Effects of Sagungsan on the Hemostasis, Intracranial Pressure, Blood Pressure and Cardiovascular System in Expreimental Animals (사궁산(莎芎散)이 실험동물(實驗動物)의 지혈(止血), 뇌압(腦壓), 혈압(血壓) 및 심혈관계(心血管系)에 미치는 영향(影響))

  • According to the original documents, Sagungsan is considered as an effective drug for controlling the hypertensive epistaxis induced by tension of autonomic nerve and it's hyperfunction. The present experiment was designed to understand the effect of Sagungsan extract on the hemostatic action, intracranial pressure, blood pressure and cardiovascular system in experimental animals. And thus the bleeding time, prothrombin time, capillary dilation, blood pressure, Intracranial pressure, and enzymatic analysis of the ATPase activities were studied. The result obtained here were as followings: 1. Sagungsan water extract reduced the bleeding time in mouse, and prolonged the prothrombin time in rabbits. 2. The drug extract increased the tail volume by capillary dilation in rats. 3. The drug extract inhibited the increase of intracranial pressure and arterial blood pressure in rabbits. 4. At the early time, the increase of arterial blood pressure by the drug extract significantly inhibited by pretreated atropin and regitine in rabbits. 5. The drug extract relaxed the smooth muscle by stimulating the Mg2+-Ca2+-ATPase activities of gastric sarcoplasmic reticulum isolated from rabbit stomach. 6. The drug extract stimulated the heart contraction by inhibiting the $Mg^{2+}-Ca^{2+}-ATPase$ activities of cardiac sarcoplasmic reticulum isolated from rabbit heart. The inhibitory mechanism was reversible and noncompatitive. 7. The drug extract increased the hepatic blood volume by stimulating the hepatic total ATPase activities and hepatic metabolism. 8. The drug extract acted as a tranquilizer by inhibiting the neural Na+-K+-ATPase activity. According to the results, Sagungsan water extract dilated the capillaries, stimulated the heart beat, and thus increased the blood flow with decreasing the intracranial pressure and blood pressure. These effects stanches the epistaxis collectively.

  • The study was aimed to investigate the effect cuscutae semen(CS) on the vascular systems including changes in blood pressure (BP), regional cerebral blood flow(rCBF) and pial arteriolar diameter of male Sprague-Dawely rats. The changes in rCBF were determinated by laser-Doppler flowmetry, and the changes in diameter of pial arteriole were measured through a closed crainal window. 1. Blood pressure was not affected by CS in rats. 2. rCBF was increased by CS in a dose-dependent manner. 3. Pretreatment with methylene blue(Img/kg), and propranolol(1mg/kg) significantly inhibited CS induced increased in rCBF. 4. Pretreatment with indomethacin(1mg/kg) did not inhibited CS induced increased in rCBF. 5. Pial arterial diameter was increased by CS in a dose-dependent manner. These results suggest that CS causes a diverse response of blood pressure, regional cerebral blood flow(rCBF), and pial arteral diameter. The increased in rCBF is also mediated by adrenergic ${\beta}-receptor $ and guanylate cyclase.

  • The effects of histamine on cardiovascular system in 6 dogs were analyzed. Mongrel dogs, 10 to 16 kg in body weight, were anesthetized with Nembutal (30 mg/kg) and arterial blood pressure, heart rate, central venous pressure, electrocardiogram were recorded and measured plasma potassium concentration. Histamine $(100{\mu}g/ml)$ was infused slowly at the rate of 0.25 ml/min through the external jugular vein until BP was 80/60 and maintained restored BP for more than 5 min. The process repeated $4{\sim}5$ times. At each time before and after infusion every items were recorded and measured. 1. Arterial blood pressure was 142/105 (mean 117) mmHg in control and decreased to 90/60 68) after histamine infusion. 2. Heart rate changed from 175 beat/min to 150 and central venous pressure from 6.2 to 5.2 cm $H_2O$. 3. Plasma potassium concentration was 4.3 mEq/L and slightly increased to 4.7 mEq/L but it was not significant statistically. 4. Most characteristic changes revealed in EKG especially in T-waves. Height, Width, Steepness, and Slimness were increased $1.5{\sim}3.7$ times than control level and Pointedness decresed 0.5 times than before.

