization to help to assess the behaviors in a more accurate and reliable manner. This study used the data collected from the 2009 Korea National Health and Nutrition Examination Survey (KNHANES), in which 7,511 of 7,893 adult participants aged ${\geq}19$ years answered the questions about injuries, and excluding 104 non-respondents, 6,258 of participants in the questionnaire survey of drinking patterns were finally analyzed. The incidence rates and specific characteristics of injuries as classified by the AUDIT categories (i.e., body regions, types and mechanisms) were assessed and estimated in terms of their relative risk using t-test, ANOVA, and logistic regression. SPSS 19.0 statistical package software was employed for statistical analyses. These analyses indicate that the incidence rates of overall injuries were significantly higher in male respondents than in female respondents. The risks of alcohol use related injuries were 8.3 times higher in male respondents than in female ones. Regarding educational background, high school graduates showed the highest rates in the AUDIT with significant difference from the other groups. The married group and the group of respondents having monthly income estimated at KRW 2.01 to 3 million also showed the highest rates in the AUDIT compared to the other groups, indicating statistically significant difference. Significantly increased in problematic drinkers and those with alcohol dependence, the incidence rate of injuries body regions was 0.0371 in the head/neck, and with respect to the AUDIT and the mechanisms of external causes of injuries, transport accidents ranked first, followed by slippage, others, crash and fall. In regard to the classified types of injuries, it was statistically significant in others (e.g., laceration, contusion, addiction, or penetrating wound). In conclusion, the mechanisms of external causes of injuries as well as injuries attributed to alcohol use are very important, and a strategy is required to reduce such the injuries in the manner of decreasing the frequency of drinking after motivation by professional counsellors.

Correlates of Problem Drinking by the Alcohol Use Disorders Identification Test on Korean College Campus (AUDIT척도에 의한 한국대학생의 알코올사용장애 실태 및 원인 분석)

  • Objectives : To survey college students with an Alcohol Use Disorder, and analyze the reasons for their disorder. Methods : The cross-sectional study was conducted at 60 four-year colleges within Seoul and 9 other provinces. The schools and students selected for the study provide a nationally representative sample, and the survey was conducted between May 15th and June 14th 2003. 2,385 cases were analyzed using questionnaires, which included a series of questions about students' alcohol use and associated problems, as well as an Alcohol Use Disorder Identification Test. Results : 42.3% of students were found to have an Alcohol Use Disorder. The probability of a student having an Alcohol Use Disorder was 1.30 times higher among male compared to female students. Those students not living with their parents or relatives were 1.40 times more likely to have an Alcohol Use Disorder. Those students where the father had a drinking problem and those who admitted that their parents drank heavily while they were growing up were 1.38 and 1.54 times more likely, respectively, to have an Alcohol Use Disorder. Those students attending a general university, joining a student club, attaining less than a B average credit score and those unsatisfied with their education were 1.60, 1.36, 1.41 and 1.27 times more likely, respectively, to have an Alcohol Use Disorder. Those students who had experience of drugs, smoking, binge drinking when they were in the last year of high school and the forceful consumption of mixed alcohol were 3.67, 1.95, 2.15 and 1.76 times more likely, respectively, to have an Alcohol Use Disorder. Conclusions : College students' with an Alcohol Use Disorder is a very severe and large problem within colleges. An Alcohol Use Disorder is determined by individual and family variables, the college environmental and life variables, as well as behavior variables.

Inter-comparison of Accuracy of Discharge Measurement Methods - A Case Study Performed in the Dalcheon River Downstream of the Goesan Dam- (유량측정 방법의 정확도 분석 -괴산댐 하류 달천 적용 사례를 중심으로-)

  • Relative accuracy of six discharge measurement methods-velocity-area method, rod-float method, ADCP moving-vessel method, ADCP fixed-vessel method, electromagnetic wave surface velocimeter (EWSV), LSPIV- is evaluated by comparing discharges measured by them with dam released discharges. Data from 39 times of concurrent discharge measurement campaigns are analyzed. Except the rod-float method, measured discharges show absolute errors less than 6.2% with dam discharges. When the four methods is evaluated by being compared with discharges measured with the conventional velocity-area method, discharges with electromagnetic wave surface velocimetry shows 7.35% of absolute errors and other three methods shows absolute errors less than 6%. The rod-float method, which shows large discrepancy compared with dam and velocity-area method, need complementary verification.

