Germ Cell Aspiration (GCA) Method as a Non-fatal Technique for Sex Identification in Two Bivalves (Gomphina veneriformis and Tegillaca granosa)

  • This study attempted to verify the possibility of using germ cell aspiration (GCA) method as a non-fatal technique in studying the life-history of equilateral venus, Gomphina veneriformis (Veneridae) and granular ark, Tegillarca granosa (Arcidae). Using twenty-six gauge 1/2" (12.7mm) needle, GCA was carried out in equilateral venus through external ligament. In granular ark, GCA was performed by preventing closure of the shells by inserting a tongue depressor between the shells while still open. The success rate of sex identification using the GCA method was 95.6% for the equilateral venus (n=650/680) and 94.3% for the granular ark (n=707/750). Mortality of equilateral venus, which spent 33 days under wild conditions, was 13.8% (n=90/650) while the mortality of granular ark, which spent 390 days under wild conditions, was 2.4% (n=17/707). Although we believe that GCA does not appear to cause death in equilateral venus or granular ark, the success rate in employing of this methodology may differ depending on the level of proficiency of the researcher and reproductive stage of the bivalve. This study concludes that GCA is a convenient non-fatal methodology, which can be employed to identify sex and investigate gonadal maturity in Gomphina veneriformis and Tegillarca granosa.

Clinical Importance of Peak Cough Flow in Dysphagia Evaluation of Patients Diagnosed With Ischemic Stroke

  • Objective To investigate the relationship between peak cough flow (PCF), pulmonary function tests (PFT), and severity of dysphagia in patients with ischemic stroke. Methods This study included patients diagnosed with ischemic stroke, who underwent videofluoroscopic swallowing study (VFSS), PCF and PFT from March 2016 to February 2017. The dysphagia severity was assessed using the videofluoroscopic dysphagia scale (VDS). Correlation analysis of VDS, PFT and PCF was performed. Patients were divided into three groups based on VDS score. One-way ANOVA of VDS was performed to analyze PCF, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and age among the different groups. Results The correlation coefficients of VDS and PCF, VDS and FVC, and VDS and FEV1 were -0.836, -0.508, and -0.430, respectively, all of which were statistically significant at the level of p<0.001. The one-way ANOVA indicated statistically significant differences in PCF, FVC, FEV1, and age among the VDS groups. Statistically significant differences in VDS and age were observed between aspiration pneumoia and non-aspiration pneumonia groups. Conclusion Coughing is a useful factor in evaluating the risk of aspiration in dysphagia patients. Evaluation of respiratory and coughing function should be conducted during the swallowing assessment of patients with ischemic stroke.

  • 전 세계적으로 혁신과 창업기업의 스케일업이 중요한 국가적 과제로 부각되고 있다. 과거 경기침체 극복을 위해 'Start-tup America', 'Start-up Chile', 'Start-up Britain' 등 스타트업 중심의 정책 패러다임을 확산시켰다. 최근 창업선진국을 중심으로 경기회복이 가시화되면서, 스타트업 중심의 정책 패러다임이 'Scale-up America', Scale-up UK', 'Scale-up Denmark' 등 스케일업 중심의 정책 패러다임으로 전환하고 있는 추세이다. 창업기업은 스타트업 단계를 넘어서 스케일업 단계로 진입해야 국가적으로 양질의 일자리가 늘어나고 경제성장으로 나아갈 수 있다. 그러므로 창업기업을 강한 중견기업으로 성장시켜 지속적으로 생존할 수 있는 발판을 마련해주어야 한다. 이에 따라 본 연구는 창업기업이 스케일업 기업으로 성장할 수 있도록 성장의 선행변수인 성장 열망에 영향을 미치는 요인들을 심층적으로 고찰하고 국가의 경제발전단계 간, 기업가 간의 차이를 실증적으로 규명함으로써 국가차원에서 창업기업이 스케일업 할 수 있는 방안을 제시하는데 목적이 있다. 본 연구는 글로벌기업가정신모니터(GEM)의 일반성인조사 3.5년 미만의 초기 기업가들을 대상으로 하였으며, 국가경제발전단계를 혁신주도형, 효율주도형, 요소주도형 경제로 구분하여 R 3.5.0프로그램을 사용하여 국가경제발전단계 간 비교를 위해 다층모형분석을 통해 연구가설을 검증하였다. 가설검증 결과는 다음과 같다. 창업기업의 혁신지향성과 성장 열망의 영향관계에 있어 국가경제발전 간, 기업가 간 차이가 있는 것으로 나타났다. 국가의 경제가 발전할수록 기업가의 혁신활동이 더 두드러지게 나타난다. 또한 창업기업은 기업가의 영향을 많이 받기 때문에, 동일한 환경 내에서 기업가가 얼마나 혁신 지향적인가에 따라 열망의 정도에 차이가 있다고 할 수 있다. 창업기업의 혁신지향성과 성장 열망의 영향관계에 있어 실패에 대한 두려움은 국가경제발전 간, 기업가 간 차이가 있는 것으로 나타났다. 기업가 및 창업에 대한 실패에 대한 두려움은 국가경제발전 단계별로 차이가 있으며, 이러한 국가적 측면에서 실패에 대한 인식은 창업활동에 영향을 미치는 중요한 요인 중 하나이다. 이러한 연구결과를 통해 규명된 창업기업이 스케일업 기업으로 성장하는데 있어 성장열망에 영향을 미치는 검증된 요인을 국가경제발전단계에 따라 적합한 스케일업 생태계를 조성하는데 활용할 수 있을 것으로 기대한다.

