Biliary ascariasis is still the leading cause of surgical complication of ascariasis, though its incidence has been dramatically reduced. Herein, we report a case of biliary ascariasis for the purpose of enhancing awareness of parasitic infections as a possible cause. A 72-year-old male visited the emergency room of Dankook University Hospital on 12 July 2015, complaining of right-upper-quadrant pain. By endoscopic retrograde cholangiopancreatography (ERCP), a tubular filling defect in the right hepatic duct was detected. The defect was endoscopically removed and diagnosed as an adult female of Ascaris lumbricoides worm, of 30 cm length. Upon removal of the worm, the pain subsided, and the patient was discharged without any complication. When treating cases of biliary colic, physicians should not neglect biliary ascariasis as the possible cause.

Ascaris lumbricoides causes one of the most common soil-transmitted helminthiases globally. The worms mostly infect the human small intestine and elicit negligible or nonspecific symptoms, but there are reports of extraintestinal ectopic ascariasis. We describe a rare case of biliary ascariasis mistaken for biliary stent in a 72-year-old female patient with a history of liver resection. She visited our outpatient clinic complaining of right upper quadrant pain and fever for the past week. She had previously undergone left lateral sectionectomy for recurrent biliary and intrahepatic duct stones 2 years ago. Besides mildly elevated gamma-glutamyl transferase levels, her liver function tests were normal. Magnetic resonance cholangiopancreatography revealed a linear filling defect closely resembling an internal stent from the common bile duct to the right intrahepatic bile duct. A live female A. lumbricoides adult worm was removed by endoscopic retrograde cholangiopancreatography (ERCP). Despite a significant decrease of the ascariasis prevalence in Korea, cases of biliary ascariasis are still occasionally reported. In this study, a additional case of biliary ascariasis, which was radiologically misdiagnosed as the biliary stent, was described in a hepatic resection patient by the worm recovery with ERCP in Korea.

We describe the imaging features of two cases of biliary ascariasis. Ultrasonography and CT showed no specific abnormal findings, but MR cholangiography clearly demonstrated an intraductal linear filling defect that led to the correct diagnosis. MR cholangiography is thus a useful technique for the diagnosis of biliary ascariasis.

한 지역사회에서 최충 감염을 반드시 관리해야 하는 궁극적 이유 또는 그 당위성에 대하여 답하기 어려운 경우가 있다. 그것은 회충 관리 결과 충란 양성률이나 감염량의 감소 이외에 실제로 무엇이 호전되었는지를 알기가 그리 쉽지 않기 때문이다. 이 연구는 우리 나라에서 전국적으로 시행해 온 회을 집단관리 사업이 성공적 결실을 맺게 됨에 따라 국내 각 종합병원급 이상의 담도(또는 담석증), 충수, 췌장 등에 대한 수술 환자 중 회충미립중(迷入症)의 빈도가 크게 감소된 점을 중시하고 이들 외과적 회충중의 감소를 우리 나라 회충 관리의 실질적 효과 중 하나로 제시하고자 하였다. 우리 나라 국민의 회충 감염률 감소 양상은 전국 학생 검변 자료(1969~1989) 및 전국민 장내 기생충 감염현황 자료(1971, '76, '81, '86)에서 파악하였다 외과적 회충증의 빈도는 주로 문헌 고찰에 의하되 1959~1990년 사이에 외과학회지 등에 발표된 담도 질환, 충수염, 췌장염, 장폐쇄 등에 관한 논문을 총 망라하여 참고로 하였다. 분석 결과 종합병원중 이상의 총 담도 수술례(또는 총 담석증례) 중 회충 미립이 차지하는 비율을 YA(또는 YB)라 할 때 YA(또는 YB)의 감소 양상은 전국 학생 회충란 양성률(X)의 감소와 매우 밀접한 순 상관관계를 나타내고 있다는 것을 알 수 있었다. 즉, 수치로 표시하면 $Y_A=0.25X^2+0.013(r=0.96),{\;}Y_B=0.19X^2+0.016(r=0.94)$의 관계를 보였다. 다른 외과적 회충증의 경우에도 담도 회충증과 비슷한 양상을 보였다. 결국 우리 나라의 회충 관리사업은 회충 감염률 및 감염량의 감소 뿐만 아니라 회충 감염시 심각한 합병증의 하나인 담도 회충증 또는 기타 외과적 회충증의 빈도를 격감시키는 데에도 크게 기여한 것으로 판단되었다.

