The Effect of Doctor's Payment Method on Practice Behavior (지불보상체계가 의사의 진료행태에 미치는 영향 : 미국사례 분석)

  • Considering the existence of asymmetric information between doctor and patient, the doctor's reimbursement method has been considered as a desirable policy device of improving efficiency of patient's use of medical care in terms of its affecting doctor's practice pattern by determining doctor's practice revenue. By using the Community Tracking Study (CTS) physician data set, which includes not only various information on doctors practice arrangements and sources of practice revenue, but also vignettes of various clinical presentations, this paper investigates doctor's reaction to the financial incentive under the control of patient's specific medical situation. Under the econometric model for exploring the effect of doctor's reimbursement method on his/her practice patterns; referring patients, recommend doctor-visit or medical tests, the Hausman's specification test was used for checking out the possibility of the doctor's reimbursement method being endogeneized explanatory variable. In the case where the endogeneity problem of doctor's reimbursement method exists, the 2SLS method was used for correcting that problem, and the multiple regression method was used in the case where the problem is found to be nonexistent. Based on the empirical results, this paper finds that doctors do appear to respond to financial incentive. The empirical results show that the doctor's reimbursement method statistically significantly affects doctor's practice pattern and are coincident with the theoretical result proposed by previous researches, This results suggest, as doctor's practice revenues are mainly determined by prospective method, such as capitation, doctors would more refer their patients to specialists, and hesitate in recommending doctor-visit or medical exam.

Different Effects of Dopamine on Differential Rotational Mobility between Inner and Outer Monolayer of Synaptosomal Plasma Membrane Vesicles Isolated from Bovine Brain

  • Fluorescence polarization of 1,6-diphenyl-1,3,5-hexatriene (DPH) was used to evaluate the effects of dopamine HCl on the range of the rotatioanl mobility of bulk bilayer structure of the synaptosomal plasma membrane vesicles (SPMV) isolated from whole bovine brain. In a dose-dependent manner, dopamine decreased the anisotropy $({\gamma}),$ limiting anisotropy $({\gamma}{infty})$ and order parameter (S) of DPH in the membranes. These indicate that dopamine increased the rotational mobility of the probe in the neuronal membranes. Cationic 1-[4-(trimethylammonio)-phenyl]-6-phenylhexa-1,3,5-hexatriene (TMA-DPH) and anionic 3-[p-(6-phenyl)-1,3,5-hexatrienyl]-phenylpropionic acid (PRO-DPH) were utilized to examine the range of transbilayer asymmetric rotational mobility of the neuronal membranes. Dopamine had a greater increasing effect on the mobility of the inner monolayer as compared to the outer monolayer of the neuronal membranes. It has been proven that dopamine exhibits a selective rather than nonselective fluidizing effect within the transbilayer domains of the SPMV.

  • To investigate the characteristics of the dynamic response of long-span cable-stayed bridges due to various dynamic loadings likes moving traffic loads, two different 3-D cable-stayed bridge models are considered in this study. Two models are exactly the same in structural configurations but different in finite element discretization. Modal analysis is conducted using the deformed dead-load tangent stiffness matrix. A new concept was presented by using divided a cable into several elements in order to study the effect of the cable vibration (both in-plane and swinging) on the overall bridge dynamics. Futhermore case of asymmetric traffic loading clustered in one direction are also considered to study the torsional response of the bridge. The result of this study demonstrates the importance of cable vibration on the overall bridge dynamics.

