인공 종양대치물을 이용한 사지구제술후의 보행 분석 (Gait Analysis of Patients with Tumor Prosthesis around the Knee)

  • Prosthetic replacement is one of the most common methods of reconstruction after resection of malignant tumor around the knee. Gait analysis provides a relative objective data about the gait function of patients with prosthesis. The purpose of this study was to compare the gait pattern of the patients who underwent limb salvage surgery with prosthesis for distal femur and that of patients with prosthesis for proximal tibia. This study included ten patients (4 males, 6 females, mean age 22.7 years, range 14-36) who underwent a wide resection and Kotz hinged modular reconstruction prosthesis replacement and six normal adult(Control). The site of bone tumor was the distal femur (Group 1) in six patients and proximal tibia (Group 2) in 4 patients. The follow-up period ranged from 15 to 82 months (mean : 33 months). The evaluation consisted of clinical assessment, radiographic assessment, gait analysis using VICON 370 Motion Analysis System. The gait analysis included the linear parameters such as, walking velocity, cadence, step length, stride length, stance time, swing time, single support and double support time and the three-dimensional kinematics (joint rotation angle, velocity of joint rotation) of ankle, knee, hip and pelvis in sagittal, coronal and transverse plane. For the kinetic evaluation, the moment of force (unit: Nm/kg) and power (unit: Watt/kg) of ankle, knee and hip joint in sagittal, coronal and transverse plane. In the linear parameters, cadence, velocity, step time and single support were decreased in both group 1 and group 2 compared with control. Double support decreased in group 2 compared with control significantly(p<.05). In contrast to our hypothesis, there was no significant difference between group 1 and group 2. 환측사용유도 고정식자전거운동이 무릎관절 기능에 미치는 영향: 무릎인공관절수술 환자를 대상으로 (The Effect of Cycle Ergometer Exercise Inducing Movement of the Affected Side on Knee Joint Function after Total Knee Arthroplasty)

  • Purpose: The purpose of this study was to examine the effect of cycle ergometer exercise inducing movement of the affected side on knee joint function after total knee arthroplasty (TKA). Methods: The primary experiment was conducted on 19 members of the cycle ergometer exercise group to measure the muscle activity of the rectus femoris, hamstring, tibialis anterior, and gastrocnemius muscles during cycle ergometer exercise that induced the affected side's movement. In the second experiment, after receiving physiotherapeutic intervention for 30 min, the general bicycle exercise group and cycle ergometer exercise group performed the corresponding exercise for 15 min, 5 times per week, for 2 weeks. The ROM, muscle strength, pain, and balance were then measured and compared between the two groups. Results: In the results of the primary experiment, cycle ergometer exercise inducing movement of the affected side showed a significantly larger increase in the activity of leg muscles (rectusfemoris, hamstring, tibialis anterior, gastrocnemius) on the affected side than the general bicycle exercise (p <0.05). In the second experiment, the cycle ergometer exercise group showed a significantly larger increase in range of movement of affected side knee flexion and muscle strength of affected side knee flexion, knee extension, and plantarflexion than the general bicycle exercise (p <0.05). No significant between-group difference was observed in pain and balance before or after the intervention (p >0.05). Conclusion: Cycle ergometer exercise inducing movement of the affected side increases use of the muscles around the affected side knee joint after TKA more than general bicycle exercise and produces better effects for enhancing muscle strength. The application of cycle ergometer exercise inducing movement of the affected side is expected to reduce the patients' unbalanced use during the early postoperative period and help them to quickly return to normal daily life through rapid muscle strength recovery.

요가용 3차원 무릎보호대 개발 및 평가 (Development of the 3D Knee Protector for Yoga)

  • This study aims to develop three dimensional (3D) yoga knee protectors that provide excellent wearing comfort. Three types of pads were modeled using 3D human data: two types of 3.0-cm-wide pads separated into top and bottom with thicknesses of 0.1 cm (TPU-1: A) and 0.2 cm (TPU-2: B); and one type with three 0.2-cm-thick separated panels (TPU-S: C). Based on these models, five knee protectors were developed using 3D patterning and 3D printing. Types A, B, and C were integrated with 0.6-cm neoprene pads. Type D was fabricated with a donut-shaped 0.6-cm neoprene pad inserted, while Type E consisted of two discrete 0.6-cm neoprene pads embedded in the protector's upper and lower sides. Wearing comfort was evaluated in terms of fit, pressure, and cushioning while in a standing and kneeling position and while in motion. The findings suggest that the fabricated knee protectors were evaluated as comfortable to the individuals with knee pain, rather than those without knee pain. The individuals with knee pain preferred the soft pads made of neoprene positioned around the knee (NEO-S: E), while those without knee pain favored the cushioned pads with a pattern structure maintained by thin 3D-printed pads (TPU-1: A).

