The purpose of this study was to compare the shape of root canal after instrumentation with some engine driven NiTi files. Thirty narrow and curved canals(15-35 degree) of mesial canals of extracted human mandibular first molars were divided into three groups. Group 1: After radicular access with Gates Glidden drill, apical shaping using step back method with Flexo file Group 2: After radicular access with Gates Glidden drill, apical shaping with Profile .04 Group 3: Canal shaping with GT file and Profile .04. Using modified Bramante technique, the root was sectioned at 2 mm from apical foramen, height of curvature, 2 mm from canal orifice. Canal centering ratio, amount of transport, amount of dentin removed, shape of canal were measured and statistical analysis is done using SPSS Program V 7.5. The results were as follows: 1. Canal centering ratio of group 3 was the lowest at coronal part, but there was no statistical difference. Centering ratio of group 2 was the lowest at curve part, and there was statistical difference between group 1(P<0.05). Centering ratio of group 2 was the lowest at apical part, but there was no statistic difference. 2. Amount of transport of group 3 was the lowest at coronal part, but there was no statistical difference. Amount of transport of group 2 was the lowest at curve part, and there was statistical difference between group 1(P<0.05). Amount of transport of group 3 was the lowest at apical part, and there was statistical difference between group 1 and group 2, group 1 and group 3(P<0.05). 3. Amount of dentin removed of group 3 was the lowest at coronal part, bur there was no statistical difference. Amount of dentin removed of group 2 was the lowest at curve part, but there was no statistical difference. Amount of dentin removed or group 2 was the lowest at apical part, and there was statistical difference between group 1 and group 2, group 1 and group 3(P<0.05). 4. The shape of the canals after instrumentation varied among the groups. The majority of canals at coronal and curve part for group 1 were round in shape(7 in 10), those at apical part were oval(8 in 10). The majority of canals at coronal part for group 2 were round in shape(7 in 10) and there was no difference in the number of shape at other part. There was no difference in the number of shape at every part for group 3. As above results, NiTi rotary instrumentation showed a trend to remain more centered in the canal than SS file instrumentation. At using NiTi file, coronal shaping with Gates Glidden drill was not statistically different from shaping with GT file. But shaping with GT file showed tapered canals, so it may be said that shaping with GT file is a safe and valuable instrumentation method.
The purpose of this study was to compare the shaping time of two shaping methods and the leakage of three different obturation techniques. Ninty three canaled human molar teeth were used, which were randomly divided into two groups of forty teeth each and ten control teeth. After working length determination, the one group was prepared crown-down technique using rotary root canal instruments of GT rotary files .12/20, .10/20, .08/20 and .06/20 taper(Maillefer Instrument SA. Switzerland). The other group was instrumented with Gates Glidden burs(#1, #2, and #3) to coronal preparation and GT rotary files .08/20 and .06/30 taper to apical preparation. Shaping time was measured. After root canals were instrumented, they were divided to three subgroups and obturated as follows : Subgroup 1, obturated with single cone method Subgroup 2, obturated with lateral condensation : Subgroup 3, obturated with continuous wave technique. Three subgroups were obturated using non-standardized gutta-percha cone(Diadent, Korea, .06 or .08 taper) and AH-26(Dentsply DeTrey, Germany) as a root canal cement. Ten unobturated teeth served as positive and negative controls. After immersion in 2% methylene blue solution for 1 month, the teeth were washed during 24h. The teeth were demineralized in 10% nitric acid and dehydrated by immersion in 80, 90 and 100% ethyl alcohol. The teeth were finally cleared and stored in 100% methylsalicylate, and apical dye penetration was evaluated under stereomicroscope(Leica M420, LC, U.S.A)at $\times$8.75 magnification. Liner measurement of dye penetration was assessed with the use of digitalized image analysing system (analySIS, GmbH, Germany) The data were analysed statistically using independent T-test and Two-way ANOVA and Tukey test. The result were as follows 1. In canal prepared with GT$^{TM}$ rotary file, shaphing time taked more than the group of using Gates Glidden drill to coronal preparation without statistical significance (p>0.05) 2. The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files showed significantly more apical leakage than those of lateral condensation and continuous wave technique regardless of shaping method (p<0.05). 3 The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files and Gates Glidden drill showed significantly more apical leakage than those of continuous wave technique regardless of shaping method (p<0.05). 4. Regardless of shaping method, The group of continuous wave obturation showed less apical leakage than those of lateral condensation without statistical significance (p>0.05). 5. The group of single cone obturation using canal preparation of GT$^{TM}$ rotary files and Gates Glidden drill showed more apical leakage than the group of lateral condensation using same shaping method with-out statistical significance (p>0.05).
