본 연구는 전국 보육교사 514명을 대상으로 아동학대 관련 경험, 아동학대 목격 시 대처방안, 아동학대 발생 원인에 대한 인식 및 아동학대 예방 및 근절을 위해 필요한 상담 관련 요구도를 살펴보고자 하였다. 연구결과, 첫째, 보육교사 중 92명(17.9%)이 어린이집 아동학대를 목격한 경험이 있었으며, 목격 후 대처방안으로 동료교사 학대의 경우, '학대 받은 영유아에게 관심을 갖고 따뜻하게 대해주었다', 원장의 경우, '아무런 조치를 취하지 않았다'가 가장 높게 나타났다. 아무런 조치를 취하지 않은 이유로 동료교사 학대의 경우, '보육은 담당교사의 권한이므로 간섭할 수 없어서', 원장 학대의 경우 '신고 후 부과될 책임이나 역할이 부담스러워서'가 가장 높게 나타났다. 둘째, 보육교사 아동학대 발생 원인은 직무스트레스, 과다한 업무, 보육교사 정신건강 문제 순으로 높게 나타났다. 넷째, 보육교사 아동학대 사례 판정 시, 필요한 후속조치로 보육교사의 경우 '교사 자격정지', 영유아 및 부모의 경우 '영유아 대상 심리평가 실시'가 가장 높게 나타났다. 다섯째, 보육교사 아동학대 전문 상담 기관이 신설된다면, 보육교사 중 457명(88.9%)이 사용할 의사가 있으며 본인의 아동학대 신고의사 결정 및 아동학대 예방과 개입에 도움이 될 것이고, 설치 장소로는 '육아종합지원센터 내 상담실'을 요구하였다. 여섯째, 보육교사 중 490명(95.3%)이 보육교사 아동학대 전문상담요원(영유아발달상담전문요원)이 필요하며, 담당 역할은 '영유아 대상 전문적 심리평가 및 사례판정', 자격요건으로는 대졸이상, 심리학 및 아동학 전공, 3년-5년 경력, 자격증 보유를 요구하였다. 마지막으로, 보육교사 아동학대 예방 및 근절을 위해 필요한 조치로는 '아동학대 관련 교육 및 전문적 상담 실시'을 요구하였다. 본연구결과를 토대로 시사점 및 추후연구를 제안하였다.
Purpose : This study was conducted to examine the degree of recognition of safety education and its execution for child-care teachers at daycare center for the disabled children in Jeollanam-do. Ninety six child care teachers filled out the questionnaire and the results were as follows. Results : 1. The possibility of accident prevention by the day care teachers was a little higher in case of nearby education institution. The most appropriate period of safety education was within one month. 2. Majority of child care teachers had received safety and first-aid educations before and the education was done for less than 2 hours. 3. The best educational method was practice-centered education of off-duty time. The contents of first-aid education were bleeding, fall, shock, and burn. 4. The education material and book should be available to the teachers when they need the knowledge. 5. By the annual schedule, safety education should be conducted and provided to the parents. Conclusion : This study suggested that reinforcement of contents related to emergency situation and actual first-aid as well as theory education in the process of parent and teacher education should be conducted on the basis of information of general conditions related to safety and first-aid education at the daycare center for the disabled children.
Introduction: The integration of theoretical material in the classroom with clinical practice in the field is an important concept in nursing education. nursing students at all levels need to acquire individual patient's needs, and applying creativity in the comprehensive nursing care. Problem solving and observation skills are important aspects in the development of nursing skills. Nursing students during their clinical work in maternal and child health observed that the major difficulties experienced by new mothers centered around psychological and physiological changes. Breast engagement and throbbing breast pain were the most frequent complaints by primiparas during the postpartum phase. In order to understand the factors affecting the discomfort, and to devise appropriate nursing care, these complaints were experimented. resent study represents an attempt to evaluate the impact of antepartal care (including breast care) on the subsequent of breast feeding difficulties. For the research purposes, hypotheses were made as follows: 1. There is a relationship between breast care and anthemata care attendance. 2. If primiparas practice treat care during their anthemata relied, they will have less throbbing breast pain during the postpartum phase. 3. If primiparas practice breast care during their antenatal period, they will have less breast engagement during the postpartum phase. Method: The researcher selected two highly specialized nurses in maternity clinical ward. They checked mother's chest circumference on the top line of breast every morning including mother's fever, caked breast, and lymphnodes on axillae. These nurses checked mother's breast while they staying hospital for four days. The total number of primiparas were 62 during June 5th to July 15th in 1971 at the Severance Hospital. For 40 days among 62 members of new mothers, 28 of them had breast care during antenatal period. Rest of them did not have breast care during antenstal period. Result: The result of this research revealed that the first hypothesis was accepted that the group which had breast care during antenatal period, had positive relationship with antenatal care. If the mothers were more educated, the more anticipated to antenatal care including breast care. For the second hypothesis, on the delivery day, there was no change on breast between two groups. On the first day of delivery, there were breast throbbing pains to the group who did not receive breast care, than the group who received the breast care. Therefore, second hypothesis was also accepted. For the third hypothesis, there was no breast engagement difference between two groups for the entire period. The third hypothesis was rejected.
