The purpose of this study was to evaluate the sealing ability of a calcium hydroxide plug treated with a bonding agent. Ninety extracted human anterior teeth and premolars with single canal were used in this study. Crowns were removed. the canal's were instrumented. and the roots were randomly divided into three groups of 30 each. In control group. a single apical seat was prepared with #60 K file 1mm short of the apex and the root canal was obturated with Gutta-percha and Sealapex by the lateral condensation method. In experimental group 1 and group 2. to prepare an apical isthmus of 1mm in length. the first apical seat was prepared with a #45 K file 1mm short of the anatomical apex and with a #60 K file 2mm short for the second apical seat. Dry calcium hydroxide powders were packed in the apical isthmus with a hand plugger and #60 K file and then. the root canal was obturated with Gutta-percha and Seal apex by the lateral condensation method. In experimental group 2. following an application of the bonding agent to the plug. the root canal was obturated in the same way. The teeth of each group were immersed in a 2% methylene blue dye solution. for 1, 2, and 4 weeks. The distance from the tip of the cone to the deepest penetration was measured using the Tool maker's microscope. The results were as follows : 1. The teeth having the calcium hydroxide plug treated with the dentin bonding agent (experimental group 2) showed the lowest leakage with 1.4705mm and the control group without apical plug(no apical isthmus) showed the highest leakage with 3.1735mm. 2. The control group without apical plug showed higher leakage than experimental group 1 having the calcium hydroxide plug treated without the dentin bonding agent(p>0.05). 3. The control group without apical plug and experimental group 1, having the calcium hydroxide plug treated without the dentin bonding agent. showed higher leakage than experimental group 2. having the calcium hydroxide plug treated with the dentin bonding agent(p<0.001). 4. The immersion time had no significant effect on the degree of leakage. In conclusion, the results showed that the calcium hydroxide plug treated with the dentin bonding agent could decrease the microleakage from the root apex effectively.
The purpose of this study was to evaluate the apical sealing ability of the Thermafil endodontic obturation technique and to compare it with lateral condensation technique. 42 straight canals from extracted human anterior teeth and 42 curved canals(> $25^{\circ}$) from maxillary and mandibular molar teeth were selected. And 80 of them were divided into four groups, 20 canals respectively. The teeth in prior two groups had straight canals and the other two groups had curved canals. The rest of four canals served as positive and negative controls. After resecting anatomical crowns, all canals were prepared using a standard step-back technique. Lateral condensation was used to obturate two groups, one group of straight ones the other curved. And Thermafil obturators were also used in the same two groups. Obturated teeth were infiltrated by India ink for a week, decalcified and cleared with 5% nitric acid and methyl salicylate. The apical leakage and the frequency of filled lateral and accessory canals were measured with stereomicroscope and also apical extrusion of sealer and gutta-percha and obturation time were checked and the data were analyzed statistically(one-way ANOVA, t-test, Chi-square test). The results were as follows : 1. There was no significant difference in the degree of dye penetration between Thermafil and lateral condensation groups(p>0.05). 2. Apical extrusion of sealer and gutta-percha occurred significantly more often with Thermafil obturators in straight canals(p<0.05), but not significantly different in curved canals(p>0.05). 3. Canal obturation time with Thermafil obturators was significantly faster than lateral condensation (p<0.05). 4. The Thermafil groups showed a higher frequency of filled lateral and accessory canals than in the lateral condensation groups. But the difference was not statistically significant (p>0.05).

Apical sealing with proper retrograde filling material and technique is the key factor for successful apical surgery. In order to provide impeccable apical sealing and healing, various mechanical and chemical characteristics of retrograde filling material is required. MTA has been used as the gold standard of retrograde filling due to its unprecedented advantages. As MTA has long setting time and difficult handling properties, premixed putty type bioceramic material has been newly developed. For efficient retrofilling with premixed putty type biocreamic material, 'Lid technique' was also proposed. The following cases present apical surgery using newly developed premixed putty type bioceramic material with Lid technique.
This study examined the influence of the apical sizes on the sealing ability of a root canal filling. Thirty-six single rooted teeth with a single canal were divided into 3 groups (n = 12) and instrumented with either the $Profile^{(R)}$ or $LightSpeed^{(R)}$ system to achieve three different apical sizes (master apical file [MAF] of #25, #40, or #60). The teeth were filled with gutta perch a using a modified continuous wave technique. The level of microleakage was determined by immersing ten teeth from each group into India ink for 1 week followed by clearing with nitric acid, ethyl-alcohol, and methylsalicylate. The microleakage was measured using vernier calipers. The data was analyzed statistically using Kruskal-Wallis one-way ANOVA and a Student-Newman-Keuls Method. Two teeth from each group were sectioned horizontally at 1, 2, 3 and 4 mm from the apex in order to observe a cross section. The apical size was significantly (p < .05) influenced the level of microleakage. In the Student-Newman-Deuls Method, MAF sizes of #25 and #40; and MAF sizes of #25 and #60, respectively showed a statistically significant difference. There was no significant difference between #40 and #60. In most cross sections, oval-shaped canals were observed, and the irregularity of the internal surface increased with decreasing apical size. There was also an increase in the area of recess, which is the area where the canal space is not filled with either gutta-percha or sealer. When the root canals are filled using a modified continuous wave technique, canal filling with more consistent and predictable outcome may be expected as the apical preparation size is increased.
