  • The aim of this study is to determine whether the 65 competencies, defined in "the role of Korean doctor, 2014", are suitable for basic medical education phase in Korea. It seeks to analyze the gap among the educational reality and expectation, assessment situation of the competencies above. We also try to define issues of the development, application and assessment phases of competency-based medical education (CBME) at individual medical schools. We designed survey tools based on the Miller's assessment pyramid (knowledge, explanation, demonstration, and performance) for each of the 65 competencies. The survey distributed to 41 medical schools in April 2015 and 38 replies were received (92.7%). Competencies that matched both the educational reality and expectation were numbers 1 (patient care), 33 (social accountability), and 49 (professionalism). However, all the other competencies ranked lower in current status than that of desirable level. 54 out of 65 competencies (83.07%) remained at the knowledge and explanation level when each competence were assessed. In the development, application, and assessment phases of CBME, common issues that medical schools commonly noted were difficulty linking competencies to curriculum preparation and student assessment, the lack of faculty's understanding, difficulty to reach consensus among faculties, and absence of teaching and learning methods and assessment tools that fits CBME. For the successful settle down of CBME, there is a need for efforts to develop the model of graduate outcomes, to share information and experience, and to operate faculty development program by the medical education communities.

신생아중환자실 간호사가 인식한 간호업무의 중요도와 수행 자신감 (Analysis of the importance of nursing care and performance confidence perceived by nurses in the neonatal intensive care unit)

  • Purpose: Neonatal nurses are expected to have clinical competency to provide qualified and safe care for high-risk infants. An educational intervention to enhance nurses' clinical competence is often a priority in the nursing field. This study was conducted to explore nurses' perceived importance and performance confidence of nursing care activities in neonatal intensive care units. Methods: One hundred forty-one neonatal nurses from seven hospitals across South Korea participated in the online survey study. The scale of neonatal nursing care activity consisted of 8 subdomains including professional practice (assessment, diagnosis, planning, intervention, evaluation, education, research, and leadership). The Importance-Performance Matrix was used to analyze the importance of and confident performance in each of the nursing subdomains. Results: Both importance and performance confidence increased as nurses' age (p=.042 and p<.001) and clinical experience (p=.004 and p<.001). Participants scored relatively higher in importance and performance confidence in the professional practice subdomains (assessment, intervention, evaluation), but scored lower in the education and research subdomains. Conclusion: To provide evidence-based nursing care for high-risk infants in neonatal intensive care units, educational interventions should be developed to support nurses based on the findings of the research.

조기진통 사정 알고리즘은 실습 시 조기진통 관련 지식, 임상수행자신감, 교육만족도에 유효한가?: 유사실험 연구 (Does a preterm labor-assessment algorithm improve preterm labor-related knowledge, clinical practice confidence, and educational satisfaction?: a quasi-experimental study)

  • Purpose: Preterm birth is increasing, and obstetric nurses should have the competency to provide timely care. Therefore, training is necessary in the maternal nursing practicum. This study aimed to investigate the effects of practice education using a preterm-labor assessment algorithm on preterm labor-related knowledge and clinical practice confidence in senior nursing students. Methods: A pre-post quasi-experimental design with three groups was used for 61 students. The preterm-labor assessment algorithm was modified into three modules from the preterm-labor assessment algorithm by March of Dimes. We evaluated preterm labor-related knowledge, clinical practice confidence, and educational satisfaction. Data were analyzed with the paired t-test and repeated-measures analysis of variance. Results: The practice education using a preterm-labor assessment algorithm significantly improved both preterm labor-related knowledge and clinical practice confidence (paired t=-7.17, p<.001; paired t=-5.51, p<.001, respectively). The effects of the practice education using a preterm-labor assessment algorithm on knowledge lasted until 8 weeks but decreased significantly at 11 and 13 weeks after the program, while the clinical practice confidence significantly decreased at 8 weeks post-program. Conclusion: The practice education using a preterm-labor assessment algorithm was effective in improving preterm labor-related knowledge and clinical practice confidence. The findings suggest that follow-up education should be conducted at 8 weeks, or as soon as possible thereafter, to maintain knowledge and clinical confidence, and the effects should be evaluated.

