  • 혈관확장성 쇼크(Vasodilatory shock)는 혈관마비 증후군(vasoplegic syndrome), 심폐바이패스후 혈관마비(post-cardiopulmonary bypass vasoplegia)라고도 불리며, 개심술 직후 나타나는 저혈압, 빈맥, 정상 또는 약간 증가한 심박출량 및 체혈관저항 감소 등을 특징으로 하며, 일반적인 수액공급이나 카테콜아민 혈관수축제(cathecolamine vasopressor)에 대한 반응이 적거나 거의 없기 때문에 개심술후 높은 이환율 및 사망율을 나타내는 상태를 말한다. 저자들은 개심술후 혈관확장성 쇼크(vasodilatory shock)로 진단되는 3명의 환자들에서 저용량의 아르기닌 바소프레신(AVP)를 사용하여 성공적으로 치료하였기에 관련된 문헌 고찰과 함께 보고하는 바이다.

Study on the Change of Catecholamine, Arginine Vasopressin and V1 Vasopressin Receptor Release in the Stressed Rat Brain

  • 스트레스가 유발된 랫드의 대뇌에서 Vasopressin-catecholamine pathway의 활성도를 알아보기 위해 면역화학염색법으로 vasopressin 호르몬의 분비와 catecholamine의 생성변화를 tyrosine hydroxylase (TH) 효소의 발현변화로 규명하고, arginine vasopressin (AVP)과 V1 vasopressin receptor의 유전자 발현변화를 in situ hybridization 방법을 이용하여 살펴보았다. 수컷 SD rat를 7시간동안 stress cage에 넣어 16$\pm$1$^{\circ}C$의 물에 수침구속 스트레스를 준 후 대조군과 함께 관류고정하여 brain을 적출하였다. Brain의 hypothalamus 부위를 중심으로하여 동결절편하여 면역조직화학 염색과 in situ hybridization을 시행하였다. TH 면역조직화학 염색에서 대뇌의 줄무늬체 부위의 꼬리조가비핵에서와 시상하부 부위의 내측등쪽시상하부와 흑색질부위에서 스트레스군이 대조군에 비해 TH 면역염색성이 증가되어 관찰되었으나 시상하부 부위의 시삭위핵, 뇌실주위핵, 뇌실옆핵에서는 두 군간의 큰 면역염색성의 차이는 보이지 않았다. AVP 면역조직화학 염색에서는 시삭위핵에 많은 수의 AVP 양성 신경세포체들이 밀집되어 있으며 뇌실옆핵에서는 스트레스군에서 AVP 면역염색성이 약간 증가되어 관찰되었으나 신경섬유의 분포양상은 비슷하였다. 중간융기에서는 모두 강한 염색성의 신경섬유들이 관찰되어 두 군간에 큰 차이는 없었다. AVP 유전자에 대한 in situ hybridization 결과 시삭위핵의 신경세포에서 AVP mRNA 양성반응을 관찰할 수 있었으나 다른 시상하부핵에서는 관찰할 수 없었으며, V1 vasopressin receptor에 대한 in situ hybridization 결과는 두 군의 대뇌에서 모두 양성반응을 관찰할 수 없었으며 V1 vasopressin receptor 유전자의 조직별 발현정도와 스트레스에 의한 발현량 조절을 관찰할 필요가 있다고 사료된다.

Spontaneously Hypertensive Rat에서 Arginine Vasopressin의 신장효과 (Effects of Arginine Vasopressin on the Renal Function in Spontaneously Hypertensive Rats)

  • There have been reports on the aberration of the control mechanisms of the blood pressure, hormone secretion, and renal functions in spontaneously hypertensive rats (SHR). However, the contribution of the renin-angiotensin system in the maintenance of high blood pressure in SHR is still controversial. Recently, it has been reported that the negative feedback short loop control mechanism of the renin-angiotensin system may be changed in SHR. In the present experiment, it was attempted to explore the possible alterations in the effect of arginine vasopressin (AVP) on the renal function in SHR. Experiments have been done in anesthetized SHR as well as in normotensive Wistar and Sprague-Dawley rats as control groups. Pharmacologic doses of AVP (10-13 mU/rat/10 min) decreased urine volume, excreted amount of creatinine and para-amino-hippuric acid. No differences in these parameters was observed between normotensive and hypertensive rats. AVP increased sodium and potassium excretion, but the responses in SHR were suppressed as compared with normotensive rats. Intravenous infusion of AVP also increased blood pressure in normotensive and hypertensive rats and a vasopressor effect of AVP was attenuated in SHR. There was a positive correlation between the changes in blood pressure and excreted amount of sodium during AVP infusion. These data suggest that the attenuated natriuretic effect of intravenous infusion of AVP may be due to a difference in renal tubular responsiveness to AVP but not due to a difference in vasopressor responsiveness.

