Effect of Resistance Exercise Intensity on the Strength and Lymphedema of Upper Extremity in Patients with Breast Cancer

  • Purpose: The purpose of this study was to determine the effects of high and low resistance exercise using an elastic band on the strength and lymphedema of upper extremity in patients with breast cancer. Methods: Seventeen female patients with breast cancer related lymphedema were randomly allocated to the high (n=9) or low (n=8) resistance exercise group. Both groups participated in the elastic band exercise program three times a week for eight weeks. For the high resistance exercise group (HR), the resistance was gradually increased, while maintaining constant intensity of exercise for the low resistance exercise group (LR). Assessments made include the upper extremity muscle strength and lymphedema before and after training. Results: After the exercise program, the HR showed significantly improved shoulder flexion and elbow flexion strength variation compared to the LR (p<0.05). Upper arm edema rate was significantly decreased in HR (p<0.05), but the difference between the two groups was not significant. Conclusion: These findings suggest that the elastic band exercise helps improve the strength and lymphedema of upper extremity in patients with breast cancer. In particular, high resistance exercise is more effective in improving muscle strength and does not exacerbate lymphedema, rather may improve upper arm edema if it is applied with a low elastic bandage or compression sleeve.

Protective Effects Wonjiseokchangpo-san has on Brain Damage and Cognitive Dysfunction in Transient Focal Cerebral Ischemia (일시적 국소 뇌허혈 흰쥐모델에서 원지석창포산의 뇌손상 및 인지기능 보호효과)

  • This study was conducted to determine the effects Wonjiseokchangpo-san on brain damage in transient focal cerebral ischemia. Rats were used for testing in the following three models: Morris Water Maze, Eight-Arm Radial Maze, and Histochemistry. In the Morris Water Maze Model, the Wonjiseokchangpo-san group showed significant decrease in the 3rd and 6th training session compared with the ischemia group. A retention test, in the Morris Water Maze Model, was performed on the 7th day without the escape platform. The Wonjiseokchangpo-san group showed significant increase compared to the ischemia group. In the Eight-Arm radial Maze model, the Wonjiseokchangpo-san group showed significant decrease in the error rate compared to the ischemia group. In the density of hippocampal CA1 cell of the cresyl violet-stained section, the Wonjiseokahangpo-san group showed significant increase compared to the ischemia group. These results suggest that Wonjiseokchangpo-san may have a significant protective effect on brain damage and cognitive dysfunction in transient focal cerebral ischemia.

The Effect of Wrist and Trunk Weight Loading using Sandbags on Gait in Chronic Stroke Patients (모래주머니를 이용한 팔목과 몸통의 무게 증가가 만성 뇌졸중 환자들의 보행에 미치는 영향)

  • Objective: This study aimed to determine the effect of wrist and trunk weight loading using sandbags in stroke patients in order to provide the quantitative data for enhancement of gait movement. Method: Twelve stroke patients, who have been diagnosed with hemiplegia over a year ago, were participated in this study. All subjects were asked to perform normal walking [N], wrist sandbag walking [W], wrist & trunk sandbag walking [WT], and both wrist sandbag walking [B] and both wrist & trunk sandbag walking [BT], respectively. Eight infrared cameras were used to collect the raw data. Gait parameters, arm swing, shoulder-pelvic kinematics, and lower extremity joint angle were calculated to examine the differences during walking. Results: As a result, there were no significant differences in the gait parameters, shoulder-pelvis, and lower extremities joint angles, but significant differences were found in the range of motion and the anteversion in arm swing. Conclusion: Wrist and trunk weight loading using sandbags affected the movement of the upper extremities only while it did not affect the movement of the lower extremities. It implies that it can reduce the risk of falling caused by a sudden movement change in lower extremities. In addition, the wrist and trunk weight loading using sandbags can induce changes in movement of the upper extremities independently and contribute to functional rehabilitation through resistance training.

  • In this paper, an algorithm to classify the 4 motions of arm and a control system to position control the prosthesis are studied. To classify the 4 motions, we use flex sensors which is electrical resistance type sensor that can measure warp of muscle. The flex sensors are attached to the biceps brchii muscle and coracobrachialis muscle and the sensor signals are passed the sensing system. 4 motion of the forearm - flexion and extension, the pronation and supination are classified from this. Also position of forearm is measured from the classified signals. Finally, A two D.O.F prosthesis arm with RC servo-motor is designed to verify the validity of the algorithm. At this time, fuzzy controller is used to reduce the position error by rotary inertia and noise. From the experiment, the position error had occurred within about 5 degree.