Clinical Audit in Radiation Oncology: Results from One Academic Centre in Delhi, India

  • The objective was to analyze the radiotherapy (RT) practice at the cancer centre of a tertiary academic medical institution in Delhi. This audit from an Indian public institution covered patient care processes related to cancer diagnosis, integration of RT with other anti-cancer modalities, waiting time, overall treatment time, and compliance with RT. Over a period of one year, all consecutively registered patients in radiotherapy were analyzed for the audit cycle. Analysis of 1,030 patients showed median age of 49.6 years, with presentation as stage I and II in 14.2%, stage III and IV in 71.2% and unknown stage in 14.6%. A total of 974 (95%) were advised for RT appointment; 669 (68.6%) for curative intent and 31.4% for palliation. Mean times for diagnostic workup and from registration at cancer centre to radiotherapy referral were 33 and 31 days respectively. Median waiting time to start of RT course was 41 days. Overall RT compliance was 75% and overall duration for a curative RT course ranged from 50 days to 61 days. Non-completion and interruption of RT course were observed in 12% and 13% respectively. Radiotherapy machine burden in a public cancer hospital in India increases the waiting time and 25% of advised patients do not comply with the prescribed treatment. Infrastructure, machine and manpower constraints lead to more patients being treated on cobalt (74%) and by two-dimensional (78%) techniques.

  • Medium size rotorcraft consist of approximately 5,000 key components. The components must satisfy the target weight and performance requirement through optimization. Therefore a rotorcraft component has approximately 100 times higher unit cost for mass production than the equivalent of an automobile's, along with huge development cost. Configuration management consists of four main parts : configuration identification, configuration control, configuration audit, and data management. In development programme, configuration management is one of the methods to save development cost, and setting a well-structured configuration management system according to the Military Standard is an important factor in success of the programme. The paper proposes a configuration management system suitable for development of various multi-use components development and presents that the configuration management was performed successfully during dual-use component development programme using the established system.

Design of Operation Management Check Items of Efficient Information System for Improvement of Business Continuity based on ISO 22301 (ISO22301 기반 비지니스 연속성 증대를 위한 효율적인 정보시스템 운영감리 점검항목 설계)

  • In this paper, we have studied the improvement of operational control for the enhancement of business continuity of information system becoming more important with the development of information technology such as big data, Iot, and artificial intelligence. The operational management and audit guidance of the current information system, which is coming in the fourth industrial age, where various services, data and industries are converged, is based on the existing general information system pattern and needs to be improved. The provision of services at fixed times is linked to the survival of enterprises and countries and serves as a key element. Therefore, it is necessary to study the application of optimized check items of the operation audits to minimize the service interruption damage of the information system and to provide the stable service in terms of business continuity management. To accomplish this, the check items presented in the operational control of the information system were derived by combining the PDCA step contents and 8 resource requirements provided in ISO 22301. From the point of view of increasing the business continuity according to the derivation criteria of the inspection items, the operational inspection check items were derived by exemplifying the improved check items and review items of the information system operation audit and the products to be checked during the operational audit. The check items were divided into management audit improvement check items for service continuity management, and operational audit improvement check items for performance and availability management. The average score of the IT professionals' survey on the suitability of the proposed checklist was 4.63, which was concluded to be appropriate.

Drinking Pattern and Nonfatal Injuries of Adults in Korea (우리나라 성인의 음주행태와 비치명적 손상의 연관성)