연하 곤란이 동반된 급성 뇌졸중 환자를 위한 장관 영양 프로토콜 개발 및 효과 (Development and Evaluation of an Enteral Nutrition Protocol for Dysphagia in Patients with Acute Stroke)

  • Purpose: The study was done to develop an evidence-based enteral nutrition (EN) protocol for effective nutritional support for dysphagia in patients with acute stroke, and to evaluate effects of this protocol on clinical outcomes. Methods: A methodological study was used to develop the EN protocol and a quasi-experimental study to verify the effectiveness of the protocol. The preliminary EN protocol was drawn by selecting recommendations from previous well-designed EN guidelines, and then developing additional recommendations based on high-quality evidence. Content validation was assessed by an expert group, and clinical applicability by care providers and patients. The scale-level content validity index of the final EN protocol was 0.99. Assessment was done of differences in percentage of caloric goals achieved and presence of undernutrition, aspiration pneumonia, and gastrointestinal (GI) complications after application of the EN protocol. Results: In the EN protocol group, the percentage of caloric goals achieved ($R^2=.24$, p=.001) and the reduction of GI complications (p=.045) were significantly improved, but the presence of undernutrition (p=.296) and aspiration pneumonia (p=.601) did not differ from the usual care group. Conclusion: Results indicate that the new EN protocol for dysphagia in patients with acute stroke significantly increased their nutritional intake and reduced GI complications.

Ultrasound-guided Evacuation of Spontaneous Intracerebral Hemorrhage in Basal Ganglia

  • Objective: Ultrasound can be used in the treatment of large intracerebral hematoma. The authors present our experiences with Ultrasound-guided catheter placement for lysis and drainage of ganglionic hematoma, with emphasis on technical aspects. Methods: The authors applied real-time ultrasonography for the aspiration of intracerebral hematoma in 6cases. Ultrasound-guided aspiration via a burrhole was performed under local anesthesia. We selected a temporal entry point instead of the frequently used precoronal approach in ganglionic hematoma. A burrhole was made 4 to 6cm posterior from posterior border of frontal process of the zygomatic bone at the level of 4 to 5cm above the external auditory meatus. Results: In all patients, the catheter was placed accurately into the hematoma target. All patients were irrigated with urokinase once to three times a day. The catheter could be removed within two or three days. The mean hematoma volume was reduced from initially 32mL to 5mL in an average of two days. There were no intraoperative complications related to the use of real-time ultrasonography and no postoperative infections were noted. Conclusion: Ultrasound allows an easy and precise localization of the hematoma and the distance from the surface to the target can be calculated. Ultrasound-guided catheter placement for fibrinolysis and hematoma drainage is a simple and safe procedure.