Objectives: The study was to compare the shaping ability of Reciproc (VDW) and WaveOne (Dentsply Maillefer) instruments compared with ProTaper, Profile and hand instrument during the preparation of simulated root canals. Materials and Methods: Five groups (n = 5) were established. Reciproc, WaveOne, ProTaper, Profile and K file (K-flexo file) were used to prepare the resin simulated canals. A series of preoperative and postoperative images were taken by a microscope and superimposed in 2 different layers. The amount of resin removed from both the inner and the outer sides of the canal was measured to the level of 10 mm from the apical tip, with a 1 mm increment. Results: The mean of resin removal from the inner canal wall was not different from the outer canal wall for Reciproc and WaveOne groups at apical third (1 - 3 mm level). There was no difference in the change of working length and maintenance of canal curvature. NiTi instruments are superior to stainless-steel K file in their shaping ability. Conclusions: Within the limitation of this present study, Reciproc and WaveOne instruments maintained the original canal curvature in curved canals better than ProTaper and Profile, which tend to transport towards the outer canal wall of the curve in the apical part of the canal.
The purpose of this study was to compare the shaping abilities of LightSpeed, ProTaper-Universal, and hybrid technique using S-series of ProTaper-Universal and LigthSpeed. The 72 simulated root canals of J-shape were used and classified as flowing 3 groups according to the instrumentation methods; Group P of 24 canal blocks were prepared with ProTaper-Universal, Group L was prepared with LightSpeed, and Group H was prepared with hybrid technique (initial shaping with ProTaper-Universal SI and S2 and apical shaping with LightSpeed from #25 to #50). A second-year resident of Endodontic department prepared the resin block canals to apical size #50 (F5 in Group P). The time lapses for instrumentation and the reduction of root canal curvature after shaping were measured. The pre- and post-instrumented root canals were scanned and superimposed to evaluate and calculate the increased canal width and apical centering ratio. The results were as followings: Group Land H showed significant less instrumentation time than Group P (p < 0.05). The ProTaper system showed greater reduction of root canal curvature and working length diminishment than other methods (p < 0.05). LightSpeed system showed best canal curvature preserving characteristics. The Group P had greater instrumented widths at all levels examined (p < 0.05). Group L and Group H showed lower centering ratio (ability to preserve the canal center; the lower ratio means the better canal center preservation) than Group P (p < 0.05). Group H had the lowest centering ratio at the 1 mm level.
The purpose of this study was to compare the initial apical file(IAF) first Ole that fits to the apex in each canal before and after early flaring to analyze if the size of file to fit to the apex would increase after flaring. Eighty anterior teeth with complete apical formation and patent foramens were selected. The samples were randomly divided into 4 groups(GG, OS, GT, PT Group) of 20 teeth each. A file was fit to the apex in each canal and that size recorded. Radicular flaring were completed using different types of instruments. After flaring a file was again fit to the apex in the same manner as before and its size recorded. The results of this study were as follows : 1. The mean diameter of IAF before flaring(file diameters in $mm{\times}10^{-2}$) was $19.81{\pm}8.32$ before and $25.94{\pm}9.21$ after(p<0.05). 2. The increase in diameter of IAF was approximately one file size for all groups. 3. Ranking of increasing diameter of IAF were GG>CT>OS>PT group. There was a statistically significant difference between before and after flaring(p<0.05). 4 Ranking of the time for flaring were GG>GT>OS>PT group. There was a statistically significant difference between GG group and other groups(p<0.05). 5. In the case without change of IAF diameter, they showed decrease in force after flaring when IAF was pulled out from root canal(p<0.05). This study suggested that early radicular flaring increases the file size that is snug at the apex, and awareness of that difference gives the clinician a better sense of canal size. Early flaring of the canal provides better apical size information and with this awareness, a better decision can be made concerning the appropriate final diameter needed for complete apical shaping.
Objectives: The present study aims to compare the obturation quality of 2 confluence confirmation techniques in artificial maxillary first premolars showing Vertucci type II root canal configuration. Materials and Methods: Thirty artificial maxillary premolars having Vertucci type II root canal configuration were made. They were divided into 3 groups according to the confluence confirmation technique as follows. Gutta-percha indentation (GPI) group (confluence confirmation using a gutta-percha cone and a K file); electronic apex locator (EAL) group (confluence confirmation using K files and EAL); and no confluence detection (NCD) group. In the GPI group and the EAL group, shaping and obturation were performed with the modified working length (WL). In the NCD group, shaping was performed without WL adjustment and obturation was carried out with an adjusted master cone. Micro-computed tomography was used before preparation and after obturation to calculate the percentage of gutta-percha occupied volume (%GPv) and the volume increase in the apical 4 mm. Data were analyzed using 1-way analysis of variance and post hoc Tukey's test. Results: Statistically significant difference was not found in terms of the %GPv from the apex to apical 4 mm. However, the NCD group showed a statistically significant volume increase compared with the EAL group (p < 0.05). Conclusions: In terms of gutta-percha occupied volume, no significant difference was observed among the 3 groups. Confluence confirmation using an EAL in teeth with Vertucci type II configuration showed less volume increase during canal shaping compared with no confluence confirmation.