Effect of Ether-Typed Alcohols on Pore Formation in Preparing an Asymmetrically Porous Polysulfone Membrane (다공성 폴리술폰 비대칭막 제조시 에테르형 알코올의 공경형성에 미치는 영향)

  • Various asymmetric Polysulfone membranes were prepared for a MBR process. Ether-typed alcohols (co-solvent) were added into a dope solution in order to control the pore size of membrane, whose effect on water permeability were investigated. Pore size of the prepared membranes were more affected by molecular-structure of co-solvent than by boiling point of theirs. With the increasing order of methoxy ($CH_3$-O-) < secondary propanol ($-CH_2$-CH(OH)$-CH_3$) < ethoxy ($CH_3-CH_2$-O-), water permeability of the prepared membrane increased. The phenomenon might attribute to the difference of molecularly steric hinderance of co-solvent (eg, Methoxy propanol, Ethoxy ethanol, Methoxy ethanol) in dope solution during the phase inversion. By the addition of ether typed alcohol into a dope solution, the pore size of MF (microfiltration) could be controlled. Also, Membrane prepared was applied to a MBR process and the system was stably operated for 2 months.

  • We investigate the effects of high input power on the performance of optical bistable symmetric self-electooptic effect devices (S-SEEDs) using extremely shallow quantum wells (ESQWs). In this study, we consider the four ESQWs SEEDs; anti-reflection (AR)-coated ESQWs S-SEED, back-to-back AR coated ESQWs S-SEED, asymmetric F뮤교-Perot (AFP) ESQWs S-SEED, and back-to-back AFP-ESQWs S-SEED. As the input power increases, device performances such as on/off contrast ratio, on/off reflectivity difference are seriously degraded because of ohmic heating and exciton saturation. On the other hand, switching speed of the device increases up to certain value and then begins to decrease. With reasonable optimization of the input power for the best switching speed operation of the devices in a cascading optical interconnection system, we simulate and analyze the system bit-rate of the various ESQWs S-SEEDs, for a mesa of $5{\times}5{\mu}m^2$ size, changing the namber of quantum wells for the external bias of 0 V and -5V.

Effects of Rotation Speed on Heat Transfer and Flow in a Coolant Passage with Turning Region ( I ) - Cross Ribbed Duct - (곡관부를 가지는 내부 냉각유로에서 회전수 변화에 따른 열전달 및 유동 특성 ( I ) - 엇갈린 요철배열 덕트 -)

  • The present study investigates heat/mass transfer and flow characteristics in a ribbed rotating passage with turning region. The duct has an aspect ratio (W/H) of 0.5 and a hydraulic diameter ($D_h$) of 26.67 mm. Rib turbulators are attached in the cross arrangement on the leading and trailing surfaces of the passage. The ribs have a rectangular cross section of $2\;mm\;(e){\times}\;mm\;(w)$ and an attack angle of $70^{\circ}$. The pitch-to-rib height ratio (p/e) is 7.5, and the rib height-to-hydraulic diameter ratio ($e/D_h$) is 0.075. The rotation number ranges from 0.0 to 0.20 while the Reynolds number is constant at 10,000. To verify the heat/mass transfer augmentation, internal flow structures are calculated for the same conditions using a commercial code FLUENT 6.1. The heat transfer data of the smooth duct for various Ro numbers agree well with not only the McAdams correlation but also the previous studies. The cross-rib turbulators significantly enhance heat/mass transfer in the passage by disturbing the main flow near the surfaces and generating one asymmetric cell of secondary flow skewing along the ribs. Because the secondary flow is induced in the first-pass and turning region, heat/mass transfer discrepancy is observed in the second-pass even for the stationary case. When the passage rotates, heat/mass transfer and flow phenomena change. Especially, the effect of rotation is more dominant than the effect of the ribs at the higher rotation number in the upstream of the second-pass.

