Objective : The goal of this study was to evaluate the biomechanical features of human cadaveric spines implanted with the Activ L prosthesis. Methods : Five cadaveric human lumbosacral spines (L2-S2) were tested for different motion modes, i.e. extension and flexion, right and left lateral bending and rotation. Baseline measurements of the range of motion (ROM), disc pressure (DP), and facet strain (FS) were performed in six modes of motion by applying loads up to 8 Nm, with a loading rate of 0.3 Nm/second. A constant 400 N axial follower preload was applied throughout the loading. After the Activ L was implanted at the L4-L5 disc space, measurements were repeated in the same manner. Results : The Activ L arthroplasty showed statistically significant decrease of ROM during rotation, increase of ROM during flexion and lateral bending at the operative segment and increase of ROM at the inferior segment during flexion. The DP of the superior disc of the operative site was comparable to those of intact spine and the DP of the inferior disc decreased in all motion modes, but these were not statistically significant. For FS, statistically significant decrease was detected at the operative facet during flexion and at the inferior facet during rotation. Conclusion : In vitro physiologic preload setting, the Activ L arthroplasty showed less restoration of ROM at the operative and adjacent levels as compared with intact spine. However, results of this study revealed that there are several possible theoretical useful results to reduce the incidence of adjacent segment disease.

A trileaflet polyurethane valve was simulated at the outflow orifice while the inflow orifice had a trileaflet or a flap valve. The numerical solutions of the simulated model show that regions of relative stasis and trapped vortices were smaller wi thin the ventricular chamber wi th the flap valve at the inflow orifice than that with the trileaflet valve. The predicted Reynolds stresses distal to the inflow valve within the ventricular chamber were also found to be smaller with the flap valve than with the trileaflet valve. Analysis of the numerical solutions suggests that geometries similar to the flap valve(or a tilting disc valve) results in a better flow dynamics within the total artificial heart chamber compared to a trileaflet valve.
The defect detection based on deep learning requires minimal loss and high accuracy to pinpoint product defects. In this paper, we confirm the loss rate of deep learning training based on disc-shaped artificial scaffold images. It is intended to compare the performance of Cross-Entropy functions used in object detection algorithms. The model was constructed using normal, defective artificial scaffold images and category cross entropy and sparse category cross entropy. The data was repeatedly learned five times using each loss function. The average loss rate, average accuracy, final loss rate, and final accuracy according to the loss function were confirmed.
터빈 로타 디스크는 디스크, keyway, bore surface, hub, rim 등으로 구성되고 이들 부분이 전형적인 균열 발생 부위이다. 이러한 부위는 기하학적 형상이 복잡해 적절한 탐촉자의 선정, wedge 설계 및 가공, 검사체의 형상과 크기에 따른 정상신호와 결함신호의 분류 및 평가가 필요하다. 본 연구에서는 디스크 keyway 및 bore surface에 인공결함을 갖는 디스크 대비 시험편을 제작하여 구조, 형상별 초음파 신호를 수집하여 분석하였고 디스크 곡률에 따른 wedge를 설계, 제작하였다. 본 연구를 통하여 형상별 신호를 정의할 수 있었고 디스크부에 발생되는 결함을 검출하고 평가할 수 있는 초음파검사 기술을 확립하였다. 또한 현장 적용에 있어서 성공적인 결과를 입증하였다.
In thIns paper, a numerical simulation of steady laminar and turbulent flow in a two dimensional model for the total artificial heart is'presented. The finite analytic numerical method was employed to obtain solutions to the governing equations in the Cartesian coordinates. The closure for turbulence model was achieved by employing the k-$\varepsilon$-E model. The SIMPLER algo rithm was used to solve the problem in primitive variables. The numerical solutions of the slulated model show that regions of relative stasis and trapped vortices were smaller within the ventricular chamber with the flap valve at the Inflow orifice than that with the trileaflet valve. The predicted Reynolds stresses distal to the inflow valve within the ventricular chamber were also found to be smaller wlth the flap valve than with the trlleaflet valve. These resu1ts also suggest a correlation be- tween high turbulent stresses and the presence of thrombus In the vicinity of the valves in the total artificial hearts. The computed velocity vectors and trubulent stresses were comparable with previ ously reported in vitro measurements in artificial heart chambers. Analysis of the numerical solo talons suggests that geometries similar to the flap valve(or a tilting disc valve) results in a better flow dynamics within the total artificial heart chamber compared to a trileaflet valve.
