Foreign Body Aspiration during Dental Treatment in the Disabled Patient (장애인 치과 치료시 발생한 이물질 흡인)

  • There are a lot of dental emergency situation. Especially on disabled or pediatric patient with negative behavior, accidental aspiration or ingestion of foreign body (ex: dental instrument) is one of the common accidents in dental procedure. Dentists and dental hygienists must have knowledge about the precaution and be ready to deal with foreign body aspiration during dental treatment, especially on the disabled. This is a case of an accident during the dental treatment of 14-year-old female patient with cerebral palsy. During scaling, prolonged retained primary tooth which had resorbed roots was fell into left bronchi. So we tried to remove the crown by endotracheal approach. Most ingested foreign bodies pass through the gastrointestinal tract spontaneously, but some of them need endoscopic or surgical removal. Moreover aspiration into broncho-trachea can be more serious events and must be treated as emergency situation.

  • A mediastinal mass was aspirated by fluoroscope-guided fine needle aspiration biopsy in a 47 years old female patient. The first aspiration smears were not diagnostic, because of hemorrhagic background and cell paucity. On the second aspiration, the smears were composed of some clusters of benign epithelial cells in hemorrhagic back-ground. Cells were arranged in mostly solid sheets and tended to form glandular lumina in part. Their nuclei were round and vesicular. Nucleoli were not prominent. These findings were suggestive of benign glandular tissue, which was finally confirmed as mediastinal thyroid gland by open thoracotomy specimen.

  • Thoracic paravertebral block is performed for the treatment of patients with chronic pain, such as complex regional pain syndrome (CRPS) and post-herpetic neuralgia. Thoracic paravertebral block can result in iatrogenic pneumothorax. Because pneumothorax can develop into medical emergencies and needle aspiration or chest tube placement may be needed, early diagnosis is very important. Recently, thoracic ultrasonography has begun to be used to diagnose pneumothorax. In addition, ultrasound-guided aspiration can be an accurate and safe technique for treatment of pneumothorax, as the needle position can be followed in real time. We report a case of iatrogenic pneumothorax following thoracic paravertebral block for the treatment of chronic pain due to CRPS, treated successfully by ultrasound-guided aspiration.

  • Aspiration, the entry of material into the airway below the true vocal folds, has been reported in various type of postsurgical head and neck patients. The important of correctly identifying the cause of aspiration in the head and neck surgical patient. Clearly each cause represents a different physiologic or anatomic disorder which requires different management strategy. We discuss about the cause of aspiration during head and neck treatment.

  • Aspiration of foreign bodies into the tracheobronchial tree is unusual in adults and it may result in Proximal airway obstruction and acute life-threatening asphyxia. It can be diagnosed by history of aspiration or visualizing the foreign body by chest roentgenogram or bronchoscopy. But it is rarely considered in adults with subacute or chronic respiratory symptoms without a definite history which suggests an aspiration. A 70-year-old woman was admitted to the hospital due to productive cough for two months and dyspnea which aggravated since the day before admission. Chest X-ray showed Pneumonic infiltration on left upper lobe and right lower lobe. Despite several days of conventional therapy, the patient complained of severe dyspnea and wheezing. We performed chest CT to rule out endobronchial stenosis, and it revealed the narrowing of left main stem bronchus compatible with endobronchial tuberculosis. Fiberoptic bronchoscopy for confirmation disclosed a $3.2{\times}0.7{\times}0.2$cm sized foreign body located longitudinally at the left main stem bronchus. We removed it with alligator forcep and it proved In be a piece of artificial denture. The patient remembered losing it while severe coughing on the day before admission. The microscopic examination of bronchial washing fluid revealed numerous acid fast bacilli. After removal of the foreign body, the patient showed marked improvement in symptom and pulmonary function test. Here we report a case of Pulmonary aspiration of foreign body which is confused with endobronchial tuberculosis with a review of the literature.

The Effect of Indwelling Silk Suture Following Aspiration in the Treatment of Chronic Lateral Malleolar Bursitis (흡입 후 견 봉합사 거치를 통한 만성 족관절 외과 점액낭염의 치료)

  • Purpose: To evaluate the drainage effect of silk suture following aspiration of the bursa as an early treatment of chronic lateral malleolar bursitis. Materials and Methods: Thirteen cases, which have over two weeks of history and over one year of follow-up, were investigated. The average duration of follow-up was 16.4 months. The average symptom duration before introduction into this study was 7.8 weeks. With an aseptic technique, the aspiration of the bursa was done with 18G needle and syringe and then the insertion of silk suture through the aspiration needle was performed. The amount of drainage was identified two or three times in a week and stitch out was done at the cessation of drainage. Over one year follow-up, recurrence, infection, pain, and limitation of range of motion were investigated by telephone interview. Results: Redness around the insertion site of silk suture was found in all cases, but there was no development of active infection or recurrence. The average duration of treatment is 10.4 days. Conclusion: The drainage with silk suture following aspiration of the bursa is less invasive and very effective method in the early treatment of chronic lateral malleolar bursitis.

