A Case Report of Traumatic Brain Injury (외상성 뇌손상 환아의 한방적 처치에 대한 증례보고)

  • Objectives : This study is to investigate the effects of oriental medical treatment of traumatic brain injury. Methods : We treated the patient with Herb medicine, acupuncture, moxa, fumigation, rubdown and negative and occupational therapy. And we evaluated the case with Modified Bathel Index, Activity Index, Jebsen Test. Results : There is improvement in his symptom (manual muscle power, finger apraxia, memory loss, dysarthria, urinary frequency), after oriental medical treatment. Conclusions : We report the good result of oriental medical treatment on this case. The more clinical study of oriental medical treatment for traumatic brain injury is needed.

  • This paper reports an compared acoustic analysis on speech produced by two Korean groups, normal and AOS, focusing on utterances of V-CV structures. Major concerns include: 1) types of errors (distortion/substitution) according to the place of articulation, 2) duration of each syllable, 3) VOTs of stop sounds, and 4) F1 and F2 of vowels. In terms of the differences in these phonetic characteristics between the two groups, we aim to clarify some characteristics of AOS and to provide fundamental criteria for diagnosing and evaluating the disease.

  • Objectives This study is to investigate the effects of oriental medical treatment for traumatic cerebral infarction. Methods We treated the patients with herbal medicine, acupuncture, aroma therapy, rehabilitation, and we evaluated these cases with Activity Index. Results There are some improvements in each patients, such as manual muscle power, finger apraxia after oriental medical treatments. Conclusions We report a change for the better effect of oriental medical treatment on this case. The more clinical studies of oriental medical treatment for traumatic cerebral infarction are needed.

A Case of Idiopathic Basal Ganglia Calcification with Dementia (치매가 동반된 특발성 기저핵 석회화 1례)

  • The case of a 66- year-old woman with coexisting idiopathic basal ganglia calcification(IBGC) and dementia was presented. The calcification was detected in bilateral basal ganglia, dentate nucleus, and thalamus by brain imaging. Serum calcium and phosphorus levels were normal. The underlying diseases of calcification of basal ganglia such as parathyroid dysfunction and other infectious, toxic, or metabolic illness were excluded. The patient had memory impairment and frontal executive dysfunction without aphasia, agnosia, apraxia, and visuospatial impairment in neuropsychological test. It suggested that the cognitive impairment might be due to the dysfunction of frontal-subcortical circuit.

Research Trends Regarding Primary Progressive Aphasia During the Last 10 Years (일차 진행성 실어증에 대한 최근 10년간 연구 동향)

  • The purposes of this study were to determine recent trends of domestic and international studies for primary progressive aphasia (PPA) patient group and to identify areas for future research. For these purposes, 171 studies on communication disorders of a PPA were chosen and sub-categories for research methods and contents were also analyzed. The sub-categories for research methods include types of research subject, research purpose and research field. As the research subject, 'agrammatism' was most frequently dealt with in both domestic and international studies; as the research purpose, 'characteristics study' accounted for the largest portion; and with respect to the research field, the international studies have covered evenly language, speech and others, while the domestic studies have handled only the field of 'language,' paying no attention to 'speech' and 'others.' In the international studies, apraxia of speech was added to the conventional three sub-types of PPA classification, so distinction between the four symptoms has been actively studied; however, patient cases were difficult to find in Korea, so the symptoms the patients had, although centering around communication disorders, have been rarely researched in the domestic studies. The results suggested that there have been extremely few studies related and their research field has lacked diversity. We suggested a direction for future research on communication disorders with PPA.

Ipsilateral Motor Deficit during Three Different Specific Task Following Unilateral Brain Damage (편측 뇌손상 환자에서 특정 과제에 한정된 동측 상지의 운동 결함 분석)

  • Impaired sensorimotor function of the hand ipsilateral to a unilateral brain damage has been reported in a variety of motor task. however, it is still the controversial issue because of the difficulty of detection in clinical situation, patients' variability(time after onset, contralateral upper extremity severity, other cognitive functions including apraxia), and the performed various motor task. The purpose of this study is to determine the presence of ipsilateral motor deficit following unilateral brain damage in three different specific tasks(hand tapping, visual tracking and coin rotation) compared with healthy age-sex matched control group using the same hand and to investigate the lateralized motor control in each hemispheric function. Findings revealed that stroke patients with unilateral brain damage experienced difficulties with rapid-simple repetitive movement, visuomotor coordination, complex sequencing movement on ipsilateral side. Also, Comparison of the left-hemispheric stroke groups and the right-hemispheric stroke groups revealed that patients with a left-hemisphere damage tended to be more variable in performing all of the three tasks. These results show that stroke patient with left hemisphere damage has more ipsilateral motor deficit, and the left hemisphere contributes to the processing of motor control that necessary for the executing actions with ipsilateral hand.

