만성질환자의 정기적 의료이용에 영향을 미치는 요인 - 고혈압, 당뇨병, 고지혈증을 중심으로 - (Factors Affecting Regular Medical Services Utilization of Chronic Disease Patients - Focusing on the Hypertension, Diabetes Mellitus, Hyperlipidemia -)

  • Objectives: This study aims to identify the factors associated with regular medical services utilization of chronic disease patients. Methods: The research selected 4,489 adults aged over 30, diagnosed with hypertension, diabetes, hyperlipidemia, hypercholesterolemia, from the Korea health panel. We analyzed states of regular medical service utilization using descriptive statistics. Multiple regression analysis was used to examine the main factors associated with regular medical services utilization in chronic disease patients. Results: In terms of socio-demographic factors, gender, age, marital status, education level, employment, household income and disability were significantly different between hypertension, diabetes, hyperlipidemia and hypercholesterolemia. Among health status and behavioral factors, number of chronic diseases, subjective health status, smoking, high risk drinking, regular meals, physical activity, obesity were significantly different. From the multiple logistic regression analysis, age, number of chronic diseases, obesity, type of chronic diseases were associated with regular medical services utilization. Conclusions: It is necessary to develop effective health education programs and individualized approach to improve continuous management in chronic diseases patients.

우리나라 노인들의 미충족 의료 유형별 관련요인 (Factors Associated with the Types of Unmet Health Care Needs among the Elderly in Korea)

  • Objectives : This study aimed to investigate the types of unmet health care needs of the elderly and factors affecting them. Methods : This study used data from the 2012 Community Health Survey. Multinomial logistic regression analysis was conducted to identify the relevance between each type of unmet health care needs and predisposing, enabling, and needs factors, based on the cases without unmet health care needs. Results : Persons with unmet health care needs were 4,460 (9.5%) of the total sample. By types of unmet health care needs, 1,171 (2.5%), 1,026 (2.2%), and 2,263 (4.8%) persons reported inaccessibility, non-accommodativeness, and unaffordability respectively. It was concluded that the there were differences in the associated factors according to the types of unmet medical needs. Conclusions : It is suggested that unmet health care needs in the elderly should be examined from diverse angles rather than from a single aspect of partial limits. In particular, diverse types of unmet health care needs for health care in the elderly according to limited accommodation shoulder be examined. Finally, strategies to decrease unmet health care needs that reflect the associated factors should be developed.

군 지역 성인의 신체활동 실천에 미치는 영향요인에 대한 다수준 분석 (Factors Affecting Physical Activity of Korean Adults in Some County Areas : A Multilevel analysis)

  • Purpose: This study was conducted to examine the individual and community level factors associated with physical activity and to identify its relative effects using a multilevel analysis among Korean adults in certain counties. Methods: A cross-sectional data of 39,547 adults (age range of 19~64 years) living in 82 counties from the 2013 Korean Community Health Survey (KCHS) was analyzed. Individual and social correlates from KCHS and physical environmental data from the Korean Statistical Information Service were collected. A multilevel logistic regression was performed using Stata 10.0 IC. Results: Multilevel analyses showed that the effect of social and physical environmental on engaging in moderate or vigorous physical activity (MVPA) was significant in comparison to the influence of individual correlates. The individual factors that were associated with participating in MVPA included gender, marital status, education, job, and household income. In the community level, social environmental factors associated with engagement in MVPA were higher satisfaction with healthcare service (OR=3.410, 95% CI=1.109~11.269), a high level of social support (OR=5.920, 95% CI=1.459~22.657) and social network (OR=1.025, 95% CI= 1.017~1.032). Conclusion: To promote moderate or vigorous physical activity in Korean adults in some counties, social environmental factors should be considered along with individual correlates.