  • Obstructive sleep apnea syndrome is associated with significant cardiovascular morbidity and increased mortality. However, it was controversial whether obstructive sleep apnea syndrome could cause pulmonary hypertension. The controversy was resolved by several studies that have shown pulmonary hypertension in 20% to 40% of patients with obstructive sleep apnea syndrome without underlying other cardiopulmonary diseases and reductions in pulmonary arterial pressure in patients with obstructive sleep apnea syndrome after treatment with nocturnal continuous positive airway pressure. Recent studies provide strong evidence for endothelial dysfunction in obstructive sleep apnea syndrome and pulmonary hypertension. Endothelin-1 is a 21 amino acid peptide with diverse biologic activity such as highly potent vasoconstrictor and mitogen regulator that may play a key role in obstructive sleep ap-nea syndrome and pulmonary hypertension. Continuous positive airway pressure therapy is moderately effective in reducing pulmonary arterial pressure. Further researches are needed to assess the therapeutic efficacy of pharmacologic therapy with agents that inhibit the action of endothelin-1 in obstructive sleep apnea syndrome patients with pulmonary hypertension.

  • The influence of essential oils from aboriginal softwoods as Pinus densiflora, Pinus koraiensis, Chamaecyparis obtusa and Chamaecyparis pisifera on the motor coordination, pulse, mean blood artery pressure and pain reducing test was investigated in mice and rats. The motor coordination of mouse was not induced by the inhalation of each oil. Furthermore, these oils did not alter the changes of the mean blood pressure and pulse rate. Chamaecyparis pisifera trand to reduce the mean blood pressure. The abdominal pain induced by acetic acid was reduced only by an essential oil from Pinus koraiensis in mouse.

  • This study was performed to evaluate the effect of electroacupuncture at some acupoint combinations on the cardiovascular system, especially on blood pressure. Electroacupuncture at acupoint combinations of CV2O(+)/GV-16(-),4(+)/GV16(-), KI1(+)/GV20(-), and HT9(+)/GV16(-) did not changed heart rates and blood pressure, but stimulation of HT1(+)/HT7(-) Increased systolic, diastolic and mean arterial blood pressure significantly in dogs anesthetized with tiletamine/zolazepam.

Availability of Capillary Blood Gas Analysis in Neonate (신생아에서 모세혈 가스분석 검사의 유용성)

  • Purpose : Arterial blood gas analysis is frequently performed in neonatal intensive care unit (NICU) to evaluate ventilation and the metabolic state of critically ill infants. In occasions when umbilical arterial catheterization is not available, frequent arterial puncture is mandatory. This requires some technical skill and may occasionally have side effects. So we studied the validity of capillary blood gas analysis which can be performed conveniently compared with arterial blood. Methods : Twenty-four neonates admitted to NICU during April to Aug. 2001 were studied. They were more than two weeks old without indwelling arterial catheters. Thirty-six times, simultaneous arterial, and capillary blood gases were drawn by puncture and the pH, $pCO_2$ and $pO_2$ of each sample was measured. Blood pressure and body temperature was checked before sampling to rule out impaired peripheral circulation. Capillary blood was collected from warmed heels. Results : There was a strong correlation between capillary and arterial pH(r=0.91, P<0.05). The absolute value of the difference between arterial and capillary pH was less than 0.05. Also capillary $pCO_2$ showed correlation with arterial $pCO_2$(r=0.77, P<0.05). Despite a statistically significant correlation between capillary and arterial $pO_2$(r=0.68, P<0.05), the absolute value of the difference was more than 10 mmHg in 92% of cases. Conclusion : Capillary blood gases accurately reflected arterial pH and $pCO_2$ and showed a relative correlation with $pO_2$. Capillary blood gas analysis can be a useful alternative to arterial blood when continuation of the umbilical arterial catheter is no longer available.

