Microvascular Anastomosis with Non-penetrating Vascular Clips in Head and Neck Free Flap Surgery (두경부 유리피판 수술에 있어서의 비천공성 혈관 클립을 이용한 미세혈관 문합술)

  • Microvascular anastomosis with suture technique is a basic skill but there are several problems such as stenosis, thrombosis and long operating time. Recently plastic surgeons have developed non-suturing mechanical coupling devices for microvascular anastomosis. The authors applied non-penetrating vascular clips (VCS clips) in the field of free flap surgery of head and neck area. Between August of 2004 and January of 2005, we performed 9 free flaps (16 vessels) using small-sized VCS clips. Four stay sutures were applied first and then VCS clips were placed between sutures about 1 mm apart. Vascular pedicle of free flap included the descending branch of lateral circumflex femoral vessel, thoracodorsal vessel, deep inferior epigastric vessel and cephalic vein. The recipient vessels were the superior thyroid artery, superficial temporal artery, internal jugular vein, external jugular vein, and superficial temporal vein. We performed 13 end-to-end (4 arteries and 9 veins) and 3 venous end-to-side anastomoses. No flap related complication occurred but we applied additional clips or sutures in two cases due to blood leakage after completion of anastomosis. Primary patency rates seemed to be good and more rapid anastomosis could be done than conventional suture technique. Advantages of VCS technique are high patency rate, low thrombogenecity and rapidity. Although the high cost of VCS instrument may be a problem, this clip could be applied safely in microvascular free tissue transfer.

Double-layered collagen graft to the radial forearm free flap donor sites without skin graft

  • Background: Radial forearm free flap is the most reliable flap for intraoral soft tissue reconstruction after cancer ablation surgery. However, unesthetic scar of the donor site and the need for a second donor site for skin graft are major disadvantages of the forearm flap. The purpose of this study was to report the clinical results of double-layered collagen graft to the donor site of the forearm free flap without skin graft. Methods: Twenty-two consecutive patients who underwent oral cancer ablation and forearm reconstruction between April 2010 and November 2013 were included in this study. Male to female ratio was 12:10, and average age was 61.0 years old (27-84). Double-layered collagen was grafted to the donor site of the forearm free flap and healed for secondary intention. Upper silicone had been trimmed at the periphery during secondary intention, and dry dressing was used. Postoperative scar healing and esthetic results and function were evaluated. Results: An average follow-up period was 34.9 months. The scar area was decreased to 63.9 % in average. The complete healing was obtained between 1.5 and 3 months according to the defect size. There was no functional defect or impairment 3 months after operation. All patients were satisfied with the esthetic results. Three patients died of recurred cancer. Conclusions: Double-layered collagen graft was successfully performed in this study. Without the thigh skin graft, patients had experienced less painful postoperative healing periods and discomfort.

Molecular Genetic Diagnosis of Genetic Endocrine Diseases (유전성 내분비 질환의 분자유전학적 진단)

  • Many endocrine disorders have a genetic component. The genetic component is the major etiologic factor in monogenic disorders, while multiple genes in conjunction with environmental and lifestyle factors contribute to the pathogenesis in complex disorders. The development of the molecular basis of inherited endocrine diseases has undergone a dramatic evolution during the last two decades. The application of molecular technology allowed us to increase our understanding of endocrine diseases, and to impact on the practice of pediatric endocrinology related to diagnosis and genetic counseling. Identification of the mutation in the particular disease by genetic testing leads to precise diagnosis in the equivocal cases and prenatal diagnosis. However, clinicians should be cautious about determining therapeutic decisions solely on the basis of molecular studies, especially in the area of prenatal diagnosis and termination of pregnancy. This review describes an introduction to molecular basis of various inherited endocrine diseases and diagnosis by genetic testing.

Delayed Parenchymal Transit During Tc-99m MAG3 Renography is a Valuable Sign in Diagnosing Urinary Obstruction in Patients with Early Hydronephrosis (초기의 수신증 환자의 요로폐쇄 진단에 있어 Tc-99m MAG3 신장 스캔시 실질통과지연 소견의 유용성)

  • Purpose: Diuretic renography (DR) can be false negative in patients with upper urinary tract obstruction due to low compliance of the renal pelvis. Delayed parenchymal transit (DPT) may be a valuable sign in case of false negative DR. We compared the diagnostic values of DR and DPT during Tc-99m MAG3 diuretic scan in adults with suspected unilateral obstructive uropathy. Materials and Methods: Fifty-four patients(male:female=30:24, age: $40.7{\pm}15.5$ yrs) who underwent Tc-99m MAG3 diuretic scan due to suspicious unilateral obstructive uropathy were analyzed. DR with a $T_{1/2}\;of\;>\;15min$ was considered as positive for obstruction. DPT was considered to be present when there was delayed appearance of radioactivity in the renal pelvis and prolonged retention of radioactivity in the renal parenchyma. The renal area ratio was defined as the ratio of pixel number of hydronephrotic kidney over that of normal contralateral at $1{\sim}2min$ images. Definition of obstruction was improved hydronephrosis after intervention, or aggravated hydronephrosis without intervention. Non-obstruction was defined as unchanged hydronephrosis over 6 months. Results: Twenty-six renal units had obstruction and 28 no obstruction. The sensitivities of DR and DPT were 69% (18/26) and 50% (13/26) respectively. Two renal units with DPT but negative DR showed the renal area ratio of <1.1. Among the 20 obstructive renal units with DPT or positive DR, 13 with DPT had lower renal area ratio than 7 renal units without DPT ($0.97{\pm}0.20\;vs\;1.30{\pm}0.41,\;p<0.05$). Differential renal function was not significantly different between these groups. DPT correctly diagnosed all renal units with non-obstruction (specificity 100%), while the specificity of DR was 89% (25/28). Conclusion: DPT during Tc-99m MAG3 diuretic scan may be a valuable sign in diagnosing urinary obstruction especially in patients with false negative DR and early HN.

