Vascular health late after Kawasaki disease: implications for accelerated atherosclerosis

  • Kawasaki disease (KD), an acute vasculitis that primarily affects young children, is the most common acquired paediatric cardiovascular disease in developed countries. While sequelae of arterial inflammation in the acute phase of KD are well documented, its late effects on vascular health are increasingly unveiled. Late vascular dysfunction is characterized by structural alterations and functional impairment in term of arterial stiffening and endothelial dysfunction and shown to involve both coronary and systemic arteries. Further evidence suggests that continuous low grade inflammation and ongoing active remodeling of coronary arterial lesions occur late after acute illness and may play a role in structural and functional alterations of the arteries. Potential importance of genetic modulation on vascular health late after KD is implicated by associations between mannose binding lectin and inflammatory gene polymorphisms with severity of peripheral arterial stiffening and carotid intima-media thickening. The changes in cholesterol and lipoproteins levels late after KD further appear similar to those proposed to be atherogenic. While data on adverse vascular health are less controversial in patients with persistent or regressed coronary arterial aneurysms, data appear conflicting in individuals with no coronary arterial involvements or only transient coronary ectasia. Notwithstanding, concerns have been raised with regard to predisposition of KD in childhood to accelerated atherosclerosis in adulthood. Until further evidence-based data are available, however, it remains important to assess and monitor cardiovascular risk factors and to promote cardiovascular health in children with a history of KD in the long term.

Virtual Non-Contrast Computer Tomography (CT) with Spectral CT as an Alternative to Conventional Unenhanced CT in the Assessment of Gastric Cancer

  • Objective: The purpose of this study was to evaluate computed tomography (CT) virtual non-contrast (VNC) spectral imaging for gastric carcinoma. Materials and Methods: Fifty-two patients with histologically proven gastric carcinomas underwent gemstone spectral imaging (GSI) including non-contrast and contrast-enhanced hepatic arterial, portal venous, and equilibrium phase acquisitions prior to surgery. VNC arterial phase (VNCa), VNC venous phase (VNCv), and VNC equilibrium phase (VNCe) images were obtained by subtracting iodine from iodine/water images. Images were analyzed with respect to image quality, gastric carcinoma-intragastric water contrast-to-noise ratio (CNR), gastric carcinoma-perigastric fat CNR, serosal invasion, and enlarged lymph nodes around the lesions. Results: Carcinoma-water CNR values were significantly higher in VNCa, VNCv, and VNCe images than in normal CT images (2.72, 2.60, 2.61, respectively, vs 2.35, $p{\leq}0.008$). Carcinoma-perigastric fat CNR values were significantly lower in VNCa, VNCv, and VNCe images than in normal CT images (7.63, 7.49, 7.32, respectively, vs 8.48, p< 0.001). There were no significant differences of carcinoma-water CNR and carcinoma-perigastric fat CNR among VNCa, VNCv, and VNCe images. There was no difference in the determination of invasion or enlarged lymph nodes between normal CT and VNCa images. Conclusions: VNC arterial phase images may be a surrogate for conventional non-contrast CT images in gastric carcinoma evaluation.

만성 뇌혈관 협착폐색증에서 아세타졸아미드 부하 다상 동맥스핀표지 자기공명관류영상으로 평가한 뇌혈류 예비능과 동맥 통과 시간의 변화 (Cerebrovascular Reservoir and Arterial Transit Time Changes Assessed by Acetazolamide-Challenged Multi-Phase Arterial Spin Labeling Perfusion MRI in Chronic Cerebrovascular Steno-Occlusive Disease)

  • 목적 만성 뇌동맥 협착-폐색증에서 아세타졸아미드 부하 다상 동맥 스핀 표지(multi-phase arterial spin labeling; 이하 MP-ASL) 자기공명관류영상을 이용하여 뇌혈류 예비능(cerebrovascular reservoir; 이하 CVR)과 동맥 통과 시간(arterial transit time; 이하 ATT)의 변화를 탐색하였다. 대상과 방법 2019년 6월부터 2020년 10월까지 새롭게 만성 협착-폐색증으로 진단되어 아세타졸아미드 부하 MP-ASL을 시행한 환자를 후향적으로 모집하였다. 부하 전후의 뇌혈류(cerebral blood flow), CVR, 부하전 ATT 및 부하 전후의 ATT 변화량을 중증 협착과 완전 폐색간, 만성 뇌경색 병변의 유무에 따라 비교하였다. 결과 5명의 양측성 협착-폐색을 포함하여 총 32명의 환자가 본 연구에 포함되었다. CVR은 완전 폐색에서 중증 협착보다 유의하게 낮았다(26.2% ± 28.8% vs. 41.4% ± 34.1%, p = 0.004). ATT의 변화는 두 군간 유의한 차이는 없었다(p = 0.717). 만성 뇌경색 병변의 유무에 따른 CVR의 통계적 차이는 미미하였다(29.6% ± 39.1% vs. 38.9% ± 28.7%, p = 0.076). 하지만, 만성 뇌경색 병변이 있는 환자에서 ATT의 단축 정도는 유의하게 작았다(-54 ± 135 vs. -117 ± 128 ms, p = 0.013). 결론 만성 협착-폐색증에서 아세타졸아미드 부하 MP-ASL 검사는 MRI 기반의 CVR 평가 도구로 사용될 수 있다.

