Noonan syndrome is a relatively common genetic disorder which is autosomal dominant. Prevalence of Noonan syndrome is varying from 0.04% to 0.1%. It is characterized by distinctive facial features, chest deformity, short stature and congenital heart disease. Oral findings in patient with Noonan syndrome include high arched palate, dental malocclusion, articulation difficulties, and micrognathia. The purpose of this case report is to describe dental treatment of a children with Noonan syndrome. 5 year old boy with Noonan syndrome visited to the Seoul National University Dental Hospital for dental treatment. Due to need for close monitoring, concern about seizure and poor cooperation, we planned to perform the dental treatment under general anesthesia. Under general anesthesia, caries treatment was successfully performed and there was no postoperative complications related to general anesthesia. High arched palate was observed which is characteristic in Noonan syndrome.
Objectives : The aim of this study is to find the acoustic characteristics of acupuncture therapy effects on post-stroke dysarthria. Methods : Acupuncture therapy was applied for four to six weeks by inserting needles into eight acupuncture points, CV23, CV24, bilateral 'Sheyu' and ipsilateral ST4, ST6 and contralateral LI4, ST36 on facial palsy side. All the speech samples were collected, pre-treatment and post-treatment, using Computerized Speech Lab. VOT and TD of each speech sample and vowel formant(F1&F2) were analyzed on spectrogram. Result : VOT and TD were decreased after treatment. F1 was decreased, and F2 was increased after treatment. Conclusions : This suggests that acupuncture therapy improves symptoms of post-stroke dysarthria by stimulating articulation organs such as tongue, lips, cheeks, larynx and pharynx.
Purpose: This study was performed to evaluate the clinical outcomes of operative treatment using porous hydroxyapatite for intraarticular calcaneal fracture of joint depression type. Materials and Methods: Twenty patients with intraarticular calcaneal fracture were followed up for more than 1 year. The period to union was calculated to evaluate the osteoconductivity of porous hydroxyapatite used as bone graft substitute. The measurement of Bohler angle, Gissane angle and the degree of articular surface depression was performed through preoperative and postoperative radiographs. The clinical evaluation was performed according to hindfoot score of the American Orthopaedic Foot and Ankle Society (AOFAS) and scale of the Creighton-Nebraska health foundation (CNHF). Results: Bohler angle and Gissane angle had improved significantly from preoperative average $10.4^{\circ}$, $117.8^{\circ}$ to average $22.6^{\circ}$, $113.5^{\circ}$ immediate postoperatively, and had maintained to average $21.2^{\circ}$ and $114.4^{\circ}$ at the last follow-up. The degree of articular surface depression had improved significantly from preoperative average 4.8 mm to 1.5 mm at the last follow-up. All cases achieved bone union, and the interval to union was average 12.8 weeks. AOFAS score was average 85.2 points at last follow-up. There were 7 excellent, 10 good, and 3 fair results according to the CNHF scale. Therefore, 17 cases (85%) achieved satisfactory results. Conclusion: Plate fixation using porous hydroxyapatite seems to be one of effective treatment methods for intraarticular calcaneal fracture of joint depression type, because of supporting the reduction of subtalar articulation by augmenting bony defect and facilitating bone formation. Further evaluation about long-term radiological changes and histological analysis on hydroxyapatite implantation site should be required.
This study analyzed 28 domestic and foreign studies related Prompts for Restructuring Oral Muscular Phonetic Targets treatment techniques from 1984 to 2020 to prepare basic data for the development of PROMPT intervention programs and examination tools. According to the analysis, continuous research has been conducted since 1984 when the prompt study was first started, and the method of research was 16 intervention studies, with the highest number of speech disorders, and the target age being 3 to 5 years old, the most frequently conducted for infancy. The treatment was the most frequent in the 16th sessions, and the activities were based on the Motor Speech Hierarchy(MSH), except for the subjects of the non-verbal autism spectrum disorder. According to the analysis of the dependent variables, 'speech production' was the most common, followed by 'speech motor control', 'articulation', and 'speech intelligibility' were highest. Combined with all these studies, it suggests that PROMPT, which are directly useful for exercise spoken word production, are effectively being used outside the country and that it is necessary to develop a PROMPT program that can be applied domestically, in Korea.

The incisors function as instruments for biting and cutting food during mastication. They also support the lips and face and maintain vertical dimension. In addition, they contribute to overall normal arch appearance. They play important role during the articulation of speech and assist in guiding jaw closure. Extraction and space maintenance are the most common treatment for a tooth with poor prognosis. However, in the mixed dentition, extraction of the upper permanent incisors results in many complications, such as resorption of alveolar bone, poor esthetics, pronunciation, and mastication. Considering these various roles of incisors in oral cavity, approach for traumatized incisors, even the ones with poor prognosis, should be considered first prior to simple extraction. The dentist must take into account the age of the patient, growth potential, occlusion, oral hygiene status, economic status and motivation towards dental health in addition to patient compliance. In this case, although the prognosis was predicted to be unfavorable due to short root and mobility, we could save the central incisor using conservative treatment, reposition by orthodontic appliance instead of extraction.