Use of an apical plug in management of cases with open apices has gained popularity in recent years. Biodentine, a new calcium silicate-based material has recently been introduced as a dentine substitute, whenever original dentine is damaged. This case report describes single visit apexification in a maxillary central incisor with necrotic pulp and open apex using Biodentine as an apical barrier, and a synthetic collagen material as an internal matrix. Following canal cleaning and shaping, calcium hydroxide was placed as an intracanal medicament for 1 mon. This was followed by placement of small piece of absorbable collagen membrane beyond the root apex to serve as matrix. An apical plug of Biodentine of 5 mm thickness was placed against the matrix using pre-fitted hand pluggers. The remainder of canal was back-filled with thermoplasticized gutta-percha and access cavity was restored with composite resin followed by all-ceramic crown. One year follow-up revealed restored aesthetics and function, absence of clinical signs and symptoms, resolution of periapical rarefaction, and a thin layer of calcific tissue formed apical to the Biodentine barrier. The positive clinical outcome in this case is encouraging for the use of Biodentine as an apical plug in single visit apexification procedures.
The purpose of this study was to compare the shaping ability of the two different Ni-Ti file systems and the two different engine systems in simulated canals. A total of four groups of each 10 were tested. Each group was instrumented with HeroShaper and Endo-Mate2 (Croup HE), HeroShaper and Tecnika (Croup HT), ProFile and Endo-Mate2 (Group PE), and ProFile and Tecnika (Croup PT). Canal preparation time was recorded. The images of pre- and post- instrumented root canals were scanned and superimposed. The amounts of increased width and centering ratio were measured and calculated at apical 1, 3 and 5mm levels. These data were statistically analyzed with one-way ANOVA and Duncan's multiple range test The results of this study were as fellows ; 1. Canal preparation time of HT group was the shortest (p<0.05). 2. The amount of increased canal width in HE group was significantly larger than PT group at apical 1mm level (p<0.05) At apical 3mm level, PT group was significantly smaller than other groups (p<0.05). At apical 5mm level, PE group was significantly larger than PT group (p<0.05). 3. The amount of centering ratio in HE group was significantly larger than other groups (p<0.05). At apical 5mm level, HT group was significantly larger than PE group and PT group (p<0.05). Under the condition of this study, torque-controlled endodontic motor is safer than no torque controlled motor, especially when the active file is used.

Objectives: The aim of this study was to evaluate the shaping ability of the TruShape and Reciproc Blue systems and the apical extrusion of debris after root canal instrumentation. The ProTaper Universal system was used as a reference for comparison. Materials and Methods: Thirty-three mandibular premolars with a single canal were scanned using micro-computed tomography and were matched into 3 groups (n = 11) according to the instrumentation system: TruShape, Reciproc Blue and ProTaper Universal. The teeth were accessed and mounted in an apparatus with agarose gel, which simulated apical resistance provided by the periapical tissue and enabled the collection of apically extruded debris. During root canal preparation, 2.5% sodium hypochlorite was used as an irrigant. The samples were scanned again after instrumentation. The percentage of unprepared area, removed dentin, and volume of apically extruded debris were analyzed. The data were analyzed using 1-way analysis of variance and the Tukey test for multiple comparisons at a 5% significance level. Results: No significant differences in the percentage of unprepared area were observed among the systems (p > 0.05). ProTaper Universal presented a higher percentage of dentin removal than the TruShape and Reciproc Blue systems (p < 0.05). The systems produced similar volumes of apically extruded debris (p > 0.05). Conclusions: All systems caused apically extruded debris, without any significant differences among them. TruShape, Reciproc Blue, and ProTaper Universal presented similar percentages of unprepared area after root canal instrumentation; however, ProTaper Universal was associated with higher dentin removal than the other systems.
Objectives: The present study aimed to evaluate the shaping ability of 2 thermally treated nickel-titanium reciprocating systems in simulated curved canals. Materials and Methods: Forty simulated canals were prepared to apical size 25 using Reciproc Blue R25 (VDW) and WaveOne Gold Primary (Dentsply Sirona) instruments. Standard pre- and post-preparation images were taken and superimposed. The removal of resin material was measured at 5 standard points: the canal orifice, halfway between the canal orifice and the beginning of the curve, the beginning of the curve, the apex of the curve, and the end-point of the simulated canal. The data were analysed using the independent sample t-test with a 5% significance threshold. Results: The canals in which Reciproc Blue R25 was used showed a significantly greater widening than those in which WaveOne Gold was used at 4 of the 5 measurement points (p < 0.05). The Reciproc Blue R25 instrument removed significantly more resin from the inner aspect of the curve at 2 of the 5 points and similar amounts at the remaining 3 points. At the 2 apical points, there was no significant difference between the Reciproc Blue R25 and WaveOne Gold Primary instruments. Conclusion: Both instruments respected the original canal anatomy; however, WaveOne Gold resulted in a more conservative shape with less transportation.