Spatial Distribution of Tidal Flats in Korea (한국(韓國)의 간석지(干潟地) 분포(分布))

  • On the basis of the topographic maps in the 1910's and 1990's and the classification map of Landsat TM satellite image photographed on Sept 1, 1996, the spatial distribution and the current situation concerning tidal flats in Korea were studied by measuring the area with GIS Arc/Info system and examining the regional condition required to develop the tidal flats. The results are as follow; The tidal flat resources in Korea cover an area of about 3800 square meters, including the reclaimed one since the 1910's. And they are widely distributed in the west coast of South Jeonla, Kyunggi bay, Asan bay, the south coast of South Jeonla, Kunsan bay, Chunsoo bay, and the coast of South Kyungsang and Pusan when put in the order from bigger area. Given the area under the construction at present, more than 50% of the tidal flats are reclaimed ones. The tidal flats are being developed especially in Kyunggi and Asan bays because they perfectly measure up to the conditions required. For Kunsan bay, a remarkably good supply system of the alluvial sedimentary materials and a favorable coastline requirement for the coast of the South Jeonla also contribute to the development of the tidal flats. In the case study of Kunsan bay, it was shown that the shape of the tidal flat is making a continuous change and the area is getting bigger in terms of the multi-temporal change of the tidal flat development. However, while in the first half of the 20th century, the increasing rate of the area was considered to be rather high, it is considerably lowered almost to a standstill in the latter half of the century.

  • This paper is to suggest a concrete way about imperiled domestic hot spring tourism and then provide some ideas to solve the problems related to public health promotion, aging society. In this paper, the author proposes the healthy tourism formation to solve such above problems. The author also proposes Asan Hot spring areas as the most healthy places for tourists. Therefore, this paper introduces that Asan Hot Spring Areas will contribute to the economic development in Korea. There will contribute to improve the nation's image and produce much more benefits by hot spring resources develop of the A-San springs area.

Distribution of zooplankton in Asan Bay, Korea (아산만 해역의 동물플랑크톤 분포)

  • Distribution of zooplankton was investigated at 30 stations in Asan Bay, an embayment in the western coast of Korea, seasonally in August and October of 1991, and January and April of 1992. Abundances of zooplankton ranged from 240 to 1,458 indiv./m$^3$. Copepods were the predominant constituent, which comprised 88% of total zooplankton abundance. Acartia bifilosa dominated from fall to spring, A. pacifica in summer and Paracalanus parvus in fall. Abundances of zooplankton were related with phytoplankton standing crops and larval fish. In spring, when phytoplankton standing crops were high, zooplankton abundances were highly correlated with phytoplankton standing crops (r$^2$=0.66, p<0.01, n=30). Results of cluster analysis based on the abundance data of zooplankton showed that the study area was divided into two areas, i.e., outer-bay and inner-bay.

A Study of Parents' Satisfaction and Needs for Foodservice Quality of the Childcare Centers in Chungnam Asan Area (보육시설 급식에 대한 부모 만족도 및 요구도 조사 - 아산지역을 중심으로 -)

  • We tried to find out parents' satisfaction and needs for foodservice quality of the childcare centers with an intention of improving it. Self-completed questionnaires were randomly collected from the parents of children in 174 childcare centers around Asan and were analyzed. Most of respondents were mothers who were housewives with college degrees (69%) in their 30's (80%). They thought nutrition (50%) and food hygiene (43%) were the most important factors for decision. Eighty percent of parents recognized the strong relationship between foodservice quality and childrens' mental and physical development. Most centers (94%) were delivering menu notices home, so parents well recognized the center's foodservice situation and problems of their kids' eating habits. The parents' satisfaction scores were 3.26 out of 5-point scale. The biggest dissatisfaction was from no variation in menu, but remarkable appreciation was from the effect of foodservice on the correction for kids' unbalanced diet habit. They had demands for improvement of nutritious menu and professional nutrition teacher in charge of foodservice and nutrition education for children and also for parents.

The Foodservice Sanitation Status of the Child Care Centers at Asan City in Chungnam (영.유아 보육시설의 급식 위생실태 - 충남 아산 지역 중심으로 -)

  • We investigated the foodservice sanitation status of the childcare centers in Chungnam Asan area to provide the information for sanitation guidelines and checklist in various operation types of childcare centers. Self-completed questionnaires were collected from the directors of 95 centers: national-public 9 (9.5%), corporation 10 (10.5%), private 52 (54.7%), and home care 24 (25.3%) types. The analyzed results are shown in foundation type; the investigated items are about critical hygienic problems and safety recognition level, presence of sanitation guideline, demand of dietian's employment and the role, hygienic management and education level, and the present and future of sanitary facility and equipment. From this study we found that home care center, which had low capacity, was comparatively poor at sanitation status, the use of hygienic standard and guideline, and safety recognition level. In our opinion, the use and application of same sanitation standard to any type and size of center is not appropriate and rational. The reestablishment of the sanitation guideline and checklist considering the conditions of various operation types would be necessary. Also periodic hygienic education by hygenic professionals, continuous parents' attention and cooperation of related government organizations are needed for improvement of foodservice sanitation status of childcare centers.