Spontaneously Hypertensive Rat에 있어서 Renin-Angiotensin계의 변조에 관하여 (Some Aberrations of the Renin-Angiotensin System in Spontaneously Hypertensive Rat)

  • Enhanced activity of renin-angiotensin-aldosterone system has been suggested as a cause of the high blood pressure in certain forms of experimental hypertension. In spontaneously hypertensive rats, however, increased activity of the system has not been found, and even suppressed renin angiotensin system has been reported in the spontaneously hypertensive rat. In the present experiments it was attempted to explore the possible alteration of the short loop negative feedback control in the hypertensive rat. Experiments have been done in the anesthetized spontaneously hypertensive rats(SHR) as well as in normotensive Wistar and Sprague Dawley rats as control. Responses of the plasma renin activity to the intravenous L-isoproterenol were dose dependent, in both SHR and normotensive control rats. Hypotensive responses to smaller do sea of L-isoproterenol were more accentuated in SHR than in the normotensive control rats. Angiotensin If given intravenously suppressed plasma renin activity in a dose dependent fashion in both groups. However, these suppressive responses were significantly attenuated in SHR as compared with the normotensive control rats. Treatment with angiotensin I-converting enzyme inhibitor did not correct the attenuated responses of the plasma renin activity to angiotensin II in SHR. Intravenous infusion of arginine vasopressin also produced a dose-dependent suppression of plasma renin activity in both groups. The responses to arginine vasopressin were also significantly attenuated to the normotensive control rats. In the sodium-depleted SHR, arginine vasopressin did not suppress plasma renin activity, whereas the suppressive responses to arginine vasopressin in the normotensive control rats were not different from the untreated control rats. These data suggest that there may be a derangement in the short loop negative feedback control of the renin-angiotensin system in spontaneously hypertensive rat.

  • 혈관 확장성 쇼크는 심장 수술 후 발생할 수 있는 치명적인 합병증이다. 이러한 혈관 확장성 쇼크는 체외 순환 후 바소프레신의 결핍으로 발생하며, 수액 공급이나 심근 수축제에 잘 반응하지 않는 경우가 많다. 저자들은 체외 순환 중지 후 혈관 확장성 쇼크가 발생한 환자들에서 바소프레신을 사용하여 성공적으로 치료하였기에 문헌 고찰과 함께 보고하는 바이다.

A Pediatric Case of AVPR2-related Nephrogenic Syndrome of Inappropriate Antidiuresis

  • Nephrogenic syndrome of inappropriate antidiuresis (NSIAD) is a rare X-linked genetic condition caused by a gain-of-function mutation of arginine vasopressin receptor 2 gene, AVPR2. We report the case of a male neonate diagnosed with NSIAD based on his DNA sequence of the AVPR2 gene and the clinical course. He demonstrated a complete correction of hyponatremia using oral urea. We suggest that (1) sequencing analysis of the AVPR2 gene ought to be done in newborns with prolonged euvolemic hyponatremia, hypo-osmolality, high urinary sodium and normal/low or undetectable AVP levels, and that (2) oral urea is a safe and effective treatment option in infants diagnosed with NSIAD until the patients are grown-up.

  • 선천성 신성 요붕증은 일반적으로 성염색체 열성 유전 양식을 취하여 보인자인 어머니로부터 아들에게 유전되고, 발생 빈도는 드문 것으로 알려져 있으며, 항이뇨호르몬의 혈장 농도가 정상이거나 상승되어 있으면서도 신세뇨관이 항이뇨호르몬에 대한 저항성 때문에 요농축능 장애가 발생하여 다음과 다뇨를 특징으로 하는 질환으로 이뇨제와 프로스타글란딘 합성억제제의 사용이 치료의 근간을 이루어 왔다. 본 증례에서 발견된 AVPR2 수용체 유전자의 돌연변이 부위는 선천성 신성 요붕증의 표현형을 보이는 것으로 외국에서는 보고된 바 있으나 국내에서는 최초로 확인된 것이며, 고용량의 항이뇨 호르몬과 이뇨제 치료에 반응을 보였다.