Effects on the Use of Two Textbooks for Four Types of Classes in a South Korean University

  • This paper determined students' ranks of difficulty on the use of materials in terms of 1) understanding the layout of the learning materials, 2) reading comprehension of the learning materials, and 3) realization on relevance to needs of the learning materials. It also determined students' 4) rank and frequency of attitude on the materials. With the data gathered through 128 survey questionnaires, 7 focused group discussions, and 10 interviews, the results were found out that there was an inappropriate assessment procedure set by this particular university. The researcher concludes that: 1) design of four types of classes by just using the two textbooks with their respective workbooks is grammar-based with limited conversation activities; 2) placement for these students in one big class size was implemented without considering their common interest and motivation and language levels; and, 3) qualification of teachers teaching these EFL students did not support students' real needs and the language program itself. Content professors who were made to teach may have the ability to input learning, but their teaching styles may differ from the ones who are real English teachers. This paper then recommends that teachers and school administration should have an appropriate placement exam before students attend the class, especially in a big class size. There could only be a few problems among students in one big class size when students' level of competence is proportioned. With this, topics and conversation activities can even be more flexible with the maneuver of art of questioning, various dimensions of thinking, strategic competence, learning attitude or behavior, etc. to ensure sustenance of communicative mode and level of interest and motivation in the classroom. Grammar-based instruction can only be taught when a need arises. Thus, the course description of each class will be able to transact the objectives ready for developing students' communication competence. Moreover, proper measurement can be utilized to validly assess the amount of students' learning and the progress of language curriculum design in terms of materials selection and teaching approach.

IT서비스에 있어서 기획자, 디자이너, 개발자의 업무 및 역할 상호 평가 비교 연구 (Reciprocal Job and Role Assessments of Planners, Designers, and System Developers of IT Services)

  • In the rapidly changing era of knowledge revolution, user-centered IT services are emerging as a very important component of modern business. However, in order to lead IT services into success, traditional capabilities and competences are not good enough. Development of IT services involve service planners and designers as well as traditional systems developers. This detailed segmentation of job and corresponding competences among involved in IT service development brings in new type of conflicts and contradictions that may require special attention for IT services to be properly development and implemented. This study aims to explore and define competences and roles of newly emerging job groups in IT services: planners, designers, and developers. In order to identify underlying competences of these emergeing groups, two stage interviews were conducted. At the first stage, general competence framework is developed across these groups with different skills for similar competence catogories. Using the categories developed at the first stage, members of each groups were asked to rate and assess the competences of other groups. Comparisons of these reciprocal assessment revealed the conceptual differences and biases across these groups. Detail differences are discussed and implications are discussed.

종합병원 간호사의 문화적 역량수준과 영향요인 (A Study on Factors Affecting Cultural Competency of General Hospital Nurses)

  • Purpose: As Korea has been rapidly transformed to be a multicultural society, it becomes essential for nurses to develop cultural competency. The purpose of this descriptive study was to assess the cultural competency level of general hospital nurses and to examine its contributing factors adopting ecological model. Method: A convenience sample of 327 nurses from six general hospitals in Seoul metropolitan area was recruited between November 14 to 28, 2011. Cultural competency was measured using the Korean version of the Caffrey Cultural Competence in Healthcare Scale (CCCHS) and Cultural Competence Assessment(CCA). Hierarchical multiple regression analysis was performed to assess the influence of intrapersonal, cultural-experience, and organizational factors on cultural competency. Results: Mean cultural competency in this study was 2.57(${\pm}0.43$). Cultural competency was associated with job position (p=.044) at the intrapersonal level. At the cultural experience level, foreign language classes experience (p=.005), fluency in a foreign language (p=.000), experience of working with foreign health care professionals (p=.000) were associated cultural competency. At the organizational level, training program (p=.000), job description (p=.002), health education materials (p=.016), insufficient job control (p=.000), and interpersonal conflicts (p=.011) showed significant difference. Foreign language class experience, foreign language fluency, experience of working with foreign health care professionals, and job control were the most consistent predictors of cultural competence and explained 20.4% of the total variance in the proposed ecological model. Conclusions: The findings suggest that to enhance cultural competency in general hospital nurses, cultural experience and organizational factors need to be more fully considered than intrapersonal factors.

생활양식과 건강수준간 상관성분석 -간호사와 비간호사 비교연구- (Correlations among Lifestyle and Health Status -A comparative study between nurses and clients-)