A heart supplies blood of about 15, 000 liters to each human organ in a day. A normal function of heart valves is necessary to accomplish these enormous work of heart. The disease of heart valve develops to a narrowness of a closure, resulting in an abnormal circulation of blood. In an attempt to eliminate the affliction of heart valves, the operative method to replace with artificial heart valves has developed and saved numerous patients over past 30 years. This replacement operation has been performed since early 1960`s in Korea, but all the artificial heart valves used are imported from abroad with very high costs until recent years. New artificial heart valves have been developed in Korea Advanced Institute of Science and Technology since early 1980`s. The first developed valve was designed with a free-floating pyrolytic carbon disk that is suspended in a titanium cage. The design of the valve was tested in vitro, and in animals in 1987. The results from this study was that the eccentrically placed struts creates a major and minor orifice when the disc opens and stagnation of flow in the area of the minor orifice has led to valve thrombosis. In this work, the design of the valve was changed from a single - leaflet valve to double - leaflet one in order to resolve the problems observed in the first - year tests. Morphological and hemodynamic studies were made for the newly designed valves through the in vitro and in vivo tests. The design and partial materials of the artificial heart valve was improved comparing with first - year`s model. The disc in the valve was modified from single - leaflet to bi - leaflet, and the material of the cage was changed from titanium metal to silicon - alloyed pyrolytic carbon. A test was made for the valve in order to examine its mechanical performance and stability. Morphological and hemodynamic studies were made for the valve that had been implanted in tricuspid position of mongrel dogs. All the test animals were observed just before the deaths. A new artificial heart valve was designed and fabricated in order to resolve the problems observed in the old model. The new valve was verified to have good stability and high resistance to wear through the performance tests. The hemodynamic properties of the valve after implantation were also estimated to be good in animal tests. Therefore, the results suggest that the newly designed valve in this work has a good quality in view of the biocompatibility. However, valve thrombosis on valve leaflets and annulus were found. This morphological findings were in accordance with results of surface polishing status studies, indicating that a technique of fine polishing of the surface is necessary to develop a valve with higher quality and performance.

Purpose: This study proposed a generative adversarial network (GAN) model for T2-weighted image (WI) synthesis from proton density (PD)-WI in a temporomandibular joint(TMJ) magnetic resonance imaging (MRI) protocol. Materials and Methods: From January to November 2019, MRI scans for TMJ were reviewed and 308 imaging sets were collected. For training, 277 pairs of PD- and T2-WI sagittal TMJ images were used. Transfer learning of the pix2pix GAN model was utilized to generate T2-WI from PD-WI. Model performance was evaluated with the structural similarity index map (SSIM) and peak signal-to-noise ratio (PSNR) indices for 31 predicted T2-WI (pT2). The disc position was clinically diagnosed as anterior disc displacement with or without reduction, and joint effusion as present or absent. The true T2-WI-based diagnosis was regarded as the gold standard, to which pT2-based diagnoses were compared using Cohen's ĸ coefficient. Results: The mean SSIM and PSNR values were 0.4781(±0.0522) and 21.30(±1.51) dB, respectively. The pT2 protocol showed almost perfect agreement(ĸ=0.81) with the gold standard for disc position. The number of discordant cases was higher for normal disc position (17%) than for anterior displacement with reduction (2%) or without reduction (10%). The effusion diagnosis also showed almost perfect agreement(ĸ=0.88), with higher concordance for the presence (85%) than for the absence (77%) of effusion. Conclusion: The application of pT2 images for a TMJ MRI protocol useful for diagnosis, although the image quality of pT2 was not fully satisfactory. Further research is expected to enhance pT2 quality.