Transforming growth factor-beta and liver injury in an arginine vasopressin-induced pregnant rat model

  • Objective: Approximately 30% of preeclamptic pregnancies exhibit abnormal liver function tests. We assessed liver injury-associated enzyme levels and circulating transforming growth factor beta (TGF-β) levels in an arginine vasopressin (AVP)-induced pregnant Sprague-Dawley rat model. Methods: Pregnant and non-pregnant Sprague-Dawley rats (n=24) received AVP (150 ng/hr) subcutaneously via mini-osmotic pumps for 18 days. Blood pressure was measured, urine samples were collected, and all animals were euthanized via isoflurane. Blood was collected to measure circulating levels of TGF-β1-3 isomers and liver injury enzymes in pregnant AVP (PAVP), pregnant saline (PS), non-pregnant AVP (NAVP), and non-pregnant saline (NS) rats. Results: The PAVP group showed significantly higher systolic and diastolic blood pressure than both saline-treated groups. The weight per pup was significantly lower in the AVP-treated group than in the saline group (p<0.05). Circulating TGF-β1-3 isomer levels were significantly higher in the PAVP rats than in the NS rats. However, similar TGF-β1 and TGF-β3 levels were noted in the PS and PAVP rats, while TGF-β2 levels were significantly higher in the PAVP rats. Circulating liver-type arginase-1 and 5'-nucleotidase levels were higher in the PAVP rats than in the saline group. Conclusion: This is the first study to demonstrate higher levels of TGF-β2, arginase, and 5'-nucleotidase activity in PAVP than in PS rats. AVP may cause vasoconstriction and increase peripheral resistance and blood pressure, thereby elevating TGF-β and inducing the preeclampsia-associated inflammatory response. Future studies should explore the mechanisms through which AVP dysregulates liver injury enzymes and TGF-β in pregnant rats.

자폐스펙트럼장애의 침치료에 대한 최근 임상 연구 동향 - RCT 중심으로 - (Recent Clinical Research on Effect of Acupuncture for Autism Spectrum Disorder)

  • Objectives The purpose of this study is to investigate recent clinical studies on effect of acupuncture for Autism Spectrum Disorder in other countries. We have analyzed the studies on effect of acupuncture for Autism Spectrum Disorder within randomized controlled trail (RCT) for 6years (from 2010 to 2015). Methods The search database includes Medline, Embase, Cochrane library and CNKI (China National Knowledge Infrastructure). To narrow the search, the following key search terms were used: 'autism or ASD or Asperger's Syndrome or pervasive developmental disorder, acupuncture'. The search was limited to the publication date from 2010 to 2015. 7 control studies in Medlin, Embase, Cochrane library and 5 control studies in CNKI were selected for analysis. Results and Conclusions 1. The acupuncture and rehabiliation treatment is more effective than only acupuncture treatment. Especially, Retention of needling is helpful. 2. It is necessary to set up standard scale in assessment of ASD patients and serum arginine-vasopressin (AVP) can be substitution. 3. Head acupuncture and tongue acupuncture is effective for ASD.

신성고혈압 백서의 신장절편에서 Renin 유리의 Negative Feedback 조절기전의 변조 (On the Negative Feedback Control Mechanism of the Renin Release in Kidney Slices)

  • Alterations of renin-angiotensin system have been suggested as one of the mechanisms increasing arterial blood pressure in experimental and clinical hypertension. But the exact nature of high blood pressure in the early and late phase of renal hypertension is still controversial. To clarify the nature of renin release in both unclipped and clipped kidney of two kidney one clip Goldblatt lypertensive rat, experiments have been done in kidney slices, which were obtained from the rats of 3 and 7 days of operation. Basal rate of renin release was suppressed in unclipped kidney slices compared to clipped kidney Norepinephrine increased renin release from unclipped kidney slices, but not from clipped kidney slices. Suppressions by angiotensin Il and arginine vasopressin of renin release were attenuated in the clipped kidney slices compared to unclipped and sham-operated kidney slices. Increases by verapamil and trifluoperazine of renin release were attenuated in the clipped kidney slices compared to unclipped and sham-operated kidney slices. These results suggest that the negative feedback control mechanism of the renin-angiotensin system by angiotensin Il and arginine vasopressin is attenuated in the clipped kidney of two kidney one clip Goldblatt hypertensive rat, and that one of the altered mechanisms may be caused by certain regulatory changes of intracellular calcium and/or calcium-calmodulin complex in the juxtaglomerular cells.