Purpose: To develop staffing levels for nursing personnel (registered nurses and nursing assistants) to provide inpatients with integrated nursing care that includes, in addition to professional nursing care, personal care previously provided by patients' families or private caregivers. Methods: A time & motion study was conducted to observe nursing care activities and the time spent by nursing personnel, families, and private caregivers in 10 medical-surgical units. The Korean Patient Classification System-1 (KPCS-1) was used for the nurse manager survey conducted to measure staffing levels and patient needs for nursing care. Results: Current nurse to patient ratios from the time-motion study and the survey study were 1:10 and 1:11, respectively. Time spent in direct patient care by nursing personnel and family/private caregivers was 51 and 130 minutes per day, respectively. Direct nursing care hours correlated with KPCS-1 scores. Nursing personnel to patient ratio required to provide integrated inpatient care ranged from 1:3.9 to 1:6.1 in tertiary hospitals and from 1:4.4 to 1:6.0 in general hospitals. The functional nursing care delivery system had been implemented in 38.5% of the nursing units. Conclusion: Findings indicate that appropriate nurse staffing and efficient nursing care delivery systems are required to provide integrated inpatient nursing care.
This study was done in order La provide basic data to a Fee System for hospital based Home Health Care services in Korea in the future. It was done by investigating activities provided to possible Home Health Care clients who could be discharged early from genera] hospitals and then estimating the nursing care fee according to each nursing activity based upon the time used for activity. The subjects of the study were selected by convenience sampling and consisted of 35 clients who might be clients for Home Nursing Care and were presently admitted to a medical- surgical ward of Y University Medical Center located in Seoul, Korea. The data collection period was from September 1, 1991 to September 30, 1991. The research in strum nets utilized for the study were a client selection criterial for Home Health Care developed by Choo(l991) and a check-list of nursing activity developed by researcher. The results of the study were as follows : 1. There were 44 different nursing activities provided in the seven days but the time was calculated for only 25 of the nursing activities. 2. Fees for the 25 different nursing activities were calculated by multipling the median of the average wage of a staff nurse having five years experience in an A grade general hospital to the Lime of the nursing activity. The results were compared with the insurance fee which the government recognized as an appropriate fee for that activity. The nursing activities with a lower calculated fee than the insurance fee were suction, catheterization, exercise education and dressing change. The nursing activities with a higher calculated fee than the government recognized fee were 1M injection and vital sign check. 3. There was a range of 1-15 nursing activities provided daily to the client. For the average number of nursing activities per day of 6.26 events the nursing care fee was calaulated at W 6136 per day. 4. Based upon the results of the study, a recommentdation for a Home Health Care fee per visit based on the nursing activities provided could be formulated for a Home Health Care fee system. The nursing care fee per visit presented in this study can be validated through a Home Health Care demonstration project.

Purpose: Immunization is one of the simplest and most effective approaches for protecting the health of children from preventable infectious disease. This study assessed the immunization statuses of children presenting to childcare centers in Korea and compared the sociodemographic factors of children with age-appropriate immunization for DTaP4 with those with delayed immunization for DTaP4. Method: A national mail survey was used to collect data on the children's immunization history and sociodemographic information. A total of 248 mothers returned completed questionnaires for their children. Results: The overall immunization coverage for HepB, a series of DTaP and polio and varicella was above 95%. However, the percentage of age-appropriate immunizations of vaccines ranged from 45.3% to 69.0% depending on the vaccines. Only the age of the child was a significant factor for delayed immunization for DTaP4. Conclusion: To fully protect children from vaccine-preventable disease, childcare providers can playa role in monitoring the immunization statuses of children in their care centers to ensure that they all receive all childhood immunizations on time.