  • In this study, we measured the blood flow on arm by non-invasive method and implemented a system to measure variation of the blood flow by estimating bio-electrical impedance and arterial pressure according to cuff pressure. The implemented system measured impedance variation according to pressure variation applied by artificial cuff pressure on the measuring position. The system consisted of pressure measuring part and impedance measuring part using 4-electrode method. Pressure measuring part was composed of semiconductor pressure sensor and electronic circuit for signal processing of sensor output signal. In addition, impedance measuring part was composed of constant current source circuit and lock-in amplifier for detecting impedance signal. We conducted experiments of impedance measuring part using standard resistance for performance evaluation of the implemented system. In addition we experimented to estimate variation of the blood flow by measuring impedances of the experimental group. We estimated ratio of the blood flow resistance using mean arterial pressure and variation of the blood flow. As a result the ratio of the blood flow resistance and variation of blood flow were in an inverse relationship with each other and the correlation coefficient was -0.96776.

  • Objective : Carthami Flos has been used as a herb to promote blood circulation to remove blood stasis in oriental medicine for many centuries. To investigate treatment of cerebral vascular disease(CVA) by promoting blood circulation and removing blood stasis(活血化瘀法), we observed the experimental Effects of Aqua-Acupuncture of Carthami Flos on the Regional Cerebral Blood Flow and Mean Arterial Blood Pressure in Rats. Method : When aqua-acupuncture of Carthami Flos(ACF) was injected into LI4, Liv3, B23, B62, GV16, experimental effects of ACF on the regional cerebral blood flow(rCBF) and mean arterial blood pressure(MABP) is not known. Therefor, this study was designed to investigate the effects of ACF on the change of cerebral hemodynamics. The changes of rCBF and MABP were tested by Laser Doppler Flowmetry(LDF). Results : When ACF was injected into LI4, change of rCBF and MABP were decreased in a injected time-dependent manner. When ACF was injected into Liv3, change of rCBF and MABP were decreased in a injected time-dependent manner. When ACF was injected into B23, change of rCBF was significantly increased in a injected time-dependent manner, MABP did not change. When ACF was injected into B62, change of rCBF was increased in a injected time-dependent manner, change of MABP was significantly decreased after injection 60min. When ACF was injected into GV16, change of rCBF and MABP were similar to the change of normal. Conclusions : In conclusion, I suggested that ACF(LI4, Liv3) has an effect that depress the blood pressure & cerebral hemodynamic acceleration and ACF(B23, B62) has an anti-ischemic effect through the improvement of crebral hemodynamics.

Effects of SoPung-Tang extract on Hypertension and Common Carotid Artery (소풍탕(疎風湯)이 고혈압과 동맥혈관에 미치는 영향)

  • This study was undertaken to define the effect of SoPung-Tang extract on hypertension in spontaneous hypertensive rat and norepinephrine- induced arterial contraction in rabbit. In order to investigate the effect of SoPung-Tang extract on contracted rabbit carotid arterial strips, transverse strips with intact or damaged endothelium were used for the experiment using organ bath. To analyze the mechanism of SoPung-Tang extract-induced relaxation, SoPung-Tang extract infused into contracted arterial strips induced by norepinephrine after treatment of indomethacin, Nu-nitro-L-arginine, methylene blue or tetraethylammonium chloride. Blood pressure was significantly decreased five days after administration of SoPung-Tang extract. SoPung-Tang extract relax arterial strip with endothelium contracted by norepinephrine, but in the strips without endothelium, SoPung-Tang extract- induced relaxation was significantly inhibited. SoPung-Tang relax arterial strip contracted by norepinephrine, but in the strips contracted by high $K^+$, SoPung-Tang extract-induced relaxation was significantly inhibited. The endothelium-dependent relaxation induced by SoPung-Tang extract was decreased by the pre-treatment of $N{\omega}$-nitro-L-arginine or methylene blue, but it was not observed in the strips pre-treated with indomethacin or tetraethylammonium chloride. When $Ca^{2+}$ was applied, the strips which were contracted by norepinephrine in a $Ca^{2+}$-free solution, arterial contraction was increased. But pre-treatment of SoPung-Tang extract inhibited contractile response to $Ca^{2+}$. We suggest that SoPung-Tang could be applied effectively for hypertension and may suppress influx of extra-cellular $Ca^{2+}$ through the formation of nitric oxide in the vascular endothelial cells.