  • Health could be maintained and promoted by pursuing an active healthy lifestyle. Life-style include health habits and behavior pattern such as exercise, diatry change, weight control, stress management et al. The objectives of this studies are (1) to analyze recognition and practice of lifestyle between nurses and non-nurses, (2) to analyze health status of clients which presented healthy lifestyle, (3) to analyze factors that affected healthy lifestyle. The lifestyle assessment questionnaire is divided into ten sections: competence in selfcare, nutritional practices, physical activity, sleep patterns, stress management, self-actualization, sense of purpose, relation with others, environmental control, and use of health care system. The major results are as follows : (1) The level of recognition and practice of lifestyle was not high. Nurses showed more higher score than non nurses in lifestyle area such as competence in selfcare, stress management, environmental control, and use of health care system. (2) Good health status and lifestyle presented positive relation. (3) In mutiple classifiction analysis, competence in selfcare, nutritional practices, physical activity, sleep patterns, stress management, environmental control, and use of health care system had significant relation to independent variables. (4) Change of concept for healthy lifestyle after this survey was higher in nurses group and the most concern area was stress management. To sum up, lifestyle pattern of nurses as health professional was not desirably high. Therefore nurses ownself should effort to practice healthy lifestyle prior to others, and then educate importance of lifestyle for health promotion and disease prevention. In conclusion, it will be useful to consider significant lifestyle factors that was be identified in this study to develop health promotion program.

대학생의 건강다이어트 역량 측정도구 개발 (Development of Health Dieting Competency Scale for College Students)

  • Purpose: This study was to develop a Health Dieting Competency (HDC) scale to assess self-rated health dieting competence of college students. Methods: The scale was developed as follows: items generated, and tests of validity and reliability. Items were developed through a literature review, review of instruments, and interviews. A panel of four experts reviewed the scale for content validity. Factor analysis, Pearson correlation, descriptive statistics and Cronbach's ${\alpha}$ were used to analyze the data (N=183). Results: In the preliminary stage, thirty-three items were generated. Final items were fourteen that were selected through exploratory factor analysis. The HDC scale was consisted with five factors and fourteen items that were Goal-Directed (5-items), Health Support (2-items), Health Self-Efficacy (3-items), Health Perception (2-items), and Escaping from Health (2-items). The internal consistency of HDC as measured by Cronbach's ${\alpha}$ was .78. Conclusion: Content validity, construct validity, and reliability of the HDC were established. The HDC scale is a reliable and valid instrument for early adulthood youth who are willing to assess health dieting competency.

예비 가정과교사의 메타버스를 활용한 교수설계 역량 증진을 위한 수업 사례연구 (A Case Study on the Promotion of Instructional Design Competencies Among Preservice Home Economics Teachers in Class Using the Metaverse)

  • This paper presents a case study in which a lesson was designed and applied to enhance preservice home economics teachers' instructional design competence using the metaverse. For 15 weeks from March 6 to June 14, 14 students enrolled on the course 'Multimedia Home Economics Education' for preservice home economics teachers used the metaverse to engage in problem-based learning to solve instructional design problems in home economics classes. In accordance with the ADDIE design model, in the analysis stage, we assessed preservice home economics teachers' level of knowledge of lessons using the metaverse, and their perceptions of the possibility, necessity, and usefulness of using the metaverse in home economics lessons. In the design and development stage, lesson plans, questions for problem-based learning, assessment tools, and teaching and learning materials were developed. The implementation was conducted in parallel with training on understanding multimedia and the metaverse, and instructional design competence was evaluated through pre- and post-testing and reflection journals. The results revealed that the preservice home economics teachers acquired a good understanding of lessons using the metaverse, learned how to design lessons for self-directed learning by applying the metaverse to their home and classroom, and gained confidence in applying it to their teaching practice or in-service work. It is expected that the results of this study will be used as support materials for prospective and current home economics teachers to design home economics lessons using the metaverse, thereby expanding the horizons of home economics education.

간호대학생을 위한 시뮬레이션 기반의 아동간호 인수인계에 관한 동영상 실습교육과 시뮬레이션 실습교육의 효과 (Utilizing Video vs Simulation Practice for Handoff Education of Nursing Students in Pediatric Nursing)

  • Purpose: The purpose of this study was to develop a model for handoff education for nursing students based on simulation using video and to identify educational effects of a simulated situation in pediatric care units. Methods: Data were collected from May 1 to 30, 2016. Participants were 84 senior nursing students in Seoul (video group: 43, simulation group: 41). Both groups were given a lecture and pre-briefing on handoff education. The simulation group had nursing practice on resolving health issues for respiratory distress using a high-fidelity baby simulator. The video group watched a video recording of a scenario based simulation, and used a summarized handoff situation to practice patient handoff to another student. Results: There was no significant difference between the two groups for handoff self-confidence, problem solving ability, handoff competence (self-assessment of students), or learning satisfaction. Self-confidence increased significantly in both groups. Handoff competency evaluated by the instructor was higher in the video group compared to the simulation group (t=2.33, p=.022). Conclusion: Nursing student education for handoff practice utilizing a video in the pediatric unit was more cost effective. Therefore, it could be a useful educational method for students in learning patient handoff practices and helpful for related research.
