A wireless optical fiber interferometer arterial pulse wave sensor system is developed for remote sensing. The wireless optical fiber sensor system consists of Zigbee communication modules and an optical fiber interferometer arterial pulse wave sensor. The optical fiber arterial pulse wave sensor is an in-line Michelson interferometer enclosed with steel reinforcement in a heat-shrinkable tube. The Zigbee communication modules are composed of an ATmega128L microprocessor and a CC2420 Zigbee chip. The arterial pulse waves detected by the optical fiber sensor were transmitted and received via the Zigbee communication modules. The experimental results show that the wireless optical fiber sensor system can be used for monitoring the arterial pulse waves remotely.
Appreciation of the large volume deficits which may occur in surgical or trauma patients due to blood loss has led to vigorous transfusion techniques designed to overt hypovolemic shock and ischemic damage to vital organs which may develop in minutes during the hypovolemic state. In a significant proportion of patients treated with massive rapid blood or fluid transfusion, hypervolemia occurs and life threatening pulmonary edema may develop. Especially, hypervolemia may occur during transfusion for preventing development of the so-called low output syndrome following cardiac surgery. However, the most effective indicator which reveals the adequate level of transfusion is not settled yet. The present study was aimed to compare the effectiveness of the indicators suggested thus far and to determine the most sensitive one. Eight dogs were experimentally studied in terms of left atrial pressure, pulmonary arterial systolic pressure, central venous pressure, mean systemic arterial pressure and heart rate before and after induced hypervolemia with infusion of 600ml heparinized homologous blood. Immediately after induced overtransfusion of the blood, pulmonary arterial systolic pressure increased 75.0%, in omparison with the control before transfusion, left atrial pressure 58.8%, central venous pressure 44.6%, and mean systemic arterial pressure 10.1%, one hour after transfusion, pulmonary arterial systolic pressure 40.0%, left atrial pressure 21.2%, central venous pressure 14.5%, and mean systemic arterial pressure 3.2%, central venous pressure 14.5%, and mean systemic arterial pressure 3.2%, respectively. Heart rate showed no significant change throughout the experiment. These result suggested that the changes of the pulmonary arterial systolic pressure is the most sensitive indicator for detection of hypervolemia during blood transfusion.
There are many things in common between hemodynamics in arterial systems and multibody dynamics in mechanical systems. Hemodynamics is concerned with the forces generated by the heart and the resulting motion of blood through the multi-branched vascular system. The conventional hemodynamics model has been intended to show the general behavior of the body arterial system with the frequency domain based linear model. The need for detailed models to analyze the local part like coronary arterial tree and cerebral arterial tree has been required recently. Non-linear analysis techniques are well-developed in multibody dynamics. In this paper, the studies of hemodynamics are summarized from the view of multibody dynamics. Computational algorithms of arterial tree analysis is derived, and proved by experiments on animals. The flow and pressure of each branch are calculated from the measured flow data at the ascending aorta. The simulated results of the carotid artery and the iliac artery show in good accordance with the measured results.
Cardiovascular effects of propofol, were assessed after premedication with xylazine(1.0 mUkg, IM) under oxygen supply(200 ml/kg/min) via a endotracheal tube. Twelve adult mixed-breed dogs were divided into four groups; 0.2(Group 1), 0.4(Group 2), 0.6(Group 3) and 0.8 mg/kg/min(Group 4) of propofol respectively. Arterial blood pressure and electrocardiogram were monitored with a physiograph after an arterial catheter was inserted into the femoral artery. pH, arterial carbon dioxide tension($PaCO_2$and arterial oxygen tension($PaO_2$) were evaluated with arterial blood collected through the inserted catheter. Diastolic arterial pressure, systolic arterial pressure and mean arterial pressure were decreased slightly in Group I IIand III, but decreased significantly in Group IV. They were increased rapidly after stopping propofol infusion in Group IV pH was maintained in normal range in Group I, II and m, but was decreased in proportion to time passing in Group IV. $PaCO_2$ was increased significantly only in Group IV but $PaO_2$ was maintained in normal range in all groups Although heart rate was recorded in normal range for 90 minutes, arythmia was noted after stopping propofol infusion in all groups. It was concluded that propofol depressed the cardiovascular system in proportion to infusion dosage, and 0.8 mg/kg/min of propofol infusion rate was not appropriated in canine anesthesia with xylazine premedication.
