Arthroscopic Treatment of the Intratendinous Ganglion of the Long Head of Biceps Brachii - A Case Report

  • A ganglion is a benign cystic mass, commonly found around a joint or tendon sheath. It frequently occurs at the wrist, foot, ankle, and knee. Intratendinous ganglion has been rarely reported, and intratendinous ganglion of the long head of biceps brachii is extremely rare. According to our literature review, this is the third case of intratendinous ganglion of the long head of biceps brachii, and the first case of arthroscopic treatment. Therefore we report a case of an arthroscopic treatement for an intratendinous ganglion of the long head of biceps brachii.

  • The pain around the posterior knee, called 'popliteal fossa', has been known to be caused by a variety of disease entities. Venous malformation is a very rare cause of popliteal area pain, and its diagnosis is frequently delayed, missed, or given incorrectly. Here, we report a case of a patient with popliteal fossa pain for 2 years and was diagnosed as intramuscular venous malformation using ultrasound.

  • Arthritis may be broadly classified as degenerative - related to defects in cartilage and other joint constituents, often age-associated - or inflammatory disease. Inflammatory arthritis called as rheumatoid arthritis (RA) is a chronic inflammatory arthropathy and characterized by a destructive arthritis. RA encompasses infectious arthritis, arthritis caused by intra-articular deposits of crystalline material (gout), syndromes associated with genetic defects (familial Mediterranean fever), and the immune-mediated inflammatory arthropathy. Degenerative arthritis called as osteoarthritis (OA), which is most frequently occurring, causes degenerative figures of knee, waist and knuckle, and accompanies severe pain around the cartilage. Also, it may cause morning stiffness, gelling effect, tenderness, bone swelling, crepitus, and motion disorders.

Comparison of the old-old aged women's pants pattern by lower body shape using 3D simulation

  • The purpose of this study was to compare and analyze the pants pattern of the lower body of 70-85 aged women. I tried to present excellent pants pattern that is convenient and suitable for the activities of elderly women. Through this, I tried to provide basic data necessary for making elderly women 's pants. As a result of evaluating the appearance of the pants pattern according to the body type, the fit of the type 2 was evaluated as the highest in the item except the position of the front waist line. For the objective evaluation of the 3D simulation, the air gap of the pants by the body part analysis showed that there was not much difference in the air gap around the waist circumference, hip circumference, thigh circumference, and knee circumference by applying the same drawing method and body size. However, in type 2, the air gap of thigh and knee circumference parts was larger than that of type 1 and type 3. Because type 2's legs were thin but it used same size of hemline. It was thought that it is necessary to adjust the space of front and back crotch length to 1 inch (2.54 cm) instead of 1.9 cm (3/4 inch) so that the waistline position of all three types can be set up a little to cover the abdomen. This study was conducted to investigate changes in body shape of elderly women and to develop appropriate pants patterns.

Effects of Different External Loads on the Activities of the Gluteus Maximus and Biceps Femoris during Prone Hip Extension in Healthy Young Men

  • PURPOSE: This study examined the effects of different external loads on the muscle activities around the hip during prone hip extension with knee flexion (PHEKF) exercise in healthy young men. METHODS: Sixteen healthy adult males participated in the study. A pressure biofeedback unit was used to provide feedback to the participants during the abdominal drawing-in maneuver (ADIM) with PHEKF. Sandbags (0 kg, 1 kg, 2 kg, and 3 kg) were used to provide external resistance. The quadriceps was contracted to maintain knee flexion 90° against resistance. Each resistance condition using a sandbag weight was given in random order. Surface electromyography (sEMG) was used to measure the electrical activity of the gluteus maximus, biceps femoris, and erector spinae during PHEKF. RESULTS: The muscle activity of the gluteus maximus was highest with the 3 kg resistance and lowest with 0 kg (F = 128.46, P = .00). The muscle activities of the biceps femoris and erector spinae were highest with 0 kg and lowest with 3 kg (F = 29.49, P = .00). The muscle activity rate of the gluteus maximus/biceps femoris was highest with 3 kg and lowest with 0 kg (F = 37.49, P = .00). CONCLUSION: The activity of the gluteus maximus was increased using a higher external weight load during PHEKF, while the activity of the biceps femoris decreased. These findings suggest that an external weight is needed during hip extensor exercise to strengthen the gluteus maximus and inhibit the biceps femoris.