생쥐 무릎관절 공간 발생에 있어 아포프토시스 세포에 관한 면역전자현미경적 연구 (An Immune-Electron Microscopic Study of the Apoptotic Cell during Mouse Knee Joint Development)

  • This study was designed to investigate the appearence and the characteristics of the apoptotic cells and the process of the joint cavity formation in mouse knee joint. Fetal mouse knee joints from 15 to 19 days of gestation were used. Paraffin-embedded serial sections, stained with H & E for light microscopic observation, Epon 812 embedded thin sections for electron microscopic observation and Lowicryl HM 20 embedded thin sections for immune-electron microscopic observation were prepared. Monoclonal antibodies to $\beta-tubulin$ and polyclonal antibodies to tissue transglutaminase were used for immune-electron microscopic study. The results obtained were as follows. 1. At 15 days of gestation, blood vessels, which have invaded in the mesenchymal cells, were present in the synovium, to form the joint cavity in the future. 2. At 16 days of gestation, the joint cleft was first appeared and several RBCs were present in the joint cleft. The invasion of blood vessels into the joint cleft was continuing, and apoptotic cells were present in the inner cell layer, adjacent to the joint cleft. Necrotic cells were also present in the outer cell layer; they were present 18 days of gestation, but apoptotic cells did not appear after 17 days of gestation. 3. In the apoptotic cells, transglutaminase were localized around vacuoles and the marginal site of the cytoplasm. 4. In the apoptotic cells, tubulin was around the endoplasmic reticulum and the marginal site of the cytoplasm. In the late stage of apoptotic cells, tubulin was localized diffusely in the cytoplasm. Tubulin was also strongly labeled around in the cytoplasm of the neighboring cell at which the apoptotic body was phagocytosed. Tubulin labeled particles were apparently increased in the seperated apoptotic bodies. On the basis of the above findings, it is proposed that during the development of the mouse knee joint, blood vessel invasion first occurs and then apoptosis and cell necrosis follow it. In the apoptotic cell, present in the synovium of the developing knee joint of the mouse. it is suggested that the redistribution of tubulin is associated with apoptotic process. And transglutaminase overexpressed in the apoptotic cell.

슬관절 주위에 발생한 연부조직 골육종 - 증례 보고 - (Extraskeletal Osteosarcoma Around the Knee Joint - A Case Report -)

  • 연부조직에 발생되는 골육종은 매우 드문 종양으로, 세계적으로 소수의 예가 보고되었으며 한국에서는 2례가 보고되었을 뿐이다. 문헌상 세계에서 최고령의 증례인 91 세 남자에서 외상, 방사선 조사, 화골성 근염, 피부 근염 등과 관련없이 슬관절 주위에 발생한 연부조직 골육종을 경험하였다. 절제술만으로 치료하였으며, 환자는 수술 후 1년 추시 상 생존해 있고 국소재발이나 전이의 징후가 없으며 슬관절의 기능도 양호한 상태이다.

인공 슬관절 치환술 후 족관절 변화에 대한 방사선학적 연구 (Radiologic Evaluation of Change of Ankle Joint after Total Knee Arthroplasty)

  • Purpose: To evaluate the incidence and describe radiologic pattern of ankle arthritis following change of mechanical loading axis by total knee arthroplasty. Materials and Methods: We reviewed radiographs of 419 cases, 243 patients underwent total knee arthroplasy from January 2002 to October 2006 retrospectively. We described radiologic parameters around the ankle joint and measured the amount of change of knee varus or valgus angle by comparing preoperative and postoperative anteroposterior standing lower extremities AP X-rays. We divided cases into two groups, one with radiologically arthritic change of the ankle joint and the other one without any radiologic change after surgery. We compared two groups in each parameters and analyzed statistically (SPSS v13.0). Results: Three hundred eighty one cases were divided into varus group and 38 cases in valgus group. 125 cases were divided into ankle arthritic change-positive subgroup among the varus group and 251 cases were in negative subgroup. The amount of varus angle correction by total knee arthroplasty showed significant difference between two subgroups. There was no significant difference in each parameters between subgroups within 38 valgus cases. Conclusion: Ankle arthritis can be aggravated after total knee arthroplasty because of the change of mechanical loading axis onto the ankle joint. Therefore it may be needed to evaluate symptoms and function of ankle joints before performing total knee arthroplasties especially in patients with huge varus deformities of knee joints.