The Effect of Business Strategy on Cost Asymmetry according to Corporate Life Cycle (기업수명주기에 따라 경영전략이 원가비대칭성에 미치는 영향)

  • The purpose of this study investigates the relationship between business strategy and cost behavior according to the corporate life cycle. To do so, I first examine the relationship between cost behavior and business strategy. And then, I analyze the effect of the business strategy on cost behavior conditional upon corporate life cycle based on listed Korean firms from 2000 to 2019. Specifically, the business strategy index is calculated by using the six continuous financial factors and the Prospector and Defender was defined on the score. Also, the corporate life cycle is distinguished based on the cash flow pattern. I find the evidence that the Prospector firms are likely to strengthen the asymmetry of cost behavior (cost stickiness), and examined that such a relationship was most strongly represented in the Introduction. This study is meaningful in that this is a more comprehensive analysis by examining business strategy and cost strategy according to the corporate life cycle and expand the application of financial information by using financial indicators to distinguish business strategies.

Power Generating Characteristics of Zinc Oxide Nanorods Grown on a Flexible Substrate by a Hydrothermal Method

  • This paper describes the power generating property of hydrothermally grown ZnO nanorods on a flexible polyethersulfone (PES) substrate. The piezoelectric currents generated by the ZnO nanorods were measured when bending the ZnO nanorod by using I-AFM, and the measured piezoelectric currents ranged from 60 to 100 pA. When the PtIr coated tip bends a ZnO nanorod, piezoelectrical asymmetric potential is created on the nanorod surface. The Schottky barrier at the ZnO-metal interface accumulates elecntrons and then release very quickly generating the currents when the tip moves from tensile to compressed part of ZnO nanorod. These ZnO nanorods were grown almost vertically with the length of 300-500 nm and the diameter of 30-60 nm on the Ag/Ti/PES substrate at $90^{\circ}C$ for 6 hours by hydrothermal method. The metal-semiconductor interface property was evaluated by using a HP 4145B Semiconductor Parameter Analyzer and the piezoelectric effect of the ZnO nanorods were evaluated by using an I-AFM. From the measured I-V characteristics, it was observed that ZnO-Ag and ZnO-Au metal-semiconductor interfaces showed an ohmic and a Schottky contact characteristics, respectively. ANSYS finite element simulation was performed in order to understand the power generation mechanism of the ZnO nanorods under applied external stress theoretically.

  • This study investigated the effect of corporate international diversification on cost stickiness. A cost behavior that indicates a lower rate of cost reduction when the volume of sales decreases than the rate of cost increase when the volume of sales is increased is called cost stickiness. This cost behavior is caused by decisions made by considering the adjusting costs of the manager, and for corporate international diversification, the decision making on the adjustment cost of the manager has been reduced by the offsetting accruals hypothesis. From the empirical results, we observed that the cost stickiness of international diversified companies decreases. It is expected to contribute to the capital market and academia by identifying whether corporate international diversification can have a significant impact on management decision making related to costs.

Contagious Effect of the Fees for the Consolidated Financial Services under the Asymmetric Information

  • Banks traditionally focus on the financial services against the uncertain future liquidity needs, i.e. saving as well as lending. As the business model of banks has been shifted from the originate to hold model to the originate to distribute model since the enactment of Gramm-Leach-Bliley Financial Services Modernization Act in 1999, the financial services encompass information gathering and generating, underwriting and risk sharing through packaging claims for the investors, in addition to the payment and settlement services. Ensued are the financial market integration and diversification of financial services, with which the accessibility to financial services is arguably significantly enhanced. Such integration and diversification necessarily entails the risk of contagion due to the non-fulfilling service over the several other financial services, which would be contained easily under the separate financial services. This paper addresses the pricing of fees for the integrated financial services through which the contagion could spread when the users of financial service are not immune to the failure to fulfill their obligation due to the economic turmoil. Consequently the information asymmetry about the clients is unavoidable. Higher fees could drive out the otherwise good clients out of the pool of customers for the financial services. Then, the risk could be exacerbated due to the proliferation of bad clients who are vulnerable to the financial distress and liquidity crunch. So the banks should take into account the interactional effect of the fees between/among the non interest based activities and interest based activities under the information asymmetry. Contrary to our general perception, the current analysis demonstrates that the bank should focus on the reduction of cost associated with good clients rather than that of bad clients.