Imaging Features of Hepatic Adenoma in a Dog with Atypical Computed Tomographic Findings

  • Computed tomography (CT) findings of hepatic adenoma in veterinary medicine are variable and unlike in human medicine, not defined clearly. A 12-year-old neutered male Shih Tzu presented after a seizure, with weight loss, salivation, and cachexia. An abdominal mass was identified on radiography, and ultrasonographic images showed a mixed echo pattern with marked vascularity. CT showed that the mass originated from caudate lobe, was heterogeneously hypoattenuated compared with the hepatic parenchyma, and had irregular margins. Contrast enhanced CT showed that the mass enhanced like the surrounding liver parenchyma. However, it contained unenhanced areas and enhanced vessels were observed in the arterial phase at the periphery of the mass. The margins of mass were more enhanced in the venous phase than the arterial phase and the hypoattenuating regions within the mass were not enhanced. Greater enhancing in the venous phase is seen with adenomas; however, the heterogeneous enhancement pattern, especially the marginal vascular enhancement and internal hypoattenuating regions, is seen with malignancy. Although this is a single case of hepatic adenoma, the atypical enhanced pattern of this case can provide useful information to predict the malignancy of primary liver tumor.

복부대동맥/장골동맥 분기혈관내 정상 및 박동성 유동의 속도와 전단응력분포 (Velocity and Shear Stress Distributions for Steady and Physiological Flows in the Abdominal Aorta/lLIAC Artery Bifurcation)

  • Steady and physiological flows of a Newtonian fluid and blood in the abdominal gorta/iliac artery bifurcation are numerically simulated to understand the etiology and pathogenesis of atherosclerosis. Distributions of velocity, pressure, and wall shear stress in the bifurcated arterial vessel model are calculated to investigate the differences of flow characteristics between steady and physiological flows and to compare flow characteristics of blood with that of a Newtonian fluid For the given Reynolds number the flow characteristics of physiological flows for a Newtonian fluid and blood in the bifurcated arterial vessel are quite different from thcse of steady flows. No flow separation or flow reversal in the bifurcated region appears downstream of a stenosis during the acceleration phase. However, during the deceleration phase the flow exhibits flow separation in the outer walls of daugtlter branches, which extends to the entire wall region.

Effect of Laser Acupuncture on Arterial Pulse

  • Laser acupuncture is defined as the stimulation of traditional acupuncture points with low-intensity, nonthermal laser irradiation. Possible advantages in using laser acupuncture are the noninvasive, painless and low risks of infection treatment. The purpose of this study is to assess the effect of laser acupuncture on the quality and waveform of arterial pulses. Ten acupuncture points were stimulated repeatedly three times in 30 individuals by laser with emission in the near infrared spectral region (808 nm) using an out power and power density of 45 mW and $143W/cm^2$. The analysis of pulse quality and waveform was performed based on the measurement of arterial pressure of the left and right wrist, using a 3-dimensional blood pressure pulse analyzer. Excess-like pulse quality of subjects before laser acupuncture changed significantly to balanced pulse quality after 10, 20, and 30 minutes of laser acupuncture; coefficient of deficient or excess, $C_{DE}$, decreased significantly from 0.68 before acupuncture to 0.61, 0.55, and 0.55 after 10, 20, 30 minutes of laser acupuncture ($$p{\leq_-}0.006$$), respectively. Other pulse qualities, floating or sinking, slow or rapid, choppy or slippery did not change significantly by laser acupuncture (p > 0.05). Pulse waveform analysis showed that amplitude of main peak (systolic function or aortic compliance, $h_1$) of left and right artery pulse waves decreased significantly after 10, 20, and 30 minutes of laser acupuncture (p < 0.05). Other parameters, duration of one cardiac cycle (T), duration of rapid systolic ejection ($T_1$), duration of the systolic phase ($T_4$), and duration of the diastolic phase ($T_5$) of left and right artery pulses did not change significantly after laser acupuncture (p > 0.05).