Objective : To clarify the benefits and therapeutic effects of intra-arterial papaverine infusion on the symptomatic cerebral vasospasm, we analyzed the results of treatment in 32 patients retrospectively. Methods : A total of 510 patients underwent surgical clipping or endovascular intra-aneurysmal treatment for ruptured intracranial aneurysm between May, 1996 and June, 1999. The delayed ischemic deficit(DID) was developed in 90 of 510 patients. Of these 90 patients, 32 developed symptomatic vasospasm inspite of using modest "3H therapy". The brain CT scan was taken before the intra-arterial infusion of papaverine. The 32 patients underwent 42 intra-arterial papaverine infusion. The symptomatic vasospasm was divided into three groups : deterioration of mental status(Group 1), appearance of a focal neurologic deficit(Group 2), or both(Group 3). We measured Glasgow Coma Scale(GCS), arterial diameters, and cerebral circulation time(CCT) at the time of pre- and postangioplasty. Results : The number of patients in group 1, 2 and 3 were 26, 7, 9 respectively. Eighteen cases showed improvement of GCS more than 2 scores, 16 more than 1, and 8 showed no change of GCS. Average cerebral circulation time(CCT) was decreased ranging from 0.0%-67.5%, and arterial diameters were increased in 21 cases ranging from 1 to 4 folds. Conclusion : Intra-arterial papaverine infusion seemed to have therapeutic effects on symptomatic vasospasm by improving the neurological signs and increasing the arterial diameter. We suggest that intra-arterial papaverine infusion would be an useful adjunctive therapeutic modality in symptomatic vasospasm.
목 적 : 동맥혈 가스 분석은 신생아 집중 치료실에서 환아의 상태를 평가하기 위해 자주 시행하는 검사이나 제대 동맥 카테테르를 이용할 수 없는 경우에는 동맥 천자를 반복해야 되는데 기술적인 어려움과 위험성이 따르게 된다. 이에 저자들은 간편히 시행할 수 있는 모세혈 가스 분석 검사로 동맥혈 가스치를 추정하는 것이 타당성을 가지는지 조사하기 위해 이 연구를 시행하였다. 방 법: 2001년 4월부터 8월까지 방지거병원 신생아 집중 치료실에 입원하여 치료받은 환아 중 2주 이상된 신생아로서 동맥내 카테테르를 삽입하지 않은 상태에서 환기와 대사 상태 평가를 위해 혈액 가스 검사를 시행한 환아를 대상으로 하였다. 대상아들은 발뒤꿈치를 가온한 뒤 모세혈을 채혈하였고 곧 이어 동맥 천자를 시행하여 얻은 동맥혈과 함께 바로 검사실로 보내어 가스 분석하여 각 수치를 비교하였다. 결 과 : 1) 조사된 24명의 신생아의 재태 연령은 32-41주로 채혈은 모두 생후 2주일 이후에 시행되었다. 2) 모세혈과 동맥혈의 $pCO_2$, pH 사이에는 높은 상관 관계가 있었다(P<0.05). 3) 모세혈과 동맥혈의 $pO_2$ 사이에도 유의한 상관관계를 보였으나(P<0.05), 절대값의 차이는 92%에서 10 mmHg 이상이었다. 4) 모세혈과 동맥혈 가스의 상관 관계는 pH, $pCO_2$, $pO_2$의 순으로 유의하였다. 결 론 : 이상의 결과로 보아 동맥혈화된 말초혈액을 이용한 모세혈 가스 분석은 동맥혈 채혈이 어려운 경우에 유용한 대체 수단이 될 수 있을 것으로 생각된다.