The purpose of this study was to evaluate the effect of soft chelating irrigant on the sealing ability of root fillings by using a glucose leakage test. A total of 45 single-rooted teeth were selected for the study. The teeth were decoronated leaving a total length of 13mm. The root canals prepared using K3 NiTi rotary instruments to an apical dimension of size 45(0.06 taper). The specimens were then randomly divided into 3 experimental groups of 13 roots each and 2 control groups of 3 roots each. Specimen in each group were prepared with different irrigation protocols : group 1, 2.5% NaOCl; group 2, 2.5% NaOCl and 17% EDTA: group 3, 2.5% NaOCl and 15% HEBP. The root canals were filled with gutta-percha and AH Plus sealer using lateral condensation. After 7 days in $37^{\circ}C$, 100% humidity, the coronal-to-apical microleakage was evaluated quantitatively using a glucose leakage model. The leaked glucose concentration was measured with spectrophotometry at 1, 4, 7, 14, 21 and 28 days. There was a tendency of increase in leakage in all experimental groups during experimental period. HEBP-treated dentin showed no significant difference with EDTA-treated dentin during experimental period. From the 21th day onward, HEBP-treated dentin showed significantly lower leakage than smear-covered dentin. HEBP-treated dentin displayed a similar sealing pattern to EDTA-treated dentin and a better sealing ability than smear-covered dentin. Consequently, a soft chelator(HEBP) could be considered as the possible alternative to EDTA.
The purpose of this study is to evaluate of apical plug materials for the contral of extrusion and sealing ability of high-temperature thermoplasticized gutta-percha in plastic root canal blocks. Seventy seven plastic blocks with canal preformed were instrumented with # 50K file 1 mm beyond apical foramen. Blocks were randomly divided into 5 groups of 15 blocks each. Group 1 was filled by high-temperature thermoplasticized gutta-percha only. The another 4 groups were placed with apical plug materials each other and then remaining space was back filled with high temperature thermoplasticized gutta-percha Apical plug materials were used as follows; Group 2: Thermoplasticized gutta-percha (Thermoplasticized gutta-percha group) Group 3 :. Calcium hydroxide powder (Calcium hydroxide group) Group 4 : Silver point (Silver point group) Group 5 : Gutta-percha cone softened by chloroform (Gutta-percha cone group) All the blocks were stored in 100 % relative humidor at room temperature for 14 days. Filling material extruded was removed carefully and then weighed in analytic balance. Each block was placed in centrifuge tube filled with India ink, and then centrifuged for 20 minutes at 3,000 rpm. Apical leakage was measured from the apical foramen to the most coronal level of dye leakage in millimeter by two examiners under a stereoscope. The data were analysed statistically by Student's t-test The obtained results were as follows; 1. The amounts of material extruded through the foramen decreased in all of groups used apical plug materials (P<0.01). 2. Silver point group and gutta-percha cone group were similar linear leakage to high-temperature thermoplasticised gutta-percha technique only (P>0..5). 3. Calcium hydroxide group and thermoplasticized gutta-percha group showed more liner leakage than high-temperature thermoplasticized gutta-percha only (P<0.01, P<0.05). 4. High-temperature thermoplasticized gutta-percha technique with silver point and gutta-percha cone as apical plugs showed less linear leakage and less extrusion of filling material.
The purpose of this study was to evaluate the apical seal produced by low temperature($70^{\circ}C$) injection gutta percha technique(ULTRAFIL) with & without sealer, warm latero-vertical cindensation technique (ENDOTEC) with & without sealer, and the lateral condensation technique with sealer. 100 extracted, single rooted human teeth were divided into 5 groups and root canals were enlarged & obturated according to the purpose of this study. Obturated teeth were immersed in 2.5% methylene blue for 48hrs. at $37^{\circ}C$ incubator and split. The apical sealing ability was evaluated by measuring the degree of dye penetration into the canal. The results were as follows: 1. All group showed varying depth of dye penetration. 2. There were no significant difference among Group I (lateral condensation), Group II (ULTRAFIL with sealer) & Group IV (ENDOTEC with sealer) (P>0.05). 3. There were less dye penetration when used in conjunction with sealer (P<0.001).