신발 뒤굽 높이가 계단 올라가기 및 내려오기 동안 내측광근과 외측광근의 근 활성도에 미치는 영향 (Effect of Shoe Heel Height on Vastus Medialis and Vastus Lateralis Electromyographic Activity During Stair Ascending and Descending)

  • This study aimed to investigate the effect of differing heel heights on the electromyographic (EMG) activity in vastus medialis (VM) and vastus lateralis (VL) during stair ascending and descending activities. A total of 26 healthy women volunteered to perform stair-ascending and stair-descending tasks with 3 heel heights: barefoot, 3 cm, and 7 cm. The EMG activities of the VM and VL were recorded during the tasks. During the stair ascending and descending tasks, the EMG activities of both VM and VL significantly changed with differing the heel heights (p<.05). Moreover, the EMG activities of VM and VL during the stair ascending task were significantly higher than the corresponding values during the stair-descending task (p<.05). However, there were no significant differences between the VM:VL EMG ratios for the 3 heel heights (p>.05). The VM:VL EMG ratios between the 2 tasks differed significantly in the 7 cm high heel condition (p<.05). Despite an increase in the EMG activities in both VM and VL during stair ascending and descending tasks, there was no change in the relative EMG intensities of VM and VL, which was measured by calculating the VM:VL ratio this result indicates that no VM:VL imbalances were elicited. The relative EMG intensities of VM and VL during stair descent were lower than the corresponding values during the ascent, suggesting that VM and VL may show an imbalance in the eccentric activation during the weight-acceptance phase. This study provides useful information that will facilitate future research on how heel height affects muscle activity around the knee joint.

신체적합성을 고려한 제깅스(Jeggings) 패턴 개발 -20~24세 여성을 중심으로- (Development of Jeggings Pattern Considering Body Fit -Focus on 20-24 Year Old Females-)

  • This study developed an ideal jeggings pattern with an improved body fit for 20-24 year old females. The results of the study are as follows. First, samples for wearing test were made using jegging patterns from three manufacturers; subsequently, one pattern with a relatively outstanding body fit was selected from the first test. The selected pattern suggested a tight knee, belly, and crotch, high front waist line, and low back waist line. The side lines were also lean to the front; consequently, the front hip width was extended for 0.5cm for more space around the belly and the extended front knee part for 0.2cm to increase moving suitability to fix the leaning side lines. A 1cm front waist line was extended for 0.5cm to fix it lower; in addition, the back waist line 3.0cm was extended for 0.5cm to heighten it. The crotch width was also extended for 0.5cm to add more space. The second wearing test for the fixed forms were evaluated for their outstanding features in appearance as well as moving functions that were proposed as final jeggings forms. The results of the study suggested a jeggings pattern with a better body fit using manufacturer patterns that can be used as basic data for pant patterns with better fitting using various stretch fabrics.

근위 경골에 발생한 악성종양 절제 후 슬관절 신전력 재건술 -증례 보고- (Reconstruction of Extensor Mechanism After Prosthetic Replacement of The Proximal Tibia)

  • 근위 경골부에 발생한 악성종양의 사지구제술은 화학요법의 발달로 인해 대표적인 치료 방법이 되었다. 다양한 술식의 보고에도 불구하고, 종양 절제 후 종양 대치물을 이용한 사지구제술에 있어 슬개건의 부착 부위가 상실되어 슬관절의 신전력을 재건 하기가 어려운 것이 공통적인 문제로 지적 되고있다. 본 연구는 경골 근위부의 종양을 절제한 후 저온열처리 한 다음 원위 대퇴골을 절제하여 저온열처리 자가경골 및 종양대치물과 결합하고, 슬개골을 자가경골과 결합된 원위 대퇴골에 고정하여 슬관절 신전력을 재건 한 2례를 보고하고자 한다.

양측 슬괵건을 이용한 후방 십자 인대 및 후외측 지대 재건술 (Posterior Cruciate Ligament and Posterolateral Structure Reconstruction using Bilateral Hamstring Tendons)