본 연구에서는 시뮬레이션을 이용하여 동맥압 파형 형태에 대한 오실로메트릭 방법을 분석하고 수축기압과 이완기압을 검출할 수 있는 새로운 알고리즘을 제시하였다. 동맥압 파형 형태에 대한 오실로메트릭 방법을 분석하고 수축기압과 이완기압을 검출할 수 있는 새로운 알고리즘을 제시하였다. 동맥압 파형 형태에 대한 오실로메트릭 방법을 분석하기 위해 동맥압 파형 형태를 쉽게 가변할 수 있는 동맥압 모델을 만들었으며, 기존의 정적인 동맥 압력-용적 지수함수 모델을 이용하여 오실로메트릭 모델의 구현 및 컴퓨터 시뮬레이션을 수행하였다. 동맥압 파형 형태와 보편화된 혈압 검출 기준인 특성비율과의 상관관계 분석을 통하여 동맥압 파형 형태와 맥압의 영향 때문에 특성비율이 수축기압과 이완기압을 결정하는 유일한 기준이 될 수 없음을 밝혔으며, 동맥압 파형 형태와 오실레이션 파형 형태의 상관관계 분석을 통하여 오실레이션 파형으로부터 동맥압 파형 형태를 추정할 수 있는 방법을 제시하였다. 최대 크기 오실레이션 파형과 동맥압 파형의 관계로부터 맥압을 구할 수 있는 맥압 표를 구성하여 수축기압과 이완기압을 검출할 수 있는 혈압 검출 알고리즘을 제시하였으며 그 결과 수축압, 이완압, 평균압의 절대편차 평균값은 각각 1.62%, 2.40%, 2.20%를 얻었다. 결론적으로 제안된 알고리즘은 정확한 혈압검출을 위한 유용 가능성을 보였다.
Congenital coronary arterial fistulae are the most prevalent hemodynamically significant congenital coronary artery malformations. Definition of congenital coronary arterial fistula is a direct communication between a coronary artery and the lumen of one of the four cardiac chambers or coronary sinus or SVC, pulmonary artery or pulmonary vein close to the heart. It is often associated with additional congenital or acquired heart disease. A 49 year old male patient was admitted with the chief complaints of anginal pain and exertional dyspnea for 9 months. He was diagnosed as the right coronary arterial fistula combined with right coronary arteriosclerotic stenosis and old inferior myocardial infarction by cardiac evaluation. The right coronary arterial fistula was communicated between the just distal portion of acute marginal branch and coronary sinus. The operative procedure was as followings; after suture ligation of fistula opening in the coronary sinus under beating heart, coronary arterial bypass grafting with saphenous vein was performed at the just proximal portion of the posterior descending branch under cardiopulmonary bypass. The postoperative course was uneventful and he was discharged without anginal pain at the 8th postoperative day.
This study deals with the accident model of arterial link section in Cheongju. The objective is to develop the accident model of arterial link section using the logistic regression. In pursuing the above, the study uses the 258 accident data occurred at the 322 arterial link section. The main results are as follows. First, Nagellerke $R^2$ of developed accident model is analyzed to be 0.309 and t-values of variable that explains goodness of fit are evaluated to be significant. Second, the variables adopted in the model are AADT, the number of exit and entry. These variables are all analyzed to be statistically significant. Finally, the analysis of correct classification rate shows that the total accident of correct classification rate is analyzed to be 72.7% at the arterial link section.
The purpose of this study is to investigate vasorelaxant effect of Epimedium koreanum(EK) extract on rabbit carotid artery. In this study, to determine vasorelaxant effect of EK extract on rabbit carotid artery, arterial rings with intact or damaged endothelium were used for experiment using organ bath, and were contracted by norepinephrine(NE). After being contracted, arterial rings were treated with EK extract in a dose-dependent manner To study its mechanism, the contracted arterial rings induced by NE were pretreated with indomethacin(IM), $N_{\omega}$-nitro-L-arginine(L-NNA), methylene blue(MB) or tetraethylammonium chloride(TEA) and 0.1 $mg/m{\ell}$ EK extract was added. To analyze the effect of the EK extract on influx of extracellular calcium chloride($Ca^{2+}$) in rabbit carotid artery, in $Ca^{2+}$-free krebs solution, krebs solution containing 1 mM $Ca^{2+}$ was infused into the contracted arterial ring by NE after pretreatment of EK extract. To measure the cytotoxicity of the EK extract, cell viability of human umbilical vein endothelial cell(HUVEC) was measured by MTT assay, and nitric oxide(NO) was measured by Griess reagent. The EK extract significantly was relaxed the arterial ring with intact endothelium contracted by NE, but the vasorelaxant effect of the EK extract was inhibited in the arterial rings with damaged endothelium. The vasorelaxant effect of the EK extract was not different between the IM-pretreatedand and non-treated arterial rings. The vasorelaxant effect of EK extract were significantly inhibited, when arterial rings were pretreated with L-NNA, TEA, MB. And in $Ca^{2+}$-free krebs solution, increasing of arterial contraction by $Ca^{2+}$ addition were also inhibited by the treatment of EK, but not significant. The treatment of EK extract was increased NO concentration in HUVEC. This study suggested that the vasorelaxant effect of EK extract would be related with EDHF and NO production and increasing of cyclic GMP.