  • Purpose: Alcohol is a major risk factor for injuries, but little is known about the relationship between chronic drinking pattern and injuries. The purpose of this study was to evaluate the incidence of alcohol-related injuries and the differences in the characteristics of the injuries between Alcohol Use Disorder Identification Test (AUDIT) categories. Methods: Among a representative sample of the third Korea National Health and Nutrition Examination Survey. 7,697 adults answered questions about injuries, drinking patterns, and socioeconomic status. Incidences of alcohol-related injuries and of non-alcohol-related injuries were calculated, and the relative ratio of injuries between AUDIT categories was analyzed. Results: The incidences of all injuries and alcohol-related injuries were 69.2 and 6.8 per 1000 person-year. After adjustment, the incidences of non-alcohol-related injuries showed no differences between AUDIT categories. But the adjusted relative ratios (RRs) of alcohol-related injuries were 3.73 (95% CI: 1.54~8.99) and 7.70 (95% CI: 3.61~16.44) for risk drinkers and for alcohol-dependent drinkers respectively. No specific body part could be identified as being frequently injured in cases of risk drinkers and alcohol-dependent drinkers. In contrast, the adjusted RRs of fractures, bruises, and open wounds were 1.76 (95% CI: 1.03~3.02), 1.73 (95% CI: 1.15~2.61), and 1.85 (95% CI: 1.17~2.93) for alcohol-dependent drinkers. Assaults occurred 4.66 times more frequently in alcohol-dependent drinkers. Conclusion: Alcohol-related injuries occur more frequently as the person becomes more dependent on alcohol. Fractures, Bruises, open wounds, and assaults occur more frequently in alcohol-dependent drinker. This study suggests that efforts should be carried out in the ED to screen and treat alcohol-use disorders associated with patients injured after drinking.

Design of Multipurpose Phantom for External Audit on Radiotherapy

  • Purpose: This study aimed to design a multipurpose dose verification phantom for external audits to secure safe and optimal radiation therapy. Methods: In this study, we used International Atomic Energy Agency (IAEA) LiF powder thermoluminescence dosimeter (TLD), which is generally used in the therapeutic radiation dose assurance project. The newly designed multipurpose phantom (MPP) consists of a container filled with water, a TLD holder, and two water-pressing covers. The size of the phantom was designed to be sufficient (30×30×30 cm3). The water container was filled with water and pressed with the cover for normal incidence to be fixed. The surface of the MPP was devised to maintain the same distance from the source at all times, even in the case of oblique incidence regardless of the water level. The MPP was irradiated with 6, 10, and 15 MV photon beams from Varian Linear Accelerator and measured by a 1.25 cm3 ionization chamber to get the correction factors. Monte Carlo (MC) simulation was also used to compare the measurements. Results: The result obtained by MC had a relatively high uncertainty of 1% at the dosimetry point, but it showed a correction factor value of 1.3% at the 5 cm point. The energy dependence was large at 6 MV and small at 15 MV. Various dosimetric parameters for external audits can be performed within an hour. Conclusions: The results allow an objective comparison of the quality assurance (QA) of individual hospitals. Therefore, this can be employed for external audits or QA systems in radiation therapy institutions.

A Review of Proximity Assessment Measurements According to Fairway Patterns and Ship Size (항로형태 및 선박크기에 따른 근접도 평가기법에 관한 고찰)

  • An acceptable proximity assessment collision probability is widely considered to be less than $10^{-4}$ in maritime traffic safety audit schemes. In the 1970s, Fujii, Macduff and colleagues introduced various models for collision probability of aberrancy in the community. Although existing studies ensured acceptable proximity collision probability, around $10^{-4}$, they were constrained by assumptions. A lack of support for the proximity probability criterion has been investigated in this study for practical use. The appropriate proximity probability for different size vessels in both straight and curved lanes has been analyzed based on GICOMS data. As a result, reasonable proximity collision probabilities were determined for various vessel traffic conditions. Accordingly, necessary improvements in the maritime traffic system have been suggested in consideration for various maritime traffic situations and conditions.

The Effect of a Time Management Program on Drinking-Habit and Self-Efficacy in College Student With Problematic Drinking (시간관리 프로그램의 적용이 문제음주 대학생들의 음주습관과 자기효능감에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effect of a time management program on self-efficacy and drinking-habit of college student with problematic drinking. Methods : Thirty subjects with problematic drinking were randomly assigned to either a experimental group or control group. The intervention applied to the experimental group was a time manage program which was consisted of two times of sessions per week and provided for five weeks. There was no specific intervention applied to the control group. Alcohol Use Disorder Identification Test-Korean (AUDIT-K) was used to measure problematic drinking behaviors and Self-Efficacy Test was used to evaluate self-efficacy of the subjects. Results : Experimental group showed a significant improvement in AUDIT-K after intervention between groups. In change scores within group, experimental group showed a significant improvement in both AUDIT-K and Self-Efficacy Test, and control group showed a significant improvement in AUDIT-K only. Conclusion : The result of this study showed that time management program is an effective method for college student with problematic drinking.
