Analysis on the Change of Hand Grip Strength as per the Circumference of Upper Arm and Forearm and the Muscle Activity of Upper Arm and Forearm as per the Duration of Gripping

  • The purpose of this study is to analyze the grip strength by the girth of upper arm and forearm and their muscle activities by duration of grip strength. The subjects were consisted of 20 healthy adults(10 males & 10 females) who had no medical history of neurological and surgical problems with their arms. Girth of upper arm/ forearm and maximum grip strength for 4sec and 30sec were measured. Muscle activity was by wireless electrode EMG system. Co-relation of girth of upper arm/ forearm was significantly high. Upper arm's muscle activity performed for 4sec and 30sec was significantly high. In this study. It suggests that training of upper arm should be performed with the training of grip strength because both of upper arm and forearm affected grip strength.

  • Chronic upper extremity hemiparesis is a leading cause of functional disability after stroke. The purpose of this study were to identify effects of a 6weeks repetitive bilateral arm training on upper motor function and the reorganization of motor network. Four chronic stroke patients participated in this study. They performed for 6 consecutive weeks, 3 days a week, 30 minutes a day. In the single group study, four 5-minute periods per session of bilateral arm training were performed with the use of a custom-designed arm training machine. The results of this study was as follows. 1. Following the 6weeks period of RBAT, patient exhibited a improvement in FMA and BBT. 2. Following the 6weeks period of RBAT, it showed improvement in reaching time, symbol digit substitution and finger tapping speed of KCNT. 3. fMRI activation after RBAT showed a focal map in lesional cortical area and perilesional motor areas. These fMRI data suggest that hemodynamics response to RBAT reflect sensorimotor reorganization in contralateral hemisphere. In conclusion, these date suggest that improved upper extremity function induced by repetitive bilateral arm training after stroke is associated with reorganization of motor network as a neural basis for the improvement of paratic upper extremity function.

The Effect of Action Observation on Motor Function of Paretic Upper Extremity in Stroke Patients: Single Subject Study (동작관찰훈련이 뇌졸중 환자의 마비측 상지기능에 미치는 영향: 단일사례연구)

  • PURPOSE: This study was conducted in chronic hemiplegic patients to examine the effect of the training of the ipsilateral arm that is identical to the model performing movements and the training of the contralateral arm on the function of the arm. METHODS: The subjects were participated total 2 patients(the subject 1 with left hemiplegia and the subject 2 with right hemiplegia). The study was conducted for 4 weeks. The action observation training were repeated 10 times in 10 days during intervention period. The evaluation of the arm function such as BBT, MFT and MAL in the each subject were examined 5 times in the baseline period, 10 times during the intervention period and 5 times during the baseline regression period. RESULTS: The results of the evaluation in each subject were presented as mean values and video graphs. The arm function of the 2 subjects were improved during the intervention period in comparison with the baseline period, and the improvement was maintained even during the regression baseline period. In addition, there were large variation ratio of BBT and MAL (AOU, QOM) in comparison with subject 1. CONCLUSION: According to the results, the action observation training was more effective in improving upper limb function of stroke patients who imitate the performed behavior of paralyed parts on the same side.

Inspiratory Muscle Strengthening Training Method to Improve Respiratory Function : Comparison of the Effects of Diaphragmatic Breathing with Upper Arm Exercise and Power-Breathe Breathing (호흡 기능 향상을 위한 들숨근 강화 훈련 방법 : 위팔운동을 동반한 가로막 호흡과 파워브리드 호흡의 효과 비교)

  • Purpose : It was to compare changes in respiratory function (pulmonary function, inspiratory function) after four weeks of inspiratory muscle strengthening training (diaphragmatic breathing with upper arm exercise, Power-Breathe breathing) for 36 healthy people. Methods : Subjects were randomly assigned to diaphragmatic breathing with upper arm exercise (Group I) and Power-breathe breathing (Group II) was conducted by the protocol for four weeks five times per week. As the main measurement method for comparison between groups For pulmonary function, Forced Vital Capacity (FVC) and Forced Expiratory Volume at One second (FEV1) were used, and for inspiratory function, Maximum Inspiratory Capacity (MIC), Maximum Inspiratory Pressure (MIP), and Maximum Inspiratory Flow Rate (MIFR) were used. Results : In changes in pulmonary function between groups, FVC and FEV1 showed no significant difference, and in inspiratory function changes, MIC showed no significant difference, but in MIP and MIFR, Group B significantly improved over Group A. Conclusion : The progressive resistance training using the Power-breath device applied to the inspiratory muscle did not show a significant difference in the increase in the amount of air in the lungs and chest cage compared to the diaphragmatic breathing training accompanied by the upper arm exercise. However, by increasing the air inflow rate and pressure, it showed a more excellent effect on improving respiratory function.

  • Background: The aim of this study was to evaluate the effect of whole brain radiotherapy (WBRT) combined with streotactic radiosurgery versus stereotactic radiosurgery (SRS) alone for patients with brain metastases. Materials and Methods: This was a retrospective study that evaluated the results of 46 patients treated for brain metastases at Dr. Abdurrahman Yurtaslan Ankara Oncology Training and Research Hospital, Radiation Oncology Department, between January 2012 and January 2015. Twenty-four patients were treated with WBRT+SRS while 22 patients were treated with only SRS. Results: Time to local recurrence was 9.7 months in the WBRT+SRS arm and 8.3 months in SRS arm, the difference not being statistically significant (p=0.7). Local recurrence rate was higher in the SRS alone arm but again without significance (p=0,06). Conclusions: In selected patient group with limited number (one to four) of brain metastases SRS alone can be considered as a treatment option and WBRT may be omitted in the initial treatment.