Background and Objectives : The current study aimed to 1) compare of Lingual function and alveolar sounds between 4-year-old Korean children with and without ankyloglossia, 2) investigate the correlation between ① maximum lingual length-protrusion(MLL-P) and percentage of consonants corrects(PCC) focused on alveolar sounds, ② lingual movement and PCC focused on alveolar sounds, ③ MLL-P and lingual movement. Materials and Method : Twenty-two 4-year-old children participated as subjects in the study: a control group of 11 normal children and an experimental group of 11 children who were previously diagnosed as having ankyloglossia. They were measured for lingual function (lingual length, lingual movement) and the performances of speech articulation. Results : Children with ankyloglossia displayed significantly shorter MLL-P than 4 year normal children. Experimental group displayed significantly worse lingual movement, lower PCC in Picture consonants test, and lower PCC of alveolar sounds than control group. Ther was significantly high correlation between MLL-P and lingual movement of experimental group. Conclusion : This paper describes clinical measure and functional aspects of the tongue. Such baseline analysis provides a more definitive appraisal of lingual function as well as a more objective basis for diagnosis and treatment of ankyloglossia.
Purpose: Tongue cancer is the most common malignant tumor of the oral cavity and the ultimate goal in treatment of the cancer is not only complete excision and meticulous closure of the wound, but also, reconstruction of a demensional and functional tongue. Our study focuses on various factors, such as defect size, extent of tumor, age, application of mandibulectomy or radiotherapy, and their influences on postoperative speech and swallowing function. Methods: Our study was based on 59 patients who underwent tongue cancer operation and reconstruction of the tongue. Speech and swallowing were evaluated according to categories documented by Sultan and Teichgraeber. Patients were classified into 3 groups as partial glossectomy, hemiglossectomy and total glossectomy groups for evaluation. The average age of the patients were 51, and the mean follow-up period was 4 years 2 months. Results: The partial glossectomy group showed statistically relevant results for speech articulation and swallowing abilities compared to the total glossectomy group. In cases of defects involving the mouth floor, the group showed decreased results compared to the group without mouth floor involvement. Increased age showed decreased postoperative results with statistical significance, while mandibulectomy and radiotherapy revealed no statistically significant data. Analysis according to TNM staging resulted in decreased functional result with advanced staging without statistical significance. Conclusion: To summarize the factors influencing the functional outcome in tongue reconstruction, younger patients and early stage cancer with minimal surgical extent revealed more satisfying results while mandibulectomy and radiation did not have influence on our analysis. Addition of various influencing factors and studies with longer follow up periods on our patient groups may provide effective data for more satisfying functional outcomes in the future.
Intrachondral articulations are the synovial joints that occur within costal cartilage in artiodactyls. The anatomical structure of Korean water deer differs from that of other deer. However, there have been no reports on the occurrence and shape of intrachondral articulations in the deer. To provide information on these articulations, we examined the occurrence and shape of intrachondral articulations in the Korean water deer by gross findings, radiography, computed tomography (CT), and histological observation. These joints often occur in the second to the tenth ribs. Morphologically, they are spheroidal joints, especially from the third to the eighth ribs, and their configuration is discernable in gross findings, radiography, CT, and histological sections. These basic results would be helpful in the diagnosis and treatment of diseases in the lateral thoracic wall of the Korean water deer.
Multiple ligament knee injury is defined as rupture to at least two of the four major knee ligament structures. Three or four knee ligament injury results in knee dislocation as complete disruption of the integrity of the tibiofemoral articulation. In multiple ligament knee injury, vascular and neurologic assessment should be performed meticulously and systematically. Emergency surgery should be needed if arterial injury is suspected. Surgical treatment rather than conservative management should be done and early surgery might be better than delayed surgery. Reconstruction of ACL and PCL, repair or reconstruction of MCL, and reconstruction of posterolateral corner are recommended, although many debates have occurred. Multiple ligament knee injury requires more aggressive management than single ligament knee injury.

Total ankle replacement (TAR) is a visible option in the surgical treatment of degenerative or inflammatory diseases of ankle joint. it is attributed to the current TAR which has improvements in surgical technique, uncemented implant fixation and minimally constrained articulation. In the clinical result, they can show promised surgical result when compared to earlier attempts in TAR. However, TAR is still not as successful as total knee replacement (TKR) or total hip replacement (THR), it needs to be note that there are limitations in concerning of long term performance of TAR, the high failure rate still associated with wear of the PE (polyethylene) component that has related with their material property and surface roughness. The aim of this study was to introduce the tribology characteristics of total ankle joint prosthesis with one of TDR model which was fabricated to try multi-axis wear test as a region of motion in ankle joint. The wear specimen of TDR was prepared with Ti-6Al-4V alloy and UHMWPE (ultra-high molecular weight polyethylene) for tibia-talus and bearing component, respectively. A wear test was carried out using a Force 5 (AMTI, Massachusetts, US) wear simulator which can be allowed to move in three axis to flexion-extension ($+3^{\circ}{\sim}-6^{\circ}$), internal-external axial rotation (${\pm}5^{\circ}$), as well as sinusoidal compressive load (1.6 kN, R=10). All tests were performed following standard ISO 14243, wear rate was calculated with weight loss of UHMWPE bearing while the specimen has tested at certain cycles. As based on the preliminary results, wear rate of UHMWPE bearing was $7.9{\times}10^{-6}mg/cycles$ ($R^2=0.86$), calculated loss weight until $10^7cycles$ was 79 mg, respectively.
