A Comparative Study according to Diagnostic Time on Meningitis (무균성 뇌막염에서 증상발현부터 진단까지 걸린 시간에 따른 시기별 유병기간의 검토)

  • Purpose : Aseptic meningitis is relatively frequent in children and caused mostly by enterovirus. The aim of the present study was to determine the effect of early diagnosis (spinal tapping) on symptom duration of childhood aseptic meningitis. Methods : One hundred fifty-three children who were hospitalized due to aseptic menigitis in the Department of Pediatrics St. Benedict Hospital from July 1996 through October 1996 were included in this study. Patients were divided to two groups according to the duration from first symptom onset to diagnosis. Early diagnosis group is diagnosed within 3 days from first symptom onset. Later diagnosis group is diagnosed after 4 days from first symptom onset. Results : 1) The average age of these patients was 4.3 years old in early diagnosis group and 4.1 years old in later diagnosis group. The sex ratio(male: female) was 2.04:1 in early diagnosis group and 2.5:1 in later diagnosis group. 2) The mean duration of diagnosis of this study was 2.04 day in early diagnosis group and 5.12 day in later diagnosis group. 3) The percentage of symptom and sign of the early diagnosis group were fever(100%), headache(88.4%), vomiting(86.9%), abdominal pain(39%), neck stiffness(36.2%), skin rash(18.8%), diarrhea(16.9%) and that of later diagosis group were fever(100%), headache(83.3), vomiting(80.9%), abdominal pain(47.6%), neck stiffness(41.6%), skin rash(29.7%), diarrhea(16.6%). 4) Initial CSF findings revealed leukocyte $146.8{\pm}386.3/mm^3$ with PMNL 38%, protein 32.47mg/dl, sugar 66.23mg/dl in early diagnosis group and leukocyte $458.1{\pm}663.2/mm^3$, protein 31.22mg/dl, sugar 64.21 mg/dl in later diagnosis group. 5) There was no statistically significant differance in the peripheral blood findings between early diagnosis group and later diagnosis group. 6) The duration of disappearance of symptom after spinal tap were 2.3 days in early diagnosis group and 2.24 days in later diagnosis group. Total symptom duration was 4.34 days in early diagnosis group and 7.36 days in later diagnosis group. Conclusions : Our results demonstrate that early diagnosis(early spinal tap) shortened duration of clinical symptoms.

A Case Mollaret Meningitis (Mollaret Meningitis로 진단한 무균성 뇌막염 1례)

  • Mollaret meningitis is a rare syndrome first described by Mollaret in 1944, which is reccurent aseptic meningitis with characteristic clinical and spinal fluid cytologic findings. No etiology has been established. Several authors suggested the association with herpes virus infection, some found intracranial epidermoid tumor eventually in patients diagnosed of Mollaret meningitis. We experienced a case of 14-year old male who had 3 episodes of recurrent aseptic meningitis during four years. The patients initially presented with clinical and laboratory feature of bacterial meningitis, however, extensive serological investigation and cerebrospinal fluid analysis failed to reveal a specific cause. Immune system studies were unremarkable. Cranial computed tomography performed during the attack and magnetic resonance imaging when the patient was asymptomatic were both normal. A tentative diagnosis of Mollaret meningitis was established at the 3rd episode. We report this case with a review of related literatures.

INTERPRETATION OF BACTERIAL CONTAMINATION OF ALLOGENEIC TISSUES OBTAINED FROM CADAVERIC AND LIVING DONORS (조직은행에서 채취한 동종조직의 세균 배양 평가)