Purpose : This study was conducted to provide for a basic resource, which can be used to set up a efficient management system in Cardiac Surgery Intensive Care Units(CSICU). Method: 1) Questionnaires were administered and observation methods were used, to examine the nursing activities performed in the CSICU after having reviewed related literatures and a review by the experts. Thus, the nursing activities were designating 254 activities and classified into 28 categories. 2)The 22 nurses in the 2 CSICUs filled out questionnaires about nursing activities from 12 April, 2002 to 17 April, 2002. The frequency of the nursing activities in the 28 categories counted and new nursing activities added by directly observing 12 nurses by two trained research staffs for 4 day. 3)In terms of validity, the 264 nursing activities were analysed by the 25 experts. As a result, 231 nursing activities were found valid and remained as appropriate nursing activities to be used for the careful analysis of the nursing activities in CSICUs. Result: The 22 categories are as below: assessment, monitoring, respiration management, nutrition management, elimination/drainage management, mobility management, sanitation management, safety management, temperature management, specimens collection, preparation and assistance of treatment, skin/wound management, infection management, medication management, education/support, dying patient care, recording/keeping, supplies management, environment management, communications, evaluations, professional development Conclusion : The manifest job description of the staff nurse will contribute to improving the efficiency of the nursing activities and to reducing the role conflicts among the medical staffs.
Purpose: This study was conducted to analyze intubation survival rates according to characteristics and to identify the risk factors affecting deliberate self-extubation. Methods: Data were collected from patients' electronic medical reports from one hospital in B city. Participants were 450 patients with endotracheal intubation being treated in intensive care units. The collected data were analyzed using Kaplan-Meier estimation, Log rank test, and Cox's proportional hazards model. Results: Over 15 months thirty-two (7.1%) of the 450 intubation patients intentionally extubated themselves. The patients who had experienced high level of consciousness, agitation. use of sedative, application of restraints, and day and night shift had significantly lower intubation survival rates. Risk factors for deliberate self-extubation were age (60 years and over), unit (neurological intensive care), level of consciousness (higher), agitation, application of restraints, shift (night), and nurse-to-patient ratio (one nurse caring for two or more patients). Conclusion: Appropriate use of sedative drugs, effective treatment to reduce agitation, sufficient nurse-to-patient ratio, and no restraints for patients should be the focus to diminish the number of deliberate self-extubations.
The critical problem of nurse scheduling in the hospital is determining the day-to -day shift assignments for each nurse for the specified period in a way that satisfies the given requirements of the hospital. As nurse scheduling involves many factors and requirements, manual scheduling requires much time and effort to produce an adequate schedule. A PC - based decision support system, developed in Turbo - C/sup ++/, for nurse scheduling was introduced. The system is composed of 4 sub-systems : 1) Entering basic information for each nursing unit : 2) Generation of an appropriate initial schedule and revised schedule for a given period, maximally satisfying each nurse's duty requests : 3) Provision of variety of statistical information, and 4) Help messages for each modular function. Icons and a mouse are used for easier graphic user interface and reducing the need for typing efforts. This system can help nurses develop quick and easy schedule generation and allow more time for the patient care.
Purpose: The purpose of this study was to develop child's health assessment tools for children in child care centers. Methods: Based on the literature review and several types of workshops participated with the child health nursing professors, doctoral students, nurses in pediatric units and pediatric psychiatric units from July to December 2006, the standards of child health assessment tools were developed. Graduate students and 4th grade students in nursing were trained for health assessment and used these assessment tools to validate the content and reliability of said tools. Results: Some record forms were newly developed, including demographic characteristics, past history, present health status, behavioral problems, and appropriate developmental screening tests in child care centers were selected. For systematic health care management in child care centers, daily care report, illness log, and referral sheet were also developed. Conclusion: In the face of growing utilization of daycare and their association with increased risk of various diseases, assessment and control measures are indispensable to the promotion of child health. Children's physical and mental health and developmental problems can be assessed using this assessment tools. They can be used for establishing the direction for developing a health care service system for young children.