Muscle Activity of Low Back Muscles During Isometric Back Extension Exercises (등척성 신전운동 시 요부근의 근 활성도)

  • Back extension exercises have been used for rehabilitation of the injured low back, prevention of injury, and fitness training programs. However, excessive loading on low back can exacerbate existing structural weakness. The purpose of this study was to compare muscle activity of low back muscles during back extension exercises. Twenty healthy male subject s were evaluated. Electromyographic (EMG) activities of low back muscles at L1 and L5 level were recorded during seven different back extension exercises and two reference tasks by surface EMG and saved for data analysis. Reference tasks of lifting 20% and 40% of their body weight were included for comparison. The result were as follows: 1) Single-arm extension and single-leg extension exercises on quadruped position appeared to constitute a low-risk exercise for initial extensor strengthening. 2) When arm extension was combined with contralateral leg extension on quadruped position, EMG activities of low back muscles were increased. 3) EMG activity of low back muscles was highest during the trunk extension exercises on prone position. 4) EMG activities of low back muscles during arm and leg extension exercises on quadruped position were less than those of reference task of lifting 40% of their body weight. These result s have important implications for progressive back extensor muscle strengthening exercises in patients with back pain.

Effect of Red Ginseng Triol Saponin Fractions on the Spatial 1 Memory Function Studied with 12-Arm Radial Maze (12-Arm Radial Maze를 이용한 쥐의 공간 인지능력에 미치는 홍삼 PT계 사포닌의 효과)

  • Male rats aged 15 months, which had been over-trained with 12 arm radial maze up to the criterion of 1 or 2 errors an average per trial, were divided into two groups and 7 months after the over-training and were studied the effect of red ginseng trial saponin fraction (PT) on the spatial memory function. The rats which could be improved the performance of 9.75$\pm$1.26 (within 185$\pm$8.2 1 sec) were classified into "normal group" (n=4) and the ones, which showed R-maze performance of 7.0$\pm$2.87 within 300 sec, "deteriorated memory group" (n=5). PT was dissolved in distilled water and injected into the deteriorated memory group intraperitoneally at 30 min before R-maze for 3 consecutive days. The injected amount of PT on the 1st day was 10mg/rat, 1mg/rat both on the 2nd the 3rd day. As results, the performance was restored to 9.4$\pm$2.0 after stopping ginseng administration, although the lower performance values (1.4$\pm$0.89, 2.8$\pm$0.83, 3.8$\pm$0.84, respectively) were exerted during the three days of PT administration than before PT administration (7.0$\pm$2.87) since 1 day after administratitan of PT. The restored performance values were continuously maintained up to the level of the "normal group". However, any lower performance values were not observed when PT was administered via intraperitoneal route to the normal group in about of 2 mg/rat except 10mg1ra1. The number of errors when the foods in arms were put at only 6 places was clearly reduced during the 9 days of consecutive administration of PT as compared with untreated control. These results indicated that PT administration may enhance their cognitive function after a long lapse of time not only in the memory deteriorated rats but also in normal ones. The reason exerted lower performance values during PT administration in this paper was discussed as compared with the results of T-maze behavior of another 3-month-aged rats. of another 3-month-aged rats.

Effects of Combination of Aerobic and Circuit Weight 12 Weeks Training on Body Composition and Body Shape of Middle Aged Korean Women (12주간의 유산소성 및 저항성 복합운동이 20-30대 여성의 체성분과 체형 변화에 미치는 영향)

  • Objectives : This study is to examine the influence of exorcise program to body shape and body composition of women. Exercise program is composed muscular resistance exercise and aerobic exorcise with 70min/time, 5day/week for 4weeks and designed for the reinforcement of muscular strength and the decease the decrease of obesity. Methods : The subjects of this study are 26 females who are from twenty until thirty-nine years old and are divided into two groups according to Obese Rate. normal weight group is 18 females under a BMI of 23 and overweight group is 8 females over a BMI of 23. we measured body compositon and body shape with body composition analyzer(inbody 3.0) measured body compositions are body weight, Lean body mass, base metabolic rate, body fat, %body fat, waist hip ratio, body mass index. measured 6body shade are upper arm circumference, chest circumference, waist circumference, hip circumference, thigh circumference. the measurements had been taken before exercise of each group, after 4 weeks, after 8 weeks, after 12 weeks. Body compositon and body shape of the measured data were analyzed by two-way repeated ANOVA followed by Dunnett's Post hoc test using SPSS. Differences were considered significant at p<0.05. Results and Conclusions : The weight was reduced at a significant level in overweight group after 4 weeks. Body fat percent was reduced at a significant level in normal group alter 4 weeks and overweight group after 8 weeks. Waist-hip ratio was reduced at a significant level in normal and overweight group after 4 weeks. Lean body mass and body metabolic rate were reduced but did not satisfy statistical significance. Circumference of all body parts showed an decrease after 12 weeks exercise program. Upper Arm, Chest, Waist, Hip Circumference were reduced at a significant level in overweight group after 4 weeks, but In normal group only Waist Circumference was reduced at a significant level after 4 weeks. Aerobic and Circuit weight training for 12 weeks reduced body weight and body fat percent, waist circumstance statistically significant without loss of lean body mass and body metabolic rate. Also these change was more effective in overweight group than in normal group.