  • Thorough screening of donors medical and social history, extensive serological and bacterial screening combined with developed processing and sterilization methods have improved the safety of the allogeneic tissues in recent decades. The risk of bacterial infection through allogenic tissue transplantation is one of the major problems facing tissue banks. The purpose study is to report the contamination rate in 358 retrieved tissues obtained strictly aseptic conditions, between 2001 and 2002 in Korea Tissue Bank. Samples from 9 donors(total 13 donors) were used in blood culture, and in 7 donors the blood culture were negative. Of the 358 tissues cultured in their entirety, 186(52%) were initially culture negative and 177(48%) were positive. Organism low pathogenicity were cultures from 20.2% of the tissues. To minimize the bacterial load, donors should be obtain in operating rooms, using aseptic techniques with only a few personnel for procurement. The procurement cultures from donors and retrieved tissues with multiple should be carefully interpreted. Blood cultures should be taken account, since these can help to find contamination not detect swab culture. A prospective cohort study is needed to determine which of the varied processing and sterilization methodologies gives the best quality.

Pneumonia Incidence Varies by Tracheal Suction Procedures among Caregivers of Patients with Home Mechanical Ventilators (가정용 인공호흡기 적용 신경근육계 희귀난치성 질환자 가족 간호제공자의 기관내 흡인 수행과 폐렴 발생과의 관계)

  • Purpose: This study aimed to survey caregivers of patients with Neuromuscular Rare and Incurable Disease (NRID) using invasive home mechanical ventilator (HMV), in order to compare the incident rate of pneumonia by tracheal suction procedures used. Method: Participants were 99 family caregivers of NRID patients using HMV. Participants were given a questionnaire consisting of 12 demographic items, 10 items about disease and HMV related characteristics, 11 items about tracheal suction procedures, and 2 items about the incidence of pneumonia. Data were analyzed using chi-square tests and t-tests. Results: The items that predicted the incidence of pneumonia were "change of irrigation saline every suction" (p=.047), "use of aseptic catheter every suction" (p=.004), and "instillation of normal saline before suction" (p=.027). In addition, these items were 47.4%, 51.4%, and 38.8% respectively. Conclusion: Family members caring for NRID patients with invasive HMV should be educated about tracheal suction, especially the necessity of changing irrigation saline after every suction, using the aseptic catheter for every suction, and instillation of normal saline before suction. Medical personnel such as home care nurses should periodically check tracheal suction procedures, and re-educate family caregivers when necessary.

Recognition and Performance on Management for Nosocomial Infections among Nursing Students (병원감염관리에 대한 간호학생의 인지도와 수행도)

  • Purpose: This study was designed to identify recognition and the performance level for nosocomial infections amongst student nurses and to provide data for preparing on efficient policy and control program for nosocomial infections. Method: 191 senior nursing student participated in this research from 5 nursing college which allowed data collection, in the city of Seoul. The questionnaire was composed of 73 items(likert scale) about eight areas; aseptic technique, disinfection, precaution, hand washing, urinary tract infection, respiratory infection, catheter related infection, and self care about nosocomial infections. Results: The mean score of recognition and performance level for management of nosocomial infection were 4.29 and 3.41 respectively. The mean score of the recognition level was significantly lower than the performance level in the eight areas. The mean score of both recognition and performance were highest in the area of disinfection. However, the mean score of recognition was lowest in the area of aseptic technique and the mean score of performance was lowest in the area of catheter related infections. The correlation of recognition and performance level was statistically significant. Conclusions: These research findings should be useful in promoting an intensive and continuous educational program on nosocomial infection for nursing students and to establish an efficient policy for preventing nosocomial infections.

  • With the development of ICT, research on smart farms is steadily progressing in the agricultural field for the modernization of cultivation facilities. However, most of the current smart farms are not specific crops, but general-purpose systems that can be used in various fields. In this paper, an environmental control device and an integrated control system capable of creating a aseptic growing environment required for mushroom cultivation were proposed, and the system was designed, manufactured, and programmed. Through this, it is possible to build a smart farm optimized for crops that is needed to maintain a precise growing environment.