일부지역 초등학생 아침 결식과 식사 관련 요인 분석 (Factors Associated with Breakfast Skipping in Elementary School Children in Korea)

  • The objective of this study was to examine relationships between breakfast skipping and ecological factors related to eating practice. Participants were 537 children (male: 274, female: 263) from two elementary schools in Kyunggi-do (School A) and Seoul (School B). Analysis included cross-tabulation of demographics, factors related to eating practice and weight status, frequency of breakfast skipping and types of breakfast and the number of foods at breakfast by schools. Logistic regression were conducted to identify the factors associated with breakfast skipping. Children in school B showed higher socioeconomic status by living environment and the type of fathers' job than those in school A. Eighty six percent of children in school B and 75% in school A ate breakfast 5 times and more per week. School and father's occupation differences correlated with the frequency of breakfast, but not mother's employ status was not. After controlling school. type of father's job, mother's employ status, eating breakfast 5 times and more was associated with eating with other family members, feeling hungry before breakfast, normal weight status, eating Korean traditional meal type and number of food eaten at breakfast. The results stress the need for intervention programs aimed at decreased skipping breakfast among elementary school children. While programs need to reach all children skipping breakfast, approaches need to be suitable to in particular those from low socioeconomic backgrounds.

대장암 조기 검진 행위에 영향을 미치는 요인 -제4차 2기(2008년) 국민건강영양조사 자료를 중심으로- (Factors Affecting Colorectal Cancer Screening Behaviors : Based on the 4th Korea National Health and Nutrition Examination Survey)

  • Objectives: This study aims to identify the factors associated with colorectal cancer screening behaviors. Methods: The nation-wide representative samples of 2,928 adults aged ${\geq}50$ years for colorectal cancer screening were derived from the fourth Korea National Health and Nutrition Examination Survey (KNHANES IV). This study investigated socio-demographic, health behavioral and contextual factors associated with colorectal cancer screening using descriptive statistics and multivariate logistic regression analysis. Results: In terms of socio-demographic factors, gender, age, marital status, occupation, monthly income, and resident region were significantly different between screening group and non-screening group. Among health behavioral and contextual factors, regular physical checkup, weight control, physical activity, smoking, drinking and having other cancers were significantly different. From the multivariate logistic regression analysis, marital status, education level, regular physical checkup and weight control were associated with colorectal cancer screening behavior. Conclusions: It is necessary to understand the importance of early detection and cancer screening. Appropriate health education and active promotion about the cancer screening should be developed based on the study findings in order to motivate people to have cancer screening. Also, these findings should be reflected in the health policy.

Factors Associated with the Use of Gastric Cancer Screening Services in Korea: The Fourth Korea National Health and Nutrition Examination Survey 2008 (KNHANES IV)

  • Objective: Despite government efforts to increase participation in gastric cancer screening, the rate is still suboptimal in Korea. Therefore, we explored barriers to and predictors of gastric cancer screening participation among a nationally representative sample. Methods: We used the Health Interview Survey sub-dataset derived from the Fourth Korean National Health and Nutrition Examination Survey 2008 (KNHANES IV) to evaluate participation in gastric cancer screening and factors associated with attendance in individuals age ${\geq}40$ years. We enrolled 4,464 subjects who completed the questionnaire and were not previously diagnosed with gastric cancer. Four groups of factors were considered potential predictors of gastric cancer screening in a multivariate analysis: sociodemographic, health behavior, psychological and cognitive, and dietary factors. Results: Overall, 41.3% complied with the gastric cancer screening recommendations. Younger age, lower education level, living without a spouse, frequent binge drinker, and current smoker were significantly associated with less participation in gastric cancer screening. Conclusions: To improve participation in gastric cancer screening, more focused interventions should be directed to vulnerable populations, such as groups with low socioeconomic status or unhealthy behavior. In addition, there should be new promotional campaigns and health education to provide information targeting these vulnerable populations.

