Immunoglobulin G4-Related Aortitis of the Abdominal Aorta

  • Noninfectious aortitis, inflammatory abdominal periaortitis, and idiopathic retroperitoneal fibrosis are chronic inflammatory diseases with unclear causes. Recent studies have shown that some cases of aortitis are associated with immunoglobulin G4 (IgG4)-related systemic disease. Herein, we report a case of IgG4-related aortitis (IgG4-RA) that was diagnosed after surgery. Our patient was a 46-year-old man who had experienced abdominal pain for several weeks. Preoperative evaluations revealed an area of aortitis on the infrarenal aorta. He underwent surgery, and histological examination resulted in a diagnosis of IgG4-RA.

  • Bacterial endocarditis is a well-recognized complication of prosthetic valve replacement. Postoperative fungal endocarditis of aortitis has been reported with increasing frequency. Aspergillus endocarditis or aortitis following open heart surgery has been reported in several patients. The difficulty in the diagnosis of this type of infection, the problems of therapy of prosthetic valve endocarditis0 and the relative ineffectiveness of antifungal agents account for the high mortality. Recently, we have experienced a patient with aspergillus aortitis after replacement of mitral valve. The diagnosis was finally established by histotogical examination of emboli removed from the femoral artery. The patient died after second open heart surgery for replacement of ascending aorta.

  • Isolated stenotic lesion located at the ostium of the left main coronary artery associated with Takayasu`s aortitis is quite rare. This report herein described a case of 25 year old woman with isolated ostial stenosis of the left main coronary artery underwent pericardial patch [fixed with 0.6 % glutaraldehyde] angioplasty. An anterior approach was used and postoperative coronary angiogram of the patient showed normal coronary ostial contour with normal runoff.

Tuberculous Aortitis Complicated with Pseudoaneurysm Formation in the Descending Thoracic Aorta: A Case Report

  • A 51-year-old male with sustained fever was diagnosed with military tuberculosis and tuberculous aortitis complicated with pseudoaneurysm formation at the proximal descending aorta. A follow-up computed tomography evaluation showed an increased size of the pseudoaneurysm in this area, suggestive of a contained rupture. Consequently, the patient underwent emergency excision and replacement of the aorta using a left heart bypass. The patient was discharged without postoperative complications on post-operative day 12. During the one-year follow-up period, the patient was free of any complications or recurrence of tuberculosis. We report a case of pseudoaneurysm of the descending aorta that was successfully surgically repaired.

대동맥판막질환의 임상적 고찰 -판막병리 및 임상성적에 대하여- (Isolated aortic valvular heart disease : analysis of etiology and surgical experience)

  • Clinical and pathologic data were reviewed in 20 patients who had have surgery for isolated aortic valvular heart disease between April 1978 and April, 1987. Hospital mortality was 10%, with no additional late mortality during a mean follow-up period of 24.1 months. Prosthetic valve failure developed in 3 patients and two had reoperation. Niety four percent of the survivors who were in NYHA Funtional class III or IV before operation are now in class I or II. Ninety percent of all patients are still alive at a maximum follow up of 9 years. The clinical histories, gross and histologic examination of valves estabilished the causes for isolated aortic valve disease: 3 rheumatic, 2 congenital bicuspid, 2 hypertention, 2 aortitis and each one case of floppy valve, medial cystic necrosis of aorta, bacterial endocarditis. But etiology was unknown in 8 cases. Sixteen patietns had myxoid degeneration, defined as significant disruption of the valve fibrosa and its replacement by acid mucosaccharides and cystic changes. Myxoid degeneration was also the primary pathologic abnormality in the patients with 2 hypertention, 2 rheumatic, 1 aortitis, 1 bacterial endocarditis, 1 floppy valve, 1 congenital bicuspid. The patients with myxoid degeneration of uncertain origin were 8. Histologic finding of all of them revealed nonspecific patients with myxoid degeneration of uncertain orgin were 8. Histologic finding of all of them revealed nonspecific chronic valvulitis with myxoid degeneration. This finding may indicate that the etiology w uld be infectious.