  • This study was designed to investigate fast ab/adduction rate of articulation valves in normal adults. The measurement of fast ab/aduction rate has traditionally been used for assessment, diagnosis and therapy in patients who suffered from dysarthria, functional articulation disorders or apraxia of speech. Fast ab/adduction rate shows the documented structural and physiological changes in the central nervous system and the peripheral components of oral and speech production mechanism. Fast ab/adduction rates were obtained from 20 normal subjects by producing the repetition of vocal function (/ihi/), tongue function (/t${\wedge}$/), velopharyngeal function (/m/), and labial function (/p${\wedge}$/). The Aerophone II was used for data recording. The results of finding as follows: average fast ab/adduction rates were vocal function(6.21cps), tongue function(7.42cps), velopharyngeal function(5.23cps), labial function (6.93cps). The results of this study are guidelines of normal diadochokinetic rates. In addition, they can indicate the severity of diseases and evaluation of treatment.

  • Purpose : The purpose of this study was conducted to find out observation at gait analysis of the stroke patient with proprioceptive neuromuscular facilitation(PNF) concept. Methods : This is a literature study with books, seminar note and international PNF course book. Results : Stroke patient gait was poor initial contact by weakness of tibialis anterior or weakness of contralateral plantar flexor, poor loading response by loss of deep sensation, poor mid stance by loss of deep sensation, weakness of tibialis anterior and weakness of plantar flexors eccentric control, poor terminal stance, pre-swing, initial swing by loss of deep sensation and stiffness fo deep toe flexors. Conclusion : Stroke patient gait determine on loss of mobility, pain, fear, trunk muscle weakness, loss of coordination, loss of deep sensation, neglect and apraxia. Therefore observational gait analysis of the stroke patient focus on gait cycle and take out hypotheses from the gait cycle. These hypotheses have to define accept or not by parameters. Treatment plan made with the hypotheses.

The Effects of Driving Rehabilitation Program on Cognitive Function in Elderly (운전재활프로그램이 노인의 인지기능에 미치는 효과)

  • Purpose : The purpose of this experiment is to find out the effectiveness which exert influence on cognitive skills by using the Driver Rehabilitation program for senior citizens who are over 65 years old and live in Busan. Method : From July first, 2014 to August 28th, 2014, we researched the 60 elderly people who are over 65 and go to community relief center which is in Busan. - 30 are experimental group and the other is control group. In the experimental group, we used Nintendo wii's driving simulation program and RC Car driving program in the model road. For estimation, we used MVPT-3(Motor-Free Visual Perception Test-3), Trail Making Test - 1, Trail making Test - 2 and LOTCA(Loewenstein Occupational Therapy Cognitive Assessment). Result : Nintendo wii's driving simulation program and RC Car driving program in model road results efficient visual perception ability. This programs results effectively in visual perception ability and space perception ability. This programs results effectively in motor apraxia ability. This programs results effectively in control ability for visual perception. This programs results effectively in thinking operation. Conclusion : Nintendo Wii's driving simulation program and RC Car driving program in model road positively influence improving for visual perceptual ability and cognitive function of elderly people. Also it is considered as being more efficient for improving visual perceptual ability and cognitive function to implement basic rehabilitation training with driving rehabilitation program than basic training itself.

Development of Measurement Method and Contents for Unilateral Neglect using Eye-tracking Technique (시선추적기법을 적용한 편측무시 측정 방법 및 개선 콘텐츠 개발)

  • In this study, using real-time gaze tracking and head tracking method, we intended to quantitatively evaluate the deviation between the patient's head and gaze direction while minimizing inspection errors due to apraxia of conventional paper-based examination respectively. As a result, we developed a software that can quantitatively measure gaze and head movement information, and computerized the line bisection and star cancelation test, which are generally used as conventional paper test. In addition, for the rehabilitation training, contents corresponding to the visual technology of Warren's visual hierarchical model lower level are implemented and can be performed repetitively and independently. This allows the patient to actively participate in rehabilitation and quantitatively compare the degree of improvement.
