VPI occurs when the velum and lateral and posterior pharyngeal wall fail to separate the nasal cavity from the oral cavity during deglutition and speech. There are a number of congenital and acquired conditions which result in VPI. Congenital conditions include cleft palate, submucous cleft palate and congenital palatal insufficiency (CPI). Acquired conditions include carcinoma of the palate or pharynx and neurologic disorders. The speech characteristics of VPI is characterized by hypernasality, nasal air emission, decreased intraoral air pressure, increased nasal air flow, decreased intelligibility. VPI can be treated with various methods that include speech therapy, surgical procedures to reduce the velopharyngeal gap, speech aid prosthesis, and combination of surgery and prosthesis. This article describes four cases of VPI treated by speech aid prosthesis and speech therapy with satisfactory result.

There have been various papers about service quality. This article is one of them. This is about the measurement of service quality in the competitive structure between department and discount store. In this paper, we tried to measure the service quality and overall satisfaction by using 6 sigma, degree of combination and top2box which is a little bit different methodology from traditional ones. The data were collected by the internet survey from 1428 and 1605 department and discount store customers respectively. The result shows the different patterns in the each retail industry. Also, there is a significant difference in terms of sigma level in the each retail industry. Finally, we showed the summarized result as the 6 Sigma Portfolio Matrix.

This article is to analyze the service efficiency of public sector using Data Envelopment Analysis(DEA). We tried to measure the public service quality and overall satisfaction by using several DEA models, degree of combination and top2box which is a little bit different methodology from traditional ones. We used CCR, Super-efficiency and Slack based measure(SBM) model in DEA to measure public service efficiency of the 16 public institutions(7 City Halls and 9 Provincial offices). Since the traditional method based on the measurement model(CCR, BCC) has the fundamental problems without taking account of the existence of slacks inputs and outputs in several efficient units, we suggest to measure the exact amount of efficiency and inefficiency of the public sector and rank analysis many efficient units exactly through the slacks-based measure model(SBM)

Hyundai Motor Company is proposing the fourth evolution of their Hyundai Simplified Model as benchmark results for the validation of CFD codes in aeroacoustics and noise transmission to the interior of a cabin. The focus of this benchmark is on variations in noise level induced by small typical geometry changes that can be found in a car development program. This article presents the noise transmission results obtained with PowerFLOW in combination with a SEA model and shows that it is possible to capture small variations in noise level with a lattice Boltzmann method based code.

Benzodiazepines are widely used for a variety of conditions in psychiatric field. In this article, the author reviewed the indications, the effects on anxiety and sleep disorders, the side effects, the drug-drug interactions, and the tapering strategies. Benzodiazepines were relatively safe and useful for the treatment of anxiety and sleep disorders. However, used clinically, benzodiazepines can induce many adverse effects (e. g. over-sedation, cognitive impairments, paradoxical effects, dependence and withdrawal symptoms, and so on). Currently available informations about their effects on the developing fetus is controversial. For this reason, pregnant women and nursing mother should be cautioned against the use of benzodiazepines. Drug-drug interactions have to be considered in combination treatments. For the tapering issues, gradual tapering was important to prevent the withdrawal symptoms. Especially, the tapering schedules have to be individualized for the each long-term benzodiapzepine users.

In this article, based on the higher-order shear deformation plate theory, buckling analysis of a rectangular plate made of functionally graded piezoelectric materials and its effective parameters are investigated. Assuming the transverse distribution of electric potential to be a combination of a parabolic and a linear function of thickness coordinate, the equilibrium equations for the buckling analysis of an FGP rectangular plate are established. In addition to the Maxwell equation, all boundary conditions including the conditions on the top and bottom surfaces of the plate for closed and open circuited are satisfied. Considering double sine solution (Navier solution) for displacement field and electric potential, an analytical solution is obtained for full simply supported boundary conditions. The accurate buckling load of FGP plate is presented for both open and closed circuit conditions. It is found that the critical buckling load for open circuit is more than that of closed circuit in all loading conditions. Furthermore, it is observed that the influence of dielectric constants on the critical buckling load is more than those of others.

The limited durability of resin-dentin bonds severely compromises the longevity of composite resin restorations. Resin-dentin bond degradation might occur via degradation of water-rich and resin sparse collagen matrices by host-derived matrix metalloproteinases (MMPs). This review article provides overview of current knowledge of the role of MMPs in dentin matrix degradation and four experimental strategies for extending the longevity of resin-dentin bonds. They include: (1) the use of broadspectrum inhibitors of MMPs, (2) the use of cross-linking agents for silencing the activities of MMPs, (3) ethanol wet-bonding with hydrophobic resin, (4) biomimetic remineralization of water-filled collagen matrix. A combination of these strategies will be able to overcome the limitations in resin-dentin adhesion.

In this study, we investigate the possibility of deploying a recently emerging Internet-based technology, called Web log or Blog, to address the problems of knowledge transfer and sharing, particularly in the case of tacit knowledge. We examined the use practice of four blogs and then identified several properties relevant to knowledge transfer and sharing. They include the specific style of blog format, content ownership attribution, posted article organization, communication tools and method, news feed function, and various links from/to outside websites. These features were argued to facilitate knowledge transfer and sharing. In particular, we discussed a great deal about the structure of comments and links as tools for collaboration and idea sharing, which enables the knowledge conversion processes (socialization, externalization, combination, and internalization), We then provide several guidelines to develop blogs as a knowledge management tool.

The concept on idiopathic pulmonary fibrosis (IPF) pathogenesis has progressed from chronic inflammation to aberrant wounding healing and even more to the current paradigms of a multifactorial and heterogeneous disease process. Despite the growth of clinical trials for IPF, most of the results, including N-acetylcysteine combination, warfarin, and bosentan, were disappointing. On the other hand, there have been a number of important developments; the foremost is the licensing of pirfenidone in Europe and Asia. In this article, we briefly review the recent knowledge of pathogenesis of IPF. We also summarize the recent clinical trials regarding the management of IPF.

This article begins by exploring recent developments in the use of the world wide webs in language testing about what a Web-based language test (WBT) is and how they are used in language testing. After a brief review of computer-based testing, WBTs are defined and categorized as low-tech or high tech. Since low-tech tests are the more feasible, they will constitute the focus of this paper. Next, item types for low-tech WBTs are described, and validation concerns that are specific to WBTs are discussed. After a brief overview of the combination of computer-adaptive and Web-based tests, the general advantages as well as design and implementation issues of WBTs are considered before examining the role that testing consequences play in deciding whether a WBT is an appropriate assessment instrument. It is argued that WBTs are most appropriate in low-stakes testing situations; but with proper supervision, they can also be used in medium-stakes situations although they are not generally recommended for high-stakes situations. Some possible ideas for future research are suggested.(169)