Objective : Recently, motion preservation has come to the forefront of emerging technologies in spine surgery. This is the important background information of the emergence of cervical arthroplasty as an alternative to arthrodesis that offers the promise of restoring normal spinal movement and reduces a kinematic strain on adjacent segments. The study was designed to evaluate early surgical outcome and radiological effects of $Bryan^{(R)}$ cervical disc prosthesis. Methods : The authors retrospectively reviewed radiographic and clinical outcomes in 52 patients who received the $Bryan^{(R)}$ Cervical Disc prosthesis, for whom follow-up data were available. Static and dynamic radiographs were measured by computer to determine the angles formed by the endplates of the natural disc preoperatively, those formed by the shells of the implanted prosthesis, the angle of functional spine unit (FSU), and the C2-7 Cobb angle. The range of motion (ROM) was also determined radiographically, whereas clinical outcomes were assessed using Odom's criteria, visual analogue pain scale (VAS) and neck disability index (NDI). Results : A total of 71 $Bryan^{(R)}$ disc were placed in 52 patients. A single-level procedure was performed in 36 patients, a two-level procedure in 13 patients, and a three-level procedure in 3. Radiographic and clinical assessments were made preoperatively. Mean follow-up duration was 29.2 months, ranging from 6 to 36 months. All of the patients were satisfied with the surgical results by Odom's criteria, and showed significant improvement by VAS and NDI score (p < 0.05). The postoperative ROM of the implanted level was preserved without significant difference from preoperative ROM of the operated level (p < 0.05). 97% of patients with a preoperative lordotic sagittal orientation of the FSU were able to maintain lordosis. The overall sagittal alignment of the cervical spine was preserved in 88.5% of cases at the final follow up. Interestingly, preoperatively kyphotic FSU resulted in lordotic FSU in 70% of patients during the late follow up, and preoperatively kyphotic overall cervical alignment resulted in lordosis in 66.6% of the patients postoperatively. Conclusion : Arthroplasty using the $Bryan^{(R)}$ disc seemed to be safe and provided encouraging clinical and radiologic outcome in our study. Although the early results are promising, this is a relatively new approach, therefore long-term follow up studies are required to prove its efficacy and its ability to prevent adjacent segment disease.
This paper considers the acoustical characteristics of the mechanical valve employed in the Korean type Artificial Heart. $Bj\"{o}rk-Shiley$ tilting disc valve was chosen for the study and acoustic measurements were performed for the artificial heart operated in a mock circulation system as well as implanted to an animal as a Bi Ventricular Assist Device (BVAD). In the mock system, three different conditions of the valve were examined which were normal, damaged (torn off), pseudothrombus attached. Microphone measurements for the BVAD were carried out at a regular time interval for 5 days after the implantation operation. Of the recorded acoustic emissions from the artificial heart, click sounds mainly originated from the valves were further analyzed using Multiple Signal Classification (MUSIC) for estimating their spectral properties. It was shown that the spectral peaks below 4 kHz and the optimal order number for MUSIC, equivalent to the number of the spectral component, might be the key parameters which were highly correlated to the physiological states of the valve like the mechanical damage of the valve or the formation of thrombus on the valves.
전남 목포시의 각 가정에서 담근 물김치에서 총 24균주의 유산균을 분리하였다. 이들 분리 유산균 중 5균주가 인공 위액에서 내성을 나타내었다. 이들 유산균은 Lactobacillus plantarum으로 동정하였고, 특히 물김치로부터 동정된 유산균 MUL-2, MUL-4, MUL-18은 인공위액에서 3시간 배양 후에도 초기 생균수에 비해 변화가 거의 없어 가장 강한 내성을 나타내었다. 이들 분리 유산균은 병원성 미생물인 Listeria monocytogenes, Escherichia coli, Staphyrococcus aureus, Vibrio parahaemolyticus, Samonella typhimurium에 대해 뚜렷한 항미생물활성을 나타내었다.