청소년 흡연과 관련된 요인 -서울시 일부 남녀 고등학생을 대상으로- (Factors related with the adolescent cigarette smoking)

  • The purpose of this study is to examine the associated risk factors for adolescent cigarette smoking. In February 1995, a total of 1793 students from 17 general high schools in Seoul were assessed with a self-completing questionnaire pertaining current smoking status and school.family.peer environmental factors. The results are as follows; 1. The overall proportion of current smoker among students in the study was 17.3%; 27.7% in males and 6.6% in females. 2. There was significant association between smoking status and all school environmental factors (ranks at school, satisfaction at school, study hours after school, extracurricular activity) examined. 3. Siblings smoking in males and family structure in females were significantly associated with the smoking status of students. 4. Functional aspect of family environmental factors, such as APGAR score, parental supervision, attachment to father or to mother were related to smoking status of students. 5. Association with friends who smoke was significantly associated with smoking status of students. 6. In males, association with friends who smoke, ranks at school, siblings smoking were significant positive predictors and APGAR score, parental supervision, attchment to mother were significant negative predictors. In females, association with friends whosmoke, ranks at school, satisfaction at school were significant positive predictors and parental supervision, attchment to father were significant negative predictors. These findings suggest that strategies that influence smoking behavior need to be directed not only to the individual student but also to their peer group, family and school environment.

  • The psychosocial stress and musculoskeletal disorders(MSDs) have been one of major health problems for hospital workers. This study tried to understand the relationship between symptoms associated with MSDs and risk factors such as working posture, job stress, psychosocial stress and fatigue. A total number of 655 hospital workers participated in this study. Specifically, REBA was applied for evaluating working posture and a checklist prepared by KOSHA(Korean Occupational Safety and Health Agency) was used for symptom survey. A questionnaire from KOSHA was also used for collecting data associated with job stress, psychosocial stress and fatigue. All these data were formulated and modeled by path analysis which was one of major statistical tools in this study. Specifically, path analysis for the data we collected came up with several major findings. The risk scores from working posture based on REBA had indirect effects via fatigue factor(MFS) as well as direct effects on symptoms. The factors associated with job stress(KOSS) and psychosocial stress(PWI-SF) had significant effects on symptoms. Specifically, indirect effect of job stress factors via fatigue factors(MFS) had bigger than that of direct effect of job stress on symptom.

Association between Upper Extremity Musculoskeletal Disorders and Psychosocial Factors at Work: A Review on the Job DCS Model's Perspective

  • Over years it has been increasingly concerned with how upper extremity musculoskeletal disorders (UEMSDs) are attributed to psychosocial job stressors. A review study was conducted to examine associations between UEMSDs and psychosocial work factors, and to recommend what to consider for the associations. For studies in which the job demand-control-support (DCS) model or its variables were specifically employed, published papers were selected and reviewed. A number of studies have reported relationships between UEMSDs symptoms and psychosocial exposure variables. For example, the findings are: higher numbness in the upper extremity was significantly attributed to by less decision latitude at work; work demands were significantly associated with neck and shoulder symptoms while control over time was associated with neck symptoms; and the combination of high psychosocial demands and low decision latitude was a significant predictor for shoulder and neck pain in a female working population. Sources of bias, such as interaction or study design, were discussed. UEMSDs were shown to be associated with psychosocial work factors in various studies where the job DCS model was addressed. Nonetheless, this review suggests that further studies should be conducted to much more clarify the association between UEMSDs and psychosocial factors.

Risk factors predicting development of complications in 72 dogs with esophageal foreign bodies

  • The aim of this study was to determine predictive risk factors implicated in complications in dogs with esophageal foreign bodies. Medical records of 72 dogs diagnosed with esophageal foreign bodies by endoscopy were reviewed retrospectively. Factors analyzed included age; breed; gender; body weight, location, dimension, and type of foreign body; and duration of impaction. To identify risk factors associated with complications after foreign body ingestion, categorical variables were analyzed using the chisquare or Fisher's exact tests and multivariate analysis, as appropriate. Complications secondary to esophageal foreign body ingestion included megaesophagus, esophagitis, perforation, laceration, diverticulum, and pleuritis. Univariate analysis revealed that the location and duration of impaction after foreign body ingestion were associated with an increased risk of esophageal laceration and perforation. Multivariate analysis showed that age, duration of impaction, and foreign body dimension were significant independent risk factors associated with the development of complications in dogs with esophageal foreign bodies. In conclusion, these results showed that longer duration of impaction and larger foreign body dimensions may increase the risks of esophageal laceration, perforation, and plueritis in dogs.