Hepatic Angiomyolipoma: Contrast Patterns with SonoVue-enhanced Real-time Gray-scale Ultrasonography

  • This study was conducted to retrospectively evaluate the pattern of contrast enhancement with SonoVue on gray-scale ultrasonography of hepatic angiomyolipoma (HAML). Imaging features of 33 pathologically proven HAML lesions in 33 patients who underwent baseline ultrasound and contrast-enhanced ultrasonography (CEUS) were assessed retrospectively. All lesions were enhanced in the arterial phase and showed whole-tumor filling in. Thirty-two of 33 (97%) lesions showed early positive enhancement in the arterial phase. Twenty-three of these exhibited isoechoic or hyperechoic features in the portal phase. HAML demonstrate characteristic manifestations with SonoVue-enhanced real-time gray-scale ultrasonography.

내독소에 의한 돼지의 급성 폐손상에서 Cyclooxygenase 대사물의 역할에 관한 연구 (The Role of Cyclooxygenase Metabolites in the Pathogenetic Mechanism of Endotoxin-Induced Acute Lung Injury in Domestic Pigs)

  • 연구배경 : 내독소 투여시 혈장과 폐의 림프내에 cyclooxygenase 대사물과 lipoxygenase 대사물이 모두 증가하고, cyclooxygenase 차단제로 초기의 혈역학적 변화가 완화되어 내독소에 의한 급성 폐손상의 기전에서 cyclooxygenase 대사물의 역할이 중요시 되고 있다. 그러나 cyclooxygenase 억제약물이 내독소 투여시 후기에 관찰되는 폐혈관 투과성 증가에 미치는 효과에 대해서는 논란이 있는 실정이고 cyclooxygenase 대사과정에 관여하는 peroxidase에 의해서 산소기가 생성된다고 알려져 있어 cyclooxygenase 대사물이 직접 폐손상을 일으키는지 또는 대사과정에서 생성되는 산소기를 통해 간접적으로 관여 하는지는 확실히 규명되어 있지 않은 상태이다. 저자들은 cyclooxygenase 대사물이 내독소에 의한 급성 폐손상의 기전에 직접적 또는 간접적으로 관여하는지를 알아보기 위해 본 연구를 시행하였다. 방법 : 총 8마리의 집돼지를 내독소 투여군(n=3)과 indomethacin 전처치 후 내독소를 투여한 군(n=5)으로 나누어 시간 경과에 따른 혈력학적 지표의 변화, 혈장내 산화 glutathione(GSSG) 및 기관지폐포세척액내의 알부민 농도를 측정하여 indomethacin 전처치가 내독소에 의한 급성 폐손상에 미치는 영향을 관찰하였다. 결과 : 1) 내독소 투여 후 phase 1(0-2 hr)과 phase 2(2-4.5 hr)에 걸쳐서 심박출양은 감소되고 평균 폐동맥압, 폐혈관저항, 폐포동맥간산소분압차는 증가되었으며 indomethacin 전처치로 두 phase의 변화가 모두 완화되었다. 2) phase 2에서 내독소에 의해 GSSG가 기저치에 비해 유의하게 증가했는데 indomethacin 전처치로는 완화되지 않았다. 3) 기관지폐포세척액내 알부민농도와 투과성지표는 내독소 투여군에 비해 indomethacin 전처치군에서 유의하게 낮았다. 결론 : 이상의 결과로 내독소에 의한 돼지의 급성 폐손상에는 cyclooxygenase 대사물이 초기와 후기 모두에 관여할 것으로 생각되고 indomethacin이 산소기에 의한 산화반응에 영향을 미치지 않는 것으로 보아 산소기를 통한 간접작용보다는 cyclooxygenase 대사물이 직접 병인론적 기전에 관여할 것으로 사료된다.

  • 기관지 동맥 색전술에도 불구하고 반복되는 대량 객혈을 보이는 동공성 폐결핵 환자에서 나선형 CT를 이용하여 Rassmusen 동맥류의 위치 및 크기를 비교적 정확히 진단하여 coil 이용한 경도관 폐동맥 색전술로 성공적으로 지혈 시킨 사례를 경험하였기에 보고하는 바이다.