Epidemics of Ascetic Meningitis in Kyoungsangnamdo from May to August, 1996 (96년도 상반기에 경상남도 중부지방에서 유행한 무균성 뇌막염에 대한 고찰)

  • Purpose : Aseptic meningitis mainly caused by enterovirus is common in pediatric population especially during summer & fall. Most of pediatric patients restore their health without any complications with proper management. Between May to August of 1996, Masan and surrounding areas of the Kyoungsangnamdo were epidemic areas for the aseptic meningitis. The purpose of this study was to determine causative virus and describe correlation between disease and clinical symptoms in aseptic meningitis patients and those with fever and characteristic rashes without apparent meningitis symptoms. Methods : Between May to August, 1996, 57 patients with high fever and characteristic feature of rashes were reviewed. From 22 cerebrospinal fluid & 57 stool obtained specimens, viral culture and detection of enterovirus RNA were conducted. Collected specimens were kept in $-30^{\circ}C$ environment until sending of specimens to labortory. The virus identified through indirect immunofluorescence. RT-PCR method was used to identify enterovirus RNA in cerebralspinal fluid. Results : 1) One hundred fifty five pediatric patients with viral infection required hospitalization. Disease occurred higher rate in male than female with ratio of 1.94:1. Examined patients' age ranged from 15days old to 15years old. But most of patients(74.8%) were under age of 5years old. The time of occurrence was between May to August of 1996. 2) All patients had high fever and physical symptoms in those patients include headache, vomiting, abdominal pain, diarrhea, and rashes. The rashes observed mainly in patients under age of 4 years and were predominantly commom patients under age of 18 months olds)<0.001). 3) Between sampled patients and non-sampled patients, clinical course was similar. Echovirus type 9 was cultivated in 41 out of 57 cases of collected stool specimens. RT-PCR that used on CSF showed positive results in 10 out of 22 cases. Three cases of positive cultivated of positive results in RT-PCR were echovirus type 9. Conclusions : Echovirus type 9 was thought to be the causative agent of aseptic meningitis that was prevalent throughout mid areas of Kyoungsangnamdo from May to August, 1996. Additionally causative agent that responsible for high fever with rashes without meningitis symptoms also thought to be the same echovirus type 9.

A Clinical Study of Aseptic Meningitis in the Busan Area in 2002 (2002년 부산 지역에서 유행한 무균성 뇌막염에 대한 임상적 고찰)

  • Purpose : There was a outbreak of aseptic meningitis in Busan, 2002. We report the clinical features and causative viruses. Methods : Two hundred seventy six children with aseptic meningitis who were admitted to the Department of Pediatrics, Maryknoll Hospital between January and December, 2002 were included. CSF, stool and throat swab viral cultures were done in 244 of these children. Results : The male to female ratio was 1.4 : 1. Age of patients varied from five months to fourteen years old. Average age was $5.7{\pm}6.2years$ old and mostly between one and four years.(41.3%) Monthly distribution revealed that the number of patients increased from May to July. The main symptoms were fever, headache and vomiting in this order. Respiratory tract infection symptom was associated from June to July and headache and nausea without fever were characteristically observed in children more than 10 years old from November to December. In peripheral blood examination, leukocytosis(WBC>$10,000/mm^3$) showed in 34.8%, ESR was increased in 56.1%, and CRP was positive value in 61%. Therefore differential diagnosis was difficult through peripheral blood examination. CSF findings revealed mean leukocyte count $86.5{\pm}180.2/mm^3$, protein $41.7{\pm}32.9mg/dL$, glucose $56.4{\pm}9.9mg/dL$. Median hospitalized period was $4.7{\pm}7.2days$ and compared with non-tapping group, hospitalized period was shorter and subsidance of symptoms was faster, therefore antibiotics injection period was shorter in the spinal tapping group. Virus was isolated in 31 cases of 244. The causative agents were echovirus 6, echovirus 9, echovirus 25, coxsakie virus B3, B4. Conclusion : There was an epidemics of aseptic meningitis in Busan, 2002; the causative agent was echovirus 6, 9, 25, coxsakie virus B3, B4.