  • Decision environments involve a high degree of uncertainty as well as multiple, conflicting goals. Although traditional goal programming offers a means of considering multiple, conflicting goals and arrives at a satisficing solution in a deterministic manner, its major drawback is that decision makers often specify aspiration level of each goal as a single number. To overcome the problem of setting aspiration levels, chance constrained programming can be incorporated into goal programming formulation so that sampling information can be utilized to describe uncertainty distribution. Another drawback of goal programming is that it does not provide a systematic approach to set priorities and trade-offs among conflicting goals. To overcome this weekness, the analytic hierarchy process(AHP) is used in the model. Also, most goal programming models in the literature are of a linear form, although some nonlinear models have been presented. Consideration of risk in technological coefficients and right hand sides, however, leads to nonlinear goal programming models, which require a linear approximation to be solved. In this paper, chance constrained reformulation with linear approximation is presented for a 0-1 goal programming problem whose technological coefficients and right hand sides are stochastic. The model is presented with a numerical example for the purpose of demonstration.

뇌졸중 후 연하곤란 사정과 중재 체크리스트 개발 및 활용정도 (Development and Utilization of Assessment and Intervention Checklist for Post-stroke Dysphagia)

  • Purpose: Purposes of this study were to develop a reliable and valid checklist to assess and manage post-stroke dysphagia and to identify its utilization. Methods: The first step was to develop checklist through systematic literatures reviews, to test the content validity from 11 clinical experts, and to test the construct validity and the internal consistency from 97 patients with post-stroke dysphagia. The second step was to identify clinical use of the checklist from the same 97 patients. Results: A total of 18 items compromised the assessment checklist and 64 items for intervention checklist. Internal consistencies of assessment checklist and intervention checklist were .84 and .98, respectively. In both checklists, level of utilization of general appearance was the highest, while that of early signs of aspiration was the lowest. The utilization levels of two checklists were significantly higher among patients who were less than 60 years old, and had liquid diet, facial paralysis, and previous history of aspiration pneumonia. Conclusion: We found that the checklists were reliable and valid. Further study is needed to develop specific strategies to improve nurses' use of assessment and intervention checklists for post-stroke dysphagia.

Using Korean Phonetic Alphabet (KPA) in Teaching English Stop Sounds to Koreans

  • In the phoneme level, English stop sounds are classified with the feature of 'voicing': voiceless and voiced (p/b, t/d, k/g). But when realized, a voiceless stop is not alwats the same sound. For example, the two 'p' sounds in 'people' are different. The former is pronounced with much aspiration, while the latter without it. This allophonic differnece between [$P^h$] and [p] out of an English phoneme /p/ can be well explained to Koreans because in Korean these two sounds exist as two different phonemes {/ㅍ/ and /ㅃ/ respectively). But difficulties lie in teaching the English voiced stop sounds (/b, d, g/) to Koreans because in Korean voiced stops do not exist as phonemes but as allophones of lenis sounds (/ㅂ, ㄷ, ㄱ/). For example, the narrow transcription of '바보' (a fool) is [baboo]. In the word initial position, Korean lenis stops are pronounced voiceless and even with a slight aspiration while in the inrervocalic environments they become voiced, That is in Korean voiced stops do not occur independently and neither they have their own letters. To explain all these more effectively to Koreans, it is very helpful to use Korean Phenetic Alphabet (KPA) which is devised by Dr. LEE Hyunbok (a professor of phonetics at Seoul National Univ. and chairman of Phonetic Society of Koera.)(omitted)

Exploring the Role of Preference Heterogeneity and Causal Attribution in Online Ratings Dynamics