Impact of Virtual Reality Based Neuromuscular Postural Control Fusion Training on Balance Ability and Jump Performance of Soccer Players with Functional Ankle Instability (가상현실 기반 자세조절 융합 훈련이 기능적 발목 불안정성 축구선수들의 균형과 점프에 미치는 영향)

  • In this study, we examined the impact on balance ability and jump performance of soccer players with functional ankle instability using virtual reality based neuromuscular posture control fusion training. Soccer players were divided into 15 people of virtual reality-based neuromuscular posture control fusion training group and 15 people of common treadmill training group and performed for 30 minutes three times a week for 8 weeks. In order to evaluate the balance of ability, using biorescue, it measured surface area, whole path length, limit of stability. In order to measure jump performance, it measured counter movement jump with arm swing and standing long jump. The results showed the statistically significant difference in the balance comparison of surface area, whole path length, limited of stability and the jump performance comparison of counter movement jump with arm swing, standing long jump. As a result, virtual reality-based neuromuscular posture control fusion training was found to be more effective to improve its balance ability and jump performance than common treadmill training.

Educational Effects of a Virtual IV Simulator and a Mannequin Arm Model Combined Training in Teaching Intravenous Cannulation for Nursing Students (간호대학생을 위한 정맥주사용 가상학습 시뮬레이터와 마네킨 팔 모형을 병합한 정맥주사 실습교육의 효과)

  • The purpose of this study is to compare the effects on nursing students' knowledge, performance confidence, and skills from combined virtual IV simulator and mannequin arm IV cannulation training against training with a mannequin arm only. A non-equivalent control group pretest-posttest experimental study was carried out. Ninety-three sophomore nursing students who were just beginning their fundamental skills training were recruited. Participants were divided into two groups (46 for the combined group and 47 for the mannequin-only group). Data were collected from March 18-29. For the experimental group, both virtual IV simulator and mannequin-arm training were provided for 30 minutes (15 minutes each). For the control group, training for 30 minutes with a mannequin arm only was provided. After intervention, there was no statistically significant difference in the knowledge score between the two groups (F=2.52, p=.116). However, there was a significant improvement in performance confidence (t=2.14, p=.035) and nursing skills (t=5.34, p<.001) in the experimental group, compared with the control. Overall, this study provides empirical evidence that the combination of virtual IV simulator and mannequin arm training may further enhance nursing students' performance confidence and nursing skills.

The Effects of Modified Constraint-Induced Movement Therapy and Bilateral Arm Training on the Upper Extremity Performance of Individuals with Chronic Hemiparetic Stroke (수정된 강제-유도운동치료와 양측성 상지훈련이 만성 뇌졸중 환자의 상지 수행 능력에 미치는 영향)

  • Purpose: The intention of this study was to investigate the effects of modified constraint-induced movement therapy (mCIMT) with bilateral arm training (BAT) on the motor performance and daily activity performance of individuals with chronic hemiparetic stroke. Methods: Sixteen subjects one year after stroke participated in this study with a control group; the pretest-posttest method was used. The subjects were randomly allocated into two groups: combination of bilateral arm training and modified constraint-induced movement therapy (n=8), and modified constraint-induced movement therapy (n=8). The mCIMT group received therapy for 90 minutes in 3 sessions per week over a period of 4 weeks. The patients receiving a combination of mCIMT and BAT were treated for the same period and frequency. The results were evaluated using the Fugl-Meyer Assessment, Action Research Arm Test (ARAT), and Motor Activity Log-Amount of Use, and Quality of Movement (MAL-AOU, QOM) assessment tools. Results: The Fugl-Meyer Assessment showed that hand and wrist performance improved significantly more in the mCIMT group than in the Combination group (p<0.05). Result from the ARAT assessment showed greater scores for gross movement in the combined group than in the mCIMT group (p<0.05). The MAL-AOU showed that there was greater improvement in the combined group than in the mCIMT group (p<0.05). Conclusion: The forced use of the more affected side can be important for the enhancement of upper extremity performance for chronic hemiparetic stroke patients during their daily activities.

  • This paper presents a force control of a arm of walking training robot. The current gait training apparatus in hospital are ineffective for the difficulty in keeping constant unloading level and constraining patients to walk freely. The proposed walking training robot is designed to unload body weight effectively during walking. The walking training robot consists of unloading manipulator and mobile platform. The manipulator driven with a electro-mechanical linear mechanism unloads body weight in various level. The mobile platform is wheel type, which allows to patients unconstrained walking.   • In first-person perspective training system, the users needs realistic experience. For providing this experience, the system should offer the users virtual and real images at the same time. We propose an automatic a persons's arm segmentation and image composition method. It consists of arm segmentation part and image composition part. Arm segmentation uses an arbitrary image as input and outputs arm segment or alpha matte. It enables end-to-end learning because we make use of FCN in this part. Image composition part conducts image combination between the result of arm segmentation and other image like road, building, etc. To train the network in arm segmentation, we used arm images through dividing the videos that we took ourselves for the training data.