Effects of Task Training for Cognitive Activation of Stroke Patients on Upper Function and Activities of Daily Living (뇌졸중 환자의 인지활성화를 위한 과제 훈련이 상지기능 및 일상생활동작에 미치는 영향)

  • The purpose of this study is to investigate the effect of task training for cognitive activation of upper extremity on upper limb function and activities of daily living and to suggest intervention in rehabilitation treatment. From January to February of 2018, nine stroke patients were arbitrated 30 minutes a day, five days a week, for four weeks. For the experimental group, the therapist has induced the group using the linguistic guidance to patients, so they utilize the cognition strategy. The control group conducted active exercises in a range of motion using the instruments and passive exercises in a range of motion to reduce the stiffness of joints and upper limbs. As muscle strengthening exercises, the patients were assigned to work on the biceps muscle of arm, triceps muscle of arm, and deltoid according to the individual patient's muscular strength level. For the experimental group, the MBI was improved by ten points at maximum, and K-AMPS motor skills showed the improvements of 1.0 logit at maximum, and processing skills showed improvements of 0.6 logits at maximum. In MFT, the maximum improvement was by two points. For the control group, MBI was improved by five points at maximum, and 0.2 maximum improvements were shown in K-AMPS' motor skills and 0.3 maximum improvements in processing skills. MFT showed no change. The conclusion is that the challenges to enable training for stroke patients give a positive impact on upper limb function and activities of daily living.

The Effect of Constraint-Induced Movement Therapy(CIMT) With Cognitive-Perceptual Training on Upper Extremity Function of Stroke Patients With Mild Cognitive Impairment (경도 인지손상을 가진 뇌졸중 환자의 상지 기능에 미치는 강제유도운동치료(CIMT)와 인지-지각 훈련의 병행 효과)

  • The purpose of this study is to examine effects of constraint-induced movement therapy(CIMT) and/or cognitive-perceptual training(CPT) on the change of hand function in cerebrovascular accident(CVA) patients and to evaluate the change in the amount and quality of use of the affected upper extremity in performing daily living tasks. The subjects of study were 10 patients who had been under rehabilitation for more than three months after CVA onset. They were all determined as mild cognition impairment according to NCSE or MVPT test. For CIMT group, to restrict the movement of the unaffected hand the subjects had been worn modified resting arm-splint in daytime for 4 weeks. For CIMT+CPT group, the subjects were performed CPT with CIMT and control group had been under conventional occupational therapy for the same period. CIMT+CPT group showed significant improvement in simulated feeding, lifting large light objects, and lifting large heavy objects of Jebsen-Taylor Hand Function Test. CIMT group also showed significant improvement compared with control group. The mean changes of the amount of use(AOU) of the affected arm had a statistically significant difference among groups (p<.05). While CIMT+CPT group had the biggest change in the quality of movement(QOM) of upper extremity of the affected side, CTL group showed the smallest change. Both CIMT and CIMT+CPT groups had statistically significant difference in the change in the quality of movement in upper extremity of affected side with CTL group(p<.05), but there was not significant difference between CIMT group and CIMT+CPT group. CIMT performed to the patients of stroke, with mild impairment in cognitive perceptual abilities showed the improvement in hand movement and AOU and QOM of upper extremity in the affected side and the combination of CIMT with CPT showed synergic effects.

Effect of Complex Training on Inflammatory Markers and Homocysteine of Obese Men (복합트레이닝이 비만남성의 염증지표와 homocysteine에 미치는 영향)

  • The purpose of this study was the effect of complex training of obese men in their 30s on inflammatory markers and homocysteine, a risk factor for cardiovascular disease. The subjects consisted of obese men (n=12) with the body fat ratio of 25% or above in their mid 30s who had no medical conditions and can follow the exercise routine required by this study. To achieve the purpose of this study measured the maximum oxygen intake (VO2max) and 1RM of 5 kinds of machine(bench press, lat pull down, arm curl, leg press, squat) as a preliminary test; based on the results, performed 8 weeks complex training (aerobic exercise + weight training); and then analyzed the variation in body composition (body weight, body fat and BMI), blood inflammatory markers (IL-6, TNF-α and CRP), and homocysteine between before and after training. The results exhibited that 8 weeks complex training reduced weight, body fat and BMI significantly (p<0.01) and also reduced inflammatory marker CRP and homocysteine, a risk factor for cardiovascular disease, significantly (p<0.05). In conclusion, 8 weeks complex training confirmed the variation in body composition, and this variation has a positive effect on the inflammatory marker and the risk factor for cardiovascular disease.