  • This study investigates when and how disagreements in online customer ratings prompt more favorable product evaluations. Among the three metrics of volume, valence, and variance that feature in the research on online customer ratings, volume and valence have exhibited consistently positive patterns in their effects on product sales or evaluations (e.g., Dellarocas, Zhang, and Awad 2007; Liu 2006). Ratings variance, or the degree of disagreement among reviewers, however, has shown rather mixed results, with some studies reporting positive effects on product sales (e.g., Clement, Proppe, and Rott 2007) while others finding negative effects on product evaluations (e.g., Zhu and Zhang 2010). This study aims to resolve these contradictory findings by introducing preference heterogeneity as a possible moderator and causal attribution as a mediator to account for the moderating effect. The main proposition of this study is that when preference heterogeneity is perceived as high, a disagreement in ratings is attributed more to reviewers' different preferences than to unreliable product quality, which in turn prompts better quality evaluations of a product. Because disagreements mostly result from differences in reviewers' tastes or the low reliability of a product's quality (Mizerski 1982; Sen and Lerman 2007), a greater level of attribution to reviewer tastes can mitigate the negative effect of disagreement on product evaluations. Specifically, if consumers infer that reviewers' heterogeneous preferences result in subjectively different experiences and thereby highly diverse ratings, they would not disregard the overall quality of a product. However, if consumers infer that reviewers' preferences are quite homogeneous and thus the low reliability of the product quality contributes to such disagreements, they would discount the overall product quality. Therefore, consumers would respond more favorably to disagreements in ratings when preference heterogeneity is perceived as high rather than low. This study furthermore extends this prediction to the various levels of average ratings. The heuristicsystematic processing model so far indicates that the engagement in effortful systematic processing occurs only when sufficient motivation is present (Hann et al. 2007; Maheswaran and Chaiken 1991; Martin and Davies 1998). One of the key factors affecting this motivation is the aspiration level of the decision maker. Only under conditions that meet or exceed his aspiration level does he tend to engage in systematic processing (Patzelt and Shepherd 2008; Stephanous and Sage 1987). Therefore, systematic causal attribution processing regarding ratings variance is likely more activated when the average rating is high enough to meet the aspiration level than when it is too low to meet it. Considering that the interaction between ratings variance and preference heterogeneity occurs through the mediation of causal attribution, this greater activation of causal attribution in high versus low average ratings would lead to more pronounced interaction between ratings variance and preference heterogeneity in high versus low average ratings. Overall, this study proposes that the interaction between ratings variance and preference heterogeneity is more pronounced when the average rating is high as compared to when it is low. Two laboratory studies lend support to these predictions. Study 1 reveals that participants exposed to a high-preference heterogeneity book title (i.e., a novel) attributed disagreement in ratings more to reviewers' tastes, and thereby more favorably evaluated books with such ratings, compared to those exposed to a low-preference heterogeneity title (i.e., an English listening practice book). Study 2 then extended these findings to the various levels of average ratings and found that this greater preference for disagreement options under high preference heterogeneity is more pronounced when the average rating is high compared to when it is low. This study makes an important theoretical contribution to the online customer ratings literature by showing that preference heterogeneity serves as a key moderator of the effect of ratings variance on product evaluations and that causal attribution acts as a mediator of this moderation effect. A more comprehensive picture of the interplay among ratings variance, preference heterogeneity, and average ratings is also provided by revealing that the interaction between ratings variance and preference heterogeneity varies as a function of the average rating. In addition, this work provides some significant managerial implications for marketers in terms of how they manage word of mouth. Because a lack of consensus creates some uncertainty and anxiety over the given information, consumers experience a psychological burden regarding their choice of a product when ratings show disagreement. The results of this study offer a way to address this problem. By explicitly clarifying that there are many more differences in tastes among reviewers than expected, marketers can allow consumers to speculate that differing tastes of reviewers rather than an uncertain or poor product quality contribute to such conflicts in ratings. Thus, when fierce disagreements are observed in the WOM arena, marketers are advised to communicate to consumers that diverse, rather than uniform, tastes govern reviews and evaluations of products.

The Effect of Performance Feedback on Firms' Decision to Form an International Strategic Alliance and Performance in the Korean Manufacturing Industry

  • Purpose - International strategic alliance has been regarded as a strategic decision made by firms' managerial problems and ensure performance growth. From the perspective of the proactive behavior for changing strategies in a global market, this study aims to identify whether performance feedback influences firms' decisions to pursue strategic alliances. This study examines the effects of performance feedback on performance when firms use strategic alliances. Design/methodology - To analyze the impact of performance feedback on forming an international strategic alliance, this study adopt the concept of performance feedback to develop a research model and our hypotheses. Thus, this study used a two-stage least squares unbalanced panel data analysis with random effects. This study is based on 24,543 observations from Korean manufacturing firms from 2007 to 2016. Findings - The results show that firms pursue the formation of strategic alliances more actively, if their past financial and R&D performance are lower than their aspiration level, based on the result of performance feedback. An in split sample analysis for examining the effect of a firm's technology sophistication based on the OECD's classification, negative innovation performance discrepancy has positive effects on the probability of international alliance in high-tech and medium-high-tech industries. Financial performance also improves when a firm decides to form a strategic alliance based on the results of performance feedback. Originality/value - This research extends recent efforts to better understand the effect of performance feedback on firms' performance when they use strategic alliances. These findings suggest that the CEOs and managers of firms should consider the performance feedback perspective when deciding to pursue a strategic alliance to improve performance. In other words, the decision-makers in a firm must analyze and consider various complex variables inside and outside the firm and expand such subjects of examination to more complex and dynamic factors.