Ascariasis symptoms penetrated into biliary tract are reported to be found much more in Korea than in foreign countries. And the present writer believes there are also many cases of biliary tract symptoms that are actually experienced but that are not reported. As a result of experiencing and nursing twenty cases of ascariasis symptoms of biliary tract, the present writer has got several conclusions, and reports them together with reference to sundry records. (1) The penetrating rate of ascariasis in male versus female was 1 to 1.5. Consequently the rate of female was higher than that of male. The highest age of the case was 67, the lowest 19, and the average age was 45.2. (2) As to symptoms, right upper quadrant abdomen pain and fat diet intolerance were the largest in number. By laboratory findings there were depressive pain on right upper quadrant abdomen in all the cases. (3) As to the location of ascariasis, choledochus occupied the largest part, and the longer illness history was, the more cases coexisting with calculus were found. (4) As to operative method, cholecystectomy and choledochotomy weres the largest in number, and choledochotomy T-tube was inserted in all cases. As to complication after operation, there were operative wound infection, excoriation through bile juice issue, etc. There were three cases of re-penetration of ascariasis and only one case of reoperation.

한국의 회충 감염률은 1970년대까지 높았으나 성공적인 기생충 관리로 매우 낮은 수준으로 감소하였다. 최근 회충 유행 지역으로부터의 인구 유입 및 해외여행 증가, 수입 농산물의 증가 등으로 인해 발생 가능성이 높아지는 가운데 성인에서 회충감염으로 장폐쇄나 췌담관 질환이 유발된 증례 보고들이 있었다. 저자들은 회충증을 의심할 만한 위험 요인이 없는 2세 남아의 기저귀에서 회충 성충이 발견된 증례를 경험하여 보고한다. 아침에 환아의 어머니가 기저귀를 갈다가 움직이는 충체를 발견하여 내원하였다. 특이 과거력 없이 평소 건강하던 환아로 이상 증상은 없었고 신체 진찰에서 특이 소견은 관찰되지 않았다. 충체는 길이 약 25 cm, 폭 5 mm의 선충으로 머리와 꼬리는 원추상을 보여 회충으로 추정하였고 대변 기생충란 검사에서 Ascaris lumbricoides의 불수정란이 확인되어 회충증으로 진단하였다. 구충제인 알벤다졸 400 mg을 1회 경구 투여 후 추적 관찰하였고 추가적인 회충 발견이나 증상 발생은 없었다. 전체적인 기생충 감염률이 감소하여 관심이 줄어드는 현실에서 회충 감염에 대한 인식을 높일 필요가 있겠다.

회충증은 Ascaris lumbricoides에 의하여 유발되는 장 질환이다. 대부분의 회충증 환자는 무증상이지만 장내 유충이 많은 경우에는 장 폐색, 폐쇄성 황달, 담관염, 담낭염, 췌장염과 같은 합병증이 생길 수 있다. 저자들은 컴퓨터단층촬영(이하 CT)에서 구불창자간막에 혈종과 활동성 출혈이 발생하여 회충증으로 진단된 74세 남자의 사례를 경험하였다. 이후에 환자가 시행한 CT에서는 이와 더불어 구불창자의 천공 발생이 확인되었다. 복강경을 통한 저위전방절제술이 시행되었고 구불창자간막에 큰 혈종이 있었으며 장간막 지방 조직에서 현미경적으로 회충이 확인되었다. 이러한 회충증의 드문 임상적, 영상의학적 소견은 기생충이 장관의 내강 안에서 복강 내로 이동하는 과정에서 발생한 순차적인 합병증들을 보여준다.

복통과 담석을 주소로 서울시립 보라매병원에 입원한 43세 여자 환자가 수술 후 담석증을 동반한 담도회충증으로 진단되었다. 환자는 어릴 때부터 '횟배'를 자주 앓아왔으며 입원 20일 전부터 식후 복통과 오심 및 구토 증세를 나타내었고 개인병원에서 시행한 초음파 검사에서 담석이 발견되어 수술을 받기 위해 입원하였다. 입원 당시 간기능 검사상 SGOT 및 SGPT는 다소 증가되어 있었고 대변검사에서 기생충의 충란은 검출되지 않았다. 환자는 담석증 진단 하에 담낭절제술을 시행받았다. 수술 전 담도조영 검사상 좌측 간내 담도는 막혀 있었으며 수술 후 어느 정도 호전되었으나 잔존 오니 (sludge)로 여전히 막혀있는 상태였다 광학현미경 검사상 담낭은 만성 담낭염의 소견을 보였고 오니에서는 심하게 변성된 회충의 충체와 비교적 잘 보존된 충란이 함께 관찰되었다. 이 환자의 경우 담석 형성은 담관 내로 기어 들어간 회충이 오랜 기간 경과하면서 변성되어 담석의 핵 (nidus)으로 작용하였을 것으로 추측되었다. 이 환자는 최근 5년 동안 국내 문헌상 매우 드물게 발견되는 담도 회충증의 증례이었다.