  • The study were to examine relationship among creativity personality, career awareness, and occupational aspiration of elementary school students. For these purpose, we analysed the data from 322 elementary school students. Finding revealed that creativity personality positively related to career awareness and occupational aspiration. And creativity personality has significantly affected career awareness and occupational aspiration. Result of structural equation modeling analysis was creativity personality and career awareness were effect to occupational aspiration. However, career awareness had no mediating effect on the relation of creativity personality and occupational aspiration. The implication for career education and counseling practice are discussed based on he result of the present study.

  • 소의 하부호흡기계 질병을 진단하고자 기침과 비루를 주증상으로 하는 31두의 환우와 임상증상을 나타내지 않는 9두의 소에 5 fr. urinary catheter를 이용한 transtracheal aspiration technique를 적용하고 이 방법의 유용성 및 분리된 병원성 세균과 세포상을 임상증상과 관련하여 조사하여 다음과 같은 결과를 얻었다. Transtracheal aspiration technique은 구강내 정상상재균에 오염되지 않은 가검물을 하부호흡기로부터 채취하는데 호흡장애와 합병증을 유발하지 않았다. 총 40두로부터 분리한 세균중 Pasteurella multocida가 48.7%로 가장 많이 분리되었으며 점액 화농성 염증이 40%로 가장 많이 나타났다. 심한 임상증상을 보인 소중 점액화농성 염증이 60%로 가장 많이 나타났으며 Pasteurella multocida가 63.2%로 높게 분리되었다. 경미한 임상증상을 보인 소에서는 염증세포에 따른 세포상이 고루 나타났으며 Pasteurella multocida가 40% 분리되었다. 세포학적 관찰에서는 정상인 경우는 섬모원주상피세포가 다수 관찰되었고 점액성 염증인 경우는 소수의 호중구 및 상피세포가 관찰되었으며 점액화농성 염증인 경우는 다수의 밀집된 호중구, 상피세포 및 점조한 삼출물이 동시에 관찰되었다. 복합세포성 염증에서는 대식구, 호중구, 임파구 및 상피 세포가 혼재하였다. 이상의 결과로부터 transtracheal aspiration technique은 소의 하부호흡기계 질병을 진단하는데 실제 임상에서 응용할 수 있는 쉽고 안전하며 유익한 방법으로 사료된다.

Phonetic Realization of Aspiration of Stops in English /Cr/ and /sCr/ Clusters and their Syllable Structure at the Phonetic Level: a Comparison between Two Speaker Groups (영어의 /Cr/과 /sCr/ 자음군 내 폐쇄음의 기식성 실현과 음성 단위의 음절구조: 두 화자집단 간 비교)

  • This study investigates the acoustic property of aspiration realized in English voiceless stops of /Cr/ and /sCr/ clusters. VOT is measured from stops in these clusters produced by two groups; one from native speakers of English and the other from Korean native speakers. Aspiration of stops in different types of clusters is compared to various phonological factors such as location of stress, syllable type, and position in word. Pursuing the idea that phonetic realization is correlated with phonological representation, attempts are made to account for the gradient nature of aspiration of stops on the basis of syllable structure at the phonetic level, which may vary in the wake of resyllabification. Voiceless stops in /Cr/ and /sCr/ clusters are further compared to results obtained in the previous study on /sC/ cluster. Variations in aspiration are also characterized in terms of segmental precedence relation of stops in the clusters, namely, post-[s], pre-[r], or both.

  • The purpose of this study was to evaluate the drainage effect of silk suture following aspiration of the bursa as an early treatment of chronic olecranon bursitis. Eleven cases, which have over two weeks of history and over one year of follow-up, were investigated. The average duration of follow-up was 17.5 months. The average symptom duration was 1.8 months. With an aseptic technique, the aspiration of the bursa was done with 18gauge needle and syringe and then the insertion of silk suture through the aspiration needle was performed. The amount of drainage was regularly checked $2{\sim}3$ days interval and stitch out was done at the cessation of drainage. At the follow-up, recurrence, infection, pain, and limitation of range of motion were investigated by telephone interview. Redness around the insertion site of silk suture was found in all cases, but there was no active infection or recurrence. The results were satisfactory in all cases and the average time for recovery was 10.5 days. The drainage with silk suture following aspiration of the bursa is less invasive and very effective method in the early treatment of chronic olecranon bursitis.