종골에 발생한 골육종 - 1예 보고 - (Osteosarcoma of the Calcaneus - A Case Report -)

  • 골육종은 슬관절 주위에 호발하며 족부에 발생하는 경우는 흔하지 않다. 20세 남자 환자가 수개월간 지속된 좌측 종골 부위의 통증으로 내원하였으며 종골의 골육종으로 진단하여 슬하부 절단술 및 술 후 화학요법을 시행하였다. 이에 저자들은 종골에 발생한 골육종 1례를 경험하였기에 이를 문헌고찰과 함께 보고하는 바이다.

  • Purpose: Muscle imbalance around the knee region, especially quadriceps imbalance, is considered one of the main contributing factors to the development and progression of knee dysfunction based on the changes observed via electromyography (EMG). This study aimed to investigate the effect of vastus lateralis (VL) inhibition taping using non-elastic tape on the EMG activity of the vastus medialis oblique (VMO) and VL during isometric knee extension. Method: Thirty-three healthy young adults (11 males and 22 females) were recruited. With and without VL inhibition taping, the participants performed isometric knee extension in the sitting position for a total of 7 seconds. Result: VMO/VL EMG ratio significantly increased after VL inhibition taping (p<.05). Conclusion: These findings suggest that VL inhibition taping may contribute to VL inhibition and may help improve VMO/VL ratio in young adults. We expect VL inhibition taping to be useful in individuals with greater VL than VMO activity

슬관절 밸런스 테이핑 요법이 노인의 무릎 통증과 관절가동범위에 미치는 효과 (Effects of Knee Balance Taping Therapy on Knee Pain and Range of Motion in the Elderly)

  • 본 연구의 목적은 밸런스 테이핑 요법이 노인의 무릎 통증과 관절가동범위에 미치는 효과를 검증하고자 시도하였다. 연구방법은 비동등성 대조군 전후설계의 유사실험연구로 자료수집은 2014년 12월 5일부터 10일까지 시행되었다. 연구대상은 노인여가복지시설을 이용하며 무릎 통증이 있는 60세 이상의 노인을 대상으로 하였다. 실험군(n=21)에게는 슬관절 밸런스 테이핑을 적용하였고, 대조군(n=19)에게는 테이핑 적용을 하지 않았다. 두 군의 무릎 통증 및 관절가동범위를 중재 전, 중재 1시간 후, 중재 24시간 후에 측정하였다. 통계분석은 SPSS/WIN 21.0 프로그램을 이용하였으며, ${\chi}^2$ test, Fisher's exact test, Mann-Whitney test, t-test, 반복측정 분산분석을 실시하였다. 연구결과 대조군에 비해 슬관절 밸런스 테이핑 요법을 받은 실험군에서 무릎 통증이 유의하게 감소하였고(F=34.03, p<.001), 무릎 관절가동범위가 유의하게 증가하였다(F=7.83, p=.006). 본 연구를 통해 슬관절 밸런스 테이핑 요법이 노인의 무릎 통증에 적용 가능한 독자적인 간호중재임을 확인하였다.

슬관절 주위 거대세포종의 치료 (Surgical treatment of Giant Cell Tumor in Knee Joint)

  • Giant cell tumors are primary bone tumors originating from non-osteoblastic connective tissue. The sites of involvement were commonly distal femur, proximal tibia, proximal humerus, distal radius and others (including os calcis, ilium and sacrum). Giant cell tumor located around knee joint has been difficult to treat because of local recurrence following curettage with or without bone graft. Although primary resections reduce recurrence of the lesion, the joint function will be markedly impaired. Marginal excision was very often complicated by a loss of joint integrity since all the giant cell tumors occupy juxtaarticular positions. Techniques involving physical adjuncts(high speed burr and electric cauterization) have been used in the hope of decreasing the rate of local recurrence and avoiding the morbidity of primary resection. A meticulous clinical, radiological and histological evaluation is needed to choose the correct treatment, keeping in mind the possibility of recurrence after each treatment modality.