대동맥-기관지루를 동반한 결핵성 대동맥염 (Tuberculous Aortitis with Aorto-bronchial Fistula)

  • 결핵성 대동맥염은 그 빈도가 드문 질환이다. 이 결핵성 대동맥염으로 인한 합병증인 동맥류의 발생이나 대동맥-기관지루의 형성은 더욱 희귀하며, 만일 대동맥의 파열이 동반된다면 그 사망률은 치명적으로 매우 높다. 객혈을 주소로 내원한 환자가 있다면, 결핵의 병력이 없더라도 즉시 흉부 컴퓨터 단층촬영을 시행해야 신속하고 정확하게 이 질환을 진단할 수 있다. 환자는 46세 남자로, 내원 하루 전부터 시작된 객혈과 흉부 불편감을 주소로 내원하였다. 시행한 흉부 컴퓨터 단층촬영상 하행성 흉부 대동맥의 동맥류 소견이 관찰되었으며, 대동맥-기관지루를 통해 좌측 폐하엽과 연결되어 있었다. 본원에서는 하행성 흉부 대동맥의 인조혈관 치환술 및 좌측폐하엽 절제술을 시행함으로써 좋은 결과를 얻을 수 있었다. 조직학적 검사 결과 결핵으로 확진되어 항결핵제 치료를 시작하였으며, 향후 장기간의 항결핵제 복용이 추천된다.

  • Bronchoartic Fistula Secondary to Pulmoanry Tuberculosis Bronch-aortic fistula is a exceptionally rare complication of pulmonary tuberculosis. We report herein, a case of 39 years woman who underwent successful repair of aor-tobronchofistula. She was admitted because of massive hemoptysis via emergency room, she had several bouts of massive hemoptysis prior to hospitalization. Thoracic-aortic pseudoaneurysm had detected by chest CT by chance. The eroded, perforated descending aorta was repaired with patch aortoplasty during temporarily clamping, followed by Left lower lobectomy and omentopexy. Pathological examination revealed pulmonary tuberculosis of superiror seg. of lerg lower lobe and aortitis. The patient had uneventful recovery was well at OPD follow-up check.

  • A rare syphilitic abdominal aortic aneurysm associated with pulmonary embolism and deep vein throbosis is reported. We have experienced a huge infrarenal syphilitic abdominal aortic aneurysm which caused venous compression at left common iliac vein. A 29 year-old female was admitted via emergency room due to several episodes of hemoptysis. Clinical evaluation for this patient revealed a couple of small ill-defined masss densities on the both lung field and abominal aortic anuerysm. Emergency wedge resections of left upper and lower lobes were performed because of a massive hemoptysis. Pathologic diagnosis showed pulmonary infarction. 20 days after thoracotomy an elective operation on abdominal aortic anuerysm was successfully carried out and post operative pathology finding showed syphilitic aortitis.

Takayasu 동맥염에 병발한 관상동맥질환 치험 1예 (Surgical Correction of the Coronary Artery Disease in Takayasu`s Arteritis)

  • Coronary artery involvement and myocardial ischemic symptom in Takayasu`s arteritis is uncommon Its presentation as coronary artery narrowing is a potentially lethal but correctable problem. In this case report, a 17-year-old woman of Takayasu`s arteritis with unstable anginal and moderate heart failure is presented. Her coronary angiogram showed that the main trunk of the left coronary artery was moderately narrowed and the proximal portion of the circumflex branch was severely obstructed and the right coronary artery was also narrowed diffusely. Simultaneously the patient had the moderate degree of mitral regurgitation. In order to save her life, the coronary bypass surgery using the saphenous veins and the modified Wooler`s mitral annuloplasty were done urgently, Immediate recovery was uneventful and the postoperative exercise capacity was markedly improved. But the long-term prognosis seems to be uncertain because of 3 reasons: 1] natural progress and complication of Takayasu`s arthritis; 2] fate of the saphenous vein grafts in a relatively young patient with aortitis; 3] residual mitral regurgitation. So long-term follow-up should be needed.

복부 대동맥류의 외과적 치료 (Surgical Treatment of the Abdominal Aortic Aneurysm)

  • We experienced 12 patients with the abdominal aortic aneurysm during last 31 years [Dec. 1958 \ulcornerSep. 1989]. Among them, 10 patients were reviewed. They were all male. The age ranged from 34 to 80 years with the mean age of 59.4 years. The etiology of the aneurysm was atherosclerotic in 8, mycotic in 1, and aortitis in 1. The location of the aneurysm was infrarenal in 8, and suprarenal in 2 cases. Aneurysmectomy and Dacron Y-graft interposition in 8 cases, and lease with Teflon Y-graft were made. In another 1 case, long thoracoabdominal bypass surgery was made. The operative mortality was 30%[3cases]. The postoperative complications were respiratory complications[3cases], acute renal failure[2cases], bleeding[lease], mechanical ileus[ lease], and peritonitis[lease].