The purpose of this study was to evaluate the effect of heated spreader on the sealing ability of lateral condensation, compared with regular cold spreader. Forty two extracted human teeth with single canal were randomly placed into 3 experimental groups, and four additional teeth were used as positive and negative controls. Each group was prepared with Ni-Ti Profile #40 using step-down technique and obturated with standardized colored gutta-percha cone by standard(cold) lateral condensation technique, warm lateral condensation technique with Endotec and hot spreader soaked in glass bead sterilizer, each with Sealapex sealer. Control groups were not obturated, but prepared. After 2 days in 2% methylene blue, the teeth were invested and made into transparent resin blocks. And then, each block was sectioned horizontally with microtome at 1, 2, 3, 4, 5 mm levels from the apex. The linear extent of dye penetration was examined with stereomicroscope at ${\times}$20 magnification. At each of 5 levels, ratio of the area of gutta-percha was obtained by calculating the area of gutta-percha to the total area of the canal. The data collected were then analyzed statistically using an analysis of variance(ANOVA) and Scheffe test. The results were as follows ; 1. All experimental groups produced the apical microleakage. 2. The mean leakage was 1.57${\pm}$0.76mm for cold spreader group, 0.86${\pm}$0.95mm for Endotec spreader group, and 0.64${\pm}$0.93mm for hot spreader group. The difference between hot spreader group and cold spreader group was statistically significant(p<0.05). 1. At the 1 mm level, the mean ratio of area of gutta-percha was 74.58${\pm}$13.15(%) for cold spreader group, 65.42${\pm}$14.62(%) for Endotec spreader group, and 80.72${\pm}$14.63(%) for hot spreader group. There was statistically significant difference between hot spreader group and Endotec spreader group(p<0.05). 2. At the 2mm level, the mean ratio of area of gutta-percha was 87.86${\pm}$11.22(%) for cold spreader group, 66.55${\pm}$14.02(%) for Endotec spreader group, and 92.93${\pm}$7.24(%) for hot spreader group. There was statistically significant difference between Endotec spreader group and other two spreader groups(p<0.05). 3. At the level 3, 4, 5 mm, there was no statistically significant difference between each group. Within the limits of the results of this experiment, warm lateral condensation technique with hot spreader soaked in a glass bead sterilizer demonstrated favorable apical sealing effect and improved density of gutta-percha mass. Thus, it is thought that this obturation technique is effective for clinical use and beneficial to reduce condensation forces, also economical and easy. The aim of this study was to compare the apical sealing ability of a new thermoplasticized gutta-percha filling technique, the Easy Filling and the Quick Obturation system with lateral condensation technique and Thermafil system to evaluate their clinical acceptabilities. Fifty-two extracted single-rooted teeth were instrumented to #35 using the .04 taper ProFile system. Four groups of 12 teeth were obturated by lateral condensation technique. Thermafil system and two new thermoplasticized gutta-percha techniques, the Easy Filling system and Quick Obturation system (Meta Dental co. Ltd. Korea), respectively. Four teeth served as controls. After the teeth were immersed in 2% methylene blue dye for 48 hours, they were resected horizontally at 1mm to 5mm level from the anatomical apex using a low-speed microtome. Each section was examined under a stereomicroscope at ${\times40}$ magnification and photographed. After each image was scanned, the leakage area was measured at each level using Brain 3 (Nosdia Tech., Korea) software. Leakage ratio was calculated for each group and was analyzed statistically to come up with the following results: 1. At 1mm level, the Quick Obturation system had the largest amount of apical leakage and it was statistically significant when compared with the lateral condensation group and the Thermafil group (p<0.05). 2. At 2mm and 3mm level, there were no significant difference of apical leakage among all four groups (p>0.05), and from 4mm level, no apical dye penetration was observed in all the groups. In conclusion, the apical seal produced by Easy Filling system and the Quick Obturation system was comparable to lateral condensation technique and Thermafil system except for the 1mm level. More improvement of the apical seal can be expected as the operator becomes skillful with the new techniques.
This study was conducted to evaluate the sealing ability of McSpadden technique compared with lateral condensation and Engine reamer technique, and according to the degree of canal enlargement and the use and nonuse of sealer. Seventy single-rooted teeth were divided into 14 groups and each tooth was enlarged and obturated according to the purpose of this study. Obturated teeth were infiltrated by Indian ink and decalcified and cleared. The apical sealing ability was evaluated by measuring the degree of ink penetration into the canal. The results were as follows: 1. All the teeth filled by lateral condensation using gutta-percha cone and sealer proved better in canal sealing ability than McSpadden technique and Engine reamer technique, showing less ink penetration. 2. In the McSpadden technique, there was less ink penetration detected in using the sealer than not. 3. In all of the experimental methods, the difference of the degree of ink penetration according the degree of canal enlargement could not be accepted. 4. There was no significant difference in ink penetration degree between Engine reamer technique using sealer and McSpadden technique without sealer.