  • 목적: 저자들은 후방 십자 인대 손상과 후외측 회전 불안정성이 동반된 환자에서 양 측 슬괵건을 채취하여 각각 후방 십자 인대 및 후외측 지대 재건술을 시행하고 그 결과를 보고하고자 한다. 대상 및 방법: 2002년 10월부터 2004년 3월까지 후방 및 후외측 회전 불안정성으로 양 측 슬괵건을 이용한 후방십자 인대 및 후외측 지대 재건술을 시행한 10명의 환자를 대상으로 하였다. 남자가 9명 여자가 1명이었고 평균 연령은 36.8세였다. 평균 추시 기간은 17개월(12개월-2년 4개월)이었다. 후방 십자 인대는 환측 슬괵건을 이용하여 횡고정 핀과 Intrafix (Mitek, Norwood, MA) 로 고정하였고 후외측 지대는 비골두에 터널을 뚫어 건측 슬괵건을 통과 시킨 후 대퇴골 외과의 등장점에 흡수성 나사로 고정하였다. 후방 전위 정도는 후방 부하 방사선 사진으로 계측하였고 이학적 검사상 회전 불안정성의 정도를 Noyes와 Barber-Westin의 분류에 따라 평가하였다. 슬관절 기능 평가는 Lysholm 점수와 International Knee Documentation Committee (IKDC) 평가 기준을 이용하였으며 건측 공여부의 평가는 IKDC 평가기준 공여부 항목과 슬관절 굴곡력으로 평가하였다. 결과: 후방 부하 방사선 사진상 술 전 13.3 mm에서 술 후 3.7mm로 호전되었으며 $30^{\circ}$$90^{\circ}$ 경골 외회전 검사상 기능적 소견을 보인 예가 7예, 부분 기능적 소견을 보인 예가 2예, 실패의 소견을 보인 예가 1예였다. Lysholm 점수는 술 전 54점에서 술 후 86점으로 호전되었다. IKDC 평가 기준상 거의 정상 이상이 8예, 비정상이 2예 이었으며 심한 비정상의 소견을 보인 예는 없었다. 건측 하지 공여부 평가상 2예에서 감각 저하를 호소하였으나 슬관절 굴곡력의 약화 소견은 없었다. 결론: 후방 십자 인대 손상과 후외측 회전 불안정성이 동반된 환자에서 양 측 슬괵건을 이용한 후방 십자 인대 및 후외측 지대 재건술은 양호한 임상 결과와 건측 채취부의 합병증이 적은 좋은 치료 방법으로 사료된다.

부자(附子)의 경구투여(經口投與)가 RAT의 척수손상(脊髓損傷)에 미치는 영향(影響) 및 비교(比較) (The Effects of Radix aconiti on the Contraction of Rat's Spinal Cord Injury)

  • Purpose In this study, We observed the recovery process in the ability to move in the hind limbs of the rats whose spinal cord injuries were treated by Radix acouniti(RA). The purpose of this study is to see the effects of Radix acouniti(RA) water extract on the contraction of rat's spinal cord injury. Procedure First, the rats were subjected to hemisectional spinal cord injuries by a scalpel blade. Those rats, then, were divided into three groups: Sham operated rats group and the experimental group, which received the Radix acouniti(RA) water extract, and the control group, which had no treatment. Their recovery in the ability to walk was observed by the Open Field Test (OFT) for 14 days after the injuries. Method The OFT was applied at four points: the hip, knee, ankle joint, and the tail. Each joint was given a movement rating of from 0 to 3, depending on the amount of movement. A movement rating of 0 designates no movement, a 1 designates slight movement, a 2 designates increased movement, and a 3 designates active movement. Slight movement is defined as a joint displaying less than or equal to 30% of that joint range, increased movement is displaying less than or equal to 60% of that joint's range, and active movement is greater than or equal to 90% of that joint's range. Tail movement is also graded on a scale of 0 to 3. A rating of 0 indicates that the tail is down 100% of the time, one of 1 indicates that the tail is down more than 10%, one of 2 shows that the tail is down less than 50% but more than 10% of the time, and one of 3 shows that the tail is down less than 10% of the time. All four ratings were added together and then averaged to arrive at a single score. Results The sham group which did not go through spinal cord injuries showed near normal results on all 3 joints and tail from right after the operation, which one would expect. The RA oral application group showed more effective recovery of movememt function than the control group around 4 days after the spinal cord injuries. However, after 14 days, both groups displayed almost the same degree of movement recovery. The results of this study are summarized as follows: 1. After 14days the spinal cord injuries, movement was recovered in sham operated group, control group, and experimented group in the hip, the knee, the tail and then the ankle of rats, in that order. 2. Around 7 days after the spinal cord injuries, the experimental group proved the effectiveness of the therapy in terms of movement recovery. 3. The level of ALT, ALP, AST in RA treated group was slightly increased. 4. The level of BUN and creatinine in RA treated group was slightly increased. The above results indicate that RA therapy at an early stage can bring about better movement recovery in patients with spinal cord injuries from traffic accidents or industrial disasters. But there is apparent side effect of RA on clinical, therefore the study on this should be continued.
